2020 SEMESTER 1
This form is for UNDERGRADUATE admissions (Level 1) only. Use a different form if applying for (1) re-admission, (2) postgraduate study, (3) or if you are a school leaver current Grade 12 or (4) if you are seeking advanced standing in a study program.
1. Follow ALL these instructions exactly. If you do not provide all requested information, your application will be rejected.
2. You can only submit ONE application for admission for entry to any semester/year. If you submit 2 or more applications, ALL will be disqualified, and you will have to wait another year to re-apply.
3. Always print in black/blue, BLOCK letters so that your writing can be quickly, easily, and accurately read by the evaluators.
4. Double check all contact information that you provide. Postal addresses provided must be a valid address. Provide valid mobile phone number that will reach you directly even if your phone is lost or stolen. TAKE NOTE THAT UNIVERSITY WILL ONLY COMMUNICATE RESULT OF APPLICATION FORM TO ALL APPLICANTS THROUGH EMAIL. IT IS COMPLUSORY FOR YOU TO HAVE A VALID EMAIL ACCOUNT.
5. The following must be attached to the application if you want to be considered for study:
* Recent passport size photo that is clear and completely filled by your uncovered head. Applications lacking photos or with poor ones will be rejected. Staple (NO glue) the photo into the designated rectangle of the form‟s starting page.
* Academic record transcripts/certificates that show grades/achievements (high school, college/university studies, vocational training, distance/open learning courses). Submit photocopies only. All these documents must be officially certified within the current year (2019) by a registered COMMISSIONER OF OATHS employed by government (including district court houses).Statutory Declaration Form are NOT accepted. The Commissioner must clearly state on each photocopy that they have inspected the photocopy of the documents and confirm that: (A) the original copy shows no evidence of fraudulent alterations, and (B) the photocopy is a true copy of that unaltered original. Your entire application will be rejected if BOTH those assessments are not clearly stated and officially stamped by the Commissioner or if you use a Commissioner not employed directly by government. * Two character references. Do NOT submit “Reference letters”! Find 2 referees who are willing to fill out the form attached to the end of this application. Referees must be established adults who are not close relatives but who know you and your capabilities well. Evaluations must be less than 2 years old. Referee‟s name and signature must be officially witnessed by a professional colleague from the same organisation and its seal/stamp inserted to officially certify the reference forms. Any document/s produce after the application been submitted will NOT be accepted.

6. Required ordering of Documents: Remove this instruction sheet, then arrange your submission in this order: (1) application form, (2) Character reference Form #1, (3) character reference Form #2, (4) all certificates, (5) all transcripts, (7) miscellaneous
attachment/s.
WARNING: Any false, misleading, or altered information found on your application or attachments (we double-check with source schools) will PERMANENTLY BLACK-LIST you from ever studying at the PNG University of Technology.
7. Nonrefundable application processing fee of K50.00, deposited into BSP Lae Branch Account Number 1000 386 581. Take the
original (no photocopy allowed!) deposit slip that clearly confirms into what account and when the deposit was made and staple it onto the upper right hand rectangle space of the application form‟s starting page.
8. Application Submission: Bring in person or mail your completed application with all required attachments to:

NOfaxed and NO e-mailed applications will be accepted. Applications must reach PNGUOT no later than 31 July 2019.Applications arriving after that day will be rejected. To better ensure that your application is received on time, (A) deliver it
in person, (B) mail it using PNG Post at least 3 weeks in advance of the deadline, or (C) use a guaranteed mail delivery service (EMS, DHL, TNT, Federal Express).
9. Initial Vetting: All applications for first time study at PNGUOT must pass a first screening for application completeness, and whether your credentials/qualifications meet the minimum requirements for the study programs you applied for.
10. Entrance Exams: Only successful applicants will receive a written notification to pay the required fee for the mandatory PNGUOT Entrance Exams. It is your responsibility to take exams in the specific subjects required by the study programs you applied for. Exams are usually given in November at these locations:
