I. INTRODUCTION
1. We set forth herein the summary material that local exchange carriers (LECs) should file on or before April 2, 1996 to support revisions to the rates in their interstate access service tariffs. A carrier's completion of the tariff review plans (TRPs) appended to this document will partially fulfill the requirements established in Sections 61.38, 61.39, and 61.41 through 61.50 of the Commissions's Rules. 1 The TRPs display basic data on rate development in a consistent manner, thereby facilitating review of the LEG rate revisions by the Commission and interested parties.2 The annual TRPs have served this purpose effectively in past years.
2. In Section II, we highlight the modifications to the 1996 TRPs and discuss other matters affecting the 1996 annual filing. In Section III, we highlight important elements of the Price Cap TRP contained in Appendix A. The rate-of-return TRP contained hi Appendix B is highlighted in Section IV. Filing guidelines for companies filing under Sections 61.39 and 61.50 are also discussed in Section IV. The attachment describes filing procedures applicable to all companies.

H. SUMMARY
3. Based on the Bureau's experience in reviewing the LECs' annual access tariff filings in prior years, and as a result of certain changes in the Commission's rules, we are modifying the 1995 TRPs.3 The modifications were developed through discussions with representatives of the telecommunications industry and from comments by other interested parties.4
A. Price Cap TRP
4. In the 1996 TRP, we make the required updates to time periods and adopt other needed adjustments hi presentation to the TRP for price cap companies (Price Cap TRP). Among those modifications are additional rows and columns to allow reporting of information for the new basket for Video Dialtone service. Further, we adjust TRP Chart PCI-1 to remove the one- time adjustment to price cap indexes, termed the PCI Factor, included hi the 1995 annual filing.
5. We also here give further guidance regarding workpapers that LECs have filed hi past years. LECs file these workpapers to support rate calculations, the results of which are reported on the TRP. This year, LECs are requested to clarify the development of any tax amounts hi calculations by displaying the applicable federal, and state and local tax rates. In addition, we provide guidance on preparing the workpapers displaying the addback calculation used in the revenue sharing procedure.
6. In the annual filing, price cap companies make an inflationary adjustment to price cap indices. In the 1995 filing, the inflation measure was the Fixed-Period Gross Domestic Product Price Index (Fixed-Period GDP-PI), using a constant base year from which to measure annual inflation. The Census Bureau recently developed a chained-period measure hi which the base period is updated each year (Chained-Period GDP-PI). The Chained-Period GDP-PI is now the published index. Although unpublished, the Fixed-Period GDP-PI is available from the Census Bureau, at least for this year. The fixed-period index should be used until we have had an opportunity to seek comment on the revised index. On a related matter, the Census Bureau recently updated its GDP-PI series to a new base year, calendar year 1992, and companies should use this new series to measure 1994 and 1995 inflation.

7. Finally, in its comments, USTA requests the right to propose TRP modifications upon completion of certain rulemakings in CC Docket 94-1.5 If the Commission there adopts changes to the price cap rules that necessitate a revision to the price cap TRP for the 1996 annual filing, we will seek comment from interested parties on proposed modifications.
B. Rate-of-Return TRP
8. Companies filing under Section 61.38 should complete the TRP for rate-of- return companies (ROR). The 1996 version is the same as the 1995 version except for the updated time periods. As usual, there is no TRP for companies filing pursuant to either Section 61.39, the small company rules, or Section 61.50, optional incentive regulation.
ffl. PRICE CAP TARIFF REVIEW PLAN
9. In the LEG Price Cap Order.6 the Commission adopted price cap regulation for LECs, effective January 1, 1991. Companies that currently file interstate access tariffs pursuant to the price cap rules include the regional Bell Operating Companies (RBOCs),7 the GTE Telephone Companies,8 Frontier Communications ~~bf Minnesota and Frontier Communications of Iowa (Frontier), Lincoln Telephone Company (Lincoln), Rochester Telephone Corporation (Rochester), Southern New England Telephone Company (SNET), and Sprint Local Telephone Companies (Sprint LTCs).9 These companies are required to make annual filings

under the price cap rules. Any other company may elect price cap regulation as part of its 1996 annual filing. All companies that file pursuant to price cap .regulation in the 1996 annual filing should file the price cap TRP outlined below. The price cap TRP is contained in Appendix A. 10
A. Modifications to the TRP and Workpapers
10. The 1996 TRP is a modified version of the 1995 TRP. In this sub-section, we discuss the reasons for these modifications and address comments received in response to the Public Notice.
1. Video Dialtone Basket
11. Last Fall, the Commission created a new basket for Video Dialtone service. 11 Additional rows and columns are incorporated in the appropriate charts in the 1996 TRP for those carriers providing this service. The summary information is consistent with that reported for the other service baskets.
2. Removal of the One-Time PCI Factor
12. In March 1995, the Commission adopted revisions to the price cap plan in the First Report and Order in the price cap performance review rulemaking proceeding. To allow companies time to implement these revisions, the Commission deferred the annual filings of price cap companies from July 1, 1995 to August 1, 1995. This meant that the price cap indexes (PCIs) applicable to the July 1995 through June 1996 tariff year would only be in effect eleven months instead of a full year. To accommodate this one month delay in implementing the new PCI levels a downward adjustment was made to the PCIs for the next eleven months. We termed this increment "the PCI-Factor" and set forth a formula on TRP Chart PCI-1 to compute the factor. 12 In addition, we required LECs to remove the eleven month adjustment prior to updating the PCIs for the tariff year starting July 1, 1996.

13. In its comments, MCI proposes a formula to remove the PCI Factor from the PCIs, 13 and USTA proposes the same formula in its comments. 14 MCI states that its proposed formula accounts for the effect of PCI revisions on the PCI Factor occurring after August 1, 1995, the initial effective date of the PCI Factor. MCI asks the Bureau to clarify the instructions in Revision to Price Cap TRP and Order in this manner. We agree that a formula to remove the PCI Factor must account for PCI revisions during the tariff year. Chart PCI-1 has been modified to permit removal of the PCI Factor, using the formula recommended by MCI and USTA.
3. Workpaper for the Addback Adjustment
14. In March 1995, the Commission amended its rules on revenue sharing under price cap regulation to require calculation of an addback adjustment. 15 In general, revenue sharing requires LECs to lower PCIs as revenues exceed thresholds based on specified interstate rate-of-return levels. By lowering the PCI (the rate cap), revenue in excess of the threshold is shared with ratepayers through lower rates. Since lowering rates may lower a carrier's rate of return, the addback adjustment adds a dollar amount equal to the shared revenue to the carrier's rate of return before calculating its next sharing obligation.
15. AT&T suggests a new TRP chart to display the addback calculation. 16 This chart would display the sharing amounts that are being added to adjust the rate of return, hi order to reconcile these figures with the sharing amounts that were originally reported in prior annual filings.
16. AT&T also suggests a new chart displaying the state and local income tax rates used to calculate a LEC's Total Expenses and Taxes on Form 492A, the annual report on interstate rate-of-return on investment. 17 LECs would document tax rates and the development of composite state and local tax rates across study areas. AT&T maintains the addback and revenue sharing calculations cannot otherwise be fully verified.
17. We agree that information on prior sharing amounts and tax rates are necessary to verify the revenue sharing procedure. Due to each LEC's unique circumstance, we

will accept workpapers rather than establish a TRP Chart. Such workpapers provide necessary details for the summary material on the TRP. LECs provided addback and sharing workpapers in their 1995 filings, but prior sharing amounts and tax rates were not always fully documented. LEG workpapers submitted with the 1996 annual filing should show clearly the development of the addback components and identify the sources of sharing amounts reported in prior annual filings. LECs should also display applicable federal, and state and local tax rates. With regard to Form 492A, LECs should use the footnote page there to explain the development of the composite tax rates.
B. TRP Charts
1. Chart IND-1
18. This chart displays current and proposed indexes, including the price cap hidexes (PCIs), actual price indexes (APIs), service band indexes (SBIs), subcategory indexes, and upper and lower bands on the SBIs and subcategory hidexes. Chart IND-1 is unchanged from the 1995 TRP except for the addition of a row for the Video Dialtone Basket.
19. Companies should continue to file a workpaper titled "IND-1 References." This workpaper identifies transmittals hi which the current index levels became effective and assists verification of hidexes.
2. Chart CCL-1
20. This chart displays the computation of the maximum Carrier-Common-Line (CCL) rates allowed under price cap regulation. CCL-1 displays the historical data that are necessary to compute these rates according to the formula in the Rules. 18 Chart CCL-1 is unchanged from the 1995 TRP.
3. Chart PCI-1
21. This chart displays the computation of the PCIs for the price cap baskets, 19 and includes the following data: 1) the Gross Domestic Product Price Index (GDP-PI),20

measuring inflation; 2) the productivity offset (X); 21 3) the change in access charge expense (Y) imputed for LEG toll services, which affects the PCI calculation in the Iriterexchange Basket; 4) the exogenous cost change (Z); 5) the base-year (1995) revenue (R) for each basket;. 6) the weighing factor (w) used in computing the PCI; and 7) the growth in minutes'per line (g).22
22. In the 1996 version of Chart PCI-1, we have replaced the row that reported the one-time, eleven-month adjustment, the PCI Factor, with a row to report the new factor used to remove the PCI Factor from the PCIs. We have also modified Chart PCI-1 to include the new Video Dialtone Basket.
4. Chart SUM-1
23. This is a summary chart displaying the revenues in baskets and categories. It displays base-year (calendar year 1995) service demand multiplied by 1) current rates and 2) proposed rates. This chart is used to calculate the difference in revenues using base-year demand, under current and proposed rates.
5. Chart EXG-1
24. This chart presents supporting data for computing the exogenous cost adjustments. There are columns arid rows in Chart EXG-1 to report exogenous cost changes attributable to: 1) long term support (LTS); 2) regulatory fees; 3) excess deferred taxes (EDT); 4) investment tax credit (ITC) amortization; 5) the sharing or low-end adjustment that was reported in the 1995 annual filing and is now being removed from the PCIs; 6) revisions, if any, to that sharing or low-end adjustment; 7) the new sharing or low-end adjustment for the upcoming tariff year; 8) Telecommunications Relay Service; and 9) "other" exogenous cost changes. These exogenous cost changes are reported by service basket.
25. As noted above, Chart EXG-1 includes columns to report: 1) the sharing amount or low-end adjustment that was reported in the 1995 annual filing and is being removed

in the 1996 annual filing; 2) revisions, if any, to that sharing or low-end adjustment; and 3) the new sharing or low-end adjustment for the upcoming tariff year.23 The removal of sharing must reflect the earnings reported in the Final Form 492A for calendar year 1994. Companies are required to base this year's sharing or low-end adjustment on their earnings in calendar year 1995. The level of sharing or low-end adjustment for the second half of 1995 should be computed based on the X-Factor chosen in the 1995 annual filing. The level for the first half of 1995 should be computed based on the X-Factor in the 1994 annual filing. Consistent with the practice in previous annual filings, companies should average the rate of returns so that the returns in the first half and second half are the same and when annualized equal to the calendar year rate of return. In some cases, companies have already satisfied their 1995 sharing obligation, including the addback adjustment, by way of a waiver of our rules that permitted those carriers to make an early PCI adjustment. In these cases, the sharing obligation in the 1996 annual filing equals zero. In the particular case of the Sprint LTCs, the nature of their waiver requires an adjustment to the PCI prior to the 1996 annual filing. The values to be reported on Chart PCI-1, Lines 233 and 235, should reflect this adjustment and the method of adjustment should be shown in a workpaper.
26. Carriers should submit their Form 492A fo>calendar years 1994 and 1995 as part of their support material. In addition, carriers should provide full documentation in their workpapers showing the calculation of the sharing or low-end adjustment amount, and the method used to allocate the amount among the baskets. As in the 1995 annual filings, carriers should remove the earnings of services excluded from price cap treatment based on the assumption these excluded services earned the same interstate rate of return as price cap services.
27. Companies should continue to include in workpapers additional data on each exogenous cost adjustment, including the calculation of the cost change, identification of total and interstate amounts, and the method of basket allocation. Particularly with respect to sharing and the addback calculation, companies should clearly display the federal, state, and local tax rates or gross-up factors as a component of the exogenous cost change or sharing amount. With respect to Form 492A for calendar year 1995, companies should report such rates on the footnote page included with Form 492A.
6. Chart RTE-1
28. This chart displays data used to compute the actual price indexes (APIs), service band indexes (SBIs), and subindexes. It displays calendar year 1995 demand, current rates, proposed rates, and revenues computed by multiplying the 1995 demand by current and

proposed rates. Chart RTE-1 enables us to verify the accuracy of "R," the revenue variable in the PCI formula that equals base period (1995) demand multiplied by rates. Demand and rate data are reported hi the aggregate under the primary rate elements of each category. The level of aggregation in Chart RTE-1 allows rapid, consistent verification of the computation of the indexes across all companies. Detailed demand and rate data underlining the information hi Chart RTE-1 are reported in the rate detail charts discussed next hi this section.
29. In then- 1995 annual filings, LECs provided a chart that gave complete rate element detail, i.e.. demand, existing rates, proposed rates, and API, SBI, and subindex computations, for every rate element. LECs provided this chart in computerized format. LECs should file this chart on a 3.5-inch computer disk. We leave unspecified the exact format of the rate detail charts because each price cap LEG has a different number of rate elements. For each rate element, however, LECs should display the rate element name, base period demand, current rates, and proposed rates. LECs may also include a rate identifying code. The aggregate revenue amounts for baskets, categories, and special and switched access services should be totaled at the end of each section of the chart, to assist in verifying the agreement between this chart and the revenue amounts in RTE-1. The variation hi the number of rate elements among LECs prevents us from specifying the row numbers, but each row of this chart should correspond to only one rate element. The rows should reflect the basket and service category sequence used hi RTE-1.
C. Miscellaneous
30. In addition to the above specifications, price cap LECs should include with their support materials a list of all currently applicable Part 69 waivers. The list should include the following information: 1) a citation to the Commission or Bureau Order granting the waiver; 2) a brief description of the waiver, including whether any new rate elements were authorized; and 3) the basket and, if applicable, service category of each rate element authorized by the waiver. Price cap LECs should also include with then- support materials a separate list of new services that are being incorporated into the APIs hi this annual filing for the first time. This list should contain all services introduced as new services under the price cap rules during calendar year 1995. To assist us hi verifying carrier compliance with the basket and service category structure established in the LEG Price Cap Order, carriers should also display, for each new service listed, the basket and service category to which the new service is assigned.
31. If a company makes a revision to its annual filing, its TRP including the computer disk containing the TRP, should be refiled hi full detail, rather than just the parts of the TRP that are changed. The latest TRP filed becomes the TRP of record. Other parts of the original filing, e.g.. portions of the Description and Justification and unrelated workpapers, may be omitted if unchanged by the revision.
IV. TARIFF REVIEW PLAN FOR NON-PRICE CAP COMPANIES
32. Carriers not electing price cap regulation are required to file based on either:

1) Section 61.38 of the Rules (rate-of-retum companies);24 2) Section 61.39 of the Rules (small- telephone companies);25 or 3) Section 61.50 of the Rules (optional-incentive-regulation companies).26 Rate-of-return (ROR) companies should file the ROR TRP contained in Appendix B. The requirements for carriers electing to file as small telephone companies are described below, as are the requirements for optional incentive regulation companies.
A. Rate-of-Return Companies
33. Companies filing under ROR regulation are subject hi part to the filing requirements adopted in the Regulatory Reform Order.27 which established a biennial filing cycle.28 The Rules require that rates be targeted to earn the authorized rate of return.29
34. The ROR TRP has been framed to place the smallest possible regulatory burden on LECs while allowing us to evaluate adequately the rates filed. The 1996 ROR TRP is the same as the 1995 ROR TRP, except for updated references to the historical period, test year, and rate monitoring period. The ROR TRP is contained in Appendix B.
35. The Rules require LECs to file one year of historical data and one year of projected test year data. All proposed rates must be based upon projections of test year data. The time frame of the past year cost-of-service (PYCOS) study and test year have been determined pursuant to Section 61.38 of the Rules. LECs are required to file the PYCOS data for the period January 1, 1995 through December 31, 1995. The period July 1, 1996 through June 30, 1997 is the test year for LECs filing the ROR TRP.
B. Small Telephone Companies
36. Companies that qualify as small telephone companies and elect to file under

the small telephone company rules are subject to Section 61.39 of the Rules.30 Companies filing under the small company rules must file access tariffs for their traffic sensitive and common line rates in odd numbered years,31 and are not required to make a 1996 annual filing hi most circumstances.
37. A company filing under the small company rules need not submit cost support at the time of an access tariff filing. Companies, however, should be prepared to submit the data promptly upon reasonable request by the Commission or interested parties.32
38. Rates under the small company rules are based on historical costs. A company filing under these rules must use the following cost support and ratemaking methods:1) for its first access tariff filing, the company must compute rates based on a cost-of-service study for the most recent twelve month period with related demand for the same period; and2) for subsequent access tariff filings, the company must compute rates based on a cost-of-service study for the two year period since the LEC's last access tariff filing, with related demand for the same period.33
C. Optional-Incentive-Regulation Companies
39. Any non-price cap company may elect optional incentive regulation for either its traffic sensitive rates only, or for both its traffic sensitive and common line rates. In contrast to ROR regulation, optional incentive regulation relies more heavily on historical costs, establishes broader earnings bands, and affords the LECs greater pricing flexibility.
40. We have not adopted a TRP for companies filing under optional incentive regulation. For any carrier planning to file in 1996 under optional incentive regulation, Tariff Division staff will provide guidance regarding specific cost support for the filing.

41. A LEG that chooses optional incentive regulation is required to compute rates for its first access tariff filing based on a cost-of-service study and related demand for the most recent twelve month period. For subsequent access tariff filings, a LEG is required to base its rates on the costs and related demand for the two year period since the carrier's last access tariff filing.
V. GUIDELINES APPLICABLE TO NECA
42. As in the past, we do not specify particular summary data for NECA to file. The level of detail in the ROR TRP should provide adequate guidance to NECA hi determining the summary data for its cost pools that it should include in its filing.
43. Our decision not to specific summary data for NECA to file regarding its rates and settlement amounts does not exempt it from providing support data hi compliance with Section 61.38 of the Commission's Rules. If requested, Tariff Division staff will provide guidance regarding specific cost support for the annual filing.
44. As in previous annual access filings, NECA should provide: 1) TRP Chart ERN for the historical and prospective periods; 2) settlements from its average schedule companies hi Chart COS-1; and 3) detailed cost support data concerning long term support calculations.
VI. GENERAL INSTRUCTIONS
45. The following general instructions apply to non-price cap LECs, and, except as noted, to price cap LECs. Except for Section VI.C., infra, the following general instructions also apply to NECA. These instructions pertain to all data filed hi support of access charges, including data that are filed in the TRP. Data should be filed no later than April 2, 1996, and should be accurate and complete.
A. Certification
46. The filing of inaccurate or incomplete data may seriously detract from the ability of the Commission and interested parties to evaluate the revised rates within the original notice period. The LECs and NECA must certify that their historical and forecast data are accurate by including a signed statement that the support data are true, correct, and complete to the best of the carrier's knowledge. This certification will apply to all data submitted hi support of revised rates, including the data that are filed hi the TRP. The text of the certification is the same as that adopted hi the 1987 Waiver Order34 and the 1988 TRP Order. The certification should be displayed as the last page hi the binder containing each company's TRP.

B. Errata
47. Carriers are also under a legal obligation to correct any inaccurate or incomplete data discovered in the TRP or other support data. As stated above, inaccurate or incomplete data may seriously detract from the ability of the Commission and interested parties to evaluate the revised rates within the original notice period. Thus, any erratum should be accompanied by a request for special permission to permit the transmittal to take effect on the originally scheduled effective date. Further, if this request is denied, rates supported by the erratum should be refiled on statutory notice, regardless of whether these rates differ from the rates originally filed. We believe that this special permission procedure is a reasonable means of encouraging carriers to improve the quality of data submitted in the annual access tariff filings. We also believe it facilitates the tariff review process. Exceptions to this rule will only be made upon a showing of good cause.
48. For the 1996 annual access tariff filings, we may specify an early cut-off date for errata. Carriers that discover errors after the cut-off may then be required to refile their tariffs with a later effective date. Any errata that result in a change to the company's TRP should be filed together with a revised TRP that reflects the changes.
C. Geographic Aggregation
49. Except for Charts RTE and ERN for rate of return carriers, all information in the TRP should be displayed at the same level of geographic aggregation to which the particular LEC's filed rates pertain. Charts RTE and ERN for rate of return carriers should be consistent with the current level of geographic aggregation. In addition, if the tariff rates are based on data from a number of study areas or operating companies, levels of aggregation associated with each study area (state) and operating company should be provided by carriers (except for Charts RTE, ERN, and REV). Price cap carriers need not provide TRP data below or above the level of geographic aggregation to which their proposed rates pertain.
D. Assistance from United States Telephone Association (USTA)
50. As in past years, to assist the Commission and interested parties in evaluating the national impact of revised rates, we request that USTA provide us with aggregated TRP data for all price cap LECs (except for those charts and portions of charts for which aggregation is inappropriate) hi both hard copy and disk format.35
E. Waivers
51. If carriers find that they are unable to provide data hi the TRP, they may file

applications for waiver. These waiver applications should demonstrate good cause for reporting a different or lower level of detail than specified and should indicate how the carrier intends to report complete TRP data in the future. If a carrier determines that it will be unable to file the complete TRP, it should file an application for waiver with the Commission no later than March 5, 1996. Opposition to applications for waiver shall be filed not later than March 12, 1996. Each carrier requesting any waiver of Commission Rules should include all such requests in this single application. Carriers should not delay undertaking development of data for the 1996 TRP in anticipation that waiver requests will be granted.
F. Technical Instructions
52. Consistent with TRP filings hi previous years, all companies that file a TRP should provide the data hi computerized form. Details regarding this technical instruction are provided hi the Attachment.
G. Compliance with the Paperwork Reduction Act
53. In compliance with the Paperwork Reduction^ Act, we estimate burden-hours and place the Office of Management and Budget form number and date on the formats included in the Appendices. We note that these TRPs were developed after informal discussions with LECs and other industry representatives and after evaluation of comments filed hi response to our Public Notice. We minimize the regulatory burden on the LECs by deleting obsolete sections of the TRP that have not proved useful, and carriers need not file historical data that have been filed in previous years. Overall, the burden this year is unchanged from that imposed by the 1995 annual access tariff filings for which we made significant reductions to reporting requirements. Smaller LECs continue to report significantly fewer data than larger companies.
VH. CONCLUSION; PROCEDURAL MATTERS
54. This document will be sent directly to all carriers that will file their own annual access tariffs on April 2, 1996, and a copy will also be sent to NECA. A copy of the document and all attachments will be available for inspection at the Tariff Division, Room 518, 1919 M Street, NW, Washington, DC, 20037. The Commission will publish the text of this document hi the FCC Record. Those parties wishing to obtain copies of Appendices A or B can do so through International Transcription Service, Inc. ATTACHMENT
PROCEDURES FOR 1996 ANNUAL ACCESS CHARGE TARIFFS TO BE FILED ON OR BEFORE April 2, 1996
1. Local exchange carriers (LECs) are required by Part 69 of the Commission's Rules, 47, C.F.R. Part 69, to file new access charge tariffs or revisions to existing tariffs to be effective July 1, 1996, on less that 90 days' notice. Therefore, the Common Carrier Bureau anticipates receiving these filings on or before April 2, 1996. In this Attachment, we provide details on the specific procedures to be followed when submitting these filings, or revisions to the filings to be effective July 1, 1996, to the Commission.
A. General
2. Interested parties are reminded that the annual access tariff filings and the associated review and analysis are not part of a docketed proceeding. Therefore, until further notice, all correspondence and pleadings (other than tariff filings) in connection with these filings should not bear any docket number but should instead bear the. designation "1996 ANNUAL ACCESS CHARGE TARIFF FILINGS." All such correspondence and pleadings should be submitted with an original and 10 copies. A copy should also be served on the Commission's Commercial Contractor, International Transcription Service, Inc. (ITS), Room 246, 1919 M Street, NW, Washington, DC.
B. Tariff Filings
3. LECs submitting tariff filings should consult the attached schedule which details the number of copies to be filed and the location to which those copies should be delivered. Carriers are especially reminded that 1) the appropriate tariff filing fee must be submitted to Melon Bank, and 2) they should serve a copy on ITS. Those annual filings submitted on or before April 2, 1996 should be delivered to ITS at the Commission in Room 535, 1919 M Street, NW, Washington, DC.
4. Carriers, whether filing their own individual tariffs or not, are reminded of the customer notification requirements as outlined in Section 61.58(a)(4) of the Commission's Rules, 47 C.F.R. Section 61.58 (a)(4), concerning rate increases in particular. Carriers are further reminded that the purpose of such notification is to afford any affected customer sufficient opportunity to comment on the tariff filings within the time period established for filing petitions in Section 1.773 of the Commission's Rules, 47 C.F.R. Section 1.773.

5. To facilitate identification and cataloging of the tariff filings, the LECs should:
a. Clearly label any package of revised tariff pages as such, e.g.: BELL ATLANTIC, STATE, TRANS., LTR. #XXX, DATA, REVISED TARIFF PAGES. These revised pages or any completely new tariff should not be submitted in a binder, but should be hole-punched for a 3-ring loose leaf, binder.
•
b. Provide a list identifying all cost support volumes submitted with these revisions and associate the list with the cost volumes and the transmittal letter.
c. Clearly label all boxes containing cost support volumes as to their contents, e.g.: BELL ATLANTIC, STATE, TRANS. LTR. #XXX, DATE, VOLS. 1, 2, 3, 4, BOX 1 of 5.
d. Submit cost support material in a 3-ring binder pursuant to Public Notice dated December 13, 1986, DA 86-337, and label the volumes of cost support on the spine of the binder with the name of the carrier, the transmittal number, the subject of the filing, the volume number and the date of the filing. For example: BELL ATLANTIC, TRANS. LTR. #XXX, 1996 ACCESS CHARGE TARIFF FILING, VOL. 2-2 of 2, April 2, 1996.
e. Each carrier should submit its respective tariff review plan (TRP) and certification in a separately bound package.
6. In addition to the paper copies, all companies should submit their TRP in three copies on 3 1/2 inch high density disks. One copy should be filed with 1) the Tariff Division in Room 514, 2) Peyton Wynns, FCC, Industry Analysis Division, Room 500, 2033 M Street, NW, Washington DC; and 3) ITS at the Commission in Room 535, 1919 M Street, NW, Washington, DC.
7. Price cap carriers should submit their TRP in Lotus, Release 3 format, using the spreadsheet provided by the Bureau. The file should be named COSAAN96 where "COSA" is the company study area code. The full rate detail spreadsheet should be provided in a separate Lotus file, named COSAA96R, where "COSA" is the study area code.
8. Rate of return carriers should follow the rules for submission of data to the Commission on magnetic media contained in the 1989 TRP. These carriers should also follow the technical documentation for the 1989 TRP, appropriately updated, for all TRP charts except COS-1(P) for which the carriers should use the documentation for ARMIS report 43-01. For filing Chart COS-1(P) on disks, carriers should follow the instructions for ARMIS report 43-01.

C. Petitions and Replies
9. Petitions concerning these tariffs will be due April 29, 1996, regardless of whether the tariff revisions are submitted on or before April 2, 1996. Replies may be filed only by the publishing carriers and will be due on May 13, 1996. Petitioning parties should reference the general designation as outlined above or the specific tariff and transmittal number, and not a docket number. Petitions should be organized where possible so as to present comments in a modular form, section by section, for ease of analysis. An original and 10 copies of any petition or reply must be filed with the Commission and a copy must be served simultaneously upon the publishing carrier or petitioner. Additionally, ITS should be served with a copy in Room 246, 1919 M Street, NW, Washington, DC.
10. Members of the general public may submit one copy of their comments, without regard to form, by letter to the Secretary. These comments should specify that they concern the 1996 Annual Access Tariff Filings or a specific tariff transmittal.
D. Public Information
11. Tariffs filed with the Commission, related correspondence, petitions, and replies concerning the 1996 Annual Access Charge Tariff Filings may be examined by the public from 1:30 - 4:30 p.m., Monday through Friday, in Room 250, 1919 M Street, NW, Washington, DC. Copies of tariffs, petitions, and other related filings may be obtained from the Commission's Commercial Contractor, ITS.
