The Physicians’ Care Clinic has much good news to share with our partners, donors, volunteers, and community friends as we report on 2014. The past year was our 22nd year of operations at Physicians’ Care, now the oldest and largest charitable clinic in DeKalb County.

When 2014 began, the Affordable Care Act (ACA) – although hotly debated politically – offered medical insurance to thousands of Georgians who previously could not afford coverage. This was good news to health professionals since people with insurance are more likely to seek needed medical care. But the Board of Directors and staff of Physicians’ Care wondered if our charitable clinic would be needed in the years to come? Will our mission continue to be relevant?

The answer is a resounding yes.

As the ACA was enacted Georgia joined other states and opted not to expand Medicaid. Although people with low incomes who do not qualify for Medicaid can purchase insurance through state and federal exchanges, there is a huge gap between what people can afford and what is available to them.

The least expensive policies include large deductibles and limited access based on geographic location.

Many thousands of our neighbors in DeKalb County fall between the cracks: they don’t qualify for Medicaid, but they can’t afford the insurance premiums, co-pays, and deductibles for office visits, tests, and medications.

Although the PCC experienced a decline in patient visits during 2014, we are already seeing a rebound in early 2015. Many patients are now returning to the clinic who are unable to afford insurance or other associated costs.

Meanwhile, the PCC outcomes for 2014 remained strong: the number of patients increased by 10% adding 142 new patients. Much more patient data is included in the enclosed 2014 Annual Report.

As we enter our 23rd year, we are deeply grateful to the over 200 volunteers, to the medical specialists who work in partnership with the PCC, to DeKalb Medical, the DeKalb County Health Department, the DeKalb Medical Society, and our partner American Health Imagining.

hiGhliGhts of the yeARThe 2014 outcomes for the Physicians’ Care Clinic reflect the impact of our clinic on the DeKalb County community:

• 1,415 patient appointments

• 539 unique patients served

• 392 referrals for medical specialty care

• 3,314 prescriptions

• 117 mammograms and breast biopsies

• 22 patient surgeries

• 150 flu vaccine vouchers for patients from Publix pharmacies

• 2,500 diagnostic tests

• 355 patients received one-on-one or classroom education for diabetes/hypertension

• 200+ volunteers provided more than 3,700 hours of in-clinic care

• $1 million of in-kind healthcare services for our patients provided by clinic partners

maintain a strong balance sheet and absorb the demand for services• Recruited 31 additional professional volunteers• Improved our chronic disease management program and quality measures

GeoRGiA ChARitABle CliniCsGeorgia has approximately 1.3 million citizens remaining uninsured under the Affordable Care Act (ACA) with many more lacking coverage as they find premiums unaffordable based on their family finances.

Patients are disproportionately affected by barriers to coverage because of an insufficient number of providers as well as a very high prevalence of chronic illnesses. The Georgia Charitable Care Network and its member clinics provide service and support as a crucial safety net for Georgians who otherwise would have no access to health care services.

Our most vulnerable citizens need access to high quality, affordable health care. Charity clinics (like Physicians’ Care Clinic), are an alternative for cost-efficient health care, a savings for taxpayers. Each clinic is different because no two communities are alike in their needs, assets and resources. Guaranteed sustainability is largely a myth; every health care organization and provider operating today must continue to work hard and adapt their financing/funding and delivery systems to an ever-changing healthcare and consumer landscape. These clinics thrive because of an entrepreneurial spirit and innovative approach to serving the community.

We know that access to care for vulnerable populations is a profound challenge not yet adequately addressed by the ACA. Georgia’s current number of 101 free and charitable clinics and health centers provide health services to Georgia’s uninsured and underserved across all regions of the state, continuing to be essential providers for these vulnerable populations.

ChARitABle CliniCs’ pAtients AnD iMpACtCharitable Clinics provide health services to Georgia’s uninsured and underserved. In 2013, 193,938 unique patients were served with total of 482,146 patient visits.

• Every $1 spent in support of charity care provides $5.25 in services, which saved taxpayers over $225 million in 2013.

• $66 million - Value of prescriptions provided• $229 million - Uncompensated care• $67.3 million - Value of specialty care visits provided• $90.48 - Average cost of 1 year of patient care• 14%-20% - Non-urgent ER usage reduction• 326,178 hours - contributed by volunteers

GeoRGiA’s ChARitABle CARe pAtients:• 90% are uninsured (100% of PCC patients are uninsured)• 75% are employed, sometimes holding more than one job• 60% of patients are living at less than 100% of the Federal Poverty Level• 78% of patients have one or more chronic illnesses. The most common diagnosis is

hypertension, followed by diabetes and COPD• 95% of clinics provide primary care services that emphasize prevention• 88%of clinics provide health education services

hoW We MAtCh upSeveral State Legislatures appropriate funds for charitable clinics, recognizing that charity care is a cost-effective model that will fill the gap where private insurance, government programs and private pay leave a void.

At this time, Georgia clinics receive no state funding. State dollars made available to these clinics would make a significant contribution towards improving Georgia’s healthcare safety net.
