Foreword

This Team Profile for Social Care and Housing Services is a brief guide to the roles and responsibilities of each of the teams in the service area, which I hope you will

find helpful.

The Social Care and Housing service is a complex business, governed within a national statutory framework, to meet local people’s needs. Many of our services are

delivered by other organisations and this guide does not attempt to describe all of those providers.

We are facing a period of significant change, with the Social Services and Well-Being

Wales Act Regulation and Inspection Act and Housing Acts, giving us new responsibilities within a context of rising demand and reduced resources. Our focus is always to safeguard and support people in the most effective and efficient way and

this means new models of service delivery and changes to current practice are required.

Some of our teams are co-located or integrated with neighbouring public sector

agencies and this is the future direction for us. This will ensure we use our resources wisely to meet future needs and it does not mean we can delegate our

statutory responsibilities to other organisations.

All of our collaboration plans will be based upon evidence of delivering benefits for Torfaen residents, no matter where a particular team or service is located.

The team profiles have been published on Sharepoint and will be regularly updated.

We have a well developed Members Enquiries system and all enquires should be

Assistive Technology Team

Lead Assessor Adrienne Street The Assistive Technology Team (Lifeline and Telecare) provides a remote monitoring

service to all vulnerable residents in Torfaen. The Telecare service objective is to empower

service user’s independence, health, safety and wellbeing in the community. Referrals are

passed to the Assistive Technology Team, to complete an individual needs based

assessment. This is necessary because there is a wide range of equipment that can be

used to support risk management and safety at home.

The team work in partnership with other agencies including the falls clinic, carers support

groups and have recently begun to support a group for carers of people with dementia. The

majority of our service users are older persons but we provide specialist support to a

diverse client group.

Brokerage Team Senior Broker Dawn Price The brokerage team was established in 2005 to provide a single access point for the

arrangement of domiciliary care packages, utilising the internal intake team and the block

providers before sending out the care package request as a mini tender on the Dynamic

Purchasing System. Brokerage monitor and report provider activity and availability

throughout the borough. One of the key service improvements has been the use of a central

point capturing relevant information, for reports, commissioning of services, budget

monitoring and contract review. This has allowed the authority to have accurate and up-to-

date market information; the team has a key role in monitoring the block contract hours and

informing commissioning strategies.

Brokerage has provided considerable savings in care management time as they no longer

have to spend time arranging care packages and respite, so can concentrate on other

aspects of their role. Brokerage is a central point of contact for finance and care managers

to provide information on a service users care package.

Brokerage continues to book block respite and monitor the utilisation of these respite beds

and also manage the short stay unit diary. Brokerage also ensures DRAIG is up to date

with services arising out of carers assessments and ensure services users attending day

activities or receiving supported living are entered onto DRAIG.

Children and Families Teams

Team Managers Sue Radford North 1 Iain McMillan North 2 Carol Dawson South 1 Natalie Poyner South 2 Torfaen Social Care & Housing Children’s Services has been designed to base teams within local communities and work with families, partner agencies and voluntary groups to achieve improved outcomes for children. The four teams cover all areas across the borough comprising of:

North 1: Blaenavon, Absersychan, Pontnewynydd, Wainfelin, Cwymynyscoy, Snatchwood and Panteg

North 2: Trevethin, St Cadocs & Penygarn, New Inn, Pontypool and Brynwern

The North and South teams both have a central ‘duty’ system which they share between them. All referrals and child concerns are processed and initial child protection processes are completed. Cases that require on-going involvement are then transferred to the responsible team. Community Meals Community Meals Manager Julie Drew The Community Meals service is highly valued, supports a high number of service users,

and provides the opportunity for a meal and a welfare check at a very low cost. The service

is delivered by paid drivers, seven days a week 365 days a year.

The service provides a hot meal and a hot or cold sweet to eligible service users throughout

Torfaen at a very reasonable charge of £4.50. There is a daily choice of menu which

changes regularly.

The daily visit allows for monitoring of service users who look forward to the calls and it

helps to reduce feelings of social isolation. Regular delivery times and regular drivers are

key to an effective and reliable service.

Day Activities Team Manager Day Opportunities Jim Wright There is a wide range of day activities available to meet the varied needs of our service users (predominantly those with learning disabilities). The opportunities available include employment training schemes, educational-based projects and activities designed for people with more specialised needs. Work training schemes offer the trainees the opportunity to develop work skills, health and safety practice, business development and customer services. Education courses are run in conjunction with the council’s adult education department and offer a choice of qualifications at entry level. Day activities cater for people with severe physical and sensory needs in collaboration with our multi-disciplinary teams which include social workers, nurses, occupational therapist and physiotherapists.

Disabled Children l Team Manager Gill Smith The Disabled Children Team, works with children (between the ages of 0-18) and families where a child has a severe learning, physical, visual or hearing disability that could cause significant difficulty in daily living. The team also work with children who have life threatening illnesses. The aim is to help a child realise their full potential while living with their family and each child and their family are met with to discuss their individual needs. Sometimes work is carried out directly with the family, or alternatively, may make arrangements for other agencies to provide care and support services. All children’s and carers’ needs are assessed before any services are provided. Carers’ needs are also assessed in recognition of their caring role and services are considered to support them following a carer’s assessment. The team work to the principles and requirements of the legislation regarding children and disabled people. The team also maintain the local authority index (register) of disabled children which helps to inform future planning. Disabled People’s Service

Team Manager Mel Firminger The Disabled People’s Service aims to help and encourage people to live as independently as possible. This is achieved by providing specialist advice, support and equipment, which makes it easier for people to live in their own home and adapt to their impairment. The service is also responsible for looking at what can be provided to support people who care for a disabled person. Occupational Therapy The occupational therapists and occupational therapy assistants are able to advise on new ways of carrying out everyday tasks. This means equipment can be provided and guidance to help people or their carers to do things more easily, or in some cases recommend adapting someone’s home. Building Surveying The building surveyors work closely with the occupational therapists to design and project manage schemes to adapt the home of a disabled person applying for a Disabled Facilities Grant (DFG). The service are able to offer this service to homeowners or tenants living in rented accommodation. The grant liaison officer will guide people to apply for the grant although this will be subject to a means test and a fee will be added to oversee the grant work. Social Work Social workers and community care workers specialize in supporting people aged under 65 who live with a physical disability or long term ill health. The service look to maximize people’s chances of independent living and whether they need someone’s support to achieve this. Also providing support to adults of all ages with a hearing or sight impairment. This can be in the form of equipment, and additionally for people diagnosed with sight loss, access to our rehabilitation worker. Carer’s Support The carers support service specialises in supporting people such as family members, friends or neighbours who provide a substantial amount of unpaid care to look after someone else. Assessments are undertaken with carers to determine what emotional and practical support they need to be able to maintain their caring role, or simply to give them information and advice. The service also run dedicated training courses for carers throughout the year to share practical ideas for looking after themselves and their cared for person, and to help to strengthen friendship networks. Direct Payments Direct payments give more flexibility in how people can access the services they need. Providing money instead of social care services gives people greater choice and control over their lives, and enables them to make their own decisions about how their care is

delivered. The Disabled People’s Service specializes in giving advice to all social work teams on how they can promote this option to service users. Duty Pod The duty pod is multi disciplinary and is the first point of contact into the adult services teams for the general public with the exception of citizens with a Learning Disability or Adults under 65years of age that experience mental health issues. The purpose of the team is to provide advice and information and where assessment is needed to provide a right time, right time response.

Family Placement Team Team Manager Andrea Yeo The Family Placement team provide and manage the council’s fostering service, i.e. foster care placements for children and young people who are looked after. The team is responsible for the recruitment, assessment and on-going support of foster carers who can meet the diverse needs of children who may need short-term foster care, permanent placements or supported lodgings placements. In addition assessments of kinship carers are undertaken to enable children to be cared for by existing family members. There is now recognition that the most important relationship a child is likely to have is with the people who care for them. Thus, the team’s priority is to recruit carers who can provide quality alternative care to birth parents for our most vulnerable children. The expectation of foster carers has grown in recent years, with increased emphasis on training and supervision. This ensures that standards are met to promote the best possible outcomes for looked after children. The team and fostering service is inspected against National Minimum Standards annually by Care and Social Services Inspectorate Wales (CSSIW). Family Resource Centre

Acting Team Manager Tina Nash The Family Resource Centre helps parents and carers to understand and meet the needs of their children in a safe, supportive and caring environment. The centre provides a range of services to children and their families which include: Supervised Contact The purpose of supervised contact is to ensure the safety and well-being of children and to improve and build upon a carer’s ability to parent by offering advice, guidance and positive role modelling. Observations of contact will be documented and given to the child’s or children’s social worker and will be used as part of the ongoing assessment and decision making process for children and their families.

Assessment and Direct Work In recognising that the best place for a child to be brought up is with their families, the team offers individually tailored packages of work. These include:

Independent parenting capacity, risk and rehabilitation assessments

Individual parenting programmes, including perceptions of parenthood work

Group work, including parenting groups for parents and carers, group work for young people to develop confidence, self esteem, and awareness of personal safety.

Practical parenting support to parents in crisis. This involves strategies and techniques for parents to help them to put advice into practice.

Young Carers Service Torfaen’s Young Carers Service works with young carers aged 8—18. A young carer is someone who is under the age of 18 and carrying out caring and household tasks because they have a parent, brother, sister, other relative or friend who has a physical disability, learning disability, mental health issue, long term or terminal illness or a drug or alcohol misuse issue. As well as individual support from the Young Carers Service. There is also a Young Carers Group and a Young Carers Forum.

Family Focus Team Team Manager Rhianydd Williams The Family Focus team believe that families are experts in their own lives and can identify their own issues and difficulties and draw on their unique strengths to address their problems. Our team members will work with families to help them make self identified changes which will enable them to stay together in a safe way. The Process The Family Focus team worker will offer intensive support to the families they work with by visiting regularly, offering practical, hands on support and supporting the family to make the changes that they identify for themselves. Gwent Wide Integrated Community

Equipment Service (GWICES)

The Gwent Wide Integrated Community Equipment Service (GWICES) Community Equipment Service has been established to maintain service user’s independence in their own home, by providing equipment to assist with daily living. Torfaen County Borough Council as lead commissioners for GWICES manage the pooled budget which was agreed though the signing of a Section 33 Agreement (National Health Services (Wales) Act 2006), which enables local authorities to exercise certain NHS functions and for local health boards to exercise certain local authority social care related functions. This Section 33 Agreement covers the five “Greater Gwent” local authorities (Torfaen County Borough Council, Caerphilly County Borough Council, Monmouthshire County Council, Newport City Council, Blaenau Gwent County Borough Council) and Aneurin Bevan Health Board. During 2010 TCBC, the lead commissioner formally tendered for the provision of a community equipment service on behalf of the partnership and the contract was awarded to Vision Products Newport (VPN) – managed by RCT County Borough Council for a five year period from April 2011. which has been extended for a further 2 years to March 2018. To ensure service users get the correct equipment, a health or social care worker will undertake a needs assessment. The range of equipment that we provide through GWICES includes:

Hospital Social Work Team

Team Manager Anne Rigon The function of the hospital social work team is to facilitate the safe discharges of Torfaen residents from all local hospitals within South East Wales. The team work with people during their hospital stay and our primary aim is to return people to their own homes. We work in partnership with service users, carers and families to:

ensure that needs and risks are identified prior to discharge

promote positive outcomes, and

provide appropriate support for a successful discharge The team works very closely with nursing and medical staff, complex care team, frailty team, district nursing and discharge teams, therapists and other social work teams, in order to ensure proportionate assessments of service users’ and carers’ needs, appropriate responses and housing support. Homeseeker & Financial Inclusion Team Manager Adam Harper Homeseeker is the Common Housing Register in Torfaen. Homeseeker is a partnership between Torfaen Council and housing partners that include Bron Afon Community Housing, Melin Homes, Charter Housing Association, Hafod Housing Association, United Welsh and Linc Cymru. Homeseeker provides a single platform for applicants to join the waiting list and access Social Housing in Torfaen. Properties are advertised on a weekly basis and a choice based lettings scheme is operated. Financial Inclusion is a support service funded through the Supported People scheme. Support is offered across all tenure - to help prevent homelessness supporting clients who are in financial difficulties. Our officers focus on maximising income and reducing outgoings. The Torfaen Homes website which provides Housing information and advice to all residents of Torfaen is continually updated and Social Media pages are continually updated. In the first three quarters of 15/16 there were over 10,000 hits on the website.

A recent scheme utilised £1.3million of Arbed ECO funding to provide energy efficiency measures to 162 homes in the Thornhill area, saving 1782 tonnes of Carbon.

Housing Renewal Team Team Manager Christine Peploe Through our Housing Renewal programme our main focus is improving private sector housing and area regeneration. In Torfaen’s 3 designated Neighbourhood Renewal Areas (Forgeside, Varteg/Garndiffaith and Northville/Southville in Cwmbran), improvements to private sector homes are targeted through the use of Housing Renewal grants, which has a positive impact not just for the owners, but also the wider community. Some recent successes The Housing Renewal Team has successfully managed the 2015/16 Specific Capital Grant programme from the Welsh Government assisting homeowners and private landlords in the Neighbourhood Renewal Areas, and they are on course to provide grant assistance for 110 projects. The team has successfully worked with Torfaen Planning department to improve the Shepherds Hill Gypsy & Traveller site.

Housing Solutions Service Team Managers Sheryl Thomas & Karen Durrant The Housing Solutions Service is a joint service run with Monmouthshire County Council Housing department, to deliver front line advice and support in relation to Housing matters. The Service has two elements, the Housing Solutions Team and the Private Sector Housing Team. The staff in the Joint Housing Solutions Team provide customers with housing advice and assistance. The new Housing (Wales) Act 2014 has a focus on preventing homelessness, and wherever possible, people are supported to remain in their own home or to find a suitable alternative. The team will look to source temporary accommodation until a more longer term housing solution is found in accordance with personal housing plans that are developed for with each client. The team will also provide housing management of the Council owned Gypsy & Traveller site at Shepherds Hill. The joint Private Sector Housing Team manage a portfolio of temporary accommodation to assist households who are homeless and in priority need for accommodation. To reduce the use of bed and breakfast, the team lease accommodation from within the private sector to provide suitable temporary housing. The team also work closely with private sector landlords to improve the quality of properties in the private sector, also encouraging Landlords to work with both Councils to help develop the private rental sector as a viable tenure option of choice. Torfaen County Borough Council initial allocation of Welsh Government Social Housing grant was £1.3m for 2015/2016 through strategic planning and working collaboratively with developing Registered Social Landlords over £4m of grant has been spent. The additional funding has been accessed through further advances to housing at Welsh Government by the Finance Minister, and slippage funding, the grant has contributed to the delivery of over 100 affordable homes. A new interest free loan was launched through the Vibrant and Viable places funding. The loan offers home owners and private landlords the opportunity to access an interest free loan of up to £25,000 to raise the standards of their properties.

Housing Supporting People Team Team Manager Bethan Covill

The Supporting People team manages the Supporting People Programme in Torfaen, that involves the strategic planning, commissioning and reviewing of housing-related support services and supported accommodation for vulnerable people. Housing-related support can include activities such as helping to set up home, improving social skills, budgeting or increasing confidence of vulnerable people. Support can be delivered in an individual’s own home or through supported housing provision such as a refuge, sheltered accommodation and group homes. The team also contributes to regional planning and development of services in partnership with colleagues from the other Gwent authorities where it delivers greater value for money and/ or more effective and efficient services. The staff are a valued source of support to service providers delivering services in the community.” Our recent success We piloted a Regional Domestic Abuse project supporting women from Black Minority Ethnic backgrounds. We have commissioned a jointly funded prisoner resettlement worker with Blaenau Gwent and Monmouthshire Supporting People teams and housing solution teams from the respective LAs. Following a successful pilot project, we have commissioned a service designed to provide housing related support to women experiencing domestic abuse with highly complex needs. Service user story - Jamie’s Story Following a period of sofa surfing with friends & relatives, Jamie had secured his own private rented accommodation. One day he returned to find that he could not get in, the landlord had changed the locks and terminated his tenancy, he had put some of Jamie’s belongings into black bags and had disposed of the rest. Jamie then stayed between his sister’s and brother’s on their sofa’s for some time and during this period built up a lot of debt through pay day loans and was at risk of offending in

order to keep up his repayments, he became clinically depressed and this resulted in him dropping out of college and signing on for Employment Support Allowance. Jamie applied as homeless to the local authority but didn’t qualify for temporary accommodation; he was advised to register with Homeseeker for housing and was referred for support. Support Received

Housing Support - to look for and apply for accommodation. Jamie was offered a one bed flat in Cwmbran, support then focussed on setting up his home and paying household bills.

Benefits Support – applying for benefits and support to attend benefit medicals.

Financial Support – applying to the discretionary assistance fund for help to carpet and furnish his flat £734 received; support to open a bank account; support to sort out pay day loan debts.

Employment and training — support to look for work and volunteering opportunities, help to apply for an open university course, Jamie is also taking part in the support providers digital inclusion project.

Support has helped Jamie to rebuild his confidence and feel hopeful about his future. He now feels part of the community. Jamie is able to budget and pay his bills and has payment plans in place for his debts. He regularly visits the local job club keeping his CV up to date. Jamie’s home is fully furnished and he is able to maintain it, he has the skills and confidence to approach his landlord should he need to. Jamie is no longer on antidepressants and has been referred for counselling. Jamie is now living independently and making plans to start a college course in ICT to increase his chances of gaining full time employment. Jamie is looking for work and has applied to volunteer at the local Citizen Advice Bureau which he hopes will increase his confidence and experience of working.

Looked After Children Team

LAC Co-ordinator Beverley Davies The team aims to provide a quality and effective service for children who become looked after by Torfaen County Borough Council. The team comprises a Looked After Children (LAC) Co-ordinator, Clinical Nurse Specialist (CNS), and Independent Reviewing Officers (IROs) and Administrators. An IRO is responsible for reviewing a child’s care plan and placement to ensure the relevant assessments are in place. The outcome of these assessments determine a child’s long-term care plan and ensures their foster care placement meets their needs in terms of a safe secure stable placement. An IRO also acts as an auditor for the reviewing process and ensures the statutory requirements are met by the child’s Social Worker. A CNS undertakes an Initial health assessment of each child and will complete an action plan which identifies any outstanding health needs. The LAC health assessments are statutory requirements which are subsequently carried out 6 monthly or annually dependent on the child/young persons’ age. The team administrators ensure all LAC reviews and LAC health assessments take place within the statutory timescales and that reports are circulated promptly thus providing a high quality of administrative service and making a positive contribution to the effectiveness and efficiency of the department. Mental Health Team Team Manager Howard Hopkins The team are a multi-disciplinary team providing social and health services to people over the age of 18 with a mental health problem resident in Torfaen. The team is based on two sites in the grounds of the County Hospital in Griffithstown. The Older Persons Mental Health Care Team (OPMHT) works with people aged 65 and over and is based in the Ty Siriol Unit. The Adult Mental Health Care Team (AMHT) works with people aged 18-65 and is based across the road in the Talygarn Unit. Both teams are also part of the Community Mental Health Teams and work collaborative community psychiatric nurses, occupational therapists, psychologists and psychiatrists to provide ‘joined up’ services to meet the needs of the people using the service. Older Persons’ Service Team Manager Sam Jones (Anne Rigon is providing Temporary cover) The service is a multi-disciplinary team comprising social workers, community care workers and a mental health nurse. The day-to-day work includes:

Performance and Information Team Leader Catherine Franklin The remit of the Performance and Information Team is wide and tasks are varied. Key tasks include:

Systems administrators for the DRAIG system. This includes setting up new users, training users, and maintaining the system in terms of data cleansing. Testing new versions of DRAIG prior to them being implemented, working with both internal and external partners during this process.

Dealing with day to day queries on use of the system.

The team is responsible for the performance management and the continued development of DRAIG and the integrated children’s system (ICS). ICS is a Welsh Government initiative which aims to provide a coherent and consistent framework for the key processes of assessment, planning, intervention and review. The team is also responsible for the design and implementation of new forms, business process, training and development of staff in the use of the Draig database. Personal Care Manager Hazel Orringe The Personal Care Team supports service users and carers to achieve greater independence. It aims to provide services that are flexible and responsive to individual needs and other aspects of daily life. The Personal Care Team is split into two functions, a Dementia Team and an Intake Team.

Dementia Team

The team provides long-term support to people with dementia.

This package of care may involve:

Personal care - such as getting up, washed, dressed and going to bed, assistance with toileting, showering and bathing, and the provision of food

promotion of independence and the provision of routine

on-going monitoring and review of medication

monitoring or administration of medication. The team concentrates on enabling people with Dementia to remain at home for as long as possible. They provide support to carers to reduce the pressure that comes from this often difficult role. For service users who live alone the team can try and build a clear picture of how people are living with Dementia and provide detailed reports to care managers. Admission to hospital/nursing homes often results because the risks of continuing to live in the community are too great. Medication administration is particularly complex in the Dementia team with over 43% of service users on the Medication Administration Scheme. Safeguarding Unit

Safeguarding Manager Deborah Davies The Safeguarding Unit is a multi-disciplinary team spanning both child protection and adult protection (POVA) work. The unit is responsible for taking forward the safeguarding agenda for both children and adults and the co-ordination and oversight of statutory multi-agency meetings to assess risk and plan investigations of alleged abuse. It therefore works closely with colleagues in Gwent Police, health, housing, education and the voluntary sector. Torfaen Community Resource Team (CRT)

Team Manager Rhian Morgan Deputy Team Manager Jeannie Davies Torfaen Community Resource Team is a part of the Gwent wide community development programme known as frailty. The team has been developed to provide:

A prevention of avoidable hospital service – rapid response health and social care services

A therapy led service which concentrates on maximising people’s rehabilitation and recovery after illness and minimising dependence on long-term health and social care services

Facilitated early supported discharge of those patients who have complex needs

Rapid response social care assessment and commissioning of emergency care to prevent avoidable admission and support early discharge from hospital.

Torfaen CRT assesses patient/client need and aims to holistically meet those needs within the team and by working with other community services. The CRT promotes independent living by:

Reducing avoidable admissions to acute hospital wards

Ensuring people can be discharged from hospital earlier when medical treatment requiring hospital admission is over

Promoting effective rehabilitation

Minimising premature or avoidable dependence on long-term care in institutional settings.

Torfaen CRT works in collaboration across health, the local authority and third sector providers to provide a need based service, delivered by health and social care professionals/registrants and non registrants in a cohesive and effective manner. Torfaen CRT staff complement Torfaen CRT is made up of staff from both the Social Services Department of the Local Authority and Aneurin Bevan Health Board. We are collocated and work closely together as one team. The team is expanding significantly to meet the needs of Torfaen residents over the coming years and the whole service is being replicated in the other four former Gwent boroughs.

The staff The mixture of professionals within the team, both registrants and non registrant’s, help to support a wide range of Torfaen residents. This can mean from a health or social care perspective and in many cases it is both. Whether it’s adapting to changes in circumstances, an accident or an illness, our multi-disciplinary team can assess, identify the issues and work together to get the best outcomes for the client. The team works with adults (over 18 years) mostly within in their own homes and our aim is to meet the identified and assessed needs of a client after a period of illness, disability or loss of confidence. The support provided helps people to regain the skills and confidence to carry out the ordinary tasks they were doing prior to their crisis, and in turn, helps them to become less reliant on social care support services. All of our team members including our mental health nurse, community liaison nurse, and social worker etc link in with our community and hospital colleagues every day ensuring the flow of information regarding our residents is constant and accurate. We work very closely with community mental health teams, district nurses, adult referral and assessment teams care providers, housing adaptations and many more to provide support in the right place at the right time to the right people. Fall management We work very closely with GPs and other health and social care professionals to provide a comprehensive approach to falls management. This includes:

Diagnostic testing (Basic and advanced)

Consultant led clinics

Prevention programmes

Interventions such as medication checks, Physio and O/T service support, social care support, etc

Reviews of interventions. Reablement We have a large team of therapists (physiotherapists, occupational therapists and highly skilled support workers) who work across the whole of the community resource team. The aim of the reablement is to assist people to regain skills for living independently, regain their mobility and increase their confidence in their own abilities. We engage clients to prevent inappropriate hospital admission and support discharges from hospitals. We provide a comprehensive assessment which identifies holistic needs (Health and social care) and a number of evidence based interventions designed to meet the agreed aims of the client. These interventions take place in the clients own home or on occasions within our step up/down facilities and can last up to 6 weeks.

Rapid response nursing The clinicians in the team, both doctors and nurses, carry out a number of advance roles. These roles are both medical and nursing ensuring residents receive a comprehensive assessment within their own home and respective treatment is provided specific to the patient’s individual needs. This service would traditionally be afforded to patients in a hospital setting but is now carried, where possible and safe, in the community, allowing residents more choice regarding their care. Doctors The team has a Consultant Geriatrician (clinical lead) and a speciality doctor who supports the assessment process and provides clinical support across the CRT. Step up/down facilities In certain circumstances we can offer the use of our six bedded step up/down facility at Plas y Garn Pontypool. This facility allows residents to spend up to 6 weeks in a residential home setting supported by the staff from the CRT and the home in order to maximise their potential to return home. We are able to support residents for short periods of time at Plas y Garn who would normally be admitted to hospital because of insufficient social support at home or who need an additional period of time between discharge from hospital and home. The opportunity was established with Intermediate Care Fund for funding of two nursing beds in Leadon Court. This will ensure that service users are prevented from having unnecessary admissions to hospital or when they do require hospital admission that their length of stay is minimised. Single point of access Any health or social care professional can refer to the CRT using the single telephone number: 01633 744284. The referred will be asked a number of essential questions regarding the patient/client, their current situation - needs and past history. The call takes a few minutes and the information is passed electronically to the team for consideration and action. This service is not currently open to the general public. Hours of business We provide this care every day of the year from 8 – 8pm, 7 days per week. We will respond within 2 - 4 hours for our rapid response nursing and social care service teams and if there is a need for reablement type services or a falls service we aim to respond within 24-48 hours. Charging policy There are no charges for the CRT services. They are time limited (6 weeks maximum) and if you require on-going care from core services a charge may be levied.

Torfaen Young People’s Support Service (TYPSS) Team Manager Chesney Chick TYPSS is a multi-disciplinary partnership, providing an integrated, one stop provision for young people experiencing difficulties in negotiating the transition to adulthood and independence. Partners involved are: Aneurin Bevan University Health Board, Children and Adolescent Mental Heath Service, Careers Wales Gwent, Citizen’s Advice Bureau, Llamau Emphasis & Mediation Projects, Choices – Drugaid, and the council’s Youth, Education and Social Care and Housing Services. TYPSS focuses on the often inter-related needs of vulnerable 16-25 year olds, including those who:

Workforce Development Team Team Manager Kate Dibble The main aim of the Workforce Development Team is to provide learning and development opportunities to ensure an appropriately skilled and qualified workforce in Torfaen, sufficient for the developing need of social care services across the whole sector. Youth Offending Service (YOS) Service Manager Jacalyn Richards The principal aim of the joint YOS (Monmouthshire and Torfaen) is to prevent offending and re-offending by children and young people, and this is facilitated by voluntary ‘preventative’ interventions, as well as through the management of statutory court orders. This includes work with offenders, parents and victims of young people’s offending behaviour. The team is multi-agency and includes social workers, a probation officer, education worker, clinical nurse specialist, substance misuse worker, a victim worker, youth worker, parenting workers, reparation workers, a police officer and support workers. The YOS is an active participant in the Community Safety Partnerships, Regional Safeguarding Children’s Board and Children and Young People’s Partnerships of both councils.
