SAMOHI BAND 2018-2019 ALL-BANDS REGISTRATION PACKET Welcome to the Santa Monica High School Bands! We are very happy to have your student in our fantastic band program. This All-Bands Registration Packet provides important information about the program and is a necessary part of registering your student for Samohi Concert Band, Jazz Band, Marching Band and/or Color Guard classes. Any information you provide will be kept by the Directors and used only to send you school and Band information throughout the year. You may register by printing these forms OR by using the online registration system on the Bands website www.samohiband.com. Either way, the Off-Campus Activity Form and Student Emergency Card must be printed, signed and returned to school. Printed forms are also available in the Band Room (M102). Marching Band & Color Guard students: Registration is required for participation in Band Camp. Please complete registration and return any printed forms before the first day of Band Camp, August 6, 2018. Concert Band students not enrolled in Marching Band: Please complete registration and return any printed forms by to the Directors by Friday, September 7, 2018. PERFORMANCE ATTIRE & INSTRUMENT / EQUIPMENT AGREEMENT

CONCERT BAND PERFORMANCE ATTIRE All four Samohi Concert Bands perform at last three concerts per year. Student participation in these concerts is mandatory and counts towards the student’s Concert Band grade. Concert Bands Performance Attire: In order to perform with the group, Concert Band students are required to wear the approved Concert Band Black Dress OR a tuxedo, black suit, or black pants and black coat with a white button-down dress shirt. Concert Band Dresses: New Concert Band Black Dresses cost $75, and the Bands will order them for you, if you decide to purchase one. They are machine washable, and made of flexible fabric that can easily be worn through 4 years of Concert Band. All dresses are very long and must be hemmed before wearing. The Bands have dresses available to lend to students. We ask for a contribution of $20/year to cover cleaning & storage. If you borrow a Black Dress, you must remove the hem when you return it. You may purchase dresses using our online forms at www.samohiband.com or pay with cash or check on the dress fitting day. Tuxedo: Tuxedos can be purchased for less than $100 from JC Penny, Amazon and other discounted Men’s stores. Tuxedos can also be rented for the year from Elite Tuxedo, which will fit students during all Concert Band class periods on a date to be announced on the Bands' website. Elite will alter the suits and/or exchange tux sizes throughout the year at no extra charge. Order forms will be sent to all Band students by email, and printed forms will be available in the Band Room shortly before the tuxedo fitting day.

CONCERT BAND PERFORMANCE ATTIRE AGREEMENT I understand that Samohi Concert Band students must wear approved formal attire to perform in mandatory Concert Band performances. At the time of his/her fitting, my student will inform the Bands if we intend to purchase or rent formal wear from the Samohi Bands. INSTRUMENT, MUSIC and EQUIPMENT AGREEMENT I acknowledge and agree that I am personally responsible for the care and safety of all school-issued instruments and equipment provided to my student. Those responsibilities include but are not limited to the following:

• being responsible for ANY damage and NECESSARY REPAIRS caused by our negligence • maintaining and caring for the instrument according to the usual recommended practices, as if it were our

own • replacing any instruments or equipment lost, stolen or damaged beyond repair with another of equal

value • forbidding anyone else to use the instrument(s) or equipment • returning all instrument(s) or equipment when requested • using the instrument(s) and/or equipment exclusively for the preparation and performances of the school

or District's classes and organizations, unless specifically authorized by the Directors I also acknowledge and agree to follow all rules regarding the care and return of all music issued to my student, and I agree to pay for any lost, stolen or damaged music. I also understand that

2018-19 SAMOHI MARCHING BAND &

COLOR GUARD UNIFORM AGREEMENT

Each Marching Band and Color Guard student will be issued a casual uniform shirt and jacket, which are the students’ to keep. If a student loses his or her casual uniform, s/he must purchase a replacement to wear at events where the casual uniform is required. Replacement shirts cost $12, and jackets cost $30. Students will also be issued a performance/competition uniform that will remain the property of Santa Monica High School. Marching Band and Color Guard students will be fitted for uniforms during Band Camp. Parent's acknowledgement: I understand that my student is responsible for the care of the issued uniforms and equipment, and I understand that the below rules apply to all uniform assignments. I give permission for my student to be issued all uniform articles, and I agree to take full financial responsibility for any uniforms or uniform parts that are damaged, lost or stolen due to my student's negligence. 2018-19 VIKING MARCHING BAND & COLOR GUARD PAYMENTS & CONTRIBUTIONS

MARCHING BAND FINANCIAL PLEDGE: THANK YOU for your support of our instrumental music program. The Samohi Band program spends an average of $750 per student in Marching Band or Color Guard. This covers the costs of training, outfitting and equipping the students all year long, beginning with Band Camp and ending with the Fourth of July Parade. To help pay these costs, the Samohi Band Parents Association asks the families of Marching Band & Color Guard students to contribute $750 to the program for school year 2018-2019. Please join us in our effort to achieve 100% participation. There is no deadline for donations. All students are able to participate in all aspects of the program, regardless of their ability to contribute financially to our fundraising efforts, and there is no required minimum contribution. However, the Band needs money soon to pay instructor salaries during Band Camp (average of $250 per student to cover costs). Families who donate more than $750 will be helping to sponsor students who would otherwise not have access to this valuable experience. All donations will be acknowledged in Samohi Concert Band programs. Donations are recognized on a cumulative basis, at the levels indicated below. Please donate or make a pledge today!

*A $1,000 donation covers the yearly class costs for 1 student participating in both the Concert and Marching Band programs. **A $750 donation pays for 1 student participating in Marching Band or Color Guard. MARCHING COMPETITION MEALS: This year, all marching musicians and color guard are asked to pay $50 per student to cover the costs of meals (typically dinners) at competitions, as well as snacks and water for competitions and football games. For many students, communal meals are their favorite part of marching season. Please use the back of this page to indicate any dietary restrictions or other information that we should pass on to our food planners. Please contribute to the cost of feeding your student. Contributions over $50 will be acknowledged as donations to help cover the costs of feeding our band director, staff, and volunteers. � I will contribute to help pay for meals (amount): $

We have a small supply of used Dinkles, but we cannot guarantee that all sizes will be available. Your student will try the shoes on for size at the uniform fitting.
