Leadership (New vs. Mature)

• The Wright Center believes there is a leader in everyone and strives to have a “Learning Culture” vested in building leadership capacity and succession plans for all employees.

• Our medical home transformation & work redistribution aims to create a healthy culture of shared accountability with all employees expected to perform at top of license.

• Medical Home workforce redistribution expanded capacity that poised Dr. Linda Thomas-Hemak, our founding Medical Director, to become President/CEO of the Wright Center for Graduate Medical Education and Wright Center Medical Group in July 2013, while still maintaining her significant primary care provider role.

• The Wright Center top management and clinical leadership team has just been restructured with appointment of new Wright Center for Primary Care Medical Directors and new Vice Presidents of Mission Delivery, Patient Safety and Quality, Clinical and Educational Operations, Human Resources and Finance.

• Dr. Gregg Novak is our newly appointed MVP Medical Director and Dr. Jignesh Sheth from this center is our new organizational VP of Patient Safety and Quality. We welcomed our new MVP Practice Manager, Jeanie Pavlovich, September 30, 2013.

• Patti Swierbinski, MA at MVP is proudly our PCT:LEAP “Emerging Leader.” • Our Spiritual Counselor, Sister Maureen Marion, was recently named “Northeast Woman.” • Our Care Manager, Kari Machelli, RN is well recognized in the PA DOH’s Care Management Leadership Team of the Chronic Care Initiative.

Our Innovative Story • MVP was the original home town, rural private practice of our President/CEO, Dr. Linda

Thomas-Hemak, a board certified Internal Medicine and Pediatrics physician, who came home after residency to join her Family Physician, Dr. Tucker Fadden Clauss. To “leverage learning,” she gifted the practice to The Wright Center in 2001.

• MVP has always been an academic practice training primary care workforce in forever expanding partnerships with numerous community resource and academic agencies, as well as amazing, engaged patients and multi-generation families with high expectations.

• Significant catalysts for our authentic PCMH and EMR MU transformation have been our: – 2009 PA DOH’s Chronic Care Initiative and the 2011 PAFP Improving Performance in Practice’s

Residency Program/Community Health Center Medical Home Initiative leading practice engagement; – 2011 pioneering, HRSA Teaching Health Center designation that brought an expanded primary care

GME curriculum rooted in an ACGME/Medical Home/EMR MU matrix and behavioral health integration; – 2011 HRSA Patient Pharmacy and Safety Collaborative “Wright Script” Team engagement and 2013

Gold Team recognition; – 2011 engagement in SAMHSA's Garrett Lee Smith Suicide Prevention in Primary Care project; – Meaningful Use Attestation for all eligible providers and role creation of on site EMR Meaningful Use

Application Specialists; – Polishing Care Management, introducing a Social Worker for Care Coordination and adding a Spiritual
