Preventing Identity Theft By Tom Orecchio

According to a Bureau of Justice Statistics report, 16.6 million people experienced identity theft in 2012 with financial

losses totaling $24.7 billion. Being a victim of identity theft can have severe financial consequences. We often find

that most people don’t realize they are a victim of identity theft until they are making a large purchase like a house or a

car and issues show up on their credit report. When someone steals your personal information and uses it, it can cause

havoc with your finances, reputation, and credit history. Resolving it can take a lot of time and money.

Here are some tips to help you prevent becoming a victim of identity theft: o Review your credit report on a regular basis to ensure that you catch any potential identity theft activity. Check the

credit report of your children as well; it’s possible that kids can fall victim to identity theft. o Use unique passwords and passcodes for online accounts and banking. Use different passwords for different sites

instead of just one common password. In addition to using unique passwords, use a password manager and enable

dual-factor authentication wherever possible. In cases where this involves security questions (“Where did you meet

your spouse?”) choose things that no one can find out about you through social media or Google. o Review your health insurance medical claims record to ensure that no one else is using your insurance card. o Don’t carry all your important documents with you! Leave your social security card, birth certificate, and passport

at home. o Don’t carry all your credit cards or any blank checks. o Check credit card statements carefully monthly to make sure you agree with all the purchases listed. o Be careful when you gets calls or emails asking for personal information. o When you order new checks, do not request them to be delivered in the mail. Go directly to the branch office. This

will prevent someone from intercepting your mail and using your checks. o Shred all paper documents that have private or sensitive information. o Chose paperless statements sent directly to your email or smartphone. o Protect your personal computer with a strong and updated firewall, an anti-virus program, and an anti-spyware

program o If you are getting rid of your computer or cell phone, make sure to delete all your personal information; the best

bet would be to restore it to factory settings. o Be careful when you shop online. While it may be easy to store your credit card on a particular retailer’s site, it also

could be vulnerable if the website is hacked. Keep a separate credit card and email account designated for online

purchases; this will make it easier to track activity and also to cancel if you see any inexplicable activity. o When you are at the ATM, make sure no one behind you who can see you type in your password; to be safe, cover

the account balance information as well as the key pad. o Educate your teenagers on how to protect themselves from identity theft. In particular, remind them not to

overshare on social media (e.g., address, phone number, etc.) and what private information should of course be

kept private.

However, that does not happen all the time, it only happens some of the time. When the market is in an uptrend, we tend to think that it will always go up, so we choose to hang around and continue to build our gains. However, those “gains” are not true gains they are only

unrealized gains. An unrealized gain is a paper gain. It is a true gain only when you sell the position and take your profits. You and I know that your gains can become “break even” or even “losses”. It’s a challenge to take your profit and move on. Wise investors understand that what goes up must come down and vice a versa. Since the market is currently at an all-time high, might it make sense to take your profits now? Before those profits disappear?

The biggest barrier to your practice success may be a set of beliefs about what constitutes

conduct becoming to a physician. Can a physician ethically target a group of best-fit

patients, engage in marketing activities and make practice choices based on profitability?

Our healthcare system is constructed around a core principle: we have a duty to deliver

medical care regardless of a patient’s ability to pay. Money has become the ultimate taboo

topic among physicians.

When I first set up my practice-- in the pre-managed care era-- I could not ask colleagues

how much they charged for a surgical procedure; if they answered, they would be in

violation of anti-trust laws. Further, I had no idea how much patients paid out-of-pocket for

the medication I prescribed.

It doesn’t take too much imagination to figure out why health care costs exploded.

It’s time to rethink our relationship to money and marketing and other taboo topics. You cannot serve patients if you

cannot meet your payroll.

If you run a profitable practice that works for you, you’re in a better position to reach out to the underserved members

of your community if that’s what calls to you.

Here are some lessons I have learned about money and marketing:

• Money is neither good nor evil; it’s a way of exchanging value.

• Patients are willing to pay for things they value.

• It’s our job to help patients understand the value of the services we offer.

• Marketing is nothing more than clearly communicating the value you deliver.

• The bigger your impact, the bigger your income.

In other words, you don’t have to choose between running a profitable practice or dedicating yourself to service. You

can have both.

And you can thrive no matter what. Practices that reinvented themselves under managed care did well. Even today

when the news spells out the economic gloom, the luxury market is rebounding.

Reinvention will become the operative word for practices that thrive in the upcoming era of the Affordable Care

Act. I'll show you how.

Roth Conversions AKA “Back Door” Roth IRA By Scott Cody, Latitude Financial Group, LLC

Many physicians seem to miss this little known planning opportunity.

Why?

In part, because so few people are taking advantage of a contributory Roth IRA to begin with. According to the

Investment Company Institute, 18.6 million U.S. households (about 15.7%) owned Roth IRAs in 2011.

What are the general rules for funding contributory Roth IRAs?

There are three ways to fund a Roth IRA--you can contribute directly, you can convert all or part of a traditional IRA to

a Roth IRA, or you can roll funds over from an eligible employer retirement plan. In general, you can contribute up to

$5,500 to an IRA (traditional, Roth, or a combination of both) in 2013 ($6,500 if you'll be age 50 or older by December

31). However, your ability to make annual contributions may be limited (or eliminated) depending on your income

level ("modified adjusted gross income," or MAGI), as shown in the chart below:

Roth IRA even if you're 70½ or older. However, your contributions generally can't exceed your earned income for the

year (special rules apply to spousal Roth IRAs).

Important changes since 2010

Prior to 2010, you couldn't convert a traditional IRA to a Roth IRA (or roll over non-Roth funds from an employer plan

to a Roth IRA) if your MAGI exceeded $100,000 or you were married and filed separate federal income tax returns.

The Tax Increase Prevention and Reconciliation Act (TIPRA), however, repealed the $100,000 income limit and

marital status restriction, beginning in 2010. What this means is that, regardless of your filing status or how much you

earn, you can now convert a traditional IRA to a Roth IRA. (There's one exception--you generally can't convert an

inherited IRA to a Roth. Special rules apply to spouse beneficiaries.) And don't forget your SEP IRAs and SIMPLE

IRAs. They can also be converted to Roth IRAs (for SIMPLE IRAs, you'll need to participate in the plan for two years

before you convert). You'll need to set up a new SEP/SIMPLE IRA to receive any additional plan contributions after

you convert.

How do you convert a traditional IRA to a Roth?

Converting is easy. You simply notify your existing traditional IRA trustee or custodian that you want to convert all or

part of your traditional IRA to a Roth IRA, and the custodian/trustee will provide you with the necessary paperwork.

You can also open a new Roth IRA at a different financial institution, and then have the funds in your traditional IRA

transferred directly to your new Roth IRA. The trustee/custodian of your new Roth IRA can give you the paperwork

that you need to do this. You can also open a new Roth IRA at a different financial institution, and then have the funds

give you the paperwork that you need to do this. If you prefer, you can instead contact the trustee/custodian of your

traditional IRA, have the funds in your traditional IRA distributed to you, and then roll those funds over to your new

Roth IRA within 60 days of the distribution. The income tax consequences are the same regardless of the method you

choose.

Calculating the conversion tax

When you convert a traditional IRA to a Roth IRA, you're taxed as if you received a distribution, but with one

important difference--the 10% early distribution tax doesn't apply, even if you're under age 59½. However, the IRS

may recapture this penalty tax if you make a nonqualified withdrawal from your Roth IRA within five years of your

conversion. If you've made only nondeductible (after-tax) contributions to your traditional IRA, then only the

earnings, and not your own contributions, will be subject to tax at the time you convert the IRA to a Roth. But if you've

made both deductible and nondeductible IRA contributions to your traditional IRA, and you don't plan on converting

the entire amount, things can get complicated. That's because under IRS rules, you can't just convert the

nondeductible contributions to a Roth and avoid paying tax at conversion. Instead, the amount you convert is deemed

to consist of a pro rata portion of the taxable and nontaxable dollars in the IRA. For example, assume that your

traditional IRA contains $350,000 of taxable (deductible) contributions, $50,000 of nontaxable (nondeductible)

contributions, and $100,000 of taxable earnings. You can't convert only the $50,000 nondeductible (nontaxable)

contributions to a Roth, and have a tax-free conversion. Instead, you'll need to prorate the taxable and nontaxable

portions of the account. So in the example above, 90% ($450,000/$500,000) of each distribution from the IRA

(including any conversion) will be taxable, and 10% will be nontaxable. You can't escape this result by using separate

IRAs. Under IRS rules, you must aggregate all of your traditional IRAs (including SEPs and SIMPLEs) when you

calculate the taxable income resulting from a distribution from (or conversion of) any of the IRAs.

Some experts suggest that you can avoid the pro rata rule and make a tax-free conversion if you take a total

distribution from all of your traditional IRAs, transfer the taxable dollars to an employer plan like a 401(k) (assuming

the plan accepts rollovers), and then roll over (convert) the remaining balance (i.e., the nontaxable dollars) to a Roth

IRA. The IRS has not yet officially ruled on this technique, so be sure to get professional advice before considering this.

Using conversions to make "annual contributions"

Unfortunately, TIPRA didn't repeal the income limits that may prevent you from making annual contributions to your

Roth IRA. But if your income exceeds these limits, and you want to make annual Roth contributions, there's an easy

workaround: you can make nondeductible contributions to a traditional IRA, as long as you haven't yet reached age

70½. You simply make your annual contribution first to a traditional IRA, and then convert that traditional IRA to a

Roth. There are no limits to the number of Roth conversions you can make. (But again, you'll need to aggregate all of

your traditional IRAs--including SEPs and SIMPLEs--when you calculate the taxable portion of the conversion.)

Employer retirement plans

You can also roll over non-Roth funds from an employer plan (like a 401(k)) to a Roth IRA. Prior to 2010, the income

limits and marital status restrictions also applied to employer plan rollovers to Roth IRAs (commonly referred to as

conversions). As with traditional IRA conversions, TIPRA removed these restrictions beginning in 2010, and now

anyone can roll over funds from an employer plan to a Roth IRA, regardless of income level or marital status. Like

traditional IRA conversions, the amount you convert will be subject to income tax in the year of conversion (except for

any after-tax contributions you've made).

Is a Roth conversion right for you?

The answer to this question depends on many factors, including your current and projected future income tax rates,

the length of time you can leave the funds in the Roth IRA without taking withdrawals, your state's tax laws, and how

you'll pay the income taxes due at the time of the conversion. And don't forget--if you make a Roth conversion and it

turns out not to be advantageous (for example, the value of your investments declines substantially); IRS rules allow

you to "undo" the conversion. You generally have until your tax return due date (including extensions) to undo, or

"recharacterize," your conversion. For most taxpayers, this means you have until October 15, 2014, to undo a 2013

Roth conversion.

Debt Free is Still the American Dream for Most Being out of debt and retiring financially secure represent the American Dream for a majority of Americans, and most

are optimistic that they can achieve it, according to the 2014 Credit.com American Dream Survey. Retiring financially

secure at age 65 represents the ultimate American dream for 36 percent of those surveyed, while getting out of debt

came in at a close second, with 25 percent of respondents choosing it as their definition of the "American Dream."

The majority of survey respondents are optimistic about their ability to achieve their version of the American Dream,

with 16 percent saying they have already achieved it and another 50 percent indicating it is within their reach. Only 24

percent say it is it not within their reach.

Other significant findings from the survey include: • Dreams are personal and varied. After retirement and getting out of debt, the other top definitions of the American Dream included owning a home (17 percent), joining the "one percent," (five percent), graduating from college (three percent), and paying off student loans (two percent). • Getting out of debt is a top priority. When asked about their top financial priority for the next year, at the top of the list was paying off credit card debt, with 19 percent of consumers choosing that option. Close behind that, another 18 percent chose "being debt-free." Other responses included investing for retirement (12 percent), saving for a major purchase (12 percent), buying a home (nine percent), paying off student loans (six percent), retiring financially secure at 65 (six percent), sending kids to college (five percent) or paying off the mortgage (four percent). • Most are fairly optimistic about their ability to pay off debt. When it comes to being debt-free, 79 percent said they are very or somewhat likely to achieve that milestone in their lifetimes, with only 18 percent indicating that it was not very or not at all likely that they would be debt-free in their lifetimes. Not surprisingly, those with student loan debt were most likely to say it was not very or not at all likely that they would be debt-free in their lifetimes (28 percent), followed closely by those who have been unemployed in the past three years (24 percent). An alternative to Deferred Compensation Are you a business owner or partner of a practice? Are you looking for additional ways to put away additional monies

while getting both a corporate tax deduction as well as a personal tax deduction?

Perhaps, you’ve maxed out your 401(k) & Profit Sharing Plan, maybe even your “Cash Balance” pension plan? Maybe

some partners of your practice cannot agree upon adding a “Cash Balance” pension plan?

Do you need to fund a buy-sell agreement? Are my business partners required to participate in order to have such a

plan? Is this Deferred Compensation? Similar, but no it’s not deferred compensation.

Consider this alternative to deferred compensation. This plan is for business owners with who have consistent

earnings. This plan offers one hundred percent deduction for dollars going into to it to the business. The individual

can deduct seventy percent of the contributions personally. The dollars inside the plan are protected federally from

litigation. The funds inside the plan are very conservatively invested and will not decline due to stock market

conditions. The plan offers a death benefit which can help with any buy-sell agreement.

What is the downside to this plan? Minimum contribution of $50,000 per year for 5 years otherwise you forfeit the

The Recruiters Corner Residents and fellows are preparing for their final year of medical training and the long and

sometimes frustrating interview process seems to begin earlier every year. Hopefully at the end

of the process lies a rewarding practice opportunity with an equally rewarding compensation

package. And because many practicing physicians have contract expiration dates that match

up with their earliest interview cycle, as many as 40,000 practicing physicians are engaged in

testing the market place for a new career opportunity as well.

In theory at least each of these journeys will end with an offer. Surprisingly fewer than half of the candidates will

negotiate the actual compensation component of the offer. Let’s set the record straight. Most offers are a starting

point for negotiation and the employer will be neither surprised nor offended with a candidate that approaches the

employment process as a mutually beneficial event. But as with most things in life there are right ways and wrong

ways to approach the process.

According to Victoria Pynchon, a career consultant and regular blogger at Forbes, one of the most common mistakes

professionals make when discussing an offer is to start with an apology. “I’m sorry to ask but is the compensation you

quoted cast in stone?” Why are you sorry? What’s the worst that the hiring authority can say?

Many compensation packages are based on the compensation plan that includes other doctors in the practice. The

practice is not going to bust the model and upset the other physicians. At the same time, starting compensation for a

new physician, especially a talented addition to the group, is often a range rather than a number. One shouldn’t expect

a practice manager to begin what they expect to be a negotiation at the top end of that range.

It’s hard to know what you can ask for if you don’t know what you are worth. And it’s hard to know what you are worth

if you haven’t done some basic research on what physicians with your training, experience and skill sets demand in the

market place. And don’t forget that geography can play a big part in pushing that number up or down.

If you find yourself bumping up against a comp ceiling that is locked in by the compensation of other members of the

group, explore the possibility of one-time, non-salary, signing bonuses for things like medical education debt

reduction, relocation or special clinical skills or certifications.

As a graduating resident or fellow it can be hard to project confidence in your true worth. And sometimes you want a

particular opportunity so bad that you don’t want to risk the offer by asking for an equitable compensation package.

But two or three years down the road you may find that number one, the opportunity has lost some of its glitter and

number two, you have locked yourself into the low end of the compensation pattern when it comes time to re-negotiate

a contract.

Remember, if you lack the confidence to protect your interests, fake it. Take a deep breath, maintain eye contact and

suggest in a firm but respectful way that you know what you are worth, you know what the market will bear and the

offer need some work. In a critical physician shortage environment like we are experiencing, a practice is not going to

walk away from talent just because they have some starch in their backbone.

Personal Finance Do your part in protecting your identity

Make it harder for someone to steal your personal information. But be prepared in case the worst happens:

Do not sign the back of your credit cards. Instead, write PHOTO ID REQUIRED.

When you are writing checks to pay on your credit card accounts, do not put the complete account number on

the Memo Line. Instead, just put the last four numbers.

Put your work phone # on your checks instead of your home phone. If you have a PO Box, use that instead of

your home address. If you do not have a PO Box, use your work address.

Never have your SS# printed on your checks.

Photocopy the complete contents of your wallet. Do both sides of each license, credit card, etc. By so doing you

will know what you had in your wallet and all of the account numbers and phone numbers to call and cancel.

Keep the photocopy in a safe place.

When traveling abroad, carry a photocopy of your passport.

In case your wallet is lost or stolen here are some things you should do WITHIN HOURS of discovering the loss:

Notify the credit card companies. The key here is having the toll free numbers and your card numbers

available. If you followed the above procedure you will have a complete copy at your disposal.

Call the 3 national credit reporting organizations immediately to place a fraud alert on your name and also call

the Social Security fraud line number. The alert will inform any company checking your credit that your

information was stolen. They will be required to contact you by phone to authorize new credit.

Here are the numbers you will need:

o Equifax: 1-800-525-6285

o Experian (formerly TRW): 1-888-397-3742

o Trans Union: 1-800-680 7289

o Social Security Administration (fraud line): 1-800-269-0271

File a police report immediately in the jurisdiction where your credit cards, etc. were stolen. This proves to

credit providers you were diligent.

Print out these instructions and file them with your photo copied information.

Real Estate – Tips & Tools

Are you ready to make a thirty year commitment?

This month’s article is really aimed at the young physician, fresh out of residency or fellowship first time buyer. Our

objective is helping you avoid some of the mistakes that first time buyers often have to live with for years. It is

remarkably easy for the search for your dream home to turn into a nightmare. So before you even begin the process

here are some words of advice.

The chances are that as a freshly minted physician you have not had time to secure the advice of a financial advisor and

put together the frame work of a long term plan. After all, the ink is still wet on your contract, children are either still a

work in progress or they are years away from college. STOP! If you are even considering signing a 30 year mortgage,

an obligation that at least in theory will last until you are in your late 50’s, without making it one of the pillars of your

financial plan you are just asking for trouble.

And don’t let anyone tell you that financial advisors won’t accept a young professional with a mountain of medical

education debt and virtually no net worth. There are dedicated professionals out there that will be happy to start the

journey as your guide and mentor. Many of them can be found at www.mdpreferredservices.com, an online resource

center for young physicians.

A financial advisor can help you calculate how much home you can afford and how much home you should afford. He

or she can make sure that you take all of your expenses into account when establishing a budget. That’s right, a

budget…even newly minted, soon to be wealthy physicians need a budget. And they can often help you find financing

even with your medical education debt. Here are a couple of useful numbers that you can compute yourself that can at

least set realistic expectation of the reception you are likely to get when seeking a mortgage.

Your monthly mortgage is always more than just the principle and interest. You are about to join the ranks of real

estate tax payers and in many counties this can really be an eye opener. In some metro areas taxes can easily run to 4%

of the value of the home. This gets divided by 12 and added to the mortgage. Hazard insurance is good for another

sizeable chunk of monthly change. And don’t forget association fees and private mortgage insurance if your down-

stroke is going to be less than 20%. Once you know what kind of monthly nut you are likely to be looking at, here is

how the money lenders are going to determine how deserving you are.

They will work from your gross income before taxes. They are going to look at two percentages of that number. First

they will determine your “front end ratio” to see if the monthly mortgage payment as described above is a suitable

percentage of your monthly gross income. The going rate is usually somewhere south of 28%.

Next they will calculate a “back end ratio”. This takes into account all of your projected monthly expenditures

including cash expenditures for minor items. This is where your financial advisor can really help. He or she will help
