Friends of Disabled Adults and Children (FODAC) is a part of an amazing journey of thousands in Georgia, while providing disaster relief equipment, supplies and excess items that help around the country and beyond.

Our goal is to restore hope, healing and life back for people facing an illness, injury or disability by bridging the gap between surplus and need of basic home medical equipment.

Government benefits and insurance previously covered many of the items we provide but cutbacks and high out of pocket deductibles have made access to

these items difficult, time-consuming or impossible.

Many of our clients are struggling to take care of their

medical issues and expenses and just don’t have

the required income to get the home medical

equipment (HME) items they need.

FODAC has one of the largest, best practice refurbishing facilities in the country with regular conferences and trainings to help other groups improve the collection, repair, sanitiza-tion and distribution of HME items. We have the privilege to engage and empower individual families, many who are

at their breaking point, and need these items for activities of daily living, rehabilitation, and pediatric physical development.

FODAC is changing lives every day, breaking the bonds of isola-tion and increasing self care, worth, health, education, freedom

and independence through mobility, fall prevention and ability to access the community.

We have seen how trials are used in people’s lives “so that the works of God might be displayed in them”. We are proud to be a part of that journey to help people thrive in their new platform of life.

FODAC’s mission is to provide home medical equipment (HME)

such as wheelchairs and hospital beds at little or no cost to

people with disabilities and their families. We seek to enhance

the quality of life for people of all ages who have any type of

illness or physical disability. Our services allow clients

to return to work, attend church, and live

more productive and fulfilling lives

because of the equipment we

provide. Since 1986, FODAC

has collected and distrib-

uted more than 35,000

wheelchairs.

FODAC provides over

$10 million each year in

equipment and services to

the community at little or no cost

to the recipients. FODAC is a 501(c)(3)

and does not have funding from Medicare/

Medicaid for the services offered. Most equipment

for people with disabilities is issued for a suggested donation

but some services have mandatory fees.

We provide home health (mobility and daily living) equipment

to people of any age or any disability, temporary or permanent,

for medically necessary and medically helpful reasons. Clients

do not have to qualify financially however, priority is placed on

medically necessary and low income individuals. We rely solely

on equipment donations to accomplish this important task.

FODAC operates as a “green” company, leading the way in recycling

and reuse in the HME community. We take in donations of new

and used home medical equipment, diverting them from a

fate of a local landfill. By taking in this gently used equipment,

we can refurbish and allow for thousands of people to receive

equipment that their healthcare plans or income would not

allow. Annually, FODAC recycles over 350 tons of equipment

including metal and betteries. We are in the process of applying

for a grant, which ups the standards of green practices to a new

level. It looks at every aspect of our operation and requires top

standards including our water use.

We strive to meet the short term needs in medically necessary

situations where the client is waiting on benefits to begin or

while trying to rehab an injury at home. Examples include

injuries with no existing insurance; a child outgrowing a

wheelchair; a back up piece of equipment; all shower

equipment and caregiver aids.

We strive to further assist individuals who have specific social

or community interests that require mobility for rehabilitation,

traveling long distances. Examples include: recreation, school,

church, shopping, or doctor’s appointments.

Items are collected, checked/repaired and sanitized, then brought to our distribution room for staff to match to client needs.

We accept phone or walk-in requests for items but work from a “first come-first serve” model.

If we hold an item request from a phone call, the client has 2 days to pickup the item.

There is a one-time $25 registration fee that is due when the client actually gets their first item from us, no equipment charges thereafter.

We make deliveries for a small fee.

We accept phone or walk-in requests for equipment. Medically necessary requests receive top priority for those with the fewest resources, followed by medicallyhelpful/quality of life requests.

Clients fill out a satisfaction survey at the time of receiving the item(s) along with safety education handouts on the item(s). Items received through the HME program become the client’s property to maintain and service themselves. The only follow up we ask is that they return the HME item when it is no longer needed.

We can only provide equipment as it is donated to us. We provide equip-

ment on a first come, first serve basis. Walkers, canes, crutches and bedside

commodes along with most medical supplies are almost always available.

Please donate your HME items to one of our 30 drop off locations

around Georgia, these can be found at: fodac.org/how-to-help/donate/

Vehicle adaptive equipment can make limited mobility a

thing of the past. However, for many disabled clients and

their families, the cost of vehicle adaptations are prohibitive

and virtually never covered by insurance. FODAC’s vehicle

modification program, Operation ReMount, installs

equipment such as scooter hitch lifts, full

size van lifts, 4-point tie downs and

more on your car for minimal

cost. Due to declining donations of used lifts and change in

vehicle shapes and sizes, we are no longer able to have a

waiting list. Please look at our website at www.fodac.org/

remount for necessary steps to follow first. Last year we

installed 20 vehicle lifts and repaired 25 lifts for our clients.

gain his or her valuable independence!

Vehicle Modifications

Home ModificationsFODAC’s Home Modification Program provided 60 projects in 2015-16 fiscal year. Help for families located in Atlanta is

possible to some who are eligible for funding based on location and income. Money Follows the Person allowed us to

assist 35 clients with 10 home modifications. The Department of Community Affairs (DCA) provided funding for 5 home

modifications. 10 Brain and Spinal Cord Injury Fund projects were provided this year too. This assistance many times gives

people the freedom to get out of their homes and participate in daily activities that otherwise they might miss out on

as well as providing better access through out vital rooms in their homes.

FODAC is one of the few organizations in the country with the resources to operate and maintain an on-site wheelchair

and HME repair facility. For a nominal fee, FODAC technicians can repair all types of HME, whether acquired through FODAC

or elsewhere. This is a vital service to the community that reduces wait time for otherwise costly repairs and helps people

maintain access to school, work, and the community. We repaired 698 items helping a combined 433 clients this year.

We receive and re-distribute donations of disposable supplies for wound care,

colostomy, urology, and a number of others to the local

community and for use in disaster relief, domestic

and internationally.

systems for basic access to the

internet for school and vocational

ventures. Computers and peripherals

are displayed and sold in the Thrift Store for

those who qualify as disabled or low-income.

FODAC is also creating more demonstration of assistive technology devices as well. We’ve had a lot of help setting up the

demonstration area from Tools for Life, the GA AT Act Program. Specialty items and devices can be accessed in this project

such as adaptive utensils, AAC devices, apps for tablets and much more.

Items that cannot be re-used domestically such

as feeding pumps, surgical kits, exam tables,

and oxygen supplies can be beneficial overseas.

Although we concentrate our efforts in Atlanta

and the Southeast items go to other

international relief agencies who

are willing to coordinate and pay all shipping

International Relief

Disaster Relief

FODAC continues to aid in disaster relief throughout the country. We provide home medical equipment

to people in need of equipment when disasters strike throughout the nation. The Georgia Emergency

Management Agency (GEMA) renewed FODAC’s contract for the upcoming year. This contract provides

for pre-staged equipment to be held at FODAC and depends on us to distribute these items in a major

disaster in Georgia.

Open to the entire community, the FODAC Thrift Store offers clothing, furniture, jewelry, books and a

variety of household items. We depend on volunteers and regular donations to meet our operating

budget. FODAC is reliant on corporate, school, and individual volunteers to help sort all donations

received each day. Virginia Commonwealth sent a group of students this year for a week to

volunteer for FODAC.

All gently used donations can be brought to our main location at 4900 Lewis Road, Fodac

can also pickup larger loads/items on our trucking routes. New scheduling and driving staff,

along with 2 new trucks, have increased our capacity

to make sure we don’t miss out on these items

that help our mission.

FODAC received accreditation from HCAA in May2016. Healthcare Quality Association on

Accreditation, which signifies that FODAC meets high standards as a Home Medical

Equipment provider.

FODAC launched a 1.5 million-capital campaign to strengthen the organization’s capacity for

long-term financial sustainability. It will allow us also to increase service for people with physical

disabilities and improve operational efficiencies.

The FODAC facility will be accessed by Southface Energy funded by the Community Foundation

of Greater Atlanta for Grant to Greens to determine if we can be considered for a grant. Improve-

ments to make us even greener will be made in the future.

FODAC successfully initiated a social media trucking funding campaign, which raised

$34.000 in matching funds we needed to allow us to purchase second truck. We now

have two new box trucks for pickups and delivering equipment to make organization

more efficient!

FODAC’s client data base has been completely revamped which will help

us serve our clients more effectively.

We worked with local clinics, hospitals and international outreach to

supply items that we are not able to issue to our clients locally.

Partners include Our Lady of Perpetual Hope, Medshare, Crutches

for Africa, and local free clinics in our area.

Expanding our statewide partnerships with area agencies on Aging (AAA) Georgia’s division of

Aging Services (DAS). Under this umbrella partnership, regional and state AAA’s will establish

individual contracts with FODAC for assistive technology, including home medical such as

wheelchairs, positioning equipment and walkers, which will help clients remain independent

and connected to their community.

A new advisory board for FODAC is being established to help get valuable advice from local

business leaders.

Rotary service projects collecting HME for FODAC continues to provide needed Home Medical

Equipment (HME). Also coordinated a project with Rotary District 6900 to fund a shipment of

HME to Nepal.

Sent 40 wheelchairs on Honor Air flight to Washington DC for veterans.

Our thrift store continues to break sales records each month, providing needed support towards

operating cost.

Total volunteer hours for 2015-16 is 18,545 valued at $148,360.00.

The Run, Walk and Roll

Run, Walk and Roll was a fantastic day for FODAC. We were

blessed with the 16th annual event with unbelievable

weather and off the chart attendance by our supporters!

FODAC Golf Classic

FODAC’s 2016 Golf Tournament was held in Suwanee at Bear’s Best Golf Course on October 12th. We had

a great group of golfers, a catered lunch from Hammerheads, and lots of raffle and auction prizes. Please mark

your calendars for the 2017 Golf Classic, October 12th It will be another great day of support for our mission

while connecting supporters.

Breakfast with Santa

FODAC held another successful breakfast with Santa at the Marriott Resort at Stone Mountain Park. We are thankful to

Tony Thomas from WSB-TV for being our emcee for the event. We had a magician, caricature drawings, sing-a-longs,

and a visit from the big man himself - Santa Claus! Please plan on joining us for this upcoming event on December 10th.

We were also voted by The Atlanta Journal Constitution top 5 unique “Breakfast with Santa” events in Metro Atlanta.

The FODAC Board is a group of committed community leaders who are passionate about the good work we accomplish.

Rusty McKellar is the current Board Chair who works close by at Pattillo Industrial Real Estate as director of land and development,

where he manages the company’s land holdings. He has served on FODAC’s board since 2010; in 2011, he chaired FODAC’s 2011

capital campaign which raised over $1.6 million to renovate its facilities. Rusty states. On a recent visit to FODAC, I passed a family

in the parking lot posing for a picture with their grandmother in her new wheelchair. In the lobby waited a family of three, having

just arrived after a 4 hour drive from Tennessee to obtain equipment for their 6 year old boy. Leaving the lobby, I encountered a

smiling young woman test driving the power chair she would be takingback to Russia.

Each and every gift of time, money, or equipment allows our donors to enhance

the lives of our clients, enabling them to return to their communities and

invest in others.

mentation of our strategic plan

is now underway. These plans

include purchasing a new truck for

donation pickups, forming new partnerships in the community, and

increasing our HME drop off locations beyond our 30 in Georgia. Finding new

partners with transportation capabilities is another goal FODAC would like to pursue, allowing us to obtain and distribute more

home medical equipment in the future. Our hospital partnerships have grown with Rockdale medical added this year; we are

staging HME each month at their location so indigent patients can return home safety. This is a model we have developed with

Grady which still exists. New accreditation steps have begun to show we meet or exceed the industry standard of HME providers

who bill insurance and government benefits for their services.

FODAC NEEDS:FODAC has a facility naming opportunity for a gift to eliminate our mortage ($928,000).

FODAC is exploring utility solutions such as solar to save thousands in operating costs.

FODAC needs a partnership with a trucking company to make pickups statewide and beyond.
