• Knows that telemedicine is THE answer for the nation’s healthcare shortage. • After graduating with a Masters of Social Work degree from Valdosta State
University she began working in the private mental health field and public school system. For nearly 15 years, she witnessed first-hand what happens to children and families when necessary medical and mental health care is out of reach.
• As more and more rural health providers disappear and fewer specialist move into rural locations, Mrs. Williams understands the critical nature of strengthening and expanding the Global telehealth network.
• By working with existing and potential partners in Georgia and beyond to implement and design sustainability strategies for telemedicine programs, she believes that all US residents will remain connected to healthcare.

Global Partnership for Telehealth, Inc. (GPT) is a
nonprofit telehealth network with the mission of
leveraging telehealth technologies to deliver
healthcare for the benefit of all. Based in rural south
Georgia, GPT is the largest nonprofit telehealth
network in the US with clinical partners in 10 states
and 4 mission projects overseas.
GPT does not have any proprietary interest in any
product, instrument, device, service, or material
discussed during this learning event.
The education offered by GPT in this program is
compensated by the HRSA Small Hospital

Program Overview
The goal of the Population Health Program is to provide training, resources and support on rural population health across the hospital
team, and to promote collaboration through the gathering and sharing of best practices and case studies of successful population
health initiatives and innovations in rural hospitals across six primary population health focus areas.
The goal of this Telehealth Transformation Series, provided by Global Partnership for Telehealth, is to provide evidence that telehealth
promotes and supports collaborative care, self-monitoring, chronic disease management, population screening which produces
improved care and outcomes, as well as cost efficiency in targeted populations.

Health systems are leveraging telehealth to bring specialty care to their patients who might go lacking if distant travel is involved.Engaged patients - Health systems, technology providers and others are responding with a host of solutions aimed at attracting, engaging and retaining patients as customers. These include mobile apps, telemedicine options, online portals for communicating with providers, and other technologies.
Reduces Errors Improved care coordination - New technologies can improve outcomes and help reduce medical errors by improving follow up care, access to specialty care, eliminating duplication of services, unnecessary tests and negative drug interactions.New support tools: Health care organizations are using new tools and determining new ways to help patients adhere to treatment paths. For example, predictive analytics is being used to improve medication adherence
Health systems are highly focused on becoming consumer-centric organizations, and viewing the integration of telehealth/virtual care/remote monitoring as part of the journey to putting consumers first.

Convenient care – Consumers want care close to home, school and work. They are willing too embrace nontraditional sites of care to include retail health clinics & online provider services.Timely care – in this age of consumerism, providers & health systems must compete to attract new patients and keep current ones. Attention is being paid to improving the patient experience. Significant areas of focus: easy access to care, little to no wait times, ease of appointments and increased respect of the person. Telehealth can help check most, if not all, of the boxes.
More Satisfying Care High quality - Consumers, particularly younger ones , increasingly expect healthcare to work the way other digital markets work, with user-friendly interfaces, clearly defined pricing and awide selection of product options designed to meet their needs.Value – Consumers want and expect access to local, high quality primary, urgent & specialty care. In this digital age of facetime, PCs and the iPhone, consumers will increasingly demand that telehealth be one of the options that community healthcare systems offer.
More Cost Effective Care
More involved – With high deductibles and more out-of-pocket expenses, consumers are more engaged and cost conscious than ever before. They want to know cost, justification for the chosen treatment and alternative less costly but effective options.Shop around – Consumers of healthcare are savvy and more motivated than ever to seek out the highest quality, most advanced healthcare at the best price. Employees especially appreciate in-house telehealth which saves Paid Time Off hours since they don’t have to leave the place of employment for an off-site appointment.
Reduces Cost of Care
Potential savings There has been a significant rise in overall healthcare costs for employers and many are looking for alternatives that bring costs down without sacrificing employee wellbeing. On-site telehealth is providing one avenue for cost savings.Better utilization of care –Company executives say telehealth is a win for the companies and the employee. If a worker gets sick with a minor illness when the doctor’s office is closed, or if the employee doesn’t have a primary care doctor, telehealth is an alternative to an urgent care facility or emergency room. Resulting in less time away from work, and cost savings for the company, the insurers and the employee.Support- telehealth can support improved outcomes for employees that are at risk or in need of chronic disease prevention or management (i.e. diabetes or coronary artery disease)
According to the Large Employers’ 2018 Health Care Strategy and Plan Design Survey by the National Business Group on Health, 96 percent of employers are set to offer telehealth services

COMMUNITY CARE AGENCIES LIKE TELEHEALTH
Limited or Under-Utilized Community Resources
Underserved or rural communities - Those who reside there are often hard-pressed to find sufficient access to medical resources. Effective and sustainable hospital-community partnerships that leverage all healthcare resources are critical to meet the challenges faced by both urban and rural communities.Insufficient health care offerings - can leave EMS providers and emergency departments overburdened. (i.e. EMS responding to falls at the same residence over & over, inappropriate use of the ED for non urgent visits)
Coordinated Care b/t Health & Community Resources
Networking & developing partnerships - between organizations is important to build coalitions, exchange/share resources, partner, and avoid service duplication.Community-based organizations - often provide services and support to the most marginalized, disadvantaged and stigmatized sections of society.
Community Care In the community- continuous digital advancements are providing countless ways for non-health entities to participate in the promotion of health & wellness for people of all ages and states of health. Entities from faith-based to senior centers to child care centers are offering wellness and disease management instructional support & activities at their facilities for the benefit of their members. (i.e. fun exercise videos for all ages from preschoolers to seniors)
Community Health Workers & Community Paramedics utilize telehealth services to receive training
& provide clinical consults to co-manage complex medical cases with providers as part of a care team
Community-based organizations - are important health system stakeholders as they provide numerous, often highly valued programs and services to the members of their community.
THE ELDERLY & CARE GIVERS LIKE TELEHEALTH
High Levels of Acceptance
Many studies report- that older patients demonstrate flexibility and interest in the novel use of technology. One ED-based telehealth program providing virtual consultations reported the delivery of safe and satisfactory care that was readily accepted by the older patients.Support for Caregivers - Telehealth can relieve some of the stress associated with caring for seniors. It provides needed support for caregivers allowing for greater access to health professional who provide crisis intervention, give advice & guidance as well as, prescribe appropriate treatments.
Improved Care Seniors are statistically more likely to experience chronic conditions - that necessitate routine monitoring from healthcare providers. Certain telehealth technologies can be utilized by providers to monitor such as glucose levels, weight and blood pressure. Remote monitoring & virtual visits – curtail the need for routine in-person visits, which can be costly, injurious and difficult for seniors who may have mobility problems and/or limited access to transportation.Lower chance of hospital admission - improved communications between providers, patients & care givers allows providers to monitor older patients' overall health and intervene early to avoid medical emergencies.
Reduces Cost of Care
Aging at home - Keeping the elderly comfortable, happy and healthy at home, instead of in a medical facility, has major financial implications. With telehealth technology, seniors can be evaluated and even treated remotely, eliminating unnecessary (and costly) office and emergency room visits. Telehealth saves time & improves quality of life for all. With a reduced need for routine in-person visits to providers, seniors & caregivers can dedicate more time to care at home and other important activities.
Telehealth: A Promising model for senior health care where a senior can receive prompt, high-quality health care at any time, without leaving their home or residence.

In Conclusion
• Telehealth plays a significant role in population health in that it
supports wellness, disease prevention & chronic disease management.
• Telehealth allows for care to be delivered in untraditional places from
the home to the work site.
• Digital and wearable devices to track and/or relay activity levels, blood
glucose, blood pressure, and other vital signs are being used to
promote wellness, manage chronic conditions & tend to the elderly at
home.
• In this digital age, telehealth technologies will continue to efficiently
and positively impact how everyone receives and delivers healthcare.
• Health Systems, Consumers, Employers, Community Care Agencies,
the Elderly and their Caregivers Like Telehealth!!!
