Project Objectives
• Mobility Management Center: coordinate human service transportation for vulnerable populations (i.e., persons with disabilities, older adults, low income individuals, etc.) and other users of community transportation.
• Emergency Operations Plan: for the mobility coordination of vulnerable populations and community transportation users in the event of a disaster and during recovery.

• 2-1-1 SCC provided by United Way. Live operator for I&R using resource database. Subcontractor located in Ventura, California. Note: OUTREACH has been a United Way organization for 30 years.
• 511 provided by the Metropolitan Transportation Commission located in Oakland, California. Note: OUTREACH has been a funding partner with MTC for 30 years for community transportation programs and technology.MTC drafted the regional coordinated human service transportation plan.

Employment Transportation Assistance Call Processing
• Eligibility: – CalWORKs/WIA or NOVA/Work2Future
• Available Services– Personal Vehicles: repairs (Jump Start), gas cards– Demand-response (Guaranteed Ride Program): transportation to work,
training, support services, school/day care, etc.– Public Transit: transportation plans, subsidized transit passes– Shared Rides, volunteer driver programs– Children’s transportation (up to 13yrs of age) & Free Car-Seats
• Transportation Providers:– OUTREACH programs, public transit, volunteer drivers, CBO’s, etc.
• Resources: – train-the-trainer; mobility management training for providers (CalWORKs
staff, community based organizations) since 1998 – Multilingual Transportation Resource Guides– Linkages with 2-1-1SCC and 511.org

Emergency Mobility Coordination
• Mobility Management training for partner agencies
• Web-based mobility management system for partner agencies to manage their members, make trip reservations, GIS mapping, accounting, reporting, as well as manage their own resources (fleet, mobility equipment, etc.) for everyday use or during and after a disaster.
• Distribution of alternative communication equipment and applications for coordination of efforts in the event of a disaster.

StandardizationNonprofit partners participate in EAP (Emergency Action Plan) and COOP (Continuation of Operations Planning) in conjunction with the County of Santa Clara, Office of Emergency Services (Homeland Security Grant). Training in Mobility Management integrated into the agency’s plans and training policies and procedures.
