The Special Kids Network System of Care (SKN SOC), in partnership with the PA Elks Home Service Program, helps children and youth with special health care needs by helping their families get the services and support these young people require in order to thrive in the community and develop to their full potential.

The SKN SOC
SKN SOC History:• Continued funding through the Federal Title V Maternal
and Child Health Services Block Grant since inception in 1996
• Has focused on children with special health care needs through age 21
• Initially began as an information and referral service

• Community systems development component introduced in 1997
• Parent Youth Professional Forums (PYPF) began in late 2008

A New Initiative
Two significant changes were introduced:
• Parents of children with special

The six regional coordinators reside in six health districts of Pennsylvania.

PA ELKS HOME SERVICE PROGRAM
The Elks Home Service Program:
• offers free in-home care coordination services;
• provides services to persons of any age with a disability that began before age 22; and,
• services can continue throughout a person’s lifetime, if needed.
PA ELKS HOME SERVICE PROGRAM
The Elks Home Service Program:
• provides home visits by a nurse or social worker;
• offers one-on-one help to navigate the system; and,
• referrals can come from any source.
