As she worked with him over the year, she taught him Diabetes Self‐ Management, the impact of diet, exercise, medicaĕons, medicaĕon adjustment, stress, and explored the possibility of Depression. Gradually, with in‐reach into the hospital seħng the hospital team began to realize it was more than paĕent non‐ adherence to the treatment plan, but a person with severe complicaĕons to chronic glucose toxicity. Paĕent was always discharged home to self‐care, and

Case Managers reach across theconĕnuum of care striving to improve

care and outcomes one "starfish" at a ĕme. I have a story to share ofcase managers reaching across the conĕnuum of care in ambulatoryand inpaĕent care seħngs.

This story is about a 24yo male diagnosed with Type 1 Diabetes as ateen. In the past year, he has had 28 hospitalizaĕons for complicaĕonsof Diabetes. Last year's medical record notes usually started with "non‐compliant 23yo male with type 1 Diabetes...." This paĕent was referred to Case Management last year and the RNCare Manager discovered, when assessing his knowledge about hisdisease that he knew very liĥle. He had been taught to administerinsulin and test his blood sugar. He knew very liĥle else.

As she worked with him over the year, she taught him Diabetes Self‐Management, the impact of diet, exercise, medicaĕons, medicaĕonadjustment, stress, and explored the possibility of Depression. Gradually, with in‐reach into the hospital seħng the hospital teambegan to realize it was more than paĕent non‐ adherence to thetreatment plan, but a person with severe complicaĕons to chronicglucose toxicity. would return with DKA, and an infecĕon, and more complicaĕonswould be diagnosed.

In 2016, the hospitalizaĕons have conĕnued, and been longer. Newdischarge opĕons were sought ‐ Adult Family Home, Increased CareGiver hours in the home through Copes ‐authorized by Home andCommunity Services, and sĕll the hospitalizaĕons conĕnued. The careteam discussed a referral to hospice, for if he was older, with thecomplicaĕons he has, it would be appropriate.

The case manager, who has visited him periodically in the hospital said,he sĕll had goals and hope for the future, and didn't think he was readyfor hospice, the inpaĕent and ambulatory MDs agreed.

Currently, looking forward to the next discharge, there has beencollaboraĕon with the inpaĕent transiĕon planner and clinic casemanager who have communicated through EPIC messages. AnInterdisciplinary care team meeĕng was set up to plan for support inthe home aer discharge and aĥended by: Hospitalist MD, TransiĕonPlanner SW, Clinic Case Manager, and Home and Community ServicesCase Worker and Supervisor, paĕent and mother.

In the conference, the paĕent cried, he said he didn't know what elseto do stay out of the hospital. The team assured him they were theretogether to figure it out, they weren't there to scold him.

Plan:

Obtain new GI consult for a gastric pacemaker to address hisgastro paresis, thought is he has gastric "dumping" syndrome,and this contributes to his large swings in blood glucose.Obtain authorizaĕon for Insulin pump aer he completes 4weeks of daily glucose monitoringHE will meet his HH RN in hospital who will observe his urinaryself‐cath technique in the home, to promote sterile techniqueand decrease his UTIs.Allow him to interview his COPES worker in the hospital, so theycan form a workable team and the service is available to himwhen he discharges

Aer the care conference, the Case Manager went to his room to seehow he felt about the meeĕng. She commented on a beauĕfulhydrangea plant at his bedside. He told the story ‐ when he was a teenhe aĥended the Boys and Girls club. He had connected with anelementary aged girl who has having a hard ĕme in her life. He sharedthat he had to fight for his life every day and gave her encouragement.He asked why she wore so much make‐up, as she was very beauĕful.She needed to let her light shine from within.

It was this girl and her father who brought him the flower. She said shehad learned that he was in the hospital, and she wanted him to knowhe had made a difference in her life. He was so happy that he hadmade a difference in someone else's life. The Case Manager talkedwith him about how he could share this gi with others in the future,support groups, perhaps volunteering at the Boys and Girls club.

Terri's entire career has involved working with and helpingpeople. After working more than twenty years in clinicalsettings for cardiac catheterization laboratories and criticalcare situations, Terri has transferred her education,experience, and passion for others into the Senior careindustry. Terri lives by one of her favorite quotes from MayaAngelou: "I've learned that people will forget what you said,

people will forget what you did, but people will never forget how you made themfeel." With an exceptional ability to work among generations, Terri builds specialrelationships with Concierge Care Advisors' clients. She has a very athletic17 yr old son, Cory, and loves watching him compete because he wants to winat everything. She is currently a Board Member of the Bellevue Network on Aging;Foss Foundation Board, and serves as Vice President of WMCMA.

Not Getting your CEUs from WMCMA?WMCMA sends all WMCMA program evaluations and CEs online via email. Attendeesshould receive an email with the link to the program evaluation and CE instructionswithin a few days of the CE presentation. Attendees have one week to completethe evaluation. Once that one week deadline has passed, attendees who requesteda CE will receive the CCM certificate via email. If you attended our programs anddid not receive the evaluation email within a few days, contact us [email protected] or (425) 329­9456.

Please note that while MOST of our educational programs have CEsattached to them, some don't due to restrictions by the speaker/sponsor.This will be noted in the event information.
