stressingtheimportanceofmaintainingasafeandproductiveworksite.AsaconditionofdoingbusinesswithSametCorporation,allsubcontractoremployeesandtieredsubcontractorsengagedonthisprojectarerequiredtoadheretoourprojectsafetyrules,regulationsandpoliciesestablishedbyourawardwinningSafetyProgram.Inadditiontoourownsafetyrequirements,ourSafetyProgramincorporatesregulationsofthecurrenteditionsoftheStateandFederallaws,includingbutnotlimitedto,thelatestamendmentsofthefollowing:Williams-SteiglerOccupationalSafetyandHealthActof1970,PublicLaw91-956;Part1910,OccupationalSafety&HealthStandards,Chapter17ofTitle29,CodeofFederalRegulations;Part1926,Safety&HealthregulationsforConstructionChapter17ofTitle29,CodeorFederalRegulations.Thissitespecificsafetyandincidentpreventionprogramwillassistprojectmanagement,supervision,subcontractorsandtieredsubcontractorsandworkersinunderstandinganincidentfreeenvironmentandthesafetyandhealthexpectationsandrequirementsofthisproject.CompliancewiththesitesafetyandincidentpreventionprogramisexpectedandaconditionofemploymentonaSametCorporationprojectsite.Safetyandaccidentpreventioniseveryone'sresponsibility.EachworkeronthisprojectsiteisexpectedtofollowallSametCorporationSafetyRulesandspecificsafetyrequirementsasoutlinedinthispolicy.Eachcontractorsupervisorandforemanisresponsibleforsafety,implementingSametCorporationsafetyprogramandtrainingemployeesinsafeworkprocedures.Superintendenthastheoverallresponsibilityforsafetyatthislocation.BruceJaworoskihasthecorporateresponsibilityandauthorityforsafetyandaccidentprevention.SametCorporationpolicyistoprovideasafeandhealthyplaceofemploymentforeveryworkeronsiteandtoabidebyaccidentpreventionregulationssetforthbythefederal,stateandlocalgovernments. Healthandsafetywillalwaysremainthetoppriorityforalllevelsofmanagement,supervision,andworkersengagedinconstructionactivities.Healthandsafetywillneverbesacrificedinlieuofschedule,cost,production,oranyothercomponentoftheworkprocess.ThisprojecthasbeendesignatedanIncidentFreeConstructionSite.TocomplywiththisphilosophyandtoachieveanIncidentandInjuryFreeEnvironment,theproject’scontractorswill:

v Thoroughlyplanallworkactivitiesandoperationssotheyareperformedsafely,aswellasefficiently.v EffectivelycommunicatethehealthandsafetyrequirementsofSametCorporationthisSiteSafety&Incident

PreventionProgramtoallcontractorsandtheirworkersatalllevelsoftheprojectthroughopencommunications,comprehensivetraining,assessments,andworkplaceinspections.

v Developanunderstanding,amongthoseinleadershiponthisproject,oftheirresponsibilitiesandaccountabilityforprovidingasafeandhealthfulworkplace.

v Coordinateworkoperationsandactivitiestominimizeoreliminatesituationswhichmayjeopardizeworker’shealthandsafetyduetoconflictingorsimultaneousworkoperationsoractivities.

v Communicatetoallworkersthatsafetyistheirresponsibilityandtheywillbeheldresponsible,accountable,andassignedtheappropriateauthorityfortheirindividualsafetyandthesafetyoftheirco-workers.

SametCorporation

SITESAFETY&INCIDENTPREVENTIONPROGRAM

ThisSiteSafetyandIncidentPreventionProgramwaspreparedtoassistprojectmanagement,supervision,contractorsandworkersinunderstandingtheincidentinjuryfreephilosophyandthehealthandsafetyexpectationsandrequirementsofSametCorporationonthisproject.CompliancewiththisSiteSafetyandIncidentPreventionProgramisexpectedandaconditionofemploymentonthisproject.Contractors’projectmanagersandsuperintendentshaveoverallresponsibilityfortheimplementationandtheexecutionofthisSiteSafetyandIncidentPreventionProgram.

SITESPECFICSAFETYCOMMITTEEAsitespecificsafetycommitteewillbeorganizedtoassistprojectteaminimplementingthisSiteSafetyandIncidentPreventionProgram.Superintendentwilldiscussthegroundrulesofthesitesafetycommitteebasedonthescopeofwork.Participationinthesafetycommitteeismandatory.Eachcontractorwhoischosentoparticipateinthesafetycommitteewillprovideoneemployee.Thepremiseofthecommitteeistoworkasateamtoidentifyandcorrectsafetyorhealthhazards,identifyunsafeworkpracticesandoffersolutionstosafetyissues.

SametCorporationexpectsallcontractorstoexecutehisorherworkonthisprojectwithavisible,proactive,andcommitmenttosafetyatalllevels.Eachcontractorshouldplantheirworkwithfocusonprotectingtheirworkersfromincidentsandinjuries.Thefollowingareactionsthateachofuscantaketoimprovesafetyperformanceonthisproject:

• Attendandactivelyparticipateintoolboxmeetings. • Discusssafetyinallmeeting.• Whenyoutalkaboutsafety,talkaboutpeople,notnumbersorstatistics.• Askwherethenextinjuryislikelytohappenandwhatcanbedonetopreventit.• Recognizeindividualsandgroupsdailyforworkingsafely.• Takepositiveactionswhenyouseesomeonedoingsomethingyoubelieveisunsafe.Talktothemaboutyourconcern

fortheirsafety,notaboutviolatingrulesorprocedures.• Takeresponsibilityforpeople’ssafetythatworkwithyou,foryouandaroundyou.• Finewaystoexpresscareandconcernforpeopleandworktoimprovethedignityandrespectpeopleexperienceon

theproject.• Makeandkeeppromisesaroundsafetyissues.

SametCorporationortheirrepresentativewillcontinuallymonitorandassesseachcontractorforcompliancewiththisSiteSafetyandIncidentPreventionProgramandappropriateregulatoryrequirements.Immediatecorrectiveactionwillbetakentoeliminateanysafetydiscrepancy,hazard,at-riskbehavior,orviolationobserved.

DESIGNATEDCONTRACTORCOMPETENTPERSON

EachcontractorwilldesignateacompetentpersonasdefinedbyOSHA29CFR1926.32(f)as“onewhoiscapableofindentifyingexistingandpredictablehazardsinthesurroundingsorworkingconditionswhichareunsanitary,hazardous,ordangeroustoemployees,andwhoasauthorizationtotakepromptcorrectivemeasurestoeliminatethem”astheirprojectsafetyrepresentative.Thisperson(s)namewillbesubmittedtoSametCorporationandthispersonmusthavetheauthorityandresponsibilitytoensuretheproperimplementationandenforcementofthisSiteSafetyandIncidentPreventionProgram.TheGeneralTradesCompetentPerson/ForemandesignatedwillbeexpectedtohaveanadequateknowledgeofOSHAconstructionstandards,pre-tasksafetyplansandrisk/severityassessment.Thedesignatedrepresentativewill,asaminimum:

• AttendOSHA10hourconstructionsafetytrainingoraminimumof2years’experienceasaForeman/CompetentPersonwouldbeacceptable.OSHA30hourconstructionsafetytrainingishighlyrecommended.

• Conductregularsafetymeetingswithworkerstoinstructthemonprojectsafeworkpracticesandrequirements.• Timelysubmissionofsafetysubmittals.• Conductdocumentedpre-tasksafetyplansandcommunicatedailytoworkerstoensurecompliancewithsafework

practices,thisSiteSafetyandPreventionProgramandOSHAsafetyregulations.

TheScaffoldCompetentPersondesignatedtooverseeerectionanddismantlingofscaffoldswillbeexpectedtohaveanaboveaverageknowledgeofOSHA29CFR1926.450SubpartL–Scaffolds,pre-tasksafetyplansandrisk/severityassessmentasitpertainstoscaffolds.Thedesignatedrepresentativewill,asaminimum:

• AttendOSHA10hourconstructionsafetytrainingoraminimumof2years’experienceasaForeman/CompetentPersonwouldbeacceptable.OSHA30hourconstructionsafetytrainingishighlyrecommended.

• AttendscaffoldcompetentpersontrainingconductedbyanauthorizedOSHAcertifiedtraineror2yearsofexperienceinscaffoldconstructionwouldbeacceptable.

• SubmitafallprotectionplanforerectionanddismantlingscaffoldstoSametCorporationsuperintendentandorSametSafetyManagerforreview.

• Conductdailyinspectionsofthescaffoldandinstructworkersinsafeworkpractices.• Submitsafetyinformationinatimelymanner.• Conductdocumentedpre-tasksafetyplansandcommunicatedailytoworkerstoensurecompliancewithsafework

practices,thisSiteSafetyandPreventionProgramandOSHAsafetyregulations.

TheTrenchingandExcavationCompetentPersondesignatedtooverseediggingtrenchesandexcavationswillbeexpectedtohaveanaboveaverageknowledgeofOSHA29CFR1926.650SubpartP–Excavations,pre-tasksafetyplansandrisk/severityassessmentasitpertainstrenchingandexcavations.Thedesignatedrepresentativewillasaminimum:

• AttendOSHA10hourconstructionsafetytrainingoraminimumof2years’experienceasaForeman/CompetentPersonwouldbeacceptable.OSHA30hourconstructionsafetytrainingishighlyrecommended.

• AttendtrenchingandexcavationcompetentpersontrainingconductedbyanauthorizedOSHAcertifiedtraineror2years’experienceasatrenchingcompetentpersonwouldbeacceptable.

• Conductdailyinspectionsoftrenchesandexcavationsandinstructworkersinsafeworkpractices.• Submitsafetyinformationinatimelymanner.Conductdocumentedpre-tasksafetyplansandcommunicatedailyto

workerstoensurecompliancewithsafeworkpractices,thisSiteSafetyandPreventionProgramandOSHAsafetyregulations.

TheFallProtectionCompetentPersondesignatedtooverseehiscompany’sfallprotectionplanwillbeexpectedtohaveanaboveaverageknowledgeofOSHA29CFR1926.500SubpartM–FallProtection,pre-tasksafetyplansandrisk/severityassessmentasitpertainstofallprotection.Thedesignatedrepresentativewillasaminimum:

• AttendOSHA10HourConstructionsafetytrainingoraminimumof2years’experienceasaForeman/CompetentPersonwouldbeacceptable.OSHA30hourconstructionsafetytrainingishighlyrecommended.

• AttendfallprotectiontrainingconductedbyanauthorizedOSHAcertifiedtraineror2years’experienceinpersonalfallprotectionsystemswouldbeacceptable.

• Conductdailyinspectionsoffallprotectionequipment,instructworkersinproperpersonalfallprotectionmethods,inspectguardrailssystemsandotherfallprotectionsystemsusedtoprotectworkerswithinyourscopeofwork.

• Submitsafetyinformationinatimelymanner.Conductdocumentedpre-tasksafetyplansandcommunicatedailytoworkerstoensurecompliancewithsafeworkpractices,thisSiteSafetyandPreventionProgramandOSHAsafetyregulations.

TheRiggingqualifiedpersondesignatedtooverseetheriggingofstructuralsteel,concretepanels,materialsorotherequipmenthoistedabovethegroundwillbeexpectedtohaveanaboveaverageknowledgeofOSHA29CFR1926.251.CompetentPersondesignatedforriggingstructuralsteelshallhaveanaboveaverageknowledgeofOSHA29CFR1926.753Hoistingandrigging.Thedesignatedrepresentativewillasaminimum:

• AttendOSHA10HourConstructionsafetytrainingoraminimumof2years’experienceasariggerwouldbeacceptable.

• Certifiedbyemployerthathe/sheisaqualifiedriggerbasedonformaltrainingandexperience.• Conductdailyinspectionsofriggingequipment.• Submitsafetyinformationinatimelymanner.Conductdocumentedpre-tasksafetyplansandcommunicatedailyto

workerstheirworkactivitiestoensurecompliancewithsafeworkpractices,thisSiteSafetyandPreventionProgramandOSHAsafetyregulations.

TheSafetyCompetentPersondesignatedtooverseethesafetyoftheiremployeesandsubcontractorswillbeexpectedtohaveanaboveaverageknowledgeofOSHAconstructionstandards,pre-tasksafetyplanning,jobhazardanalysis,risk/severityassessment.

• AttendOSHA30HourConstructionsafetytraining.• Haveasaminimum5years’experienceincommercialconstruction.• Conductdailyinspectionsoftheworkareas,conductregularsafetymeetingswithworkerstoinstructthemonproject

safeworkpracticesandrequirements.• Submitsafetyinformationinatimelymanner.Conductaccidentandnearmissinvestigationsreportsandlessons

learned.• Conductdocumentedpre-tasksafetyplansandcommunicatedailytoworkerstheirworkactivitiestoensure

compliancewithsafeworkpractices,thisSiteSafetyandPreventionProgramandOSHAsafetyregulations.

EachcontractormustsubmitaMonthlySummaryReport.Thisformmustbesubmittedevenifthesubcontractorhasnoincidentstoreport.Informationtoreportonthisformincludes:

• Monthlytotalmanhoursworked. • Firstaidcases.• OSHAmedicaltreatmentcases.• OSHAlostworkdaycases.• Restrictedworkcases.

Allincidentsincludingnearmissesmustbereportedandinvestigated.ThereportmustbesubmittedtoSametCorporationforreviewandimplementationoflessonslearnedtopreventfurtherincidentsfromoccurring.Contractorscanobtainapplicablesafetyformsfromprojectsuperintendent.

SAFETYREGULATIONS

SametCorporationandcontractorswillincorporate,asaminimum,OSHA29CFR1926ConstructionSafetyStandards,OSHA29CFR1910GeneralIndustryStandards(asapplicable),specificstatesafetyregulations,specificownerrequirements,projectsafetyrules,andthisSiteSafetyandIncidentPreventionProgramwhendeterminingthesafeworkpracticesandprotectionofallworkers.Ifanyofthesestandards,requirements,orproceduresconflict,themorestringentrequirementshallprevail.

CONTRACTORSAFETYSUBMITTALSPriortobeginningworkeachcontractorshallsubmittoSametCorporationSuperintendentorhisrepresentativethefollowing:

• ExecutedContractorSafetyCertificate• Contractor’swrittensitespecificsafetyprograms• Contractor’swrittenhousekeepingplan• Contractor’ssignedHousekeepingCommitmentAgreement• Contractor’swrittenhazardouscommunicationprogram.• Contractor’swrittensubstanceabuseprogram.• Contractor’swrittensilicaexposurecontrolprogram(ifapplicable).• SafetyDataSheetsforallchemicalsandmaterialsusedorstoredonsite.• Namesofdesignatedcompetentpersonsasrequiredbytheirscopeofworkfortrenching,scaffolding,rigging,

electricalandetc.• Namesoftrainedandqualifiedequipmentoperatorsasrequiredbytheirscopeofworkforcranes,forklifts,aeriallifts

andetc.• NamesofemployeestrainedinFirstAid.• Trainingverificationforcompetentpersons.• EnergizedWorkPermitifrequiredtoworkonenergizedcircuits.• Detailedjobhazardanalysis/Pre-TaskSafetyPlanforthescopeofwork• PersonalProtectiveEquipmentHazardAssessmentandCertification• Annualcraneinspectionsofanycranebroughtontothesite.• VerificationofOSHAandorprojectrequiredtrainingasnecessary.Employeetrainingshallbeverifiedbycontractor’s

seniorrepresentativeanddocumentationoftrainingsubmittedtoSuperintendent.Examplesoftrainingmayinclude:o OSHA10hourconstructionsafetytrainingo Fallprotectiono Pre-tasksafetytrainingandriskassessmento Ladderso Scaffoldso Aerialliftso Forkliftso Trenchingo Generalconstructionhazardso Cranesignalpersono Confinedspaceso Respiratoryprotection

o Lockouto Rigging

Throughoutthecourseoftheprojecteachcontractorwillberequiredtosubmitvariouson-goingsafetydocumentstoSametCorporationSuperintendentasrequiredbythescopeofwork.Thesesubmittalsmayincludebutnotlimitedto:

• MonthlyIncidentSummaryReport.• Accidentandincidentinvestigationreport(within24hours).• DailyWorkSiteSafetyInspections.• Dailydocumentedscaffold,trench,crane,aerialliftandforkliftinspections.• Weeklysafetytoolboxmeetingtrainingrecords.• Dailypre-tasksafetyplan• Airsamplingdataforrespiratoruseifapplicable• Inspectionsofriggingequipment• Inspectionsofpersonalfallprotectionequipment• Initialinspectionsofallweldingmachinesandgenerators• Dailyinspectionsofheavyequipment(backhoes,dumptrucksandetc.)• Inspectionsofhandandpowertools.• Dailyinspectionsofladders• Dailyinspectionsofmaterialhandling/hoistingequipment.

Safetyformscanbeobtainedfromprojectsuperintendent

ViolationofSafetyandHealthRequirementsViolationsofstatutoryhealthandsafetyregulationsortheprojectsafetyrulesandpoliciescontainedinthesitesafetyplanwillnotbetolerated.Allhazardsidentifiedaretobeabatedimmediately.Whenahazardcannotbeimmediatelycorrected,awrittenexplanationistobesubmittedtoSametCorporationSuperintendent.Failuretocorrecthazardsmayresultindisciplinaryactionsorsuspensionofpartorallwork.

DISCIPLINARYPROGRAM

At-riskbehavioronthisprojectthatcontributestoanincidentorinjurywillnotbetolerated.Eachworkerhasanindividualresponsibilitytoworksafelyandminimizeunsafeactions.SametCorporationreservestherighttodisciplinecontractorbasedonsafetyviolationscommittedbytheiremployees.Thediscipline/sanctionswillnotbebasedsolelyonanindividualemployee’ssafetyviolationsbutonthecompany’ssafetyviolations.SametCorporationhasestablishedaprogressivedisciplinaryprogramforthoseactsorpracticesnotconsideredImmediatelyDangeroustoLifeorHealth.CommittinganunsafeactandorpracticethatisnotImmediatelyDangeroustoLifeorHealthwillresultinthefollowing:

• Firstoccurrence:Verbalwarningwithanotetofile(SafetyTicket)• Secondoccurrence:Writtenwarning,re-training,orsanctionstoincludebutnotlimitedtosuspensionfromproject,

holdingmonthlyinvoicechecksandetc.• Thirdoccurrence:Writtennotificationofsanctions,possibleterminationfromprojectsite.Atthetimeofthethird

violation,SametCorporationreservestherighttodiscipline/sanctionacontractortoincludebutnotlimitedtoremovalofemployeesfromthesite,retraining,meetingwithsubcontractor’scorporateleadershiptodiscussasafetyimprovementplanandetc.

Other-than-serioussafetyviolationsmayconsistof,butnotlimitedto:

• Failuretofollowfallprotectionsrequirements.• Removingguardrailsandnotputtingthembackinplace.• Workinginanunprotectedtrenchgreaterthan5feetdeep.• FailuretofollowtheSubstanceAbusePolicywillresultinafineandremovalfromthejob.• Possessionoffirearms,explosivesordangerousweapons.• Violationofprojectsecurityrulesandprocedures.• Fighting,horseplay,practicaljokingorgambling.• Enteringaconfinedspacewithoutfollowingprocedures.• Failuretofollowlock-out/tag-outprocedures.• Workingonenergizedcircuitswithoutanenergizedhotworkpermit.

Itisimpossibletopublisheverysafetyruletocovereverycircumstance.However,ifworkersfailtofollowsafeworkpracticesnotcoveredbythispolicy,disciplinaryactionswillbeassessedbasedonSametCorporationonsiteSuperintendentandProjectManager’sassessmentoftheviolation.SafetyViolations Non-serioussafetyhazardscanresultindisciplinaryactionsorsanctionsbasedonSuperintendentandProjectManager’sdiscretion.Serioussafetyhazardscanresultindisciplinaryactionsorsanctionsincludingpermanentremovalofemployee(s)fromtheprojectbasedonSuperintendentandProjectManager’sdiscretion.

SUBSTANCEABUSEPOLICY

Thisprojectiscommittedtoprovidingasafe,drugfreeworkplaceforallemployees.ThispolicyappliestoallSametCorporationcontractors,venders,andotherthirdpartyemployees.Drugandalcoholabuseonandoffthejobcancontributebothtoincidentsandtogreaterriskforallindividualsemployedonthisproject,aswellasthegeneralpublic.Constructionworkisdangerous;thereforeallworktasksonthisprojectwillbeconsideredsafetysensitive.Theuse,sale,offertosell,purchase,andtransfer,distribution,orpossessionofdrugparaphernalia,anydetectableamountsofalcoholorillegaldrug,firearm,orotherdangerousweaponsbyanyemployeeonthisprojectisprohibited.EachcontractorwillpromoteaDrugFreeWorkplacewiththeiremployeesandcommunicateduringthesafetyorientationwhatconstitutesprohibitedactivities.Everyworkerinvolvedinanincidentshallhaveapostincidentdrug/alcoholtestperformedwithinthree(3)hoursaftertheincident.Anyworkerontheprojectsitewhoisreasonablysuspectedofbeingundertheinfluenceofalcoholoracontrolledsubstanceshallbetested.ContractorswilltransporttheirworkersinvolvedinanincidenttoacollectionfacilityselectedbySametCorporation.Workersthatrefusetotest,stalltobetested,areuncooperativewithcollectors,orattempttoalteraurinespecimenwillbeconsideredpositiveandimmediatelyremovedfromtheproject.

WORK-REALATEDINJURIES,ILLNESSESANDINCIDENTINVESTIGATION

Anincidentisdefinedasanyunplannedorundesiredeventthatresultsinorhasthepotentialtoresultinawork-relatedinjury/illness,propertydamage,ordisruptionofbusinesswherethecausewasfromhumanerrorsoromission.Everyincidentwillbeinvestigatedtodeterminetheprobablerootcauses(s)andstepsrequiredpreventingasimilaroccurrencefromhappeninginthefuture.Intheeventofawork-relatedinjuryorillness,theworkeristonotifyhisorhersupervisor.Allworkrelatedinjuries/illnessesandincidentsmustbereportedtoSametCorporationimmediately.

Firstlinesupervisionwillberesponsibleforconductingtheinvestigationoftheincidentimmediately.Asafetyrepresentativemayassistthefirst-linesupervisorintheinvestigationbutwillnotsolelyconducttheinvestigation.TheincidentnotificationandinvestigationreportformwillbesubmittedtoSametCorporationwithin24hoursoftheoccurrence.

IncidentInvestigationProcedure

Thesubcontractorcompetentpersonmusttalkwitharepresentativeofthedepartmentandmustreport:theemployername,locationandtimeoftheincident,numberofemployeesinvolvedtheextentofinjuriesorillness,abriefdescriptionofwhathappenedandthenameandphonenumberofacontactperson.• DONOTDISTURBthesceneexcepttoaidinrescueormakethescenesafe.Wheneverthereisanincidentthatresultsindeathorseriousinjuriesthathaveimmediatesymptoms,apreliminaryinvestigationwillbeconductedbytheimmediatesupervisoroftheinjuredperson(s),SametCorporationsuperintendent,anemployeerepresentativeoftheprojectsafetycommittee,andSametCorporationsafetydirector.Projectsuperintendentwillhaveavailablearenecessarytools,materialsandequipmentforuseinconductinganincidentinvestigation.Itemssuchascellphonewithcamerasandvoicerecordingcapabilities,tapemeasures,markingpaints,flagsandetcwillbereadilyavailabletoaidintheinvestigation.SametCorporationSafetyDirectorwillactastheincidentcommanderandwillworkwiththeinvestigationteam.

Theinvestigationteamwilltakewrittenstatementsfromwitnesses,photographtheincidentscene,andconductmeasurementswithtapemeasuresandequipmentinvolved.Theteamwillalsodocumentassoonaspossibleaftertheincident,theconditionofequipment,weatherissues,noise,andillumination,physicalfactorspertainingtotheinjuredperson

(s)andanyanythingelseintheworkareathatmayberelevant.Anyevidenceincludingphysicalevidencepertainingtotheinvestigationwillbecatalogedandsecureduntiltheinvestigationiscompleted.Theteamwillmakeawritten“IncidentInvestigationReport”ofitsfindings.Thereportwillincludeasequenceofeventsleadinguptotheincident,rootcauses,conclusionsabouttheincidentandanyrecommendationstopreventasimilarincidentinthefuture.Thereportwillbesubmittedwithin24hrstobereviewedbythesafetycommittee,SametCorporationsuperintendentandSafetyDirector.SametCorporationSafetyDirectorwilluponreviewoverseelessonslearnedandoradditionaltrainingrequirementsasrecommendedbytheincidentinvestigation.Whenasupervisor/competentpersonbecomesawareofanemployeeinjurywheretheinjurywasnotseriousenoughtowarrantateaminvestigationasdescribedabove,thesupervisorwillwritean"IncidentInvestigationReport"toaccompanythe"Employee'sInjury/IllnessReportForm"andforwardthemtoprojectsuperintendentandSametCorporationSafetyDirectorforreview.Wheneverthereisanincidentthatdidnotbutcouldhaveresultedinseriousinjurytoanemployee(anear-miss),theincidentwillbeinvestigatedbythesupervisor/competentpersonorateamdependingontheseriousnessoftheinjurythatwouldhaveoccurred.The"IncidentInvestigationReport"formwillbeusedtoinvestigatethenear-miss.Theformwillbeclearlymarkedtoindicatethatitwasanearmissandthatnoactualinjuryoccurred.ThereportwillbeforwardedtoprojectsuperintendentandSametCorporationSafetyDirectortorecordontheincidentlog.Intheeventofawork-relatedinjuryorillness,theworkeristonotifyhisorhersupervisor.Allworkrelatedinjuries/illnessesandincidentsmustbereportedtoSametCorporationimmediately.Intheeventofawork-relatedinjuryorillness,theworkeristonotifyhisorhersupervisor.Allworkrelatedinjuries/illnessesandincidentsmustbereportedtoSametCorporationimmediately.Firstlinesupervisionwillberesponsibleforconductingtheinvestigationoftheincidentimmediately.Asafetyrepresentativemayassistthefirst-linesupervisorintheinvestigationbutwillnotsolelyconducttheinvestigation.TheincidentnotificationandinvestigationreportformwillbesubmittedtoSametCorporationwithin24hoursoftheoccurrence.

SAFETYPLANNING

JobHazardAnalysis(CompletedbyContractorSuperintendentandProjectManager)Priortostartingworkonthisproject,eachcontractorwillsubmitawrittenJobHazardAnalysis(JSA)fortheirscopeofwork.TheJSAcanbeincludedintheSiteSpecificSafetyPlan.TheJobHazardAnalysismustidentifyandoutlineeachworkcomponentoractivity,listthepotentialsafetyhazards,risk/severityassessmentandhealthhazardsassociatedwitheachactivity,anddescribewhatsafetycontrols,PPE,toolsandequipmentwillbeimplementedandrequiredtomitigatetherecognizedhazardsandsafelycompleteeachactivity.Pre-TaskSafetyPlanning(CompletedbyContractorForemanorFirstLineSupervisor)EachForeman,designatedsupervisorandorworkerswillanalyzeeachtasktobeperformedforeachscopeofworkandidentifytheworksequences,hazards,andcontrolsnecessarytoprotectworkersfromtheidentifiedhazards.ThePre-TaskSafetyPlanwillbecommunicateddailytoeachcrewperformingworkonthisproject.EachemployeewillsignthePTPformacknowledgingthesafetyprocedureswhileengagedinthetask.Asamplepre-taskcanbeobtainedfromprojectsuperintendent.IncasesofachangedconstructionactivitythatwouldrequireachangeinPPEuse,oralapseinproperuse,insufficientskillorknowledge,theemployeeorsubcontractorcompetentpersonmustassessthechange(s)andretrainhisemployeesanddocumentthatre-traininginhisdailypre-tasksafetyplanandfieldreport.

SAFETYTRAINING

Safetyandhealthtrainingisarequirementandmandatoryforallandcontractorworkersassignedtothisprojecttopromoteandensurethatanincidentandinjuryfreeenvironmentexists.SafetyOrientation:Allprojectmanagement,supervisors,andworkersshallattendsite-specificsafetyorientationtraining.SametCorporationSuperintendentorhisrepresentativewillconductsafetyorientations.Noworkerswillbeallowedtostartworkuntiltheyhaveattendedthesafetyorientation.Asaminimum,thesafetyorientationwillincludeviewingSametCorpSafetyvideo,ProjectSafetyRules,sitesafetyorientationreport,emergencyactionplans,fireextinguishertraining,safetytrainingrequirementsandprocedurescontainedinthismanual.Thesite-specificorientationwillcommunicateeachworker’sresponsibilitytobeincompliancewiththisproject’sSiteSafetyandIncidentPreventionProgram,ProjectSafetyRules,regulations,accountability,andthedisciplinarypolicy.Uponconclusionof,allpersonnelwillbegivenanumberedhardhatstickerverifyingthattheyhavebeenthroughtheorientation.

Anemergencyplanisasetofrulesorprocedurestobefollowedbyallpersonnelintheeventofaprojectemergency.Asitespecificemergencyactionplanwillbewrittenandallsubcontractorcompetentpersonswillbeprovidedwithacopyofthesitespecificemergencyactionplanwhentheymobilizeonsite.Theplanandprocedureswillbediscussedduringtheprojectsafetyorientationmeeting.The emergency plan ismaintained in the Samet field office and is under the direction of the project superintendent. Theemergencyplandeterminestheproperaccess/egressofemergencyequipmentand/orpersonnelintooroutofthesiteincaseofemergency.Projectsuperintendentwillactivateemergencyactionplanbytheuseof3longairhornblastsandorphonecommunicationtosubcontractorcompetentpersons.Supervisorswillbedirectedtokeylocationsonthesitetoassistinanemergencysituation.Eachemployeeisexpectedtofollowdirectionofsupervisorsandcooperateinanyemergencyactioneffort.Personnelshouldevacuatethesiteinanorderlyfashionifinstructedtodosobysupervisors.Ifyoubecomeawareofanemergencysituationoraninjury,notifyasupervisorimmediately.Personnelarestrictlyforbiddentodiscussprojectconditions,incidentsoremergencieswiththemedia,pressoranypersonnotassociatedwiththeproject.

PROJECTSITESECURITY

“NoTrespassing”signsshouldbepostedattheprojectsitetopreventcasualentrybythepublic.Allconstructiontrafficwillenterthroughdesignatedareas.Onprojectsthatarefencedin,allconstructiontrafficwillenterthroughdesignatedcontrolgate(s).Subcontractorsenteringtheprojectincompanyvehiclesshallregisterthevehiclewiththeprojectsuperintendent.Parkingareaswillbedesignatedforprivatelyownedvehicles.Subcontractorsareresponsiblefordirectingtheiremployeesandvendorstousespecifiedgatesandparkingareasasrequired.Subcontractorsareresponsibleforsecuringandmaintainingtheirownequipment,officetrailersandstorageareastoincludeafterhours,weekendsandholidays.Ifthesubcontractorelectstostoretoolsorothervaluableitemsonsite,alltoolsorothervaluableitemsshouldbelabeledwiththeownersnameandlockedinasecuremetaljobboxorstoragecontainer.Ataminimum,locksshouldbehightensilesteelsecuritylocksets.Mechanizedequipmentshouldbelockedandorsecuredinstoragecontainers.Allworkersonsiteshallreportsuspiciousbehaviortotheirrespectivesupervisors.SubcontractoremployeesmaybesubjecttoSametCorporationdisciplinaryproceduresforviolationofprojectsecuritymeasuresandforcertainoffenses,maybesubjecttolegalaction:

FIRSTAIDPOLICY

Intheeventanemployeeisinjuredonthejob,FirstAidkitsareavailablefortheemployeetotreattheirowninjuries.Firstaidkitswillbe located in thevicinityof theworkareaandcontentsof thekit inspectedwhenbroughtonsite. SubcontractorForemanwillnotifyproject superintendentorhis representativeif employeesuse firstaid items. In theeventofa seriousinjury,911willbecalled.

HEATSTRESSWork involving high air temperature, radiant heat sources, high humidity, direct physical contact with hot objects orstrenuousphysicalactivitieshaveahighpotentialforinducingheatstressinworkersengagedinconstructionactivities.Age,weight,degreeofphysicalfitness,degreeofacclimatization,metabolism,useofalcoholordrugsandavarietyofmedicalconditionsallaffectaworker’ssensitivitytoheat.Eventhetypeofclothingtheworkerwearsmustbeconsidered.Priorheatinjurypredisposesaworkertoadditionalinjury.It isdifficult topredict justwhowillbeaffectedandwhen,becauseaworkerssusceptibilityvaries. TrainingEachemployeewhoworkswithorispotentiallyexposedtohazardouschemicalswillreceiveinitialtrainingontheHazardCommunicationProgramandthesafeuseofthosehazardouschemicals.Additionaltrainingwillbeprovidedforemployeeswheneveranewhazardisintroducedintotheirworkareas.Thetrainingwillemphasizetheseelements:• Asummaryofthestandardandthiswrittenprogram.• Hazardouschemicalpropertiesincludingvisualappearanceandodorandmethodsthatcanbeusedtodetectthepresence

ContractorEmployeesProject superintendentwill advise subcontractor’s foremanofSametCorporationhazardouschemical listduring thesafetyorientation.Inaddition,thecontractor’sforemanwillbenotifiedofthelocationandavailabilityofSDSs.EachcontractorbringingchemicalsonsitemustprovideacopyoftheirwrittenHazardousCommunicationProgramincludingall SDS’ to Samet Corporation superintendent prior to mobilization on the jobsite. The superintendent will maintaincompany’sHazcomprogramuntilsubcontractorfinishestheirwork.CommunityRighttoKnowEachprojectlocationwillcooperatewithcityandcountyofficialstocomplywithrequirementsoftheOSHAstandardsregardinghazardousmaterialsonsite.

FALLPROTECTION

SametCorporation,subcontractors,vendersorotherthirdpartyindividualswilltakeallpracticalmeasurestoeliminate,prevent,andcontrolfallhazards.Allworkwillbeplannedwiththeintenttoeliminateidentifiedandpotentialfallhazards.Samet Corporation’s fall protection policy and OSHA 29 CFR 1926.500 Subpart M govern the requirements to protect workers exposed to falls. Additionally, Samet Corporation fall protection policy is 100% fall protection 6 feet or greater above a lower level. The use of conventional fall protection systems are the only means to protect workers from falls to lower levels. Workers wearing personal fall arrest systems shall not free fall more than 6 feet or never contact a lower level. AwrittenfallprotectionandpreventionplanmayberequiredasdeemednecessarybySametCorporation.Contractorsengagedinthefollowingshallsubmittheirfallprotectionplanforapprovalpriortobeginningworkonsite:Steelerection,concrete(castinplace),woodframing,drylaidmasonrywall(segmented),pre-castconcretewalls,tilt-upconcretewalls,androofingwork.Fallprotectionplanmustbeapprovedpriortobeginningwork.Thedesignatedcompetentpersonmustoverseethecompany’sfallprotectionplantoensureitmeetsSametCorporation’sfallprotectionpolicies.Subcontractorssubmittingafallprotectionplanshallincludearescueplanifapplicable.SametCorporationiscommittedtothephilosophyof100%continuousfallprotection,wheneverthepotentialexistsforaworkertobeexposedtofallhazardsofsixfeet(6’)orgreaterabovealowerlevel.Acceptablefallprotectionsystemsincludethefollowingconventionalsystems:

Workersexposedtofallhazardsshallbeuniformlyequipped,trained,andgivenperiodicrefreshertraininginfallprotectionatspecificintervalstominimizetheadverseeffectsofaccidentalfalls.FallprotectiontrainingrecordswillbemaintainedontheprojectandavailableforreviewbySametCorporation.100%FALLPROTECTIONMEANSPROTECTEDFROMFALLSATALLTIMESWHENWORKINGATORABOVESIXFEET.Theuseofduallanyardsshallbeusedtoensure100%protection.Thismeansitismandatoryforalltrades,including:

GeneralfallprotectionrequirementsWorkerswillnottieofftoaperimetercableorwireropeguardrailunlesstheperimeterguardrailhasbeenproperlydesignedasahorizontallifeline.Horizontallifelinesmustbedesignedbyaqualifiedperson.Whenwireropeisusedtoconstructguardrailsystemsatleast3/8”diametercableshallbeusedwiththreecableclampsperconnection.Guardrailsystemsmustbeconstructedsuchthatthetoprailis39”-45”highandiscapableofwithstandinga200lb.forcewithoutdeflectingbelow39”.Lanyardswillnotbetiedbacktothemselvesunlessapprovedbythemanufacturer.Onproperlyconstructedscaffolds,elevateddecks,andelevatedplatformsthathaveperimeterguardrailsystemsconsistingofatoprailandmidrail,workersarenotrequiredtotieoff.Personalfallarrestsystemswillberequirediftheperimeterguardrailsystemmustberemoved.Anycontractorthatcreatesafloorholeorpenetrationlargerthan2incheswillberesponsibleforprotectingthatopeningandproperlymarked,“HOLEORCOVER-DONOTREMOVE.”Anycontractorthatmustremoveaguardrail,holecoverorotherfallprotectionsysteminthecourseoftheirworkwillberesponsibleforimmediatelyreplacingtheprotectivesystem.

SCAFFOLDSANDAERIALLIFTS

Contractorshall identifyacompetentpersoninchargeoferectinganddismantlingallscaffolds. ThecompetentpersonshallensurethatthescaffoldiserectedandusedaccordingtoOSHAregulations(29CFR1926subpartL-Scaffolds)andCodesofSafePractice(ScaffoldIndustryAssociation).Recordswillbemaintainedforscaffoldtrainingandbeavailablefor review by Samet Corporation Superintendent. The Competent person shall submit to Samet CorporationSuperintendentorhisrepresentativeafallprotectionplanforerectinganddismantlingscaffolds.Employeesworkingonscaffolds6feetabovea lower levelshallbeprotected fromfallingbyeitherastandardguardrailsystemorpersonalfallarrestsystem. Whenrespiratorsarerequiredtobeused,theemployermustestablishacomprehensiverespiratoryprotection program,asoutlinedinOSHA’sSmallEntityComplianceGuideforRespiratoryProtectionandasrequiredintheOSHA respiratoryprotectionstandard[29CFR1910.134and1926.103]. Importantelementsofthisstandardare:

• Periodicexposuremonitoring,• regulartrainingofpersonnel,• selectionofproperNIOSH-approvedrespirators,• anevaluationoftheworker'sabilitytoperformtheworkwhilewearingarespirator,• respiratorfittesting,and• maintenance,inspection,cleaning,andstorageofrespiratoryprotectionequipment.

Therespiratoryprotectionprogramshouldbeevaluatedregularlybytheemployer.

Ifaworkerdesirestovoluntarilywearafilteringfacepieceandarespiratorisnotrequired,thefirst-line supervisorisrequiredtoinformtheworkeraboutthespecificrespiratoranditslimitations.Alldisposabledustmasks mustbeNIOSHapproved.ContractorsshallreadandhaveemployeesignAppendixDtosection1910.134 (Mandatory)InformationforEmployeesUsingRespiratorsWhenNotRequiredUndertheStandard.AppendixDis containedintheappendixofthismanual. UseofRespirators

Donotuserespiratorsastheprimarymeansofpreventingorminimizingexposurestoairbornecontaminants. Instead,useeffectivesourcecontrolssuchassubstitution,automation,enclosedsystems,localexhaustventilation, wetmethods,andgoodworkpractices.Suchmeasuresshouldbetheprimarymeansofprotectingworkers.However, whensourcecontrolscannotkeepexposuresbelowOSHApermittedexposurelimits(PEL),controlsshouldbe supplementedwiththeuseofrespirators.

LADDERSAFETY

Thefollowinggeneralrequirementsapplytoallportableladders.SametCorporationrequiresallportableladderstoberatedheavydutytype1,1A,or1AA.TypeIIorTypeIIILadders(<225Lbs.)areProhibited.JobmadeladdersifusedshallcomplywithANSIA14.4.Theuseofaluminumladdersisprohibitedonthisproject.CompetentPersonshallevaluatetheuseofpersonalfallprotectionsystemswhileonladdersgreaterthan6feetup.• Theuseof ladderswithbrokenormissingrungs,brokenorsplit siderails,orother faultyordefectiveconstruction is

prohibited.• Portableladderfeetshallbeplacedonasubstantialbase,andtheareaaroundthetopandbottomoftheladdershallbe

keptclear.• Laddersshallnotbeusedinahorizontalpositionasplatforms,runways,orscaffolds.• Laddersshallbeusedonlyforthepurposeforwhichtheyweredesigned.• Ladders shall not be placed in passageways, doorways, driveways, or any location where they may be displaced by

off.• Aftertheextensionsectionhasbeenraisedtothedesiredheight,checktoseethatsafetydogsorlatchesareengagedand

thattheextensionropeissecuredtoarungonthebasesectionoftheladder.• Extensionladdersmustbeoverlappedaminimumofthreerungs.Stepladders• Stepladdersshallalwaysbeopenedandsetlevelonallfourfeet,withspreaderslockedinplace;theyshouldneverbeused

likeastraightladder.• Neverstandonthetopofastepladderorplacetoolsormaterialsonthesteps.

ELECTRICALSAFETY

The following regulations apply to electrical installations used on this Project site, both temporary and permanent.Electriciansworkingonexposedlive(50to280volts)partsshallweartheappropriatelevelofpersonalprotectiveequipmentrequiredunderNFPA70e.

• Extensioncordsusedwithportableelectricaltoolsandappliancesshallbe#14AWGorgreaterandbethree-wiretypedesignedforhardorextra-hardusage.Groundsarenevertoberemovedfromtheextensioncords.

• Allflexiblecordspluggedintoageneratorwithanoutputof5KWorgreaterandallflexiblecordspluggedintothepermanentwiringofthebuildingshallbeprotectedbyagroundfaultcircuitinterrupter(GFCI).

• AnyreplacementplugendsinstalledonflexiblecordsshallbeUL/FMapprovedforitsintendeduse.Note:Openconstructionsitesareconsideredwetlocations.UL/FMapprovedwaterresistantreplacementplugendswouldbeacceptable.

• Temporarylightsshallbeequippedwithguardstopreventaccidentalcontactwiththebulb.Temporarylightingcircuitsshallbepermittedwithincableassemblies,orwithinmulti-conductorcordorcableofatypeidentifiedforhardusageorextra-hardusage.

• SametCorporationprohibitssplicingorrepairingflexiblecordswithelectricaltape.• Electricalandextensioncordsorcablearenottobelaidonfloors,inwalkways,etc.,unlessitisimpracticaltodo

otherwise.Theyshouldbesuspended,orprotectedinsuchawayasnottoblockorhanginwalkways,doorways,orworkareas.

• Allenergizedpanelboxesshallbeequipmentwithalockablecover,allholessealed,andcircuitslabeled.Panelboxes shall have anapprovedcover on themat all times,exceptwhenbeing serviced;andwhena temporarycoverisinplace,itwillbemarked“DANGER-HIGHVOLTAGE”todenotelivecurrent.

• ItisSametCorporationpolicythatelectricalpanelsshallbede-energizedandlockedoutpriortobeingworkedon. However, if anyworkonenergizedcircuits is requiredwithpanels removedanEnergizedWorkPermit and Safety Plan shall be submitted and reviewed by Samet Corporation Safety Director and SeniorSuperintendent. Compliance with NFPA 70E is mandatory. PPE requirements shall comply with NFPA 70EHazardRiskClassificationTable130.7(c)(9)and130.7(c)(10).

• Samet Corporation requires all 125-volt, single phase, 15-, 20, and 30-ampere receptacles that are eithertemporaryorpermanentwiringofthebuildingorstructureandthatareinusebypersonnelshallhaveground-fault circuit interrupter (GFCI) protection for personnel. Additionally, receptacles other than125-volt (240v),singlephase,15-,20-,and30-amperereceptaclesthatiseithertemporaryorpermanentwiringofthebuildingorstructure and that are in use by personnel shall have ground-fault circuit interrupter (GFCI) protection forpersonnel.ElectricalcontractorshallinspectGFCIreceptaclesdailyandlogtheresultsonthedailySubcontractorDailyWorkSiteSafetyInspectionform.FormiscontainedintheAppendixofthismanual.

• Perhapsthemostabusedtoolontheconstructionjobsiteistheelectricalpowercord.Theyarekinked,twisted,cut,pulledandcrushedalmostconstantly.

• Unfortunately,thesedamagedcordsalsotaketheirtollininjuriesandevenfatalities.Inoneincident,aworkerwasinstallingventilationductsandreceivedaslightshockfromtheexposedconductoronadamagedelectricalcord.Theshockdidn’tkillhim,buthefellbackwardfourfeetoffascaffoldandstruckhishead.Thefallparalyzedhimpermanentlyfromthewaistdown.

• Don’ttakeelectricalcordsforgranted.Theycanbeabighelptous,buttheycanalsohurt.Keepthesepointersinmind:

• Visuallyinspectthecordfordamagedandexposedconductors.Ifthecordisindamagedcondition,don’tuseit,takeitoutofservice.

• Inspecttomakesurethegroundprongisingoodconditionandthecordprovidesasatisfactorygroundfortheelectricaltoolsbeingused.

• Don’t drag cords over rough surfaces and don’t use them to lift of pull materials. Electrical cords were not

designedtofunctionasropes.• Don’tdisconnectcordsbyjerkingthemout.Theyshouldbedisconnectedatthereceptacle.• Don’tstringelectricalcordsthroughwateroroilandgrease.Also,don’thammernailsorstaplesintocords.• Whennotinuse,theelectricalcordshouldbeneatlycoiledandstored.• Onlyroundcordsthatareratedforheavydutyuseareallowed.Neveruseflatpowercords.• Withjustalittlerespect,electricalcordscanprovideuswithlongandsafeservices.

TRENCHING&EXCAVATIONSAFETY

• Priortoanyexcavationortrenchingonthisproject,thefollowingmustbeperformed:• Anycontractorengaged in trenchingoperationsdeeper than5 feet shalldesignateacompetentpersonand inform

• Protective systemsdesigned to be placed in trenches such as trench boxesmust have tabulated dataavailable forreviewasnecessary.

• Spoilpilesandothermaterialswillbeplacedaminimumof2feetfromtheedgesofalltrenchesandexcavations.• Intrenchesdeeperthanfourfeet,locatemeansofegress,suchasladdersorstepsorramps(45degreeslope),sothey

arenomorethan25feetoftravelfromanyoneintrench.• Acompetentpersonmustinspectthetrench,adjacentareas,andanyprotectivesystemsforpossiblecave-ins,failure

MOBILECRANESAFETYANDRIGGING

Any contractorwho uses a crane on this Project Site shall adhere to the requirements of 29 CFR 1926.1400 Cranes andDerricks in Construction. All crane operators shall fill out Samet Pre-Erection Crane Analysis and provide requireddocumentationsuchasannualinspectioncertification,operator’slicenseandsignalmantraining.MobileCranesandRigging• Nocranewillbebroughtontotheprojectwithoutacurrentannualinspectionandapplicableloadcharts.• Crane operators will perform daily crane safety inspections. Crane operators are to turn in the Daily Crane Safety

ChecklisttoSametSuperintendent.ADailySafetyCraneChecklistisprovidedintheAppendixtothismanual.Note:Anequivalentformmaybeused.

CONCRETEANDMASONRY

• Freestandingmasonrywallsover8feetinheightwillbeadequatelybracedtopreventcollapse.Limitedaccesszones

willbeestablishedasrequiredbyOSHAtoprotectworkersfromthehazardsassociatedwithcollapsingmasonrywalls.

• Allrebardowels,electricalconduitsorsimilaritemswhichareconsidereda“potentialimpalementhazard”shallbecapped(protected)atalltimes.Thisincludesverticalandhorizontalimpalementhazards.

• Workerscuttingmasonrymaterials shall not be exposed to airborne concentrations of respirable crystalline silicadust that exceeds OSHA permissible exposure limit (PEL) of 50 ug/m3 of air. Subcontractor is responsible fordeterminingtheexposurelevelofrespirabledustinandaroundtheiremployeesbreathingzone. WetcuttingsawseitherstationaryorhandheldequippedwithanintegratedwaterdeliverysystemthatcontinuouslyfeedswatertothebladeforcuttingwouldbeanacceptablepracticetokeeprespirabledustfromexceedingOSHAPELlimits.Handheldgrindersanddrillsusedonmasonrymaterialsarerequiredtobeequippedwithacommerciallyavailableshroudanddustcollectionsystem.Dustcollectormustprovide25cubicfeetperminute(cfm)orgreaterofairflowperinchofwheel diameter and have a filter with 99% or greater efficiency and a cyclonic pre-separator or filter-cleaningmechanism.

SametCorporationisthecontrollingcontractoronthisprojectandwillnotifysteelerectioncompanythatanadequatelaydownarea,setuparea,andadequateaccessexistspriortothedeliveryofstructuralsteeltotheprojectsite.SteelErectionCompanywillbenotifiedthatconcretehasattained75%ofitsdesignstrength,allanchorboltsareproperlydesignedandinstalledaccordingtobuildingplansandspecificationspriortobeginningsteelerection.

• SteelErectionCompanyshallsubmittoSametCorporationsuperintendentawrittensteelerectionplan.Theplanmustincludeallaspectsoftheprocessforunloadingmaterialstoinstallingpermanentfloors.SteelerectionproceduresshallfollowOSHA29CFR1926.750SubpartR–SteelErectionstandardoranysupplementalrequirementsrequiredbySametCorporation.Thefollowingrequirementshallbeincorporatedintotheplan:

• Donotuserespiratorsastheprimarymeansofpreventingorminimizingexposurestoairbornecontaminants.Instead,useeffectivesourcecontrolssuchassubstitution,automation,enclosedsystems,localexhaustventilation,wetmethods,andgoodworkpracticesasindicatedin29CFR1926.1153Respirablecrystallinesilica.Suchmeasuresshouldbetheprimarymeansofprotectingworkers.However,whensourcecontrolscannotkeepexposuresbelowtheOSHAPEL(50ug/m3)controlsshouldbesupplementedwiththeuseofrespiratorsasrequiredbyTableI.Theemployershalladheretotherespiratoryrequirementsof29CFR1910.134and29CFR1926.1153.

INSTALLINGANDSANDINGSHETROCK

Thisprocedureoutlinesthesafetyrequirementsforinstallingandsandingsheetrockinapartmentcomplexesandcommercialbuildingunderconstruction.

• Contractormustensurethateachworkeristrainedandfullyunderstandshisorherduties. Workersmustpickuploosepiecesofsheetrockandorganizetheminpilesanddisposeofalldebrisdaily.

Whilewearingstilts,workersareprohibitedfromwalkingupanddownstairs.Whenworkerswearingstiltsarewithin10feetofstandardguardrailstheymustextendthetoprailanadditional2feettoensureworkerisproperlyprotected.

• Workershandsandingsheetrockjointscan,onavoluntarybasis,wearadisposablerespirator(dustmask) ratedN95.WorkersmustbetrainedandsignAppendixDtosection29CFR1910.134“VoluntaryUseofa DisposableRespirator”.

• Workersengagedinmechanicallysanding(poweredorbitalsander)sheetrockjointcompoundshallnotbe exposedtoairborneconcentrationsofrespirabledustabovetheOSHApermissibleexposurelevel(PEL) of5mg/m3ofair.Subcontractorisresponsiblefordeterminingtheexposurelevelofrespirabledustin andaroundtheiremployeesbreathingzone.Theuseofavacuumattachedtopoweredorbitalsanderswould beacceptablemeanstoreducerespriabledustbelowtheOSHAPELfromworkersbreathingzone.

• Workerswhowouldbeexposedtorespirabledustthatisgreaterthan5mg/m3inandaroundworkers breathingzonemustsubmitacomprehensiverespiratoryprotectionprogramthat complieswith29CFR 1910.134iftheyrequiretheiremployeestowearrespiratoryprotectionwhensandingsheetrock.• Respirabledustthatislessthan5mg/m3inandaroundtheworkersbreathingzonedoesnotrequirerespiratory

protection.However,workerschoosingtowearadisposabledustmaskonavoluntarybasismustcompleteAppendixDtoSection1910.134(Mandatory)InformationforEmployeesUsingRespirators WhenNotRequiredUndertheStandard.AppendixDiscontainedinthissitespecificmanual.

LOCKOUTPOLICY

• Thisprocedureestablishestheminimumrequirementsforthelockoutofenergyisolationdeviceswhenevermaintenanceorservicingisdoneonmachinesorelectricalequipment.Itshallbeusedtoensurethatthemachineorelectricalequipmentisstopped,isolatedfromallpotentiallyhazardousenergysourcesandlockedoutbeforeemployeesperformanyservicingormaintenancewheretheunexpectedenergizationorstart-upofthemachineorelectricalequipmentorreleaseofstoredenergycouldcauseinjury.

• Lockoutisthepreferredmethodofisolatingmachinesorelectricalequipmentfromenergysources.Toassistemployersindevelopingaprocedurewhichmeetstherequirementsofthestandard,thefollowingsimpleprocedureisprovidedforuseinlockoutprograms.Thisproceduremaybeusedwhentherearelimitednumbersortypesofmachinesorelectricalequipmentorthereisasinglepowersource.Formorecomplexsystems,amorecomprehensiveprocedurewillneedtobedeveloped,documented,andutilized.

• Allemployeesandcontractoremployeesarerequiredtocomplywiththerestrictionsandlimitationsimposedonthemduringtheuseoflockout.Theauthorizedemployeesarerequiredtoperformthelockoutinaccordancewiththisprocedure.Allemployeesandcontractoremployees,uponobservingamachineorpieceofelectricalequipmentwhichislockedouttoperformservicingormaintenance,shallnotattempttostart,energize,orusethatmachineorelectricalequipment.

Responsibility

• Appropriateemployees(contractor)shallbeinstructedinthesafetysignificanceofthelockoutprocedure.• Acompetentpersonwillconductasurveytolocateandidentifyallisolatingdevicestobecertainwhichswitch(s),

valve(s)orotherenergyisolatingdevicesapplytotheequipmenttobelockedout.Morethanoneenergysource(electrical,mechanical,orothers)maybeinvolved.SeetheattachedSametCorporationLockoutProcedureschecklist.

Lockoutsystemprocedure

• Notifyallaffectedemployeesthatalockoutsystemisgoingtobeutilizedandthereasontherefore.Theauthorizedemployee(contractor)shallknowthetypeandmagnitudeofenergythatthemachineorelectricalequipmentutilizesandshallunderstandthehazards.

• Ifthemachineorelectricalequipmentisoperating,shutitdownbythenormalstoppingprocedure.• Operatetheswitch,valve,orotherenergyisolatingdevice(s)sothattheequipmentisisolatedfromitsenergy

source(s).Storedenergy(suchasthatinsprings,elevatedmachinemembers,rotatingflywheels,hydraulicsystems,andair,gas,steam,orwaterpressure,etc.)mustbedissipatedorrestrainedbymethodssuchasrepositioning,blocking,bleedingdown,etc.

• Lockouttheenergyisolatingdeviceswithassignedindividuallock(s)andtag(s).• Ensurethattheequipmentisdisconnectedfromtheenergysource(s)byfirstcheckingthatnopersonnelareexposed,

thenverifytheisolationoftheequipmentbyoperatingthepushbuttonorothernormaloperatingcontrol(s)orbytestingtomakecertaintheequipmentwillnotoperate.Returnoperatingcontrol(s)toneutralor“off”positionafterverifyingtheisolationoftheequipment.Themachineisnowlockedout.

RestoringEquipmenttoServiceWhentheservicingormaintenanceiscompleteandthemachineorelectricalequipmentisreadytoreturntonormaloperatingcondition,thefollowingstepsshallbetaken.• Checkthemachineorelectricalequipmentandtheimmediateareaaroundthemachineorequipmenttoensurethat

nonessentialitemshavebeenremovedandthatthemachineorelectricalequipmentcomponentsareoperationallyintact.

• Checktheworkareatoensurethatallemployeeshavebeensafelypositionedorremovedfromthearea.• Verifythatthecontrolsareinneutral.• Removethelockoutdevicesandreenergizethemachineorelectricalequipment.• Notifyaffectedemployeesthattheservicingormaintenanceiscompleteandthemachineorelectricalequipmentis

readyforuse.

CODEOFCONDUCT/WORKPLACEVIOLENCE

NothingismoreimportanttoSametCorporationthanthesafetyandsecurityofitsemployeesandcontractors.Threats,threateningbehavior,oractsofviolenceagainstemployees,contractors,visitors,guests,orotherindividualsbyanyoneonCompanypropertyorprojectssiteswillnotbetolerated.Violationsofthispolicywillleadtodisciplinaryaction,whichmayincludeterminationofemployment.Anypersonwhomakessubstantialthreats,exhibitsthreateningbehavior,orengagesinviolentactsonSametCorporationpropertyorprojectsiteswillberemovedfromthepremisesasquicklyassafetypermits,andshallremainoffSametCorporationpropertyorprojectsitespendingtheoutcomeofaninvestigation.SametCorporationwillinitiateadecisiveandappropriateresponse.Thisresponsemayinclude,butnotlimitedto,suspensionandorterminationoranybusinessrelationship,reassignmentofjobduties,suspensionorterminationofemployment,andorcriminalprosecutionofthepersonorpersonsinvolved.IncarryingouttheseSametCorporationpolicies,itisessentialthatallpersonnelunderstandthatnoexistingSametCorporationpolicy,practice,orprocedureshouldbeinterpretedtoprohibitdecisionsdesignedtopreventathreatfrombeingcarriedout,aviolentactfromoccurring,oralifethreateningsituationfromdeveloping.AllSametCorporationemployeesareresponsiblefornotifyingtheirsupervisorortheSafetyDirectorofanythreats,whichtheyhavewitnessed,received,orhavebeentoldthatanotherpersonhaswitnessedorreceived.Evenwithoutanactual
