National Data Clearing House

รมต.นพ.ประดิษฐ์ สินธวณรงค์ ทิศทางการทํางานในระบบหลักประกันสุขภาพชาติ ใน 10 ปีต่อไป

การพัฒนาระบบข้อมูลข่าวสารให้เป็นระบบและมีการนํา

เทคโนโลยีมาใช้เพื่อเพิ่มประสิทธิภาพและคุณภาพ เพื่อใช้เป็น

ฐานข้อมูลระดับชาติ (National Health Information) และ

ภายใน 1 ปี ตั้งเป้าหมาย มีหน่วยงานกลาง ในการจัดทําธุรกรรมการ

เบิกจ่ายระดับชาติ (National clearing house) เกิดขึ้น โดยมี

คณะกรรมการดูแลระบบที่มีส่วนร่วมของผู้เกี่ยวข้องทุกภาคส่วน หน่วย

บริการจะส่งข้อมูลทุกระบบผ่านหน่วยงานเดียวระบบเดียว28

คณะรัฐมนตรีมีมติ

๑. เห็นชอบตามมติคณะกรรมการหลักประกันสุขภาพแห่งชาติ

ครั้งที่ ๑/๒๕๕๖ เมื่อวันที่ ๗ มกราคม ๒๕๕๖ ตามที่

รัฐมนตรีว่าการกระทรวงสาธารณสุข ประธานกรรมการ

หลักประกันสุขภาพแห่งชาติเสนอ ดังนี้

๑.๑ ให้คณะกรรมการหลักประกันสุขภาพแหง่ชาติปฏิบัติหน้าที่อืน่ตาม

มาตรา ๑๘ (๑๔) แห่งพระราชบัญญัติหลักประกันสุขภาพแหง่ชาติ พ.ศ. ๒๕๔๕ ใน

การควบคุมดูแลสํานักงานหลกัประกันสุขภาพแห่งชาติ (สปสช.) ให้ปฏิบัติหน้าที่อืน่

ได้แก่ การทําหน้าทีห่น่วยงานกลางจดัการธุรกรรมการเบิกจ่ายและระบบข้อมลู

บริการสาธารณสุข (National Clearing House) ตามนโยบายการบูรณาการและ

เพิ่มประสิทธิภาพระบบประกันสุขภาพของรัฐบาล เพื่อบริหารการเรียกเก็บ

ค่าบริการสาธารณสุข (Claim center) ของสถานพยาบาลต่าง ๆ ที่ให้บริการผู้มี

สิทธิสวัสดิการรักษาพยาบาลข้าราชการและบุคคลในครอบครัว และผู้ประกันตน

รวมทั้งข้าราชการ/พนักงานส่วนท้องถิ่น และหน่วยงานรัฐอื่น ๆ ที่สมัครใจ โดย

อาศัยอํานาจแหง่พระราชบัญญัติหลักประกันสุขภาพแห่งชาติ พ.ศ. ๒๕๔๕ มาตรา

๒๖ (๑๔)

๑.๒ ให้หน่วยงานที่เกี่ยวข้อง ได้แก่ กรมบัญชีกลาง

กระทรวงการคลัง และกระทรวงแรงงานโดยคณะกรรมการ

ประกันสังคม ให้ความร่วมมือ และให้ สปสช. ทําหน้าที่เป็น

หน่วยงานกลางจัดการธุรกรรมการเบิกจ่ายและระบบข้อมูล

บริการสาธารณสุข (National Clearing House) ของ

สวัสดิการรักษาพยาบาลข้าราชการให้กับกรมบัญชีกลางและสิทธิ

ประโยชน์ด้านการรักษาพยาบาลของผู้ประกันตนให้สํานักงาน

ประกันสังคม

๑.๓ เห็นชอบให้มีกลไกคณะทํางานเพื่อผลักดันการพัฒนาระบบ

อย่างมีส่วนร่วมของกองทุนประกันสุขภาพทั้งสามระบบ ได้แก่ ระบบ

ประกันสังคม ระบบสวัสดิการรักษาพยาบาลข้าราชการ และระบบ

หลักประกันสุขภาพถ้วนหน้า โดยให้รองปลัดกระทรวงการคลังท่านหนึ่ง

เป็นประธาน และให้มีหัวหน้าหรือผู้แทนหน่วยงานราชการที่เกี่ยวข้อง

เป็นกรรมการ รวมถึงการร่วมลงนามบันทึกความร่วมมือระหว่าง สปสช.

กรมบัญชีกลาง กระทรวงการคลัง และสํานักงานประกันสังคม กระทรวง

แรงงาน รวมทั้งหน่วยงานรัฐอื่น ๆ ที่สมัครใจ เพื่อผลักดันการดําเนินงาน

ให้เป็นรูปธรรม ตั้งแต่ปีงบประมาณ ๒๕๕๗ เป็นต้นไป โดยมีระยะ

เตรียมการในปี ๒๕๕๖

๒. ให้ สปสช. รับความเห็นของกระทรวงกลาโหม กระทรวงศึกษาธิการ สํานักงบประมาณ และ

สํานักงานคณะกรรมการพัฒนาการเศรษฐกิจและสังคมแห่งชาติ ที่เห็นควรเตรียมความพร้อมด้าน

บุคลากร ด้านระบบข้อมูล และการประสานงาน การกําหนดสิทธิประโยชน์ของกองทุน

ประกันสังคม กองทุนสวัสดิการรักษาพยาบาลข้าราชการ และกองทุนหลักประกันสุขภาพถ้วน

หน้า กลไกการจ่ายเงิน และอัตราการชดเชยตามกฎหมาย ระเบียบปฏิบัติแต่ละกองทุนกําหนด

การสื่อสารทําความเข้าใจกับหน่วยงานที่เกี่ยวข้องและประชาชนให้ได้รับทราบขอ้เท็จจริง

ประโยชน์ และผลกระทบของการทําหน้าที่เป็นหน่วยงานกลางในการจัดการธุรกรรมการเบิกจ่าย

และระบบข้อมูลบรกิารสาธารณสุข (National Clearing House) รวมทั้งเห็นควรมีระบบ

ตรวจสอบการดําเนินงานที่ชัดเจนและเป็นกลางเพื่อควบคุมแนวทางการจัดสรรทรัพยากรทั้งใน

เชิงประสิทธิภาพและความเป็นธรรมให้กับผู้ที่อยู่ภายใต้แต่ละกองทุน และหาข้อตกลงเกี่ยวกับ

แนวทางการดําเนินงานให้เป็นที่ยุติ ก่อนเสนอคณะรัฐมนตรีพิจารณาให้ความเห็นชอบ ไป

พิจารณาดําเนินการด้วย

• พรฎ.ว่าด้วยวิธีการแบบปลอดภัยในการทําธรุกรรมทางอิเล็กทรอนิกส์ พ.ศ. 2553▫ วิธีการแบบปลอดภัย มี 3 ระดับ (พื้นฐาน, กลาง, เคร่งครัด)▫ จําแนกตามประเภทของธุรกรรมทางอิเล็กทรอนิกส์ (ธุรกรรมที่มีผลกระทบต่อความมั่นคงหรือความสงบเรียบร้อยของประเทศ หรือต่อสาธารณชน) หรือจําแนกตามหน่วยงาน (ธุรกรรมของหน่วยงานหรือองค์กรที่ถือเป็นโครงสร้างพื้นฐานสําคัญของประเทศ หรือ Critical Infrastructure)

“วิธีการแบบปลอดภัย”

ธุรกรรมทางอิเล็กทรอนิกส์ ประเภทต่อไปนี้• ด้านการชําระเงินทางอิเล็กทรอนิกส์• ด้านการเงินของธนาคารพาณิชย์• ด้านประกันภัย• ด้านหลักทรัพย์ของผู้ประกอบธุรกิจหลักทรัพย์• ธุรกรรมที่จัดเก็บ รวบรวม และให้บริการข้อมูลของบุคคลหรือทรัพย์สินหรือทะเบียนต่างๆ ที่เป็นเอกสารมหาชนหรือที่เปน็ข้อมูลสาธารณะ

• ธุรกรรมในการให้บริการด้านสาธารณูปโภคและบริการสาธารณะที่ต้องดําเนินการอย่างต่อเนื่องตลอดเวลา

วิธีการแบบปลอดภัยในระดับเคร่งครัด

•ผลกระทบต่อจํานวนผู้ใช้บริการหรือผู้มีส่วนได้เสียที่อาจได้รับอันตรายต่อชีวิต ร่างกาย หรืออนามัย▫ ต่ํา: ไม่มี▫ ปานกลาง: ผลกระทบต่อร่างกายหรืออนามัย 1-1,000 คน▫ สูง: ผลกระทบต่อร่างกายหรืออนามัย > 1,000 คน หรือต่อชีวิตตั้งแต่ 1 คน

•ผลกระทบด้านอื่นๆ• ถ้ามีผลประเมินที่เป็นผลกระทบในระดับสูง 1 ด้าน ให้ใช้วิธีการแบบปลอดภัยระดับเคร่งครัด

ระดับผลกระทบกับวิธีการแบบปลอดภัย

• อ้างอิงมาตรฐาน ISO/IEC 27001:2005•มีผลใช้บังคับเมื่อพ้น 360 วัน นับแต่วันประกาศในราชกิจจานุเบกษา (19 ธ.ค. 2555) คือ 14 ธ.ค. 2556
