Aboriginal Health Worker (AHW) Child and Family Allied Health Team
Service description The AHW works closely with other AHW’s within Tamworth Community Health and community members, men’s health and women’s health and well-being groups. This is to facilitate delivery of culturally appropriate health services and to support members of the Aboriginal Community to access Community Health Services to improve the health outcomes of Aboriginal children and their families living within Tamworth Local Government Area.

Aboriginal Maternal Infant Health Strategy Primary Care Services
Service description The Aboriginal Maternal Infant Health Strategy (AMIHS) provides an Aboriginal specific service to promote maternal and infant health and wellbeing through consultation, case management, group work, education, referrals and supportive counselling in the antenatal period and immediately postnatal for 6-8 weeks. The AMIHS service provides two drop in antenatal clinics at Coledale Community Centre and Tamworth Aboriginal Medical Service each week and also a home visiting service.

Adult Community Occupational Therapy Integrated Aged Care Team
Service description Community Adult Occupational Therapy provides assessment, intervention to improve individual functioning with daily activities at home. This may include home modifications and equipment provision, referral to community services, activity retraining and support services to adults living in the community within Tamworth Regional Council Area.
Who can use the service? Clients must be over 18 years of age or have left school and must reside in Tamworth Regional Council Area. Clients with any diagnosis are provided a service except paediatrics, or those presenting with; traumatic brain injury, mental health, current ADHC clients, and clients returning to work as the primary issue. These clients are provided with a service by the following teams: Child and Family Health Team, New England Brain Injury Rehabilitation Service, Tamworth Community Mental Health Team, ADHC, ACAT and Occupational Rehabilitation providers.
Service hours Monday – Friday, 8.00am to 4.30pm
Who can refer? Anyone can refer; a medical letter is not necessary but may be helpful to the therapist. Common referrers are: clients, family, General Practitioners, Community Nurses and community services.
How to refer? For clients under the age of 65, or under the age of 50 for Aboriginal or Torres Strait Islander people referrals can be provided in writing by phoning the Occupational Therapist and requesting an OT referral form. Clients over the age of 65, or over the age of 50 for Aboriginal or Torres Strait Islander people should be referred via My Aged Care gateway; http://www.myagedcare.gov.au/ or by telephoning 1800 200 422.

Aged Care Assessment Team (ACAT)
Service description The ACAT helps older people and their carers decide what kind of care will best meet their needs when they are no longer able to manage at home without assistance. ACAT provide information on suitable care options and can help arrange access or referral to appropriate residential or community care services. An ACAT assessment and approval is required before people can access Commonwealth funded residential aged care and Community packages. Service provided to Quirindi, Walcha, Werris Creek, Manilla, Barraba, Gunnedah and Tamworth.
Who can use the service? Persons with complex age related care needs for access to relevant services.
Service hours Monday – Friday, 8.00am to 4.30pm
Who can refer? Anyone may refer a client with the client’s or guardian’s consent. The client needs to be informed that a referral has been made.

Aged Care Day Centre Integrated Aged Care Team
Service description Provides an environment in which the socially isolated senior citizens of Tamworth can make new friends and hone their skills, thus enhancing their ageing process and allowing their Carer time to attend to other matters. A range of activities are provided for day centre clients, including:
Card and number games
Bus outings on Wednesday for lunch or morning tea.
Music and Entertainment
Celebration of special days e.g. Christmas in July, St Patricks Day, Easter, St Valentines Day, Seniors Week.
Educational talks e.g. Diabetes, Falls Minimisation.
Men’s Social Group outings on Thursday
Who can use the service? Older and frail people aged over 65 years who live in the community and younger people who have moderate, severe or profound disabilities. Carers of people who have a disability.
Service hours The Centre operates 4 days per week – Monday, Tuesday, Thursday and Friday – from 9.30 am to 2 pm. Bus outings on Wednesday.

Building Strong Foundations Team Primary Care Services
Service description The Building Strong Foundations Team is an Aboriginal specific Child and Family Health service which provides culturally appropriate primary health care to families with children aged 0-5 years regarding feeding, nutrition, growth and development, behaviour, immunisation and post-natal depression management for mothers and parenting advice. Child Protection Counselling Service (CPCS)
Child and Family Allied Health Team
Service description The Child Protection Counselling Service (CPCS) assists children and young people to deal with the effects of abuse and neglect. This is through counselling and therapy, group work, support for carers, support and advocacy for children and families, education for health staff, community education, court preparation and support, referral to other services.
Who can use the service? Children, young people, their families and carers/ foster carers where physical abuse, emotional abuse, neglect or domestic violence have been investigated and confirmed by the Family and Community Services.

Children Under 10 with Problematic or Harmful Sexualised Behaviour
Child and Family Allied Health Team
Service description Specialised counselling for children under 10 years who present with problematic or harmful sexualised behaviour and/or children who present with complex developmental, behavioural and psychosocial difficulties. The service works closely with the family of the child by proving support, advice and education regarding sexualised behaviours.
Who can use the service? Children under 10 who are exhibiting sexualised behaviours and their families or carers who are caring for the child or/and children.
Service hours Monday to Thursday 8:00am to 16:30pm.
Who can refer? Anyone can refer to this service. Referrals to this service come from various sources including parents, family members, carers, medical services, schools and other services in the community.
How to refer? By contacting Tamworth Community Health Service on 6767 8100 and asking to speak to the Clinical Lead, Children Under 10 with Problematic or Harmful Sexualised Behaviour or a Paediatric Psychologist.

Chronic Care for Aboriginal People Service description The Integrated Chronic Care for Aboriginal People Program (ICCAPP) attempts to improve health outcomes and quality of life for Aboriginal people, their families and carers, with chronic health conditions within Peel, Mehi and Tablelands Sectors. The aim of the service is to prevent, detect and treat the progression of chronic diseases including diabetes, renal, respiratory and cardiac disease. Services include assessment, early intervention, secondary prevention, self-management and education programs, and short term case management of individuals. Phone support is available as well as the follow up within 48 hours of discharge of Aboriginal patients admitted with chronic disease to a HNE hospital.

Community Nursing Primary Care Services
Service description Community based nursing provide assessment, monitoring, support and advice, health education, clinical nursing procedures, supervision and administration of medications, wound care, and palliative care. Services are provided either in the home or a clinic setting. Community nurses work in partnership with other health professionals and community groups to provide culturally appropriate service provision to designated community groups. By working in partnership with the hospital discharge planners, clients are assisted to maintain their independence and avoid prolonged hospitalisation by having clinical procedures attended in the community setting. Home visits or Clinic appointments (at hospital or Coledale Centre) are offered.
Who can use the service? Any member of the community in need of nursing care in the Tamworth area.
Service hours Monday – Friday 7.30am to 4.00pm. A limited service is available at the weekend.

Connecting Care: Integrated Chronic Care
Service description The service assists adults with chronic illnesses to improve their ability to manage their condition and increase access to health services, helping people to remain living well in the community. The service is predominantly telephone based although face-to-face assistance is available for people needing extra support.
Who can use the service? Connecting Care assists people over 16 years. People who have an identified self-management or health behaviour issue or a health access issue will benefit most from the service. Dietitian Child and Family Allied Health Team
Service description Service providing outpatient clinics and small group education. Medical Nutrition Therapy involving assessment, counselling, development of nutrition care plans, individualised education and follow up offered to paediatric clients 0-18yrs. Case types may include (but not exclusive) to: failure to thrive, food allergy and intolerance, cystic fibrosis, reflux and infant feeding, gastrointestinal issues, and oral and enteral nutrition. Services can be provided at Tamworth Community Health Centre, client homes, or other community setting based around enhancing a family centred approach. Project management for workforce capacity building strategies, or development of collaborative resources also conducted.

Drug and Alcohol Clinical Service
Service description Drug and Alcohol Clinical Services (DACS) provide general drug information and referral, assessment, counselling for individuals and families/members, consultation with the local hospital, local doctors and other health professionals. Other services provided are; withdrawal management advice, rehabilitation advice/referral, pharmacotherapy program and referral to other services. DACS health professionals provide strategies to assist clients with; moderated drinking, relapse prevention, abstinence support, lifestyle changes and cessation of smoking. DACS is committed and understands the importance of providing appropriate cultural services to our local Aboriginal communities, therefore Aboriginal Health Workers are essential and accessible to support and assist clients throughout their journey.

Falls Prevention Service Connecting Care
Service description The service aims to reduce the health, social, psychological and economic impacts of falls on individuals, families and the greater community through screening, assessment and falls risk and injury management. This is done by offering falls risk assessment (including QuickScreen) to identify level of falls risk, assessment of falls risk factors, jointly planned intervention strategies to reduce the risk of falling again, phone coaching after the fall occurs, education and referral to community services.
Who can use the service? Individuals aged over 50 years of age residing in Peel, Mehi and Tablelands Sectors. Exclusion criteria exists and includes: syncope and/or collapse; individuals that reside in a residential aged care facility (RACF); fractures, alcohol and/or drug related; work related; complex medical issues; falls from ladders, bikes, roofs etc.; individuals admitted to hospital; representations for review; and individuals deemed at risk for unsighted telephone based assessment and referral .

Genetic Counselling Service Child and Family Allied Health Team
Service description The Genetic Counsellor provides information, counselling and support to people in the New England Area who are concerned about the possibility of a genetic condition in their family. Information and education on inheritance and genetic conditions is also provided to community groups and other health professionals. The Genetic Counsellor co-ordinates genetic clinics for the visiting Geneticist. The service covers Peel, Tablelands and Mehi Sectors. Who can use the service? HNE Diabetes Service - Tamworth Connecting Care Services
Service description Diabetes management, education and support to help people with diabetes self-manage their condition and to assist with the prevention of long term complications of diabetes. Joint Investigation Response Team (JIRT) – Health

Service description NSW Health is an equal partner in Joint Investigation Response Teams (JIRT) working jointly with NSW Police Force and the Department of Family and Community Services (FACS) to provide a response to children and young people who experienced sexual assault and/or serious physical abuse and neglect that meet the JIRT referral criteria and their families in each Local Health District. The role of NSW Health in JIRT is:
Decision making with regard to investigation and assessment.
Responsibility for the provision of medical and forensic examinations.
Arranging services to respond to the medical and psychosocial support needs of children, young people and their families as early as possible after abuse or neglect has been reported.
The JIRT Senior Health Clinicians (SHC) lead the NSW Health response in the JIRT program and participate in development of a Local Planning and Response (LPR) with the other two partners. The JIRT SHC work closely with Sexual Assault Services and other Health professionals.

Service description The Sexual Assault Service provides counselling, group work, casework, court preparation and support, advocacy, community education, professional training and consultation. The service also operates a Crisis After Hours Service for children, young people and adults requiring urgent forensic medical examination and over the phone crisis counselling.
Who can use the service? Women, men, children and young people who have experienced sexual assault either recently or in the past and their non-offending family members or carers. Referrals are not accepted for people who have perpetrated sexual assault.

Service description The NSW Statewide Infant Screening - Hearing (SWISH) Program is aimed at identifying all babies born in NSW with significant permanent bilateral hearing loss by 3 months of age, and for those children access to appropriate intervention by 6 months of age. Service description The Peel Cluster Transitional Aged Care program is designed to address the needs of senior people who, in the absence of the program, would require residential aged care. It is a time-limited (12 weeks) period of support and low intensity therapy in a community setting. It provides senior people who have been assessed by an Aged Care Assessment Team (ACAT) whilst in hospital as eligible, with an opportunity to optimise their functional capacity and determine their appropriate long-term care requirements. Provision of a goal-oriented therapy program which aims to help patients achieve their goals in their own home environment or a home-like environment. Support is provided for activities in which the patient is not fully independent. Services are tailored according to each individual client's requirements and are adjusted as the client's function improves. Who can use the service? Senior people who:
At the completion of their acute/sub-acute hospital episode and whom are ready for discharge and have the capacity to benefit from a period of low-intensity therapy and who have been assessed by the ACAT team as eligible for at least low-level residential aged care.
Are in the process of making a decision about their long-term care options after a stay in hospital.

Monday – Sunday 7.30am to 4.30pm

Women’s Health Nurse, Consultant Integrated Aged Care Team
Service description To provide a Women’s Health consultancy service to the Hunter New England Local Health District.
To provide clinical and educational services to women in Hunter New England. This includes Pap testing, Breast examination and contraceptive information etc.
To work with women and other health workers to ensure better health outcomes for women in the area
To provide resources and a consultancy service to other health workers, women as clients and other women’s groups
To provide clinical stream leadership to Women’s Health/ Gynaecology stream (WHAM).
Who can use the service? All females
Priority clients Aboriginal, isolated rural women and low socioeconomic especially targeted but will see all females.
Service hours Monday – Friday, 8.00am to 4.30pm by appointment
