GOV All persons applying for SNAP food benefits will need to provide proof of: If you cannot find proof, ask your SNAP worker for help. INCOME You may qualify for more benefits if you pay child support, have medical expenses or other eligible expenses. Examples of what you can show as proof of expenses: If you are paying child support, bring: • Court or administrative order (to prove your legal obligation); and • Evidence of withholding from your wages or unemployment; or Statement from the custodial parent for your support payment. If you have medical costs, bring: • Health insurance policy showing premium amount; • Statement from a pharmacy; • Billing statements; • Bills for a visiting nurse, homemaker, home health aide, etc.; • Itemized receipts; and/or • Bills/receipts for prescribed equipment, medical supplies, etc. You need to prove all income sources for each person in your household. Examples of what you can show as proof of income: • Employer’s records/statements; • Self-employment bookkeeping records; • Pay stubs; • Tax records; • Award letters (Veterans, financial aid, Bureau of Indian Affairs, etc.); • Workers’ compensation letter; • Statement from anyone giving you money (gifts/loans); and/or • Unemployment compensation letter. Show one piece of identity for the head of the household and the alien status for any person who is not a U.S. citizen. Examples of what you can show as proof of identity: Show one piece of identity such as: • Driver’s license/photo identification card; • Work or school ID card; • Certificate of Degree of Indian Blood (CDIB) or Alaska Native; • U.S. military record; • Wage stubs; or • Birth certificate.
