QFFICE ORDER Consequent to the approval accorded by the Board of Management (BoM)of the Institute vide Agenda Item No. 40.18 in its 40th meeting held on 19.03.2021, the following Hospitals/Diagnostic Scan/Imaging Centres are hereby empanelled with immediate effect initially for a period of two years, extendable for another 02 years on satisfactory performance to provide medical facilities to the regular employees and their families in terms of the Memorandum of Understanding (MoU}) to be signed between the Institute and the Hospitals/Diagnostic Scan/Imaging Centres & to be read in conjunction with Central Service (Medical Attendance) Rules as applicable for SLIET

The above empanelment is subject to the following terms and conditions:-

Referral from Medical Officer of SLIET Health Centre would be mandatory for a) taking Scan/Imaging Centre restricting the reimbursement as per CGHS rates.

b) In the event of joining/relieving of any doctor(s) from the list of doctors submitted at the time of approval of empanelled hospital, as the concerned hospital will inform SLIET administration about the same, along with new doctor's full particulars and specialization for up-dation of record. The ambulance services will not be provided whenever any patient is referred for treatment to Empanelled Doctor/Hospital/Diagnostic Scan/Imaging Centre by Medical Officer of Health Centre except in case of emergency.

No Travelling Allowance will be provided to the patient for treatment from

Empanelled Doctor/Hospital/Diagnostic Scan/Imaging Centre.

All the employees of the Institute will be allowed to avail the facilities of above

Hospitals/Diagnostic Scan/Imaging Centres as per above mentioned terms and conditions.

This bears the approval of the competent authority dated 11.05.2021.
