SAP Appeal Instructions Federal regulations require Midway University to establish Satisfactory Academic Progress (SAP) standards. Students must maintain minimum SAP standards to maintain eligibility for financial aid. Students who fail to meet SAP standards are suspended from receiving financial aid until regaining eligibility or until an appeal is approved by the Financial Aid Department. Note: Taking pre-requisite courses for admission into your program is not considered an extraordinary circumstance. APPEAL REQUIREMENTS 1. Complete the SAP Appeal Form (steps 1-3), which includes a written letter describing the circumstances that prevented you from meeting SAP (example: illness, injury, second degree, loss of job, etc.), as well as the steps you will take to ensure better performance in the future (using a tutor, change in medicine, counseling, etc.). If you had multiple issues - select the issue that was the most impactful. You must provide supporting documentation for all issues you list that impacted your performance. 2. Complete the Timetable of Remaining Coursework for Program Completion form (step 4) indicating the coursework needed and term in which you will complete it. Please consult the Midway University Catalog for specific degree requirements. You can work with your academic advisor, the Registrar's Office, or the Student Development Center to determine which courses you still need to fulfill your degree requirements. Sign and return the completed form along with all supporting documentation (steps 1-5) to the Financial Aid Office. APPEAL GUIDELINES Before completing and submitting this appeal, review the SAP Policy at www.midway.edu/admissions-aid/cost-aid/ my-aid-awards/satisfactory-academic-progress/ to ensure you understand the necessary requirements. Lack of knowledge of the SAP Policy is not grounds for the approval of an appeal. Be specific when explaining your circumstances. Lack of information will result in a delay or potential denial of your appeal. Refer to the SAP Appeal procedures for details regarding acceptable types of SAP appeals and documentation you are required to provide with your submission. Do not discuss your need for financial aid in your appeal, as this is not a valid reason for approval of an appeal. Complete all items on the appeal form. Incomplete appeals will delay decisions and potentially result in a denial. The appeal form is two (2) pages (not including this cover sheet). If you have questions about completing the appeal form, please contact the Financial Aid Office. APPEAL PROCEDURE Submit your appeal to the Financial Aid Office. Appeals are due within two (2) weeks of the date of your ineligibility notification. Late submissions may jeopardize your financial aid. Please note that filing a SAP Appeal does not guarantee continued eligibility for Financial Aid. Students who have been suspended from receiving financial aid may appeal that decision by completing a SAP Appeal. The appeal must contain a description of the circumstances which led to their unsatisfactory SAP status and what actions they will take to improve their performance moving forward. In addition, if the student has been suspended for Max Time Frame, they must submit a completed Timetable of Remaining Coursework for Program Completion form. The Financial Aid Office will review all appeals and may refer cases to the SAP Appeals Committee. The student will be notified in writing via their Midway student email of the outcome within three (3) weeks of submission of a complete appeal. If the appeal is from a former student, the decision will be mailed to the address on file. If your appeal is approved, any extension of financial aid eligibility will be limited to only those courses that are required to complete your academic program. This appeal is for financial aid purposes only. If you remain enrolled in your courses once the semester begins, you will be responsible for payment of your fees regardless of the appeal decision. The Financial Aid Office cannot guarantee a decision will be rendered prior to the start of the term. To avoid the risk of class cancellation, satisfactory payment arrangements should be made with the Business Office.

Attach a signed, and dated written statement explaining the reason for your appeal with a description of the steps you have taken (or will take) to address the above circumstance(s) and ensure better academic performance in the future.
