Abstract
Childhood obesity is a growing issue within the United States. Caregivers can prevent children
from becoming overweight by adapting health eating habits, daily exercise, and developing skills
to making good choices. The Start Healthy Fitness and Nutrition Program is a strategic planned
project to assist families to become aware of their health and become active in a healthy lifestyle.

Start Healthy
Start Healthy is a fitness and nutrition program that is designed to assist children who are
overweight to become healthy through becoming active and eating right. Children learn best by
example. Caregivers are important in supporting their children to be healthy and active. This
program also teaches caregivers how to become actively involved in making healthy food
choices and fitness activities.
Environmental Trends
Children who become overweight can face intermediate and long term life threatening
conditions. According to the Food Research and Action Center (FRAC), report “about a quarter
of 2-5 year olds and one-third of school age children (including adolescents) are overweight or
obese in the United States” ( FRAC, 2014). Kids Health reports that the percentage includes one
out of three kids are overweight or obese (2014, par 1.). Children are becoming less active as
they spend more time in front of the television watching movies and playing with electronics.
This seems like a harmless activity however it comes with serious side effects. The Centers for
Disease Control and Prevention (CDC) reports that children and youth obesity may lead to the
following health problems: heart disease caused by high cholesterol and/or high blood pressure,
type-two diabetes, asthma, sleep apnea, and social discrimination (CDC, 2014). The listed health
conditions can be prevented through healthy life styles that include being physically active,
healthy decision making, and eating balanced meals.
Similar Services
We are aware of the availability of other information such as nutritional and physical
activity information that provided by the Center for Disease Control (2014). Additionally, area
hospitals provide services such as Beaumont's Healthy Kids Program, and Henry Ford’s “Let’s

get healthy!” (Gibson, 2012). At the national level, the “Let’s Move” program introduced in
2010 by First Lady Michelle Obama is “dedicated to solving the challenge of childhood obesity
within a generation, so that children born today will grow up healthier and be able to pursue their
dreams” (Let’s Move, n.d.). We wish to acknowledge the value of information provided by
these sources while encouraging the benefit of the one-on-one service provided by our
customized Start Healthy program, designed to support the development life-long healthy
choices.
Mission Statement
Start Healthy is committed to assessing, monitoring, and providing community resources
that encourage healthy lifestyles for children and the caregivers that support them. Licensed
staff, such as nutritionists, physicians, and registered nurses, will provide a holistic approach to
meet the needs of patients and caregivers in their journey to a healthier lifestyle.
Philosophy
The philosophy of the Start Healthy Program is to empower children to achieve healthy
lifestyles. This program takes a holistic approach that incorporates the cultural and spiritual
needs of the child and caregivers. The program aims to maintain a safe environment for children
to become excited about fitness and make healthy choices. We provide personalized assessments
to assist our clients in developing achievable individual goals. We strongly encourage caregivers
to become active in the child’s learning process. We provide activities that include fitness,
nutrition, cooking, and skill building. We are committed to providing tools and education
through the Start Healthy program that can be used throughout our client’s lifetime.

Program Goals and Objectives
The goal of Start Healthy fitness and nutrition program is to provide children and
caregivers the education and community resources to live healthy lifestyles. As previously
discussed, childhood and adolescent obesity is a growing problem within the United States. This
program plans to address healthy lifestyle education, nutrition and physical activity with the goal
of providing a healthier future for individual families. Yoder-Wise (2014) defines “goal-setting
[as] the process of developing and formalizing … targets or objectives” (p. 315). The following
list includes the goals and objectives of the Start Healthy program regarding education, activity,
and nutrition:
Education and Resources
Strategic Goal 1: Provide participants with education to identify appropriate resources
and independently conduct research to find needed information.
Objectives:
Within one month of being enrolled in the Start Healthy program, the child and
caregiver will identify online resources for recommended healthy food choices
and activity levels.
Within one month of being enrolled in the Start Healthy program, the child and
caregiver will identify local resources that include nearby sources of healthy food,
including food banks, food distribution sites, and local groceries.
Within two months of being enrolled in the Start Healthy program, the child and
caregiver will identify local resources that include state and local parks,
recreational facilities, and playgrounds that can be utilized to increase activity.

Diet
Strategic Goal 2: Provide participants with education and resources regarding healthy
food choices.
Objectives:
Within two months of being enrolled in the Start Healthy program, the child and
caregiver will demonstrate understanding of the FDA food pyramid by providing
examples of healthy proportions according to food type, as well as appropriate
serving sizes.
Within three months of being enrolled in the Start Healthy program, the child and
caregiver will give examples of healthy snacks and how they have incorporated
these foods into their lifestyle as indicated by food diaries.
Within six months of being enrolled in the Start Healthy program, the child and
caregiver will demonstrate cooking skills that include limiting sugars, reducing
unnecessary fats, and substituting healthy alternative food choices where
appropriate.
Activity
Strategic Goal 3: Provide participants with education and resources regarding the
importance of exercise, as well as providing tools and resources that can be used to set and reach
personal activity goals.
Objectives:
Within one month of being enrolled in the Start Healthy program, the child and
caregiver will be able to provide examples of the benefits of exercise and regular
physical activity.

Within three months of being enrolled in the Start Healthy program, the child and
caregiver will demonstrate participation in regular physical activity as indicated
by daily logs.
Within five months of being enrolled in the Start Healthy program, the child and
caregiver will identify measurable progress by tracking their success through
weight, blood pressure, and body mass index, as compared to the same identifiers
when beginning the program.
Program Structure
Start Healthy Fitness and Nutrition Program will be overseen by a Board of Directors that
includes a Director, a Community Liaison, a Marketing Director, an Education Director, and a
Financial Director. These positions will be supplemented with additional clinical staff as needed
to maintain the program objectives of the organization. A “service-line [organizational]
structure” [will be used initially, with the expectation of incorporating a] matrix structure”
(Yoder-Wise, 2011) to accommodate future program growth. Job descriptions will be used to
identify leadership relationships, roles, and responsibilities within the Start Healthy Fitness and
Nutrition Program. These job descriptions will be used to identify the “chain of command [and]
span of control” (Yoder-Wise, 2011) for the individuals performing that role. The information in
this presentation does not specifically address, but has the expectation, for clinical and other staff
to be included with the future growth of the program.
Organizational Chart
An organizational graph or chart represents work units and reporting relationships, relates
to the division and specialization of labor functions attended by specialists” (Yoder-Wise, 2011,
p. 145). This chart is a useful tool as it provides a visual aid of chain in command within an

organization. The chart listed below (Table 1) provides an outline of the leadership roles within
the organization that are required in order to function successfully.

Job Descriptions
Job descriptions are important tools for employers and the primary organization. Job
descriptions will be used to provide clear, concise and accurate role definitions to applicants in
order to prevent “role ambiguity [or] role conflict” (Yoder-Wise, 2011). They will also outline
expectations for education, experience and physical requirements. These job descriptions
include duties, responsibilities, and “competency-based requirements” (Yoder-Wise, 2011)
linked to the position. The following are job descriptions maintained within the Start Healthy
Fitness and Nutrition Program. All employees of the Start Healthy program are required to
successfully pass pre-employment drug testing, a background investigation, and to maintain a
valid driver’s license throughout their employment (LRBOI, 2014).

Director
Job description for the Director of Start Healthy: the Director of the Start Healthy Fitness
and Nutrition Program will provide leadership and direction for the childhood obesity prevention
program within the guidelines of the organizational objectives.
Job Responsibilities:
Provide leadership, oversight, and direction for the Start Healthy Fitness and
Nutrition Program.
Oversee planning, implementation and evaluation of Start Healthy services.
Develop relationships to promote the Start Healthy Fitness and Nutrition
Program.
Monitor day-to-day delivery of program and services to improve or maintain
quality.
Develop staff competencies, program standards and protocols to ensure quality
service delivery.
Coach and mentor staff as appropriate to improve performance and meet program

Job description for the Community Liaison of Start Healthy: the Community Liaison of
the Start Healthy Fitness and Nutrition Program will provide for professional relationships with
individuals, organizations, businesses, and schools within the Start Healthy Fitness and Nutrition
Program service area as specified in the guidelines of the organization (Career builder, 2014).
Job Responsibilities:
Develop relationships with participants, individuals, and families within the Start
Healthy service area.
Develop relationships with organizations, businesses, community leaders, and
school officials within the Start Healthy service area.
Provide information about the Start Healthy Fitness and Nutrition Program to
legislators, organizational and community leaders, and other interested parties.
Provide participants with transportation options and assist with program
procedures.
Proved case management expertise to qualified adolescents and family members.
Requirements for this position:
Bachelor’s degree with emphasis on health care.
Two years in a teamwork/communication role.
Excellent communication skills, including use of technology.

Job description for the Marketing Director of Start Healthy: the Marketing Director of the
Start Healthy Fitness and Nutrition Program will maintain marketing, communications, and
public relations for the Start Healthy Fitness and Nutrition Program within the guidelines of the
organizational objectives (Joyaux & Ahern, n.d).
Job Responsibilities:
Develop, maintain, and measure the effectiveness of program marketing
objectives.
Coordinate organizational advertising, including print and electronic newsletters,
brochures, and memos.
Develop short and long term goals related to marketing, communication, and
public relationships.
Requirements for this position:
Bachelor’s degree with emphasis on marketing.
Two years of marketing/professional communication experience.

Health Educator
Job Description for the Educator of Start Healthy: the Educator of the Start Healthy
Fitness and Nutrition Program will develop educational materials for participants and interested

Start Healthy Fitness and Nutrition Program, using organizational objectives (My plan, 2014).
Job Responsibilities:
Assess individual, family, and community needs.
Promote healthy lifestyles, provide workshops, and promote increased awareness
to improve health.
Develop and implement health education programs.
Identify community resources and make necessary referrals for participants.
Conduct and present research to community members.
Oversight of staff clinical skills and documentation.
Requirements for this position:
Bachelor’s degree in health education or health promotion.
Two years of education experience.
Knowledge of community health needs in the Start Healthy service area.

Financial Director
Job description for the Financial Director of Start Healthy: the Financial Director of Start
Healthy Fitness and Nutrition Program will manage program funds, with a focus on maximizing
financial returns within the guidelines of organizational objectives. The finance manager will
insure adequate control of financial gains while adhering to state and government regulations
(Accounting tools, 2014).

Work with program director and staff members to prepare a comprehensive
budget.
Research funding sources, oversee the development of fund raising and grant
writing.
Provide accurate accounting and assure funds are used appropriately.
Maintain organization compliance with required taxes, insurance, and payments.

Maintaining organizational budget requirements.
Developing long-term and short-term financial goals.
Work with staff and Board members to secure adequate funding for operation.

Clinical RN (future position)
Job description for clinical Registered Nurse of Start Healthy: the clinical Registered
Nurse of Start Healthy Fitness and Nutrition Program will provide a full range of duties specific
to the Start Healthy program. The RN will assess, monitor, and implement appropriate measures
related to specific conditions and needs of the participant, and provide education to participants,
families, and interested parties.

Implement appropriate care plan related to assessment, and evaluation of
participants. Monitor, record and provide communication regarding condition of
participants as appropriate.
Oversee care of participants while attending program(s). Delegate tasks to

health, comfort and safety of participants.
Identify expected outcomes for individual participants using Nursing Outcomes
Classification (NOC) standards.
Participate in community/school related councils, associations, task force and/or
health service focus groups related to childhood obesity and appropriate
nutritional needs.

Advertising
One aspect of marketing includes advertising that should be designed to attract the
attention of staff and volunteers who enhance the mission and values of the organization (Yoder-
Wise, 2011). Yoder-Wise (2011) makes it clear that marketing should be “strategic[ally]
plan[ned … and designed] to meet the needs of the population being served [and to assure]
customer satisfaction (p. 319; p. 318). The following information has been collected to assist the
board of directors of the Start Healthy Fitness and Nutrition Program in determining an
advertising budget.
Help wanted advertisements for open positions will be posted to MLive. This website
provides digital marketing linked to news, weather, and employment opportunities (MLive,
2014). The price for online advertising is listed as $195 for 30 days or one fourteen (14) day
online job posting for $120 (MLive, 2014). See appendix A for an example of a Start Health
advertisement.
The American Nursing Association (ANA) advances the nursing profession by fostering
high standards of nursing practice, promoting the rights of nurses in the workplace, projecting a
positive and realistic view of nursing…(ANA, 2014). This organization provides nurses peer-
reviewed articles and research on broad topics for nurses to review. The price of $2905 includes
a 30-day, 1/5 of a page advertisement (ANA, 2014).
The Ludington Daily newspaper is a local newspaper for the Mason County, Michigan
area. This paper is distributed Monday through Saturday to paid subscribers. The price for
advertising a position is $195.00 for thirty days (Extra, 2014).
Budget Proposal

Budgets are critical to the success of organizations and businesses. Yoder-Wise (2011)
indicates that there are several types of budgets to consider, that include an “operating budget
[for] day to day [operations, as well as] capital expenditure [and] cash budget[s]” (p. 243-244).
Bradford (n.d.) “Operating budgets are focused on facilitating income. The first and perhaps
most crucial component of the operating budget is the sales and collections budget. This is
followed by the projected cost of goods sold budget, the inventory and purchasing budget, and
the budget for operating expenses” (para 2). The Start Healthy Fitness and Nutrition Program
projects the following estimates as an initial budget, with the expectation of changing with the
needs of the growth of the organization. The total yearly budget of Start health program is
estimated to be $307,500.00. See Appendix B for the program’s itemized expense.
Human Resources
The Start Healthy program will follow an operating budget to manage daily functions
intended to meet program objectives. Operating budgets are financial plans for the day to day
activities of an organization (Yoder-Wise, 2013) or a combination of known expenses, expected
future costs, and forecasted income over the course of a year (Bradford, n.d.).The program will
include six key positions: community liaison, program director, marketing director, education
director, financial director, and clinical Registered nurse. Supportive staff will be hired based on
grants obtained for funding the program. Employee’s salaries will be budgeted and based on
competitive pay scales. Future positions in the program will depend on the growth of the
program and any positions needed will be budgeted accordingly and advertised to meet the needs
of the program.

The Start Healthy and Nutrition Program will be open five days a week, Monday through
Friday, hours of operation will be from eight o’clock in the morning until five o’clock in the
evening. The clinic will be closed for recognized state and Federal holidays.

Volunteers NeededStart Healthy is a fitness and nutrition program designed to assist children and their families to develop healthy lifelong diet and activity habits. We are looking for volunteers who want to make a difference for our children. You do not need to be a fitness trainer or nutritionist to be an important part of our program.
