INTRODUCTIONIndemnity-The recipients of, and attendees to, this safety seminar shall indemnify and save Sunbelt Rentals fromany and all claims, demands, actions and causes of action against Sunbelt Rentals whether groundless or not, inconnection with any and all injuries, losses, damages or liability of any kind whatsoever arising, directly or indirectly,out of the use, distribution or sales of products or services by or through the Recipient or its affiliates whether ornot the claims, demands, actions, or causes of action are alleged to have resulted in whole or in part from negligentacts or omissions of Sunbelt Rentals or from acts or omissions of such persons for which they or any of them wouldotherwise be strictly liable. OSHA and ANSI

OSHA stands for Occupational, Safety & Health Administration. OSHA writes federal regulations governing the safepractices of industry and construction. OSHA also polices these regulations with field inspectors who write citationsand levy fines.

ANSI stands for American National Standards Institute and is a voluntary organization made up of users, uniongroups, manufacturers, rental companies, government officials, and industry representatives. ANSI writes standardsfor many areas in our and other industries. We will be addressing the ANSI standards as they apply to aerial workplatforms. ANSI cannot fine you for not following their standards.

How do OSHA and ANSI interact?OSHA references ANSI in several codes governing our industry namely CFR 1910 for industry and CFR 1926for construction. OSHA references A92.2-1969 ANSI Standards for Aerial lifts under CFR 1926.453 and CFR1910.67. A92.2-1969 has since been rewritten and is now A92.2, A92.3, A92.5, A92.6-1992. Therefore OSHAcan fine an employer for violation of ANSI standards. Please be advised that OSHA can write a citationreferencing Section 5 (a) of the Occupational Safety and Health Act of 1970, commonly known as"the general duty clause" as well.

This clause states "Each employer shall furnish to each of his employeesemployment and a place of employment which are free from recognizedhazards that are causing or are likely to cause death or serious physical harmto his employees."

OWNERS, USERS, OPERATORS, QUALIFIED PERSONDEFINITIONS:

OWNERS - A person or entity who has possession of an aerial platform by virtue of proof of purchase.USERS - A person or persons or entity that utilizes or puts into operation an aerial platform.OPERATORS - A qualified person who controls the movement of the aerial platform.QUALIFIED PERSON - A person who by reason of knowledge, experience, or training is familiar with theoperation to be performed and the hazards involved.

DANGER, WARNING, CAUTIONFollowing are definitions of labeling you might encounter on aerial platforms:

DANGERDANGER - Immediate hazards which WILL result in severe

personal injury or death.

WARNINGWARNING - Hazards or unsafe practices which COULD result

in severe personal injury or death.

CAUTIONCAUTION - Hazards or unsafe practices which COULD result in

minor personal injury or product or property damage.

RESPONSIBiliTIES OF OWNERS, USER & OPERATORAnnual and frequent Inspections per ANSI

• Annual Inspections, are to be performed within 13 months of the last inspection.• Frequent Inspections, are to be performed every 3 months or 150 hours on the hour meter• Pre-delivery Inspections are required before an AWP is delivered to the end-user for purchase, rent, lease

or delivery. At Sunbelt Rentals this pre-delivery inspection is evidenced by the "Ready-to-Rent" tag attachedto the machine. If purchasino an AWP you should receive a copy of the Operator's Manual, Maintenance

Manual, all Maintenance Records and copies of all inspections• Aerial Work Platform mechanicsltechnicians must be trained by the manufacturer on the specific make and

model of AWP to perform these inspections and to perform any adjustments or repairs.

Record Retention per ANSI

The user shall retain the following records for at least four (4) years:• Records of the operator(s) trained on each model of an aerial platform.• Written records of the frequent and annual inspection shall be kept by the user when he performs the

inspection(s). The Records shall include the date of the inspection, any deficiencies found, the correctiveactions recommended and identification of the person(s) performing the inspection.

• Written records of all repairs accomplished on the aerial platform shall include the date of any such repair, a -.description of the work accomplished and identification of the person(s) performing the repair.

DELIVERYDelivery Procedures:Each machine is different. Do not assume that you understand the operating controls, prestart inspection procedures,manual location, etc., just because you operated a machine like this one last week. Our drivers are trained to showyou all the features of each machine, features which include:

• Where the operating manual and ANSI Manual of Responsibility are located.• How to start the machine• How to move the machine• How to steer the machine• How to activate the deck extensions• How to use the outriggers or extendable axles• Where the tie off points are (required on boom lift ONLY)• Guardrail gates/chains• Battery maintenance and charging• Grease points• Fuel fill location and type of fuel• Hydraulic oil fill location• Prestart inspection procedures• Oscillating axles and how they operate (boom lifts)• Pothole protection and how it operates (scissor lifts)• Any other features

DEliVERY• Oscillating axles and how they operate (boom lifts)

- The oscillating axle locks when the turret is rotated out of the '1raveI position'; designedto keep the machine from tipping when rotated.

-If you travel when the axle is locked, the high wheel will float in the air.- To unlock the axle and get all wheels on the ground, rotate the turret to the 'travel position:'- ''Travel position" means that the basket is positioned so the steering wheels are leading

the machine. See diagram below.- A function test of the oscillating axle should be performed every 4 months.- If you rotate the turret when on level ground, the axle will lock also.

RESPONSIBILITIES OF THE USER & OPERATORANSI A92 User & OperatorOPERATOR TRAINING:User- Whenever a user directs or authorizes an individual to operate an aerial platform, he shall ensure that theindividual has been trained in accordance with the manufacturer's operating and maintenance manual and the user'swork instructions and requirements listed in section 8 of this standard before operating the aerial platform.

User Model Training· The user shall be responsible for the operator being trained on the model of the aerial platformthat he will be operating. Such training shall be in an area free of obstructions, under the direction of a qualifiedperson for a time sufficient to determine that the trainee display proficiency in knowledge and actual operation of theaerial platform. Only properly trained and authorized personnel shall be permitted to operate the aerial platform.

Operator- The operator shall be trained on the same model of aerial platform or one having operating characteristicsconsistent with the one to be used during actual work site operation. Under the direction of a qualified person, traineeshall operate the aerial platform for a sufficient period of time to demonstrate proficiency and knowledge in the actualoperation of the aerial platform. Care shall be taken to ensure the area of such operation has a minimum of obstaclesand is conducive to training. Only properly trained and authorized personnel shall be permitted to operate the aerialplatform.

1. Position the basket within 12" of the elevated suriace and even with the suriace and the floor of the basket.2. Shut off the lift and push the emergency stop button.3. Snap your second snap hook on your double lanyard to the other elevated suriace anchorage point (5,000 lb.

anchorage point), while leaving yourself tied to the lift.4. Disconnect your other snap hook from the lift basketattachment point.5. Open the guardrail gate.6. Carefully step from the basket to the other elevated suriace.DO NOT JUMP!7. Close the guardrail gate.8. Always remain tied off or behind a guardrail when elevated. .

It would appear by reading the above OSHA regulation that OSHA would allow this practice. But that is not true if theforklift manufacturer does not allow the practice or doesn't allow lifting workers with their forklifts.

• Most rough terrain forklift manufacturers will not allow this practice:- Lull, JCB, Skytrak, Gradall, Square Shooter, Teledyne, Case, and Caterpillar do not allow the lifting of

personnel with their forklifts (unless using the manufacturer of the forklift's basket with their instructions).- Always consult the operating manual and warning labels on the forklift.

• There are some warehouse forklifts that will allow this practice- Consult the operating manual and warning labels on the forklift- ANSI requires a kill switch in the basket

These manuals are considered an integral part ofthe aerial platform and are vital to communicate the necessary safety information to users and operators.Operator - The operator shall be aware that the operating and safety manuals, including the manual which definesthe responsibilities of dealers, owners, lessors, lessees, users, and operators, are stored on the aerial platform andthe location where they are stored. The operator shall be familiar with the manuals stored on the aerial platform andconsult them when questions arise with respect to the aerial platform.

• Two manuals are required on all aerial work platforms1. The ANSI Manual of Responsibilities and2. The Operating Manual for that make and model lift.

Note: The operator and user must know where the manuals are located on the lift.

Power Pump and lower boom with function switches.35. Switch ground control switch to platform controls and pull red emergency stop button.36. Enter platform, close guardrail gate or attach guardrail chain, attach lanyard (boom lift) to anchorage

point, and make sure all personal are secured.37. DO NOT place foot in foot switch. Pull emergency stop button and start machine.38. Place foot in foot switch and make sure all functions work.39. Drive machine a short distance in both directions and make sure brakes stop machine when joystick is neutralized.

With the ground control switched to "Ground Controls" and the red emergency stopbutton pulled and with the help of another person to monitor the "Chassis Out of Level" indicator light on the platformcontrol console, manually activate the indicator light by compressing anyone of the three tilt sensor mounting springs.If the "Chassis Out of Level" light does not illuminate, shut down the machine and contact a qualified JLG technicianbefore operating.

• Both Red Emergency Stop buttons must be pulled in order for the machine to start and run from the basket.• DO NOT step on the enable foot switch when starting the machine (gas, propane or diesel).• Be sure key switch is switched to appropriate basket or ground controls.• Batteries are dead - Charge the batteries.• Out of fuel - If gas, refuel. If diesel, call the service department.

What to do if a machine will not move:• If electric machine, check both Red Emergency Stop buttons. They must be pulled in order for the machine

D Bumps and floor obstructions - look around, a change of elevation of only 3 inches on the ground can mean adifference of 3 to 5 feet 40 to 80 feet up

D Debris - pick up and clean up the area you are going to work

D Overhead obstructions and high voltage conductors - these machines are not insulated and will conductelectricity

D Hazardous locations, gaseous atmospheres - these lifts do not have explosion proof switches

o Inadequate surface and support to withstand all load forces imposed by the aerial platform in all operatingconfigurations - this includes checking the tire inflation. (tire inflation 45 to 95 psi, look for decal indicating pressure)

D Wind and weather conditions - if winds are greater-than 25 MPH do not operate an AWP

D Presence of unauthorized persons - keep the area below clear of people

horizontal unless the lift is secured to the truck/trailer. Rotating the lift or raising the boom above horizontal while on atruck/trailer can cause the truck/trailer to tip and may cause the lift tofall off the truck/trailer if the lift is not chained down to the trailer.

User & Operator - TRAVEL SPEED. Under all travel conditions, theoperator shall limit travel speed according to conditions of groundsurface, congestion, visibility, slope, location of personnel, and otherfactors causing hazards of collision or injury to personnel.

User & Operator - ELEVATED DRIVING REQUIREMENTS. Beforeand during driving while the platform is elevated, the operator shall:

a) Maintain a clear view of the path of travel.b) Maintain a safe distance from obstacles, debris, drop-offs, holes, depressions, ramps, and other hazards

- Take the key out, remove the control box (scissor lifts), chain and lock the machine to other machines.

Operator - MISUSE AS A JACK. The boom and platform of the aerial platform shall not be used to jack the wheels offthe ground unless the machine is designed for that purpose by the manufacturer.

- Some larger boom lifts may use the boom to elevate the axles off the ground to extend the axles. Be sureand follow the operator manual's instructions for this procedure.

- DO NOT use the boom to push the machine. This can do considerable damage to the boom sections.

Operator - DRIVING ON SLOPES. The aerial platform shall not be driven on grades, side slopes or ramps exceedingthose for which the aerial platform is rated by the manufacturer.

Operator - SNAGGED PLATFORM. If the platform or elevating assembly become caught, snagged, or otherwiseprevented from normal motion by adjacent structure or other obstacles such that control reversal does not free theplatform, all personnel shall be removed from the platform before attempts are made to free the platform using groundcontrols.

- If you find that the boom or platform or basket is caught, do not try to force the machine out of the situation.When the platform is caught and you operate the hydraulics, this can cause a very dangerous "loading ofthe hydraulics" situation. If the platform does suddenly get loose from the structure it is caught on, it couldcrush you against an object. STOP - Figure out what movements are needed to get the basket loosefrom the structure it is caught on, and then very carefully operate the controls to get the basket uncaught.NEVER let anyone operate the ground controls when you are in the basket. Do not operate controls tocause one or more wheels to leave the ground.

RESPONSIBILITIES OF THE USER & OPERATOREmergency ControlsManual - most AWPs are equipped with some type of manual platform descent controls. You must be familiar withthese controls on the model of lift you are operating.

READ THE OPERATOR'S MANUAL.

Some are manually activated by pulling on a handle located at the base of the lift, that slowly lowers the platform.Some have valves that need to be opened in a certain order to lower the boom by gravity to the ground. Each lift isdifferent. These Emergency Controls should only be used when there is total power loss.

Auxiliary Power - Boom lifts only - These controls are found on both control panels, the ground controls and basketcontrols. It is a simple toggle switch that activates an electric pump, powered by the battery, that will give you enoughpower to then activate the boom functions necessary to lower the platform to the ground. The Red Emergency Stopbuttons must be pulled and the enable foot switch must be depressed.
