FOR SMCHS STUDENT & PARENT/GUARDIAN:
I request that SMCHS allow my son/daughter to attend and participate in SMCHS activities and
events, including the Back to School Dance (grades 10-12), Homecoming Dance (grades 9-12),
Senior Cruise (grade 12 only), Winter Formal (grades 10-12), Spring Dance (grades 9-12) and
Prom (grades 11-12; students in grade 10 may attend only if invited by student in grade 11 or
12). Non-SMCHS students (guests) may only be invited to attend the Homecoming Dance,
Winter Formal and Prom. Non-SMCHS students (guests) must have a separate guest
contract on file with SMCHS Activities and must check-in at events they are invited to with
their SMCHS date.
I understand that these are not a mandatory activities and I agree to direct my student to
cooperate and conform to the directions of the supervisory personnel at these school functions. I
am aware of the rules and consequences for all activities and events should my son/daughter
violate these rules.
Our goal is to provide a safe and secure environment for your student to enjoy the evening.
To that end, by signing this contract, I understand and agree to the following:
• My student could be the subject of alcohol and or drug screening
• My student could have their personal belongings searched
• My student could be “patted down”
• Refusal by my student to submit to any of the above listed items will result in
their not being allowed into the dance. I agree to respond at that time to pick up
my student.
• “Freak” dancing will not be allowed. “Freaking” will result in removal from the
dance.
• No “Rave” paraphernalia allowed. (Glowsticks, lights, pacifiers)
• Dress code will be enforced. Failure to adhere to the dress code will result in
removal from the event.
• All guests, (non SMCHS students), are required to adhere to all the above listed
items.
Final interpretation regarding appropriate attire and behavior will be determined by
SMCHS staff.
Important - The searches and “pat downs” will be conducted by professionals trained in
such activity (male and female personnel). The purpose of searches and “pat downs” is to
ensure that students are not in possession of any contraband. Contraband is defined as
anything illegal to possess. This includes but is not limited to: drugs, alcohol, and weapons.

DRESS CODE
Each activity/event covered in this contract has a specific dress code that is clearly outlined. It is
the responsibility of each student to review that dress code included in this document and the
Parent Student Handbook, know the appropriate dress for the event they are attending and to
communicate that dress code to any guest they have invited to attend. Please review and keep
the attached dress code picture handout. Attendees (guest included) unable to comply with
the Dress Code will be refused admittance. Students with concerns about the appropriateness of
their attire may submit a picture of themselves wearing the outfit (front and back) to the Deans
Office for approval prior to the event. Please note that bandage/body conforming, stretchy skirts
and dresses made of lycra, spandex, etc. are strictly prohibited. Students and parents must initial
each page of this contract after reading.
SMCHS students are expected to keep in mind that although social activities do not require
uniforms, the same standards of personal appearance emphasizing modesty, good taste,
neatness, and safety must be followed. In the case of dress for extracurricular events, it is
the right and obligation of the Dean’s Office to determine appropriateness of fashions.
Students in violation of the Formal/Semiformal dress code will either be asked to leave the
event or, if there is an appropriate loaner outfit available, the student may wear a loaner
outfit for the event but they will receive a dress code detention and will have to dry clean
the outfit and return it within one week.
BEHAVIOR CODE
SMCHS rules and consequences will be enforced at all events covered in this contract. Please
refer to the parent/student handbook for all policies and consequences for conduct.
Certain types of dancing (moshing, freaking) are considered inappropriate or dangerous
and are therefore prohibited. Attendees (guests included) who engage in these types of
dancing will be subject to school consequences – removal, parent contact, loss of future
dance privileges.
Use or possession of alcohol, drugs or any nicotine delivery system or any other illegal mood-
altering substance is strictly prohibited before/during this event. Attendees (guests included)
deemed to be in possession or under the influence of any banned substance will be removed
from the dance. School consequences will be implemented. Law enforcement officials may
be contacted.
My son/daughter and I have read and understand the SMCHS Student Activity and Event
contract detailing the rules and consequences for these school functions. In case of emergency,
supervisory personnel have my permission to seek any medical treatment deemed necessary.

BACK TO SCHOOL DANCE AND SPRING DANCE

DRESS CODE
Students must meet the minimum casual dress attire (see below) in order to be admitted. The
Back to School and Spring Dances are considered casual dances, and thus you are expected to
follow the SMCHS guidelines for casual attire. SMCHS requires that students keep their shirts
and shoes on during the event.
Men Casual Dress:
• Shorts/pants must have at least a 6” inseam
• Hair well-groomed and clean shaven
• Closed toe, closed heel shoes (i.e. tennis shoes, Converse, Tom’s, etc.)
Women Casual Dress:
• Shorts/pants must have at least a 6” inseam
• Modest cleavage, no plunging necklines /open backs
• No bare midriffs

• Dress slacks and dress shirt with tie (mandatory)
• Dress shoes (hard soled) or sneakers (i.e. Converse, Toms, etc.) and socks
• Hair well-groomed and clean shaven
• Dress shirts and shoes must be worn at all times
Women:
• Dresses may not come any higher than 4 inches from the middle of the knee
• Slits on the dress can only be to the knee
• Dress length is measured from the bottom of solid material

SENIOR CRUISE
(Students in grade 12 complete this page. Grades 9, 10, 11 do not need to

All students must check in by 7:45 pm with their current SMCHS ID card.
Students will not be allowed to leave the event before 10:30 pm.
All students must check in to the yacht by 7:45 pm. The boat leaves promptly at 8:00 pm
and will not be held for anyone who is late. Only SMCHS students in the Class of 2018
may attend this event. Tickets will not be sold at the door. No guest passes will be accepted
for this event.
DRESS CODE
Costumes must be in good taste and appropriate for an SMCHS function. This includes:
• Shorts must have at least a 6” inseam.
• Dresses/skirts must be not shorter than 4” from the middle of the knee.
• Modest cleavage.
Inappropriate costumes will not be tolerated and you will not be allowed on the boat if your
costume is deemed to be so by an SMCHS adult chaperone. This includes, but is not limited to:
• Costumes that are racially insensitive to any race, group, or culture.
• Costumes cannot cover your face for safety reasons, masks and props will be stored for
pick up afterwards.
• Costumes that are not modest in appearance either by length of short, skirt or pant.
• Bandage/body conforming, stretchy skirts/dresses made out of material such as lycra,
spandex, or similar material are prohibited.
