Credit Statements for the SE Kansas

Regional Trauma Program All participants are required to complete and sign a

“verification of attendance” form. After the program, a certifi-

cate of completion will be provided to activity participants based

on documentation of actual attendance time.

Physicians: This activity has been planned and implemented in

accordance with the Essential Areas and policies of the Accredita-

tion Council for Continuing Medical Education through the joint

providership of the University of Kansas Medical Center Office of

Continuing Medical Education, the Southeast Kansas Regional

Trauma Council, and the Kansas Trauma Program. The Universi-

ty of Kansas Medical Center Office of Continuing Medical Educa-

tion is accredited by the ACCME to provide continuing medical

education for physicians.

The KU Medical Center Office of Continuing Medical Education

designates this live activity for a maximum of 4.5 AMA PRA Cat-

egory 1 Credits TM. Physicians should claim only the credit commen-

surate with the extent of their participation in the activity.

APRNs: The University of Kansas Medical Center Area Health

Education Center West, as an approved provider of continuing

education by the Kansas State Board of Nursing, presents this

offering for a maximum of 5.5 contact hours credit applicable for

relicensure of APRNs. Kansas Provider Number LT0086-1149.
