GREATEST SOCIAL CHALLENGE OF OUR TIME

Two- thirds of people admitted to hospital are over 65

Readmissions within 30 days costs the NHS £2.2 billion per year

In 10 years re-admissions have risen 88% for those 75+

AVOIDING UNHAPPY RETURNS
