does every branch of the medical profession throughout theEmpire, and with full knowledge of the grave issuesinvolved, feels bound to place on record its earnest hopethat the Dogs’ Protection Bill will not pass into law, as ifenacted it will place this country at a great disadvantageas compared with all other civilised countries, and willpractically compel our ablest workers to seek opportunitiesfor preventive and curative research abroad."

CREATION OF DIRECTORATES OF HYGIENE AND PATHOLOGY

WITHIN THE ARMY MEDICAL DEPARTMENT.

THE sanction of the Treasury has just been given to ascheme put forward by the Director-General, Army MedicalService, with a view to linking up, under a definitely plannedorganisation, the activities of the different departments andindividuals hitherto concerned with the various problems ofpreventive medicine, pathology, and tropical diseases bear-ing upon the health of the Army in peace and war. Theneed for such a reorganisation had long been felt by manyof those officers of the R.A.M.C. engaged in hygifmic orpathological work, and efforts had already been made, priorto the war, towards an improved system. It was not, how-ever, until the progress of the war brought wider recognitionof the far-reaching benefits of scientific research, and afuller realisation of the amount of training, work, andorganisation required to obtain the best results from suchresearch, that the urgent need for a better system becamemanifest.The principle objects which were in view in the formula-

tion of the scheme outlined below were as follows -

1. To utilise to the fullest extent the benefits of newknowledge, as these become available, through closecooperation between those working within and outside theArmy and by initiating and controlling research work inconnexion with problems affecting the health of the troops.

2. To raise the standard of sanitary and pathologicalwork in the Army by creating within the Army MedicalDepartment such an organisation as will ensure this coopera-tion and will, at the same time, furnish inducement toofficers who have specialised in these subjects to continue towork therein. It is also hoped that the improved prospectsnow opening up may encourage young medical men whosebent and inclinations lie in these directions to enter theService.

The scheme to which approval has now been given may besomewhat briefly outlined as follows :-

1. Two new Directorates, of Hygiene and Pathologyrespectively, have been created under the Director-General,Army Medical Services, as a part of the War Office organisa-tion of the Army Medical Department. These Directorateswill be directly responsible to the Director-General for allmatters relating to their respective spheres, and they willtake over from the various branches of the Army MedicalDepartment the sections of this work which have hithertobeen distributed among those branches.

2. The following administrative staff appointments havebeen authorised, conforming as regards conditions of appoint-ment and tenure to those of D.M.S., D.D.M.S., A.D.M.S.,and D.A.D.M.S. :-

(a) Director of Hygiene.D.H.

(b) Deputy Dir. of Hyg.D.D.H.

(c) Assistant Dir. of Hyg.A.D.H.

(d) Deputy Assist. of Hyg.D.A.D.H,

Director of Pathology.D.P.

3. The above appointments will carry with them thefollowing ranks :-

(a) Directors-Brigadier-General (but may be held by aMajor-General if selected for promotion to this rankin the ordinary course).

(b) Deputy Directors-Colonel.(c) Assistant Directors-Lieutenant-Colonel.(d) Deputy Assistant Directors-Major.

Officers selected for appointment to any of these chargesif not already holding the substantive ranks indicated willbe given the appropriate temporary rank, together with thepay and allowances of that rank for as long as the appoint-ment is held.

4. Establishment.-Sanction has been given to the newDirectorates and to the following establishment of the cadresin the Commands for a period of two years, at the end ofwhich time the latter will be subject to revision, in thelight of the experience by that time acquired as to itsadequacy. The appointments will be filled up as sooc as thefinal disposition and strength of the troops to be stationed athome and abroad have been settled, and as suitably qualifiedofficers become available.

5. Advisory Committee.-It has been decided to appoint suchcommittees in connexion with each Directorate to assist theDirector in technical matters. They will be composed of bothmilitary and civilian members, and the following are theconstitutions which have been approved :-

6. Organisation for War. - A similar scheme for thecoordination of hygiene and pathology work in the field,together with the establishments considered necessary undervarious circumstances, has also been drawn up, but need notbe described here.

NEW RATES OF PAY IN THE INDIAN MEDICAL SERVICE.

The Secretary of State for India in Council, in announcingrecently the introduction, with effect from Dec. 1st, 1918,of improvements in the rates of pay for permanent officersof the Indian Medical Service, stated that the detailed ratesof pay to give effect to this decision were being worked outin India. (See THE LANCET, Feb. 15th, 1919, p. 278.) Thedetailed rates of pay for the military side have now beenreceived and are as follows (the rates of pay in force beforeDec. 1st, 1918, for officers in permanent medical charge ofregiments are given in column 2 for comparison):-

These new rates have been arrived at by adding approxi.mately 3313 per cent. to that portion of the old rates ofconsolidated pay for these appointments which’ representedgrade pay. ROYAL MATERNITY CHARITY OF LONDON.-Thischarity, which was founded in 1857 for the purpose of pro-viding midwives and medical attendance to poor marriedwomen in their own homes, has now issued its report for theyear 1918. The report shows that 369 cases were attended,and that 382 infants were born, of whom 213 were males and169 females. There were no deaths of mothers, and all butsix infants survived. The ages of the mothers ranged from19 to 49. The average cost per patient was £3 17s. 8½d.Since the outbreak of the war 1320 wives of sailors andsoldiers have been attended by the midwives of the charity.As extra demands have been thrown upon the society inconsequence of the war, with a corresponding increase ofexpenses, an earnest appeal for financial help is issuedwith the report. The total amount of fees paid to themidwives during the year is stated in the report as;E155 3,. 2d., or an average of just under 8.. 5d. per case.The public might subscribe more liberally to a charity notpractised at the expense of the midwife.
