Hillsborough County SchedulesNeighborhood Cleanup forSugarcreek, Buffalo Estates

Hillsborough County has scheduled a neighborhood cleanup fromFebruary 18-21 and February 25-28 in the Sugarcreek and BuffaloEstates subdivisions, located in the East Lake-Orient Park area.

The cleanup boundaries are Martin Luther King Jr. Boulevard onthe north, East 26th Avenue on the south, Orient Road on the west, andDanny Bryan Boulevard on the east.

During the cleanup, a central collection site will be located at 8008 E.Chelsea St. in Tampa. Dumpsters will be available from 8 a.m. to 5:30p.m. Residents who live within the cleanup boundaries may dispose ofvarious items at no charge by showing proof of residence, such as aFlorida Driver’s License, Florida Vehicle Registration, or a utility bill.

Hillsborough County LibraryServices Welcomes New Director

Andrew S. Breidenbaugh, who began working at the Tampa-Hillsborough County Public Library system almost 18 years ago as alibrarian, took over as director on Monday, February 9.

Breidenbaugh began his library career as an adjunct librarian inthe University of South Florida’s Tampa Campus Library. He startedworking for the Tampa-Hillsborough County Public Library systemin April 1997 and has held a variety of positions, includingCoordinator of the Electronic Reference & Information Department;Reference & Information Services Coordinator; and Chief Librarianfor the South County Region, South Tampa Region and the John F.Germany Public Library.

Working in public libraries at a time when the Internet was still anovelty and long before social media platforms were created,Breidenbaugh has been instrumental in bringing online databases,eBooks and downloadable/streaming movies, music, and courses tocustomers. Most recently, he was Manager of Library Technology &Collections, coordinating print and digital collections, and onlineaccess through hcplc.org - the Online Library.

As director of Library Services, Breidenbaugh will be overseeing alibrary system with a $36.5 million annual operating budget, whichprovides materials and resources to customers through a network of 25libraries and two mobile outlets. Services include both traditional andelectronic books, Internet access, Talking Books for the visuallyimpaired, DVD’s, music, online databases, cultural and educationalprograms, collaborative technology and facilities for communitymeetings. In 2014, the library system circulated more than 10 millionitems and hosted 8 million user visits. Breidenbaugh’s annual salarywill be $128,000.

Breidenbaugh has a Master of Library Science from USF, a master’sdegree in Middle Eastern History from the University of Maryland,and a bachelor’s degree in History & Asian Studies from TowsonUniversity in Maryland. He has also served as an adjunct facultymember at USF’s School of Library & Information Science. He is anactive member of the American Library Association and the FloridaLibrary Association. He also volunteers with First Robotics, judgingfor First Tech Challenge events in the Tampa Bay area.

Hillsborough County is launching the new HELP(Health, Education and Life Planning) and DomesticPartnership registry tomorrow. Residents can nowdesignate a “support person” or domestic partner to makedecisions regarding hospital and correctional facilityvisitation, healthcare and burial, emergency notificationand education for dependent children. Information will beaccessible online at any time by medical, education, anddetention staff.

The registries will help ensure timely visitation anddecision making power of the designated person for anincapacitated person, eliminating confusion and delay.The registry is especially helpful for unmarried couples,adults caring for elderly friends and relatives, andindividuals who want to designate someone other thantheir next-of-kin as a decision maker.

The HELP and Domestic Partnership Registry isadministered by the Hillsborough County Clerk ofCourt, who receives and records registration. The Cityof Tampa’s Domestic Partner Registry will be rolledinto the Hillsborough County registry. New registrantscan apply at either the Tampa City Hall or at anyHillsborough County clerk location. Individuals whoare already registered with the City of Tampa mustapprove the transfer of their records by filing a requestwith the City Clerk.

How does it work?There are two affidavits you can download from the

Hillsborough County Clerk’s website. Choose the one thatis right for you. The filing fee for either form is $50.

• Option #1 is the HELP Affidavit. It is designedmainly for relatives or acquaintances. This affidavitallows you to specify which powers you want to grantyour surrogate, known as your “support person.’’ Forexample, you may want to give him/her the power tomake medical decisions for you, but not the power tomake education decisions for your children.

• Option #2 is the Domestic Partnership Affidavit. Thisis for unmarried couples who are in a committedrelationship, share a residence and are financially co-dependent. It automatically grants an array of rights,including hospital and correctional facility visitation,medical decisions, burial decisions, and educationdecisions for dependent children. Instructions on howto fill out the forms and register them are on theHillsborough County Clerk’s website.

What is Domestic Partnership?A Registered Domestic Partnership is the entity formed

by two persons who have met the criteria listed inHillsborough County Ordinance No. 14-32 and filed anAffidavit of Domestic Partnership with the Clerk. Therequirements are as follows:

• Each person is at least eighteen (18) years of age andcompetent to contract;

• Neither person is married, nor is a partner in adomestic partnership relationship or a member of acivil union with anyone other than the co-applicant;

• They are not related by blood as defined in FloridaLaw;

• Each person considers himself or herself to be amember of the immediate family of the co-applicantand jointly responsible for the maintenance andsupport of the domestic partnership;

• Applicants reside together in a mutual residence;• Each person declares the co-applicant to act as his/her

healthcare surrogate as provided in Chapter 765,Florida Statutes and, if determined incompetent, to actas their preneed guardian pursuant to Chapter 744,Florida Statutes; and

• Each person declares the co-applicant as his/her agentto direct the disposition of their body after death;

• Each person agrees to be jointly responsible for eachother’s basic food and shelter; and

• Each person agrees to immediately notify the Clerk’sOffice, in writing by filing an Affidavit ofTermination, if the terms of the Registered DomesticPartnership are no longer applicable or if one of thedomestic partners wishes to terminate the domesticpartnership.

What is the purpose of the HELP Registry?If you are married, your spouse would probably take

over as decision maker if you were incapacitated. But whowould fill that role if you are not married? TheHillsborough County HELP Registry is designed to allowyou to identify a surrogate to make decisions for youwhen you are incapacitated or otherwise unable to makethem yourself. These include decisions regardingvisitation in hospitals or correctional facilities, emergencynotification, health care and burial decisions, andeducation decisions for your dependent child or children.

Where and how do I register?Registration for both HELP and Domestic Partnerships

will begin on February 12, 2015, at 8:00 a.m. and will beavailable thereafter on all days and times provided for bythe Hillsborough County Clerk of the Circuit Court andthe City of Tampa. The Health, Education and LifePlanning (HELP) Affidavit is available on the Clerk’swebsite and in the Clerk’s offices and City of Tampa’soffice listed below.

The declaration of registered domestic partnership isrequired to be signed by both domestic partners under thepains and penalties of perjury, signed by two (2) witnessesand notarized.
