Induction Into Porter Davis Safety Requirements

Porter Davis Inductions

It is the responsibility of every supplier to ensure that every employee and/or subcontractor is inducted into the contents of this Supplier Safety Pack prior to commencing work on Porter Davis sites.

Construction Induction Training

All Supplier employees/subcontractors must have completed the Construction Induction Course prior to working on Porter Davis Sites. Construction Induction Cards must be carried on site and be produced when requested by a Porter Davis Employee.

Supplier Performance

Key Performance Indicators (KPIs)

The KPIs for Porter Davis suppliers are based on cost, innovation, service, quality and safety. The safety performance of suppliers makes up 35% of the overall assessment. In the safety assessment, WorkSafe Notices, Incidents and Breaches of Porter Davis Safety requirements are taken into account. Poor safety performance will result in a review of the Supplier Agreement and the quantity of work may be reduced or the Supplier Agreement may be terminated. An OH&S Site Instruction will be issued for any of the following breaches:

¨ No Construction Industry Induction Card ¨ No SWMS for High Risk Construction Work

¨ No Porter Davis Induction ¨ Equipment used is not safe

¨ Scaffold altered to an unsafe condition ¨ Unsafe work methods

¨ Equipment used incorrectly ¨ Working at height in unsafe manner

¨ Electrical equipment not tested and tagged ¨ Alcohol or drugs consumed on site

¨ House keeping not to standard (still on site) ¨ Housekeeping not to standard (finished job and left site)

Non-Conformance Procedure

Where any supplier’s direct employee, contractor or subcontractor's employee does not comply with any site safety requirements they will be counselled by the Site Manager, OH&S Field Officer (or appropriate person delegated by Porter Davis Management) and verbally advised of their obligation to comply with site safety requirements. This will be recorded in the Site Managers diary, on the OH&S Field Officer checklist and an OH&S Site Instruction may be issued.

If the supplier’s employee, contractor or subcontractor continues to work unsafely then they will again be counselled and may be required to leave the site. This will be recorded in the form of an OH&S Site Instruction and the Supplier will be contacted immediately.

NOTE: Serious Safety Breaches will be handled in accordance with the gravity of the breach. Consideration will be given to removing the offending person from site immediately if the breach is of a serious nature.

Working at Heights

Where there is a risk of a worker falling from a height of 2 metres or more (or into an excavation of 1.5 metres or more) fall prevention control measures must be put in place.

Wherever possible, arrangements should be undertaken for tasks to be carried out on the ground or on solid construction to reduce the risk.

Other control measures may include:

• Properly erected scaffolding • Guard railing incorporating mid-rail and toe-board • Catch platforms • Access towers • Mobile scaffolding • Elevated work platforms • Penetration covers • Barricading • Fall arrest equipment • Ladders • Administrative controls • Or a combination of any of the above.

Floor openings, penetrations, voids and similar openings must be protected against persons falling and falling objects at all times.

If fall protection is not in place, the supplier’s employees /subcontractor are not to carry out the work. They should contact the site manager for further instructions.

Ladders

All ladders are to be of Industrial Grade with a minimum safe working load of 120 kg complying with relevant Australian Standards.

Only minor works shall be performed from a ladder. The use of power tools on a ladder should be restricted to those that are easily operated one-handed.

A person should always have two hands free to ascend and descend a ladder (i.e. all material and tools which cannot be safely secured from the worker’s belt should be independently transferred or hoisted to the work location).

Access ladders shall extend 1 metre above the stepping off point.

Ladders are to be secured against movement (top and bottom) and are to be supported on a firm, level, non-slip surface and be placed at a slope of 4 to 1.

Work involving restricted vision or hot work (such as welding or oxy-cutting) should not be performed from a ladder.

Where handrail has been removed or tampered with, it must be reinstated. Failure to leave handrail in a safe condition will result in back charges to the supplier.

Electrical

Overhead Power

Where there is a possibility of delivery plant or material breaching the 6.4 metre “No Go Zone” around overhead power lines (as defined by Energy Safe Victoria Guidelines), the delivery will not proceed unless the following controls are in place:

1. The Porter Davis Site Manager has assessed the site and implemented appropriate

controls, as outlined in the Site Managers Training Manual 2. A qualified spotter is in attendance and has completed a Safe Work Method Statement.

(The spotter will be organised by the Porter Davis Site Manager unless contrary arrangements are included in the Supplier Agreement)

3. If there is a possibility that the load or material will breach the 3 metre “No Go Zone”, the delivery is not to proceed without a “Permit to Work” issued by the electricity supplier and that all requirements of the permit are put in place

The assessment of risks associated with the delivery where overhead power lines exist, must be included in all supplier site risk assessments and Safe Work Method Statements.

Electrical Equipment and Tools

All electrical equipment will, when in use, be protected by residual current devices (RCDs). All electrical equipment will be well maintained, fully serviceable and inspected and tested at three monthly intervals by an electrician or suitably qualified person. Traffic Management

Local (Minor) Roads (Brown or orange in the Melways)

In accordance with Porter Davis’s OH&S Policy, Porter Davis requires that our Suppliers and their individual representatives observe and fully comply with all relevant OH&S Laws, Regulations, Standards and Codes of practice.

Therefore, it is expected that the Supplier delivering goods from the road comply with the legal requirements of the Road Management Act. For Local (Minor) Roads, the Supplier is to provide traffic management controls. In areas where traffic is present, the Supplier must meet the minimum requirements set out in the Act. All delivery trucks must carry a “Man at Work” sign and three traffic cones as a minimum, at all times. Where appropriate, they should be supplemented with warning signs for pedestrians and additional traffic cones where the operator is working on the traffic side of the truck.

It is the responsibility of the Supplier to ensure that traffic management controls are set up by their delivery drivers.

Major Roads (red, black or green in the Melways)

In the case of a Major Road, a more rigid and complex traffic control system is required. In these circumstances, Porter Davis will engage an accredited traffic control supplier to manage these high risk sites.

Welding & Gas Cutting

No gas cutting, gas welding or electric arc welding is to be carried on outdoors on a day of total fire ban unless a permit has been obtained from the local fire authority.

Gas bottles are to be stored upright and securely chained to a solid object to prevent them from falling and transported in accordance with relevant guidelines (vented area in vehicle).

When electric arc welding is carried out, appropriate welding screens are to be provided to protect other worker's eyes from "welding flashes'.

If gas cutting or welding is required to be carried out then:

• Flash back arrestors are to be provided on the gauge end of gas-cutting equipment • Appropriate eye protection is to be worn • Other appropriate burn protection is to be worn • An appropriate fire extinguisher is to be available and • Where necessary, a fire spotter is also to be made available

Dangerous Protrusions

All star pickets and reinforcement bar protruding from the ground must be covered by a yellow plastic cap. Yellow plastic caps are not a protection for impalement. If there is a risk of anyone falling onto reo bar or star pickets, the area must be barricaded off.

Brick cutting, painters/renderers wash down areas are to be situated in a position where run-off is not permitted to leave the site. If necessary the trade is to construct a sump with bunding around it The effected trades should carry a hose (so that equipment can be washed away from the front tap), and a shovel, so that a bunded sump can be dug

Excessive noise leaving the site

Construction site noise should not exceed the background level by no more than 10dB(A). Trades can reduce noise by observing the following requirements:

• Radios are to be kept to an acceptable level and not turned on before 7.00 am

• Where possible, noisy operations such as sawing, planning etc should be carried out inside or in the garage. The walls will muffle the sound

• All equipment is to be in good working order and fitted with the manufactures silencers etc

NOTE: Most Council’s now have a requirement for temporary fencing on all sites. The reason for these fences is to reduce the amount of rubbish leaving the site as well as restricting access to the main entry point, thereby reducing the amount of mud leaving the site.
