Trust Policy Foreword
South Western Ambulance Service NHS Foundation Trust (SWASFT) has a number of specific corporate responsibilities and obligations relating to patient safety and staff wellbeing. All Trust policies need to appropriately include these.
Health and Safety - SWASFT will, so far as is reasonably practicable, act in accordance with the Health and Safety at Work etc. Act 1974, the Management of Health and Safety at Work Regulations 1999 and associated legislation and approved codes of practice. It will provide and maintain, so far as is reasonable, a working environment for employees which is safe, without risks to health, with adequate facilities and arrangements for health at work. SWASFT employees are expected to observe Trust policy and support the maintenance of a safe and healthy workplace. Risk Management - SWASFT will maintain good risk management arrangements by all managers and staff by encouraging the active identification of risks, and eliminating those risks or reducing them to the lowest level that is reasonably practicable through appropriate control mechanisms. This is to ensure harm, damage and potential losses are avoided or minimised, and the continuing provision of high quality services to patients, stakeholders, employees and the public. SWASFT employees are expected to support the identification of risk by reporting adverse incidents or near misses through the Trust web-based incident reporting system. Equality Act 2010 and the Public Sector Equality Duty - SWASFT will act in accordance with the Equality Act 2010, which bans unfair treatment and helps achieve equal opportunities in the workplace. The Equality Duty has three aims, requiring public bodies to have due regard to: eliminating unlawful discrimination, harassment, victimisation and any other conduct prohibited by the Act; advancing equality of opportunity between people who share a protected characteristic and people who do not share it; and fostering good relations between people who share a protected characteristic and people who do not share it. SWASFT employees are expected to observe Trust policy and the maintenance of a fair and equitable workplace. NHS Constitution - SWASFT will adhere to the principles within the NHS Constitution including: the rights to which patients, public and staff are entitled; the pledges which the NHS is committed to uphold; and the duties which public, patients and staff owe to one another to ensure the NHS operates fairly and effectively. SWASFT employees are expected to understand and uphold the duties set out in the Constitution. Code of Conduct and Conflict of Interest Policy - The Trust Code of Conduct for Staff and its Conflict of Interest and Anti-Bribery policies set out the expectations of the Trust in respect of staff behaviour. SWASFT employees are expected to observe the principles of the Code of Conduct and these policies by declaring any gifts received or potential conflicts of interest in a timely manner, and upholding the Trust zero-tolerance to bribery. Information Governance - SWASFT recognises that its records and information must managed, handled and protected in accordance with the requirements of the Data Protection Act 1998 and other legislation, not only to serve its business needs, but also to support the provision of highest quality patient care and ensure individual’s rights in respect of their personal data are observed. SWASFT employees are expected to respect their contact with personal or sensitive information and protect it in line with Trust policy.

1. Introduction 1.1 South Western Ambulance NHS Trust (the Trust) is committed to ensuring that
every patient receives the highest standards of quality patient care. Within the pre-hospital environment, a wide range of medical devices, equipment and consumables are utilised by clinicians on a daily basis to support the clinical assessment process.
1.2 It has long been acknowledged that by standardising practices, patient safety and
patient outcomes are maximised. With the Trust operating a significant number of vehicles of different types, it is essential that all vehicles are equipped to one common standard.
1.3 In the case of time critical patients, the absence of one single item has the potential
to adversely affect the quality of care delivered. Conversely, the overstocking of vehicles negatively impacts on the clinical working environment, leads to unnecessarily wasteful use of Trust resources and can result in poor infection prevention control.
1.4 It is equally vital that all bags used by the Trust support staff welfare and enable
equipment to be carried in such a way as to minimise manual handling implications.
2. Scope 2.1 The scope of the policy is to specify the range and quantity of all medical devices
and consumables carried on all Trust vehicles and to clarify the minimum standard of equipment that must be taken to the patient.
2.2 The policy covers the following vehicle types:
Double Crewed Ambulances (DCA)
Rapid Response Vehicles (RRV)
Patient Support Vehicles (PSV)
Air Ambulances
Ambulance Responding Officer Cars
Urgent Care Service (UCS) Cars
HART resources
3. Duties and Responsibilities 3.1 Chief Executive 3.1.1 The Chief Executive holds ultimate responsibility for the provision of all medical
devices. 3.2 Director of Operations 3.2.1 The Director of Operations is responsible for ensuring that the policy is applied fully
across all areas of the Trust, and that monitoring procedures are in place to identify and rectify any vehicles who fail to meet the agreed specification.

3.3 Head of Operations 3.3.1 The Head of Operations are responsible for ensuring that the policy is applied fully
within their geographical area and that local monitoring procedures are in place to identify and rectify any vehicles who fail to meet the agreed specification. They are responsible for providing written reports as requested by the Executive Director of Delivery to provide assurance against the policy.
3.4 Deputy Clinical Director/Clinical Development Managers 3.4.1 The Medical Directorate are responsible for maintaining and authorising any
changes to the policy, to ensure that it continues to reflect the latest clinical evidence and to evaluate opportunities for further development.
3.5 Clinical Development manager (East) 3.5.1 The Clinical Development Manager (East), in conjunction with Procurement
department is responsible for the maintenance of equipment and consumable catalogues and for reviewing opportunities for maximising efficiencies within these areas.
3.6 Head of Logistics 3.6.1 The Head of Logistics is responsible for ensuring that core numbers of vehicles to
meet activity are available for the required shift cover and that all operational vehicles have the appropriate, maintained medical devices and equipment available to ensure compliance with this policy.
3.7 Operations Managers/UCS Field Managers 3.7.1 Operations Managers are responsible for ensuring that the policy is applied fully
within their geographical area and that local monitoring procedures are in place to identify and rectify any vehicles who fail to meet the agreed specification.
3.8 Operations Officers/Lead Paramedics 3.8.1 Operations Officers are responsible for ensuring that the policy is applied fully at
their station/s and that local monitoring procedures are in place to identify and rectify in conjunction with their OM any vehicles who fail to meet the agreed specification. OOs are responsible for ensuring that all staff are aware of and practicing according to this policy.
3.9 Clinicians 3.9.1 Clinicians (for the purpose of this policy, this includes all staff who engage in clinical
duties, ACA, ECA, Technician, Paramedic, Specialist Paramedic, Nurses, Doctors) are responsible for ensuring that all vehicles (and premises where appropriate) on which they work are thoroughly checked, and any deficiencies which prevent compliance with the agreed specification are reported to the appropriate line manager. Clinicians are responsible for ensuring that all vehicles carry the appropriate , maintained in date, charged and functioning medical devices and equipment in accordance with the equipment checklist and in particular confirm provision of critical equipment as detailed in Appendix 30.
3.9.2 Clinicians are responsible for reporting any equipment default or adverse incident
through Trust reporting mechanisms.

3.10 Medical Device Coordinators (MDC’s) 3.10.1 The MDCs are responsible for establishing and maintaining an up to date
database of all diagnostic and therapeutic equipment introduced to the Trust. 4.6.2 The MDCs are responsible for servicing and maintaining all diagnostic and
therapeutic equipment in line with manufacturer’s guidelines and instructions, through approved providers.
4.6.3 The MDCs will co-ordinate and report any fault in any item of diagnostic and
therapeutic equipment, both internally, and to the Medicines and Healthcare products Regulatory Agency (MHRA).
4. Governance 4.1 Vehicle, Uniform and Equipment Working Group 4.1.1 The Vehicle, Uniform and Equipment Working Group are accountable for the
approval of and revisions to the policy. 4.2 Clinical Effectiveness Group 4.2.1 The Clinical Effectiveness Group are responsible for endorsing the policy following
approval at the Vehicle, Uniform and Equipment Working Group. 4.3 Directors Group 4.3.1 The Directors Group are ultimately responsible for the provision of equipment and
for approving the required vehicle specification and equipment list.
5. Procedure 5.1 Staff are required to ensure that any vehicle (and premises where appropriate) on
which they work meets the agreed specification detailed within this policy. At the commencement of each shift, or at the earliest opportunity, staff must conduct a Vehicle Daily Inspection (VDI), which must include vehicle, medical device, medication and consumables. This inspection MUST be recorded in the vehicle log book and is a legal requirement. Appendix 30 illustrates the Vehicle Checklist.
5.2 Operational SOP OP001 provides detailed guidance on the process for
commencing a shift. It is accepted that resources may be tasked to an emergency call before a full VDI has been completed. Staff must therefore commence each shift on an emergency vehicle with an immediate critical equipment check, which includes the following:
Defibrillator
Oxygen (at least x1 CD cylinder)
Drug bag (Not required on a PSV check)
Response bag with at least the following present: bag-valve-mask adult and paediatric.
At least one of the following oxygen mask: high concentration adult/paediatric, medium flow adult, nebuliser adult/paediatric and a T-piece
Electric suction unit. (Not required on a PSV check)

5.3 If the vehicle does not have all applicable working items on the critical equipment
check, and the shortfall cannot be rectified immediately, the lead clinician on the vehicle must contact the clinical hub via Airwave or telephone. The vehicle will be booked unavailable whilst attempts are made to ensure the vehicle meets the minimum standard. If this is not possible within ten minutes the duty appropriate Operations Officer must be informed.
5.4 The critical check must be annotated in the vehicle log book. Following completion
of the critical check, the remaining equipment must be checked as part of the VDI. The vehicle log book must be signed to confirm that the VDI has been completed and no deficiencies have been identified.
5.5 This vehicle log book must be completed at the start of every shift and with every
change of driver. The vehicle log book must be annotated to record the date the shift was commenced, the start mileage and whether at the start of the shift there are any known defects.
5.6 Appendix 1 details the medical devices, equipment bags and consumables that
must be carried on each type of Trust vehicle. The subsequent tables detail the exact contents list of each individual bag or item, together with the required consumables.
5.7 If any discrepancies are identified during the VDI, the senior clinician on the vehicle
is responsible for trying to rectify the issue during the shift. Consumable are available from all Trust stations, and medical devices are available from the Medical Devices Exchange Store, which are available 24/7 on selected stations. If the deficiency affects the ability of the resource to respond and provide a safe level of care, then the Logistics desk must be informed via the Clinical Hub and an incident report submitted.
5.8 If it is not possible to rectify the issue by the end of the shift, it must be reported to
the appropriate Medical Device Coordinator (Logistics Desk OOH) and line manager at the earliest opportunity, entered into the vehicle log book and the station log book.
5.9 The Medical Device Coordinators, alongside the Operations Officers and/or the
appropriate line manager should then work to ensure that any deficiencies are rectified in a timely manner.
6. Minimum equipment to scene 6.1 Minimum Equipment to Scene
6.1.1 The Trust acknowledged some time ago that we needed to review and standardise
the provision of response and drug bags. 6.1.2 The bags that have been purchased have been thoroughly tested and piloted and
feedback incorporated into the final design. The bags have high levels of infection prevention and control within the material and also incorporate a thermo regulatory ability to maintain contents at an ambient temperature. All bags are very robust and

are designed with the ambulance service in mind, they are bespoke and are not “off the shelf”.
6.2. Design and Rationale 6.2.1 The theory and rationale behind the response bags has always been to reach a far
more practical common sense approach to the carriage of equipment to scene. The basis upon which these bags have been developed is such that we split the response bag into a green response bag, that contains sufficient equipment and consumables for the vast majority of jobs that we attend.
6.2.2 The new bag design incorporates a separate large red Therapy and Rescus Bag
and this bag has been created in a modular design so that individual components can be removed. It would be highly unusual for the whole red bag to be required at any incident. This again reduces the burden on manual handling and makes the requirement to carry the whole bag very unlikely.
6.2.3 The oxygen cylinder within the red bag can be accessed from the end without
having to open up the bag and again this can be removed as required. 6.3. Initial Equipment to Scene 6.3.1 The provision of the new bags creates an opportunity to review the carriage of
equipment by front line operational personnel and to reduce the impact of manual handling operations.
6.3.2 When responding to any RED 1 incident, or an incident where the details provided
make it clear that there is the potential for a cardiac arrest the following minimum list of equipment should be taken to the scene;
Diagnostic bag
Resuscitation Pouch from Resuscitation and Therapy Bag (There is no requirement to take the whole bag)
Defibrillator
Oxygen 6.3.2 When responding to any other incident a sensible pragmatic approach should be
taken. The carriage of the Diagnostic Bag is required at all incidents, but further items should be selected on an as required basis based on the balance of probabilities at the time. For example, when attending an asthmatic patient, the Nebuliser Pouch from the Resuscitation and Therapy Bag may be required, along with the oxygen cylinder. When attending a paediatric patient, the Paediatric Pouch from the Resuscitation and Therapy Bag may be required.
6.3.3 It is the lead clinicians responsibility to determine the likely items required
and to convey such items to the patients side. Other factors to consider include:
If the vehicle is parked outside the patient’s location, additional items can be retrieved without undue delay and difficulty.
Is there a significant distance to the patient from the vehicles location, in which case additional items may be taken to prevent repeated return to retrieve any additional items.
Are others present who can assist with any items to reduce any manual handling burden.

6.4. Manual Handling 6.4.1 The new Response Bag and Diagnostic Bag configuration is being provided in order
to mitigate some of the manual handling requirements of ambulance clinical practice. The emphasis is now being placed on the clinicians professional judgement as to what equipment is, on the balance of probabilities, required at each incident and for this dynamic risk assessment to be undertaken.
6.4.2 The vast number of ambulance activations do not require the carriage of additional
items outside of the new Diagnostic Bag and therefore it is hoped that the manual handling implications will be demonstrably reduced.
7. Medical Devices
7.1 Procurement
7.1.1 The standard equipment selection, procurement and funding process has been introduced to ensure that the Trust provides clinicians with the most clinically and cost effective consumables, equipment and medical devices available.
7.1.2 The process has been developed to ensure that there is a formal and consistent approach to the selection and procurement of ambulance equipment which is in accordance with legislation and best practice. The process will ensure the all aspects have been adequately considered, in order to maintain the safety of patients and staff, whilst promoting the achievement of best value for money purchasing decisions. The focus of the process is to provide the best possible care to patients.
7.1.3 The process applies to all single use items and medical devices proposed or used within the Trust, including those donated through charitable funds.
7.1.4 The group responsible for governing this process and agreeing the Medical Devices is the Vehicle, Equipment and Uniform Working Group (VEUWG)
7.1.5 In expressing its commitment to the safety of medical, diagnostic and therapeutic equipment this policy seeks to promote the safe and effective use of all devices and ensure that whenever a medical device is used, it is:
Suitable for its intended purpose
Properly understood by the user
Maintained in a safe, reliable and clean condition (see infection control policy for cleaning guidelines)
7.2 Training
7.2.1 The Trust provides information and guidance to staff on the use of medical devices, to ensure they are able to provide the best treatment and patient care at all times.
7.2.2 In the event of any incident involving medical, diagnostic devices which compromises patient or staff safety, then staff will complete an incident report form in accordance with the Trust’s incident reporting policy.

7.2.3 The Trust’s training departments are responsible for ensuring that all relevant medical device operating instructions are incorporated in training courses and assessment instruments as training is a key element in device safety.
7.3 Medical device failures – MHRA 7.3.1 The Trust’s MDCs will report to the MHRA all adverse incidents involving medical
devices, ensuring that the device and all associated leads/connections are quarantined, and a detailed report has been obtained from the staff member reporting the incident. The device and report will be held by the MDC until either the MHRA or the manufacturer request the items in order to perform a detailed investigation into the device fault/failure.
7.3.2 Once the investigation has been completed by the MHRA or manufacturer, the MDC will obtain a full report of the investigation and provide this to the Trust’s serious untoward incident panel (if appropriate) for consideration when completing the Trust’s investigation.
7.4 Contaminated, Maintenance and Repair
7.4.1 All medical devices held by the Trust will be maintained/serviced and repaired in accordance with manufacturers’ guidelines and coordinated by the Trust’s MDC(s) for each division.
7.4.2 The Defect Reporting System as illustrated in Appendix 31 outlines the system for operational staff to report and record a defective piece of equipment.
7.4.3 The Trust’s Fleet department carries out the service and maintenance of all stretcher trolleys and carry chairs in house. Each service technician must undertake an accredited Manufacturer course before being able to work on these items of equipment.
7.4.4 All Trust equipment will be labelled with an identifying bar code label and once serviced by external engineers the equipment/device will be labelled with the company’s inspection label containing contact details, date of last service and date of next service.
7.4.5 All contaminated Medical Devices will be recorded and maintained in line with the Trust Decontamination process as illustrated in Appendix 24
7.5 Monitoring of Medical Devices
7.5.1 The Trust’s VEUWG will receive an annual report from the Medical device Team which will demonstrate:-
Whether all those with responsibilities for diagnostic and therapeutic equipment are fulfilling their duties under this policy;
Implementation of the systematic inventory of all reusable medical devices also giving assurance that equipment is properly maintained and used within the Trust;
The process for ensuring that all medical devices and equipment are properly maintained and repaired has been implemented;
Details of the reporting of faults/failures in equipment which have been reported internally, and to the MHRA;

The process for ensuring that all medical devices have been calibrated within Trust and manufacturer timescales has been implemented in accordance with the procedure for delivery and acceptance of equipment;
The process for ensuring that all medical devices have been serviced within specified time frames has been implemented;
Implementation of an inventory (or links to an inventory) of diagnostic equipment used within the organistion, and the maintenance of an updated register of authorized users;
The process for determining the training required to use the equipment identified in the inventory has been implemented (ie training needs analysis); and
The process for ensuring that the identified training needs of all permanent staff is being met and implemented;
8. Monitoring 8.1 Operations Managers are responsible for providing written assurance to their Head
of Operations each quarter that all vehicles within their areas of responsibility continue to comply fully with this policy.
8.2 The Head of Operations are responsible for collating the written assurance provided
by their Operations Managers each quarter. An assurance report will be presented to the A&E Service Line Group on a quarterly basis.
8.3 The Vehicle, Equipment and Uniform Working Group (VEUWG) will monitor
compliance with the policy through routine monitoring of the incident reporting system.
8.4 Temporary changes to the policy due to short term supplier shortages or the issue
of new items may be authorised by the Deputy Clinical Director, as Chair of the VEUWG, as detailed within the Terms of Reference of the group. Any changes to the policy must be ratified at the next VEUWG meeting.
8.5 The policy will be reviewed bi-annually by the Vehicle, Equipment and Uniform
Working Group (VEUWG).
