A Brief History of the ClinicThe clinic was developed with a grant from the Department of Health and Human Services to start a pilot clinic to serve the migrant and seasonal farm workers of the area along with uninsured and underinsured. Community Health Partnership of Illinois (CHP) began as the clinic’s parent organization and fiscal agent, but an advisory consortium of CHP, the Mental Health Board and the Health Department helped to ensure that services are not duplicated and to realize the goal of an integrated clinic for physical and mental health.

The Clinic officially opened its doors to patients on April 1, 1996. It was expected that the Clinic would see approximately 500-1000 visits each year. The Clinic now sees over 750 visits each month.

In early 1998, the FHPC received Community Development Block Grant (CDBG) funds for rent and utilities to open a satellite clinic in the Woodstock area. Other fund-raisers and donations supported this effort and a merger with the McHenry Free Clinic run by Dr. James Mowery was announced. The Woodstock clinic opened its doors in January 1999.

In October 1999, the Clinic officially spun off of its parent organization to become its own free standing 501 c 3. This change was a planned one, designed for the long-term future of the clinic. By becoming independent, the Clinic has more control over finances and program planning. The advisory board became the Board of Directors.

In January 2000, the board made the decision to close the Harvard clinic because of space concerns as well as financial ones. The Clinics consolidated into the Woodstock Clinic on March 1, 2000.

In August 2003, the Clinic moved its entire Clinic to a larger location at 13707 W. Jackson, Woodstock. This was facilitated through a generous offer by Centegra Health System who owns the building. The larger space enables the clinic to expand programs and serve more patients.

In October 2009, the Clinic opened a satellite site in the city of McHenry through a generous grant from the Blowitz Ridgeway Foundation. That satellite has continued to serve the needs of the patients in the eastern section of the county.

The clinic continues operation with foundations, grants, corporate and community support as well as patient support from sliding scale fees.

Along this journey, staff and board have realized that the sheer numbers of people needing medical care far exceed what we can provide, but we are supported by the commitment and resources of people throughout the area. They help us keep our focus on our primary concern - our clients, old and young.

As we seek to meet the growing needs of the community, we are looking to enlarging our capacity to provide care to more people. We hope you will embrace us as we move through this exciting phase.

Dear Friend of the Clinic,

Health care has been the topic of discussion everywhere this year – in the newspapers, in the boardrooms and especially in Congress. What we tend not to hear is the discussion taking place in the exam room, between the patient and his health care provider. But at the Clinic , we hear those voices pretty strongly. This is what the discussion sounds like:

“ I just lost my job and I can’t afford my medications – what am I going to do?”

“My wife needs this surgery, but I have coverage only for myself at work.”

“I haven’t been to a doctor in 5 years – I just can’t afford it.”

And then, there’s another voice: “We can help you.”

This report to you is a snapshot of the work that you allow us to do on your behalf in our community. Your involvement in the work we do is critical. Because we do not receive federal funding, we depend on our community - and you - to help make a difference in the lives of the over 40,000 uninsured people in McHenry County.

We as a board are grateful for that trust and investment, and we work diligently to maintain strong fiscal responsibility, excellence in clinical care and transparency in the work we do. Through support from our community, our funders and our patients themselves, we are working to expand access and provide even better care to our patients. We look forward to continuing to partner with you as we undertake some significant projects this year to enhance services.

Why is the Family Health Partnership Clinic Important in Our Community?

“Ana” made an appointment for a breast exam, even though she had just had a normal mammogram less than four months before. Ana was concerned because she felt a lump in her breast. After an initial exam, the clinic made an appointment for her to get a mammogram even though the recent one was normal. Based on the results, the physician requested an ultrasound and guided biopsy of the breast. Within forty-eight hours, surgeons were able to biopsy the tissue and send the labs on to an oncologist. Ana returned to the Clinic for the results which indicated cancer. The Clinic collaborated with the county Department of Health, Centegra Health System and the oncologist, and Ana began chemotherapy. Happily, she is on her way to recovery. This swift response and positive outcome is just one example of the strong relationship the Clinic has with partner organizations.

In March, “John”, a 30 year old man, came to the clinic complaining that he was unable to keep his diabetes under control. During the initial exam, he mentioned he had not been taking his medication regularly because they were too expensive. He also made an offhand remark to the nurse that he had a small sore on one toe that would not seem to heal. Our nurse immediately instructed him to remove his sock and was appalled to see a diabetic ulcer that covered not only his toe, but a good portion of the ball of his foot. Because of diabetic neuropathy, the patient had been unable to feel the pain that most people would have found unbearable. Our team sprang into action, gathering the necessary diabetic medications as well as the medications and dressings needed to attend to his foot. He was scheduled for repeat visits every few days to monitor the wound and change the dressings. It started to improve, but without warning, infection rapidly set in and the staff was afraid he would lose his foot. The Clinic set up a consultation with a surgeon, where it was decided to remove his toe in order to save his foot. It was a difficult decision, but this young man is still able to walk, to work and had significantly less complications because of the quick response. He lost his toe, but was able to save the foot.

Pamela found her way into the capable hands of our nurse practitioner after the emergency room told her to not come back for her on-going hip pain. The pain had intensified to the point that Pam thought she could no longer go on. Concerned not only about her physical pain but the emotional toll this pain was taking, our nurse practitioner worked with Pam on a comprehensive perspective and was able to make sure that Pam was seen for her myriad of medical issues beyond her hip pain. Guiding her through the process, our staff helped Pam to file for a charity care application at a downtown medical center. She was accepted and received a total hip replacement at a cost she could afford. Pam realizes she will remain a patient of the clinic until she finds adequate employment with affordable insurance or until she is able to qualify for Medicare. “I am the person who is in the gap, the hole of the medical system. I am so grateful for the Family Health Partnership Clinic and the care I get!”

heAlTh CAre reForM – What’s in it for the Clinic?

Using models developed by the Urban Institute in June 2011, the Family Health Partnership Clinic expects there to be an estimated 20,000 still uninsured in McHenry County post health care reform. The Clinic currently sees about 3500 individuals, still leaving a significant gap in resources.

Across the nation, 26,100 people between 25 and 64 died prematurely due to lack of health coverage in 2010. Families USA, June 2012.

Uninsured women over the age of 50 were about half as likely to have gotten a mammogram in the past two years as insured women. (42% vs. 79%) Commonwealth Fund, March 2011

Support for Family Health Partnership Clinic Takes on Many FormsCorporateSnap-On Tools of Crystal Lake has been supporting the clinic for over five years. The company has been an on-going sponsor for the Care4 Breast Cancer Run/Walk for several years. Most recently, Snap-On raised their commitment to the clinic and became a Gold Sponsor of our event. The employees are involved with the event from a volunteering and fundraising perspective. We are grateful for the efforts of Deborah Witthoft and Ilene Mraz of the Snap-On team who introduced Snap-On to FHPC!

IndividualsJoel and Linnea Kooistra of Woodstock are long time supporters of the clinic. They have made it their mission to be philanthropic through farming, “bushels of corn for charity” as they call it. Each year, they farm their corn fields and the yield from those harvests are then turned into dollars for charities that they hold close to their hearts. The Clinic has been the recipient of the farming philanthropy for many years and we are grateful for their dedication to our services and our patients!

Elizabeth Phelps of Spring Grove, has donated over 400 hours to the clinic. She performs a variety of tasks on a weekly basis from filing charts to organizing and helping with several events. In September, Elizabeth accepted the volunteer position of Chief of the Finish Line area for the Care4 Breast Cancer Race and made sure that all timing and volunteer operations ran smoothly. This year, she has spent most of her time assisting in our Pharmacy Program, logging in the medications from the

patient prescription drug program and preparing them to be dispensed to our patients. She faithfully helps twice a week and has assisted and volunteered at our PicklePalooza and Gala events.

Carole Gulbransen of Immanuel Evangelical Lutheran Congregation of Crystal Lake writes….”Enclosed is a donation from our school’s Girls on the Run Bake Sale. These 3 – 5th grade girls recently completed their 5K at Huntley Healthbridge. The bake sale was part of their community service project which they organized, planned & implemented. They had a lot of fun doing it and were excited that the proceeds could help others here in our community. I pray this will be a blessing and thank your organization for all you are doing here in McHenry County.”

CommunityAptar Cary chose to raise money for the clinic’s breast cancer fund by selling pink lanyards within the company. Employees purchased lanyards that were available throughout the month of October. Aptar Cary employee, Jenny Rehberg, orchestrated the sales and suggested Family Health Partnership Clinc receive the monies. Together, they raised over $500.00 for breast cancer services at that clinic! Aptar has also sponsored the Care4 Breast Cancer 5K Run/Walk!

Sage Products, Inc., Cary, A copier was donated to the clinic by Sage Products, Inc. when the old copier was ready to be replaced. A Sage employee learned about the need for a new copier and decided to do something about it. He knew the copier at Sage was being replaced and the “old” copier was going to be resold to another entity. He chose to present the company with an opportunity to make a donation and help out the clinic by giving the copier to the clinic. The copier is far better than what was in-house and the money saved for a copier could be used for direct patient care instead.

Woodstock Chamber of Commerce held a two-day music event organized by Kraft Foods and the Chamber called Picklepalooza. The event highlighted many area bands and played up on the hometown Claussen pickle factory in Woodstock. This first year, the organizers chose three non-profit agencies to receive proceeds from the event. The Clinic was one of the recipients. Thank you to the Shari Gray, Chamber Director, the Chamber and its members for thinking of the clinic and bringing such a fun festival to the area!
