Skyward Training for Nurses

Vision, Hearing, Scoliosis & AN Screens BMI and BMI% Setting Defaults Mass Add/Delete State Reports Health Reports for Vision, Hearing and Scoliosis
Menu Path: ST/SP
New change to the pathway from last year! Begin by clicking the health tab you wish to go into. Make sure that you
have the student that you want to enter (hidden behind the black)
You can flow through skyward with the side tabs
Let’s click on vision to go into this student’s vision screens
Tabs let you go where you want to go Shows what students file you are in What action you want to take
When you click on vision the word “Vision” is bold
2
Click add to add a new vision entry
You can add edit void change defaults or log from this screen. I have my defaults set up so I don’t have to enter everything. This default is set up as if everything is with in normal limits for this student. I would change the date if I am not entering it on the day it was done.
Optional way of
charting
3
I would change the information only if the student could not see well. Since most of the student’s pass the vision
screen this prevents me from having to input all the information that is WNL. (But there is even an easier way that I will
show you soon.)
I can also edit a previously entered Vision screen here or I could void this previously entered screen.
I could add a secure note, change my defaults, or log
Voiding does not make it go away it just voids it and it is supposed to turn red when you void something in Skyward.
4
Enter your reason for voiding this screen. Forever you will be able to see the reason it was voided
5
Always save your work. We do the same in hearing. You can add, edit a previous screen or void a previous screen or
add a default to this screen.
Hearing Defaults have been set up for this screen. Most white spaces have data in them once the default is set.
Once again if you are entering data into a blank white space, you do not have the defaults set so make sure the
pass/fail option is picked
Indicate if this is a rescreen.
Individual % of loss if you know it
You can add the ear intensity and once you have this set it will stay unless you change it.
You can edit all the thresholds or delete the thresholds
You can also have a note added
6
Since my defaults are set up I do not have to pick the observation, school, examined by. So when entering one by one it
makes entry faster
Editing the Thresholds screen looks like this and don’t forget to save. This is like a little default inside your vision screen. If your student fails you need to indicate it.
They will remain the thresholds unless you change them yourself
7
Scoliosis
Because I have my defaults set up and this student is in the normal limits. I can just save this. If the student did not pass
I would change the information to fail and indicate the correct results. Once again the Pass/Fail is very important and
builds your state report.
The BMI and BMI % is the new part. Physicals, BMI and BMI%, AN (so easy now)
Here you can enter Ht, Wt (skyward populates the students BMI), BP, risk for diabetes, if this was a sport physical, AN
present, if the student has been referred or treated by a physician, degree of AN and a note. In order for skyward to
populate the BMI you have to put the Ht and Wt on the left where you see the (0)
8
When you enter the Ht and Wt Skyward will automatically populate the BMI and BMI % for you. Love that!
Defaults
You can set your defaults for any office visit, vision, hearing, scoliosis or Physical Screen (used for AN) screen by clicking
the default tab on the right side of the screen. You must be in the vision, hearing, scoliosis or AN screen in order to set
the defaults
9
This is the default I have set up for my vision. You can set it up any way you want. I go to two different schools so I have
chosen to leave the School blank. As in all of skyward the grey areas are already populated by skyward based on what
you tell it to do in skyward.
This is the default screen for Hearing
This is the default screen for Scoliosis
Defaults for Health Test were set up for my (before I knew where to put them) AN but you already saw the Physical
screen for this. But you can see that you can set up a default for any test you like and you can name the test

Mass Add and Mass Void- OF/HR/PS/UT
Pick Mass Add
Caution: Make sure you are really comfortable in doing this or have your IT department folks help you. A mistake is a
BIG mistake and you have to be extremely careful when voiding also.

Add
Set up your screen (like you would your default)
Then Save and Run
Once this is complete you can print.
Remember that the entire 7th grade student shows that they were all normal on the date that you ran this mass Add for
vision.
All you have to do now is go in to the student’s vision screens that did not pass and update it to reflect the correct
information. This is a handy little step if you know how to use it correctly.
Health Reports: Pathway: OF/HR/RE(Using the old pathway here)
12
The pathway above gives you most of your reports options on this screen. When you are sending a report to another
school district you need to consider the ALL Health at the top. If you did an office visit today and you want to send a copy with the parent to the physician you can do an OV report. Let’s pick all health
You can Add, Edit, clone or delete this report. We are going to edit this report
Click on the individual student or you have the option to run by a range
Let’s edit all the ranges. Do you want to add Specific Secure note?
Those secure notes for me are used in highly sensitive situations.

Just look at your report before you give it to folks and make sure this information needs to be there. Then
systematically go down the list to see the ranges you want set. I don’t usually print both the (Report and summary). Do
it for a while and decide for yourself.
Remove the previous student then. Then enter the last 3 digits of the student’s last name and then Click Add, ok and
then back. This report will stay the way that you set it up. I just edit this report all year and change the students’ name.
Then I print it.
Since this is going to be an all health report, edit your ranges in all the areas below. We will look at them each on the
computer
Just systematically go in and edit the ranges for all area’s and then use this same report every time with only having to
change the name of the student
14
I like to sort by grad year and the then sometimes the gender
These little arrows are draggers. Click on them. Drag to the arrangement you want. Remember to Save, Save and Print
Click on your report and it will open. Here is what the report looks like.

State Reporting
The state does not care where we get our information from they just want that information downloaded into their data
base now. They never wanted the reports that were printed in skyward. So use skyward to make sure everything is
correct. Then enter it in the state web site
