intervention and devices; pathobiology of vascular diseases; and the management of the associated

about sVMThe Society for Vascular Medicine is dedicated to improving the recognition and care of vascular diseases; fostering education and research in vascular medicine; and providing leadership to government, industry and the profession regarding issues related to vascular health. SVM includes among its membership the major investigators and opinion leaders in vascular medicine. The field of vascular medicine encompasses a broad spectrum of arterial, venous and lymphatic disorders, as well as the associated medical disorders of hypertension, hyperlipidemia, insulin resistance and diabetes mellitus. This diversity and complexity of vascular diseases requires knowledgeable practitioners of vascular medicine to provide cost-effective care.
This activity has been planned and implemented in accordance with the Essential Areas and Policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint sponsorship of the Colorado Foundation for Medical Care (CFMC) and SVM. CallforSeniorInvestigators: Senior Investigators are encouraged to submit abstracts for a new Senior Investigator portion of the poster session. The abstract should indicate where your research is going and how you have guided it: in other words, tell your career story. It should also present and discuss your research as a whole — an overview. “Where I’ve been and where I’m going.”
CallforMemberLecture: For the topic that’s near and dear to your heart, but missing from the agenda, here’s the chance to give a 13-minute presentation on the topic of your choice during the Scientific Sessions on Thursday, June 13, 2013. The subject must be of interest to an audience of vascular medicine practitioners.

foReign VisasTravelers coming from qualified countries to the United States for tourism or business for 90 days or less may be eligible to visit the United States without a visa. Currently, 34 countries participate in the Visa Waiver Program (VWP). Visitors entering on the VWP cannot work nor study while in the United States and cannot stay longer than 90 days or change their status to another category. Visitors entering on the VWP must have machine-readable passports. Travelers without machine-readable passports will need to apply for a visa to enter the United States. Applicants for visitor visas should generally apply at the American embassy or consulate with jurisdiction over their place of permanent residence. Though visa applicants may apply at any U.S. consular office abroad, it may be more difficult to qualify for the visa outside the country of permanent residence. Meeting RegistRationRegistration for the 2013 SVM Annual Meeting is open to SVM members and nonmembers, including applicants for membership and anyone with an interest in vascular medicine. If you wish to attend the MOC Learning Session without attending the full meeting, SVM members pay $125 and nonmembers pay $175.

PayMent confiRMationAfter your registration is processed, you will receive a confirmation letter via email. Keep a copy of the registration form for your records. Due to registration and payment-processing time, we are unable to confirm your registration until two weeks after it is submitted.
Caution: If you submit your registration form more than once, it may result in a duplicate charge on your credit card. Send your registration using only one method of payment. Faxed registration forms that do not contain credit card information do not qualify as an early paid registration. Telephone registrations are not accepted.
cancellation PolicyNotification of cancellation must be submitted in writing via email, fax or postal mail. Cancellations received by May 23, 2013, will be subject to a $100 cancellation charge.
