INTRODUCTION

i) Immediate Action:

It is important to note that the Client for the scheme, Liverpool City Council, has been managing the safety of buildings and properties within the site boundary for a number of years. This includes utility disconnections to individual properties, boarding-up and general security measures. These buildings are in a state of disrepair and in a hazardous condition.

As a result of this process NO information is available on the current state of existing buildings and properties other than that provided as part of the survey and demolition process therefore the Principal Contractor must proceed with caution at all times.

The Contractors attention is drawn to the need to visit the site during the Early Contractor Involvement stage.

ii) Basis of Information Pack:

The Plan should be read in conjunction with the Specification and Drawings and references have been made to this document where appropriate.

This Information Pack prepared by the CDM Coordinator is based on the design prepared by the Designers and on information provided by the Client and/or Designers Significant hazards are identified in relation to the works and the methods foreseen by the Designers. Additional hazards may be identified by the Principal Contractor and these should be included in the development of the Information Pack

It has been assumed that a competent Principal Contractor will prepare and operate his own safety management system taking into account the Site Rules, the Client requirements, the perceived hazards and project information provided in this document.

iii) Development of the Plan

The Principal Contractor has a duty to develop the Information Pack prepared by the CDM Coordinator to set out the means by which he will control the hazards identified and to take account of the following: -

a) Any changes to the design

b) Aspects of Health and Safety specific to the Principal Contractor's method of working

c) Principal Contractors detailed Health and Safety procedures.

d) All current relevant legislation

e) Hazards identified by the Principal Contractor and other Contractors.

f) Hazards identified by Contractors with a design responsibility

g) The Clients specific requirements

h) New risks, which may arise during the course of the work

Any changes to the Plan resulting from design changes must only be done by agreement with the CDM Coordinator.

These may include specific items with respect to site rules and the control of Health and Safety hazards.

The Information Pack for the work should be developed as early as possible to avoid delays and a satisfactory response is required prior to commencement on site. -

Thereafter -the Information Pack will be updated by the Principal Contractor to incorporate further hazards identified as work proceeds.

iv) Duties as Designer, Contractor and/or Principal Contractor.

Where part of the contract involves an element of design and construction, the Principal Contractor should note that Designers’ duties under CDM will apply.

1.1 DESCRIPTION OF PROJECT - Description and Programme details

1.1.1 Location of site

Edge Lane West Kensington Liverpool Merseyside L7

1.1.2 Nature of construction work to be carried out

1.1.2.1 Summary of construction work to be carried out

The proposed Highway and Environmental improvements, designed and project managed by Mouchel and 2020 Liverpool, will continue the high quality dual two-lane urban boulevard theme along Edge Lane. These high standard environmental improvements will integrate a new highway with better pedestrian facilities at new signalised junctions, soft landscaping, extensive tree planting and the highest quality of public realm. Public art will play an important part of the overall concept and theme of Edge Lane West, which could include sculptures and lighting based artwork. There will be a new communal square at the centre of Edge Lane West, bordered by a mixed use of residential, commercial, educational and community facilities.

The Edge Lane West Project will also require extensive housing clearance and demolition of approximately 371 properties. Currently these will be delivered under a separate contract being procured by Liverpool City Council, but it will be necessary for the approximate 125 highway affected frontage properties to be demolished and cleared by the Highway Contractor, in advance.

A major part of the highway works will be the extensive diversion of Utilities apparatus. Currently these are estimated at £2.5M and taking upwards of three months to complete at the beginning of the construction programme. The successful Contractor will be required to coordinate all Utilities activities, with orders to be placed directly by the Council.

1.1.2.2 Notification to the Health and Safety Executive

Initial notification of the Project has been made by the CDM Coordinator. The CDM Coordinator will

notify the appointment of the Principal Contractor following the award of the Contract. On award of the Contract, the Principal Contractor shall provide the following information to the CDM Coordinator to enable him to complete the notification to the HSE:

i) Name and address of the Principal Contractor

ii) A declaration on Form 10 (Rev) signed by or on behalf of the Principal Contractor that he has been appointed as such

iii) Date Planned for the start of the construction phase

iv) Planned duration of the construction phase

v) The estimated maximum number of people carrying out construction work on the site at any one time

vi) Planned number of Contractors on the construction site (if known).

vii) Name(s) and address(es) of any Contractor(s) already chosen

1.1.3 Timescale for completion of the construction work

1.1.3.1 The programme for the proposed construction works is to be developed by the Principal Contractor, The Client and the Designer during the proposed ECI stage of this Contract.

It is a requirement of this Information Pack that the programme proposed for the construction works must demonstrate the adequate allocation of competent and adequate resources. Reference should be made to the Preliminaries and the Principal Contractors requirements.

1.1.3.2 The proposed works will be delivered under one single phases, with demolitions and utilities diversions being front end activities. This will be followed by the main works contract, with accommodation works to existing boundaries, main carriageway highway works, junction improvements and School Lane Link Road. It is envisaged for construction purposes that the length of Edge Lane West will be divided into manageable sections ie. between and Botanic Road and Holt Road junction, Holt Road to Jubilee Drive, etc.

1.1.3.3 The Principal Contractor is required to determine the resources he will require to sufficiently develop the details of the Information Pack and propose any alteration to the above periods to allow for this.

Equally, in proposing the programme for the construction works, the Principal Contractor must include and illustrate the allocation of resources for all Health and Safety concerns.

1.3 DESCRIPTION OF PROJECT - Extent and Location of Plans and Records

1.3.1 Drawings and Documents

Initial Drawings and documents for these works are contained within Volume 3 of the Tender package and will be supplemented with additional information in advance of the proposed ECI period.

1.3.2 Services Drawings

The statutory authorities have been contacted and the location plans for services have been received for water, gas, electric, foul & surface water and BT / Telewest. The location drawings are provided in Volume 3.

1.3.3 Reports

There has been a SI report carried out by Mouchel and a copy of the report will be with the ECI documents. Also there has been an ecological survey carried out and are waiting for the report. This will be sent out as soon as it is received.

A Type 3 asbestos survey has been carried out and ACM’s have been found within some buildings

that are to be demolished. It is understood that they are non notifiable ACM’s however the full report will be attached to the ECI/ contract documents.

1.3.4 Existing Health and Safety file

There is no existing H&S File for this project or the existing properties / buildings on site.

1.4 EXISTING ENVIRONMENT

1.4.1 Surrounding land uses and relevant restrictions

The surrounding land use is predominately residential; there are a number of businesses, petrol station and religious buildings fronting the carriageway and to the north of the site Kensington County Primary School. The area is prone to high level of vandalism and it would be advised for the Principal Contractor to work closely with the local police in terms of security and for themselves to have security present within their site outside working hours.

1.4.2 Existing Services – Underground and Overhead

See section 1.3.2

1.4.3 Existing traffic systems and restrictions

Local residential roads have had traffic calming measures installed. There is a one way system around the roads to the north of the site. A banksman should be present for all construction traffic entering and leaving the site and a detailed traffic management plan should be included in the Principal Contractor’s Construction Phase H&S Plan to demonstrate their proposals for access, egress and management of all on-site movements.

1.4.4 Existing Structures

The majority of existing buildings and properties on-site to be demolished have basements. There are no other known structures within the site boundaries.

1.4.5 Ground Conditions

Refer to the Stage 1 and 2 Ground site Investigation report included in Volume 2.

2.1 CLIENT CONSIDERATIONS AND MANAGEMENT REQUIREMENTS - Client Considerations

2.1.1 Safety Goals

The Client is committed to supporting the Health and Safety Strategy in the context of 'Rethinking Construction, Respect for People' and Revitalising Health and Safety. The Client will employ Contractors who will implement a safety strategy with a target of zero accidents of all types.

The safety strategy will comprise of a number of separate components:

2.1.1.1 Employee competence

All management, supervisory staff, tradesmen and operatives will have a demonstrable skill level incorporating Health and Safety training. The requirements are to be confirmed by an independent audit.

• All operatives on site MUST CSCS cards • All demolition personnel to have CCDO cards (of various levels) • All plant operators to have CPCS cards • All MEWP operators to have IPAF cards

• All safety net operatives to have FASET cards • All Traffic Management operatives MUST have NRSWA accreditation • All operatives working within areas of traffic management MUST have NRSWA basic training

2.1.1.2 Site induction

Everyone working on the site or supervising works will go through a site specific Health and Safety induction process.

2.1.1.3 Consultation with the workforce

Demonstrate a commitment to worker consultation on Health and Safety issues.

2.1.1.4 Identifying, agreeing and adopting best practice

To have a system to identify, agree and adopt best practice and also to rapidly disseminate lessons learnt from Health and Safety incidents.

2.1.1.5 Publication of safety performance

Provide an annual report on company safety performance, highlighting the positive achievements of the year and also the accident statistics for the overall group.

2.1.2 Arrangements for monitoring and review

There will be monthly construction progress meetings throughout the duration of the project, which will include monitoring, and reviewing Health and Safety.

2.1.3 Emergency procedures

Emergency procedures should involve the following elements.

2.1.3.1 Emergency Plan

The Principal Contractor must prepare a suitable emergency Plan detailing the procedures to be taken in the event of serious and imminent danger, (fire and/or explosion, collapse of excavations, flooding in excavations). The procedures may require evacuation of the site or involve the rescue of an injured person. Details should be included in the Construction Phase H&S Plan. A sufficient number of competent persons should be nominated to implement those procedures.

When planning emergency procedures, escape routes and exits, etc. consider the type and location of work being undertaken, i.e. persons working in deep excavations and the number of persons, likely to be present on the site at any one time.

The emergency Plan must be agreed with the emergency services along with agreeing access restrictions for them in the event that they have to enter the site un-escorted.

2.1.3.2 Emergency escape routes

Emergency escape routes on the site should be kept clear, signed and adequately lit. The Health and Safety (Safety Signs and Signals) Regulations 1996 set standards for these signs.

2.1.3.3 First aid

The Principal Contractor must provide adequate first aid facilities and sufficient numbers of qualified first aiders based on an assessment of the risk of injury and ill health on the site.

The Principal Contractor should ensure that any necessary contacts with external services are arranged, particularly with regard to first aid, emergency medical care and rescue work.

2.1.4 Storage and unloading arrangements and areas / compounds

The Contractor must provide or arrange for adequate space on site for vehicles to be loaded and a designated ‘crushing’ area for the demolished material. The Principal contractor should draw up a traffic management plan, along with the areas for storage and welfare etc. and submit this proposal to the client / CDM-C for comment and approval. Choose delivery times to avoid busy pedestrian or traffic periods. Use a banksman to direct vehicles and assist with loading and unloading. Materials should be stored within the area agreed with the Client.

2.1.5 Welfare facilities

The Principal Contractor will be responsible for ensuring that suitable and appropriate welfare facilities are provided on the site in accordance with the Construction (Design & Management) Regulations 2007

The welfare facilities should be sufficient for everybody who is working on the site, or providing supervision and can be arranged separately or jointly with others. The facilities should be kept clean, warm and properly ventilated and have to be set up and fully functional Day 1 of the principal contractor being on site.

2.1.6 Activities on or adjacent to the site during the works

The Principal Contractor is reminded that the works are taking place in a predominately residential area and consideration must be given at all times to the requirements of the local residents especially in relation to safe access and egress to and from the site and levels of noise and dust created by the demolition works . There is also a School and Hindu Centre in the vicinity of the site which will be in use during the demolition and main works period so the above considerations will also have to be adhered to.

2.1.7 Arrangements for liaison

The construction phase Health and Safety Plan must include arrangements for on going communication and regular liaison between all parties on site, consultation with the workforce, exchange of design information between the Client, Designers, CDM Coordinator, Contractors on the site or with adjacent works/site(s).

2.1.8 Security arrangements

The Principal Contractor must take reasonable measures to ensure that no unauthorised persons enter the work area(s). Only people who are explicitly authorised by the Principal Contractor should be allowed access to the site. Authorised people should have the relevant site rules explained to them and undertake any necessary safety induction training. The perimeter hoarding should be of sufficient height and strength with regular spaced signage to prohibit and deter unauthorized access.

NB The Construction Stage Health and Safety Plan will need to indicate firm proposals for how site security will be addressed.

2.2 CLIENT CONSIDERATIONS AND MANAGEMENT REQUIREMENTS - Management Requirements

2.2.1 Development of site rules

2.2.1.1 Site rules are primarily to be developed by the Principal Contractor.

2.2.1.2 The site organisation and Client areas/overlap rules are as specified.

2.2.2 Rules required by the CDM Coordinator

2.2.2.1 Openings in floors and roofs are to be properly protected and perimeters of floors and roofs are to be protected.

2.2.2.2 The Principal Contractor shall ensure there is an effective fire alarm procedure in place for the duration of the works and that all operatives, and those visiting site, are aware of the evacuation procedure. Regular fire drills should be undertaken.

2.2.2.3 Ensure that any alarm bells etc. can be heard in all enclosed or covered work areas.

2.2.2.4 Temporary lighting must be adequate in all areas.

2.2.2.5 Ensure that properly designated escape routes are kept clear at all times, and adequate lighting and signs are provided as necessary.

2.2.2.6 Adequate temporary emergency lighting is to be installed.

2.2.2.7 The area in the possession of the Principal Contractor must be properly secured at nights, weekends and bank holidays.

2.2.2.8 Building operations must not cause risks to the public e.g. temporary footpaths and crossovers out of level or insecurely fixed.

2.2.2.9 The public must be protected from all work and associated activities along the perimeter of the site by properly secured hoardings. The type of hoarding must be approved by the Clients Representative prior to commencement on site.

2.2.2.10 Where Calor gas or air bottles (or similar pressurized bottles) are used on site, they are to be secured in a vertical position within a caged enclosure in an open-air location. There may be adverse reactions between various gases. This should be checked and such gases kept in separate locations.

2.2.2.11 Adequate fire points are to be provided by the fire officer at agreed locations on the site, the number of fire points required must be agreed with the fire officer. A fire point will consist of a minimum of two fire extinguishers and two fire buckets, which must be checked at regular intervals. The type(s) of fire extinguisher(s) must be appropriate to the type(s) of fire risk.

2.2.2.12 Work on electrical services, hot works, and other hazardous activities as may be determined must be adequately controlled using a permit to work system, (details of the Principal Contractor's permit to work systems to be included in the construction phase Health and Safety Plan).

2.2.2.13 The Principal Contractor may include reasonable rules for the management of construction work in the Information Pack, which others on the site have to follow. These may cover issues such as site induction, restricted areas, permit to work systems and emergency Plans. Any site rules must be set out in the Plan in writing.

3.1 OVERLAP WITH CLIENT'S UNDERTAKINGS

3.1.1 Division of responsibilities for Health and Safety

The Principal Contractor will be responsible for maintaining Health and Safety in areas in his possession, which is to be agreed between the Client and the Principal Contractor.

3.1.2 Inform site operatives

All site operatives must be kept informed at all times of any changes to temporary means of escape routes by the Principal Contractor before they are implemented.

3.1.3 Circulation and escape routes and protection

Drawings marked up to show temporary means of escape must be included with the Health and Safety Plan during the construction phase.

All temporary means of escape must be kept clear of obstructions at all times and provided with adequate lighting and signage at intervals on long routes and changes of direction, located for good visibility and securely fixed.

3.1.4 Signs

The Principal Contractor will be responsible for developing proposals and providing signs for the construction site to cover day to day access, information and emergency matters.

3.1.5 Permit to Work

The operation of a 'Permit to Work' system to control the isolation of, access to and handover of elements work, which require extra safety controls in place to safely execute the task in hand. E.g. working near live services, excavations etc.

The Principal Contractor is to highlight all areas of work that will need a permit to work and include this in their construction phase plan.

3.1.6 Identifiable Hazards to Future Occupiers and Maintenance Operatives.

All significant hazards to future occupiers and maintenance operatives, if applicable, are contained within the Designers hazard elimination sheets.

3.1.7 Environmental Restrictions and Existing On-Site Risks - Health Hazards

The Principal Contractor must provide (if appropriate) a list of materials or substances with significant Health and Safety hazards or risks

The Contractor will select appropriate materials as part of any Contractors design and must satisfy himself that he is aware of all precautionary information that suppliers are, by law, required to provide.

Other products contained in the tender documents such as cement and oil based paint, will require specific precautionary measures but are considered to be in common usage in the construction industry and therefore it is deemed unnecessary to specifically draw these to the Contractors attention. However, the Contractor should ensure that the requirements of the Control of Substances Hazardous to Health Regulations 2004 (COSHH) are fulfilled.

The Contractor is required to carry out a COSHH Assessment for all substances used in the execution of the project, this will include where appropriate a consideration of dusts and fumes created by the construction operations. Records of the assessment and monitoring of safeguards must be maintained by the Contractor.

4.1 SIGNIFICANT DESIGN AND CONSTRUCTION HAZARDS

4.1.1 Principles of design: precautions and sequences to be observed during construction

4.1.1.1 The general precautions and sequences to be observed during construction, where necessary, have been incorporated by the Designer within the hazard elimination sheets. These are included in Appendix B.

4.1.1.2 In addition to the precautions and sequences referred to above, the information below is included to assist the Principal Contractor in assessing their Health and Safety requirements.

The Principal Contractor is required by current legislation (Management of Health and Safety at Work Regulations 1999) to identify hazards, assess the risks and where appropriate, prepare method statements, for all operations during construction in order to eliminate, minimise or control risks.

Where it was considered appropriate, individual design disciplines have indicated on their risk assessments that the Principal Contractor must prepare a method statement for a specific activity. These are detailed below and in Appendix B if included.

The Principal Contractor must prepare initial method statements for the following works/activities:

i. Traffic management plan and phasing ii. Demolition sequence iii. Methods for reducing the levels of noise and dust iv. Competence of site operatives v. Asbestos removal

The Principal Contractor must ensure that a management system is in place to approve S/C method statements prior to relevant activities being undertaken on site and that these are incorporated into the construction phase Health and Safety Plan.

4.1.3 Construction materials

Hazards associated with specific materials and the controls required to minimise risk are dealt with in final section concerning Hazard Identification and Risk Analysis and detailed in Appendix B if included.

4.1.4 Co-ordination of future design work

4.1.4.1 Arrangements should be in place to deal with on-going design work and handling any resulting changes to the design.

4.1.4.2 Overview of Procedures

Arrangements to facilitate ongoing communication and liaison between all parties will need to be established. Construction and design team meetings are to be held at regular intervals throughout the duration of the project. The meetings will cover developing design issues and changes during the construction phase.

4.1.5 Hazard identification and elimination

4.1.5.1 Hazard identification checklists

The checklists identify the most commonly occurring hazards associated with the activity/elements of design listed, and need to be read together with the hazard elimination sheets.

The Principal Contractor is required to carry out a detailed hazard identification exercise to state required controls, provide necessary method statements and incorporate these into the construction phase Health and Safety Plan.

4.1.5.2 Overview of Risks and Control Measures

The significant hazards, risks and/or their resultant control measures are contained with the hazard elimination sheets. These have been identified by the Designers as a risk to construction workers Health and Safety, which have not reasonably been practicable to eliminate by design.

4.1.5.3 Hazard elimination sheets

The elimination sheets take into consideration previously identified hazards, assess the severity of the risk and where appropriate indicate the consideration given by Designers to Health and Safety. The Principal Contractor is required to fully address the issue of risk analysis in developing the Construction Phase Health and Safety Plan.
