TThe battle over the bulgeis never easily won.There is always the latest dietand/or exercise craze to help youlose those unwanted pounds.Sometimes, though, no matter howhard you fight it there are those fewinches that never seem to want to“come off” even with diet andexercise. Often it is a person’sgenetic makeup that prevents themfrom shedding those unwantedsmall bulges.

Liposuction is an option to removesmall bulges that won’t improve withdiet or exercise. Body sculpting can beaccomplished with very little downtime or expense. Liposuction,however, does not remove cellulite,only fat.

The best candidates for liposuctionare those individuals who are onlymildly above their ideal body weight,have firm, elastic skin, are in goodhealth and have concentrated pocketsof fat that are resistant to diet andexercise.

The methods utilized to performliposuction have changed drasticallysince its development in the 1970s.Traditional liposuction is performedunder general anesthesia andrequires a surgeon to insert a largehollow tube called a cannula into theareas you would like the fat removed.The surgeon then uses the cannula tobreak up the fat cells in each area byusing a firm back and forth motion.Finally, the broken up cells aresuctioned out. The technique andinstruments of traditional liposuctionare more aggressive, leave largerscars, require longer recovery periodsand are not ideal for small pockets offat removal.

More recently tumescentliposuction has been performed.Using this technique, the surgeon willinject a solution composed of localanesthesia into the fatty areas to beremoved. The solution not onlyincludes a local anesthetic butepinephrine, a drug that contractsyour blood vessels. The solution aidsthe surgeon in removing the fat aswell as decreasing the amount ofbruising associated with traditionalliposuction. The ease in which the fat

can be removed isessential to makingliposuction a safeand minimallypainful procedure.

In order toimprove the ease offat removal,ultrasound-assistedliposuction wascreated. Ultrasound-assisted liposuctionis a techniqueutilized to helpliquefy the fat to make its removalless traumatic and easier for thesurgeon. More recently, techniquesinvolving laser-assisted liposuctionhave gained popularity. The theorybehind their use is more consistentwith ultrasound guided liposuction.Instead of using ultrasound to liquefythe fat, however, laser wavelengthsare used. The most advanced laser-assisted liposuction machinesselectively melt the fat and tighten thetissue for optimal results.

The down-time with theseprocedures is much shorter, with lesspain, less bruising, and overallsmoother more natural results.Patients that have had traditionalliposuction under general anesthesiaare more prone to complications, morepain, greater bruising, and contourdeformities. There has never been todate a death related to tumescent,local liposuction. All complicationshave been minor, unlike traditionalliposuction, which has been linked todeath under anesthesia, as well assignificant post-operativecomplications including pain.

The laser-assisted liposuctionwhich can be performed completelyunder local anesthesia offers such agreater advantage for patients it issure to be the wave of the future. Theprocedure is performed by numbingsmall areas of the skin, injecting localanesthesia, and filling the fatty tissuewith the tumescent numbing solution.Once the tissue is numb the laserprobe is inserted directly into thefatty tissue. The laser selectively heatsup and melts the fat. Oncesatisfactory melting has beenaccomplished, suction is used toaspirate the fat. The patient remains

awake during the entire procedurewhich adds a significant level ofsafety. If a patient is extremelyanxious a mild sedative may be given.

In addition to the fat removal, thelaser is able to deliver additionalwavelengths to tighten the tissue.Tissue tightening is somethingtraditional liposuction has historicallybeen unable to accomplish. The fullresults of the fat removal and tissuetightening are seen 3-6 months afterthe procedure.

Liposuction is currently the mostpopular type of cosmetic procedure.Making the technique safe andaffordable for patients is the optimalgoal. I believe the laser-assistedliposuction techniques will be seenmore and more as people get beautifulresults and have fewer complications.

In its “Behavioral Risk in theRetirement Red Zone” report,Prudential Financial, Inc. sites fivedominant emotions and tendenciesthat can influence investmentdecisions: fear, regret, inertia,aggressiveness, and susceptibility.The study also found:• Three of four investors — 76percent — are affected by theiremotions to a moderate or highdegree.

TThere’s no doubt about it,the eligibility rules forMedicaid are complicated,especially for the middle-class. Theescalating cost of long-term care hasforced many Long Islanders to turn toMedicaid as the only viable means topay for Nursing Home care and HomeHealth aides. Along with theincreased reliance on Medicaid,several pervasive myths have added tothe confusion.

Myth #1. Medicare will pay fornursing home costs. Medicare coversSeniors 65 and over, and provideshealth insurance covering doctors’visits, hospital stays, and someprescription medicines. It providesminimal long term care. After ahospital stay, Medicare will pay for upto 20 days in a Nursing Home forrehabilitation. It will also paypartially for up to 80 more days,provided the patient continues tomake progress in rehabilitation. Oncethose 100 days are over, Medicare willnot pay for nursing home costs. Theonly way to pay for it is private funds,

long term care insurance, andMedicaid.

Myth #2. Only poor people qualifyfor Medicaid. The law limits aMedicaid applicant’s assets to $13,800of "countable assets," but middle classSeniors should not be discouraged.There are many exceptions thatpermit applicants to restructure theirassets in order to qualify for Medicaidbenefits. For example, under certaincircumstances, an applicant cantransfer a house to an adult child orto a sibling. Countable assets may beconverted into non-countable assetsunder some circumstances. A marriedapplicant can also transfer certainassets to a spouse. Even afteradmittance to a Nursing Home,thousands of dollars can be preservedfor your family if you consult anexperienced Elder Law attorney toprepare the Medicaid Application.

Myth #3. Giving $13,000 a yearto the younger generation is aneffective means to qualify forMedicaid. This myth sprang from amisapplication of estate planningadvice. Medicaid, however, was

created for thepoor, not forpeople with estateplanning issues.With limitedexceptions, aMedicaid applicantcannot give awaymoney and expectthe government topay for theirNursing Homecare.

Myth #4.There is a five-year look-backperiod for allMedicaid. Thereare severalmisconceptions inplay here. In orderto dispel the myth,we mustdistinguishbetween two typesof Medicaid: (1)Nursing Homecare and (2)Community, orHome Care. Ingeneral, NursingHome Medicaidhas a look-backperiod, which is a

thorough audit of all your financialaccounts. An application for NursingHome Medicaid must be accompaniedby complete financial statements forevery account open during theapplicable look-back period. Everylarge cash expenditure should besupported by receipts. If you apply forMedicaid in 2009, (and you don’t havea trust), your look-back period will bebetween three years and four years,depending on the date of yourapplication. The look-back period islengthening and it will reach five-years in 2011.

Currently, Community Medicaid,which includes Adult Day Care as wellas Home Care, does not have a look-back period. Someone in need of thiskind of care can transfer assets andqualify for Medicaid immediately.

Myth #5. I’ll Never Need LongTem Care. Experts predict that 65% ofpeople over age 75, and 97% of peopleover 85, will need long term care.Smart people, concerned about theirhome and their investments, takeaction to protect their hard-earnedassets. Procrastination can cost youmoney. A Nursing Home stay willrender most people penniless withinone year. Many people can benefitfrom an Asset Protection Trust, but itmust be created and funded at leavefive years before a Nursing Homebecomes unavoidable. Are you willingto bet your home and all your assetson the hope of staying healthy formore than five years?

Over time, your familysituation may have changed,through death, divorce, remarriageor the birth of new children, so youshould periodically review all yourbeneficiary designations.� Examine your insurancecoverage. When you have a youngfamily, you need a certain amountof life insurance coverage to providefor some major expenses - such asyour mortgage, college for yourkids, perhaps some retirementfunds for your spouse. But whenyour children have grown, yourmortgage is paid and your spousehas decades' worth of retirementsavings, your insurance needs maychange considerably. At the sametime, you may find other uses forinsurance. Take some time andreview your insurance coveragewith your financial advisor.

Osteoporosis is one health issuethat a person can take control ofthrough diet and exercise. If thedisease is too far advanced, thenmedical attention may have to be theprimary approach – hence, theimportance of getting a bone densityscan sooner rather than later. As forosteopenia or early osteoporosis, awell-structured resistance exercise

program, coupled with an increase indietary calcium, can work wonders.

Studies show that if aerobicexercise is your only source ofexercise, it can be detrimental to bonedensity. Low level activity increasescortisol levels, which breaks down thebody. Higher intensity activityincreases the hormone levels that buildup the body.

Increasing the body’s calciumuptake is somewhat complex. It takesproper exercise, plus a number ofvitamins, minerals, and hormones –especially calcitonin, to generate thisprocess. Hormones are the key factorsin making things happen. Vitaminsand minerals are the building blocksfor the process. If the exercise that isused is stimulating enough to thebone, then the brain is notified bysensors surrounding the bone tosecrete the hormones and locate thematerials to start increasing bonestrength. Low intensity exerciseprovides too little tension on thetendon attachment on the bone. Highintensity exercise – resistance or

weighttraining –generatesadequatetension on thetendonattachmenton the bone tostimulate thesensors torelease thenecessary hormones for bone building.

Increased resistance exercise anddiet are an excellent prescription forreversing bone loss, but it must be ajoint effort between your medicaldoctor and your exercise instructor.Research is continually concludingthat gym therapy, not drug therapyalone, is a very valuable approach tothis bone loss disease..

TTo Implant... or not to ImplantTwo prospective patientscame into the officerecently with the sameemergency situation.Both were edentulous (noteeth) and were wearingold, ill-fitting dentures.Their dentures were loose;they had trouble eatingfoods they would haveliked; their faces had thatsunken-in look and theywere in constant pain.

Their previous dentistsrecommended implants tostabilize the dentures. Theproblem was that as theywere both retired and onfixed incomes they couldnot afford implants. Whatto do? Yes, implants are awonderful service andhave revolutionized whatwe can do to improvelives. I love implants! Butdue to the laboratory andmaterials costs involved,they are expensive.

My approach isdifferent from most otherdentists. I make new,custom-fitted dentures forthe patient and tell themthat if the new denturedoesn’t fit and functionwell, we can then addimplants. My experienceis that the denture patientdoes very well with thenew dentures and rarelydecides to go ahead withthe implants. Implants arealways an option but theydon’t have to be the firstoption. I believe thatpatients like these can bewell served with well-fitting dentures.
