pharmcotherapy in adults with simple ADHD and ADHD plus comorbid psy -chiatric disorders.If we’re going to leave you with a single message for today, once you havemade a diagnosis of ADHD I want to encourage you to treat the condition.Undertreat ment is a significant problem and the impairments from thedisorder are quite profound.

Pharmacotherapy in AdultsWhen we talk about thepharmacotherapy of ADHDin adults the medi cationsbreak out basically into two classes. They are the stimulant med ications:methylphenidate and theamphetamine based com -pounds and the FDAapproved non-stimulant,

ato moxetine. Additionally, we will be talking about pharmacotherapy ofadults with ADHD and comorbid conditions — that is off label. FDA approvalis for the medicines noted above for treating ADHD alone. However,ADHD tends to occur commonly with comorbid disorders and, therefore,discussing the treatment of ADHD with another mental health disorder isquite important.

SuspectedNeurochemicalPathophysiology of ADHD (1)This slide depicts thesus pected neurochemicalpatho phy siology of ADHDlooking at the presynapticneuron and the post -synaptic neuron at the trans- porter, at exotitic release and at the receptors on thepostsynaptic neuron.

SuspectedNeurochemicalPathophysiology of ADHD (2)In this slide we also look athow the medications act.Amphetamines and methyl -phenidates, the stimulants,block reuptake. Atomox -etine blocks the nore- phine phrine transporterand amphetamine, making

it slightly more potent. Atomoxetine also causes increased dopa mine release.

Stimulant Medications Studied for ADHDThis slide depicts the stimulant medications studied for adult ADHD. Weshould note that there are four FDA approved medications which are allsustained release versions of the psychostimulants. But let’s review thedifferent medications and how they have been studied. The immediate release methylphenidate compounds or other preparationsthat are not oral sustained release include: Ritalin®, Metadate® which isan intermediate acting methyl phenidate compound, and Daytrana®, theforms vary with their brand. The duration of action for the immediaterelease methylphenidate is only three to four hours. It is longer for theintermediate acting Metadate® and Daytrana®. Mixed amphetamine saltsin the immediate release form ( brand name is known as Adderall) is afour salt combination known as mixed amphetamine salts comes in avariety of strengths from 5 mg up to 30 mg. The noted duration of effectis anywhere from four to six hours. We will also present data showingimprovement in ADHD symptoms in adults with this preparation; however,it is approved for children but not approved for adults with ADHD.

Oros methylphenidate is Concerta®, and is a methylphenidate preparation.It involves a pill that has a methylphenidate coating. There are twochambers on the inside part of the pill with a membrane barrier and alaser drilled hole with a reservoir of methylphenidate in the bottom. Thereis a mechanical sustained release preparation whereby the pressure fromthe gut fluid slowly pushes the methylphenidate out through themembrane. It comes in a variety of different dosage strengths: 18mg, 27mg, 36, mg, and 54 mg. The FDA label goes up to 72 mg a day for adults.In fact, the usual starting dose in clinical practice varies from 18 to 30 mg.The duration of action is up to 10 hours and again this is FDA approvedfor adults with ADHD.Dexmethylphenidate extend ed release is Focalin®XR. It is the d-methylphe -nidate in a sustained release preparation and it is a beaded mechanicalsustained release preparation. All methyl phe nidate com pounds, formallyas Ritalin®, are d and l preparations and in fact Oros methylphenidate is acombi nation of a dextro and levo methylphenidate. Here we isolated thed form of methylphenidate and it is in a mechanical sustained releasepreparation. It comes in a variety of dosage strengths from 5 to 30 mgwith the 30 mg strength being a fairly recent addition to the portfolio. Theapproximate duration of effect is anywhere from 10 to 12 hours. Again, itis FDA approved for adults with ADHD.Mixed amphetamine salts extended release is Adderall XR®. Again, it is amechanical sustained release preparation with short and long-acting beads.It has been studied in adult ADHD and is FDA labeled. The dosagestrengths vary between 5 mg and 30 mg although the FDA approved labelgoes to 20 mg. in adults. The approximate duration of effect is again 10 to12 hours and it is FDA approved. Lisdexamphetamine dimesylate is Vyvanse®. This is a pro-drug. It has achemical sustained release which we will talk about. The dosage strengthsvary between 20 and 70 mg. Usual starting dose is 30 mg. Titration goeson up to 70 mg. It is FDA labeled up to 70 mg with an approximateduration of effect up to 12 to 13 hours and it is FDA approved for adultswith ADHD.

Studies of Stimulants inAdult ADHDLet’s look at some studiesof stimulants in adultADHD. We will actually gothrough studies that showeach of these com poundsas men tioned in the priorslide.

Study ofMethylphenidate(MPH®) in Adult ADHDThis slide depicts a studydone by Tom Spencerusing meth ylphenidate inadult ADHD. It is a compar -ison between immediaterelease methly phenidateand a placebo. There area couple of things to notebefore we discuss the

actual data. The rating scale used here is a DSM-IV ADHD symptomchecklist which is quite similar to the ADHD RS. Actually, the symptomthreshold you’ll notice is slightly above 20, in part, because the datacollected used DSM-III and DSM III-R and extrapolated out to DSM-IV.Classically, the ADHD RS will have the eighteen symptoms in DSM IV ratedzero to three on a severity basis. Looking at the data here you’ll see thatmethylphenidate given three times a day will drive down ADHD symptomsbelow the symptomatic threshold for ADHD. These improve ments occurstarting at week two and continue all the way out with improvement toweek six.

Study of MixedAmphetamine Salts inAdult ADHD These data come from astudy of mixed amphe -tamine salts immediaterelease in a study of adultADHD con ducted by TomSpencer. Focus on the thefirst portion of the study,looking again when symp -toms are driven below

subthreshold for ADHD on an ADHD symptom checklist similar to theADHD RS. You see the effects of mixed amphetamine salts over placebostarting at week one with significant effects by week two and then drivingdown below the symptom threshold by week three. So, even thoughneither immediate release methyl phenidate nor immediate release mixedamphetamine salts are approved for adults with ADHD — this use wouldbe off label — we do have data that show they are efficacious. Both ofthese compounds are approved for use in children.

Study of MixedAmphetamine SaltsExtended Release in Adult ADHDThis is the registrationstudy looking at mixedamphe tamine salts extend -ed release in adults withADHD. It is a dose ranging,multi-center, double blindstudy. It is a fairly largestudy with 248 patients.

The studies we’re going to be showing now have larger samples than thesingle site studies we showed previously. This study showed that theonce daily mixed amphetamine salts was well tolerated and there weresignificantly lower scores out to twelve hours on the Conners Adult ADHDRating Scale (CAARS) and the primary outcome showed statisticallysignificant improvement in the ADHD RS. The CAARS is a self-reportversion which measures similar symptoms to the ADHD RS,but it is slightlydifferent. There was significant improvement in the clinician GlobalImpression of Severity Rating Scale—CGI scores. Note here, the FDA labelon this study is only at 20 mg because the study did not find doseresponse relationship. Even though subsequent studies have found onseverity analysis that patients with more severe ADHD did somewhat betteron higher doses, the FDA label on mixed amphetamine salts XR is 20 mgper day.

ADHD Rating Scale: Mean Total Score at Endpoint (ITT)What you see looking at in this four week study, compared to placebo yousee an average change on the ADHD RS of a decrease of 6.6 points. Thenyou see improvements across the range of mixed amphetamine salts from20 to 40 to 60 mg with improvement mean scores going from decrementsof 12.6 to 14.4 points.

Study of OROSMethylphenidate in Adult ADHDThis slide depicts a singlesite study of Oros methyl -phenidate in adult ADHDpublished by Joe Bieder -man from Mass General.Subse quently, there hasbeen a registration studythat was published withsimilar results. The current

study looked at changes in the AISRS which is a semi-structured scale thatmatches the eighteen items of the DSM-IV with specific ADHD promptsand probes embedded. But again, it is an 18 item, clinician administeredscale showing improvements in Oros methyl phenidate starting at weekone with significant improvements by week three going all the way out toweek six. Oros methylphenidate is FDA approved for adults with ADHD.

Open Label Study ofDexmethylphenidateExtended Release in Adult ADHDThis is an open label exten -sion study of dexmethyl- phenidate XR in patientsthat come out of a doubleblind study. The doserange here was 20 to 40mg per day. On the ADHDRS what you see are patients

coming out of treatment with placebo had a decrement over 10 points onthe ADHD RS, and those who had come out of the dexmethylphenidateXR group had an improvement of 8.4%. The drug obviously worked.Similarly they have data from the controlled phase of this study that showssignificant effects of dexmethylphenidate XR over placebo. This was a longerstudy that looked at open label treatment as compared to the double blindstudy. Dexmethylphenidate XR is FDA approved in ADHD with adults.

LisdexamfetamineDimesylate ChemistryLisdexamfetamine dime -sylate is a prodrug that is thera peutically inactiveuntil it is converted toactive d-amphetamine inthe body. Lisdexam feta -mine consists of dextro-amphetamine (d-amphe -tamine) bonded to anamino acid lycene. It is a

chemical sustained release mechanism whereby, once it is absorbed intothe bloodstream, it is too large to cross into the brain, the body recognizesit as actually a protein and proteases actually cleave it apart leaving thelycene, which is the amino acid and then the d-amphetamine. This givesyou the sustained release mechanism that is fairly long, up to twelve hours.Once the d-amphetamine is in the blood it can obviously cross the bloodbrain barrier into the brain.

LisdexamfetamineDimesylate in AdultADHDThis shows the studydesign in the registrationstudy of lisdexamfetaminein adult ADHD. It was adouble blind, placebo con -trolled, random ized parallelgroup, four week studywith a forced dose esca -lation. The adults ranged

from18 to 55 years of age and were diagnosed by DSM-IV criteria. Whatyou see is there is a placebo group, a 30 mg cohort, and a group thatstarts at 30 and moves up to 50, and then a group that starts at 30 thento 50 and then 70mg in this four week study.

ADHD-RS Total Scores: ITT PopulationHere are the results of the LDXStudy. What you see aresignificant im prove ments in alldosage strengths: 30, 50 and70mg of lisdexam phetaminevs placebo. The magnitude of changes are fairly large,varying between mean scoreson the ADHD RS of 16.2 to

18.6 points across the dosage strength of lisdexamphetamine.

Side Effects in Stimulant StudiesWhat are the side effects ofstimulants? The side effectsare fairly similar across theentire class. Common thingsthat are seen are listed here.Most of these can be man -aged by dose adjustment andtime on medication.

With stimulant treatment you will see some dry mouth which can betroubling. A clinical pearl here is sometimes having patients use sugarlessmints and candies, and maintaining good hydration is really critical.Insomnia can be a problem, especially if the medicine is taken late in theday and if it is a sustained release preparation. Although many adults withADHD have trouble sleeping before they take a psychostimulant, manypatients will have no change in their sleep pattern or actually some haveimprovement in their sleep when they are treated. Appetite suppression is common. It’s usually an early side effect. It canbe managed by having patients eat early and late and at minimum eatinglunch even though they may not be hungry. Headaches can occur. Someedginess, tension, and mild agitation may also be observed. Occasionallystimulants can cause significant agitation and psychosis but this is veryuncommon. Both stimulant and non-stimulant medications may cause cardiovasculareffects when used to treat ADHD. They can all cause minor changes inblood pressure and pulse on the order of about three millimeters ofmercury and about five beats per minute in the heart rate. AHA guidelinesfor children with ADHD, although somewhat controversial, recommendgetting a screening cardiogram. Many adults have had a cardiogrampreviously and if they haven’t, it is not unreasonable to order one beforeinitiating a psychostimulant. In addition, their adult blood pressure andpulse should be regularly monitored. Also some clinicians recommendscreening for family history of early or sudden cardiac death or an individualhistory of a heart murmur or synchopy. If those things are found, furtherworkup and monitoring the individual more carefully if they are beingtreated with a psychostimulant is in order.

Prescribing Information FDA Approved Stimulants for Adult ADHDHere is some prescribing information for the four FDA labeled compoundspre viously reviewed: Oros methyl phenidate, dexmethyl-phenidate XR,mixed amphe tamine salts XR, and lisdex am fetamine dimesylate. Thesemedications come in a variety of dosage strengths which are noted here.

Starting dose on Oros methylphenidate can be 18 to 36 mg a day, up to72 mg with an approximate duration up to 10 hours. DexmethylphenidateXR usual starting dose is about 10 mg, recommended maximal dose inthe studies was 20mg and there’s a 30 mg tablet though that is approved.The duration of effect is 10 to 12 hours. Mixed amphetamine salts XRgeneral starting dose noted was 20 mg although most clinicians I think willstart at somewhat of a lower dose so probably 10 mg a day. The FDA labelin the studies actually list 20 mg, but it is actually higher in kids which iswhy the package insert will actually note up 30 mg a day and theapproximate duration of effect is 10 to 12 hours. Finally, dosages forlisdexamfetamine generally starting at 30 mg once in the morning up to70 mg with a duration of 12 to 13 hours.

Studies of FDA Approved Non-Stimulant Medication for ADHD in AdultsThere is one FDA approved non-stimulant in adult ADHD calledatomoxetine hydrochloride or Strattera®. It comes in a variety of dosagestrengths varying between 10 and 100 mg a day. The FDA label does goup to 100 mg per day. Duration of effect can be up to 24 hours, but isdosed on a qd or bid basis, and it is approved for adults with ADHD.

Study of Atomoxetine inAdult ADHDThis is a registration studyconducted by David Mich -el son of the use of ato -moxetine in adult ADHD.It’s an FDA approved nore-pine phrine reuptake in -hibitor. You think aboutwhat a SSRI is, whichprimary care physicianscommonly use for the

treatment of depression — this is a norepinephrine reuptake inhibitor ascompared to a serotonin reuptake inhibitor in the SSRI’s. It is not anantidepressant. The registration study included two identical, combinedmulti-site, randomized, placebo-controlled studies. The samples included280 in one and 256 in the other, for a total of 536 adults. In this largestudy there was a target dose up to 120 mg a day. Please note, the FDAlabel is 100 mg here. The mean dose was about 92 mg a day, and thiswas a 10-week study.

Efficacy of Atomoxetinein Adults with ADHDStudy results show signi -ficant improvement start ingby week two with ongoingimprove ment to week tenbased on the CAARS ADHDscore. These are the 18symp toms on the CAARS,which are extracted tomatch the DSM-IV symp -toms and you see im prove-

ment going from a little less then 32 down to about 23 over the course oftreatment. One thing to note is that atomoxetine’s effect, because it is areuptake inhibitor, is somewhat slower although it shows significant effectsat week two. You do notice this ongoing improvement going out to weekten and because it’s a reuptake inhibitor it does take two to three weekseach time the dose is titrated to get the maximal effect.

Side Effects ofAtomoxetine in Adults with ADHDWhile there are similaritiesin the side effects there aresome differences betweenthose seen in psychostimu - lants. We talked about drymouth and insomnia. Thenausea tends to be anearly effect and taking themedi cation with food will

diminish this problem. Sometimes ginger can actually help that nauseaand that’s a nice clinical pearl. While decreased appetite can be observed,it is sometimes transient and can be improved if it is taken with food. Onething that you see sometimes with atomoxetine that you don’t see asmuch with psychostimulants are decreased libido or erectile issues. It’simportant to talk to your patients about that. A delay in the urinary streamis also something that you can see with atomoxetine which is not seenwith psychostimulants. Dizziness can occur. The blood pressure changeswere similar to those noted with psychostimulants. I should note that in the post-marketing surveillance there have been acouple other warnings put on atomoxetine. There is a warning for anallergic type hepatitis that tends to occur one in a million times. There’sno way to screen for this. If patients develop this, they’ll become jaundicedand complain of flu like symptoms. Certainly it is important to discuss thatwith them. It’s a rare side effect and in the cases that have been observed,stopping medication has been effective in relieving there effects.Additionally, it is important to note that atomoxetine carries a warning onit for suicidal thinking in children, but not in the full adult population.

Prescribing Information FDA Approved Non-Stimulantfor Adult ADHDThe prescribing information on atomoxetine is that it’s available in anumber of dosage strengths: 10 mg, 18 mg, 25 mg, 40mg, 60mg, 80mg,and 100mg. The recommended starting dose is 40 mg a day on the label.I will say that many clinicians start at a lower dose of either 18 or 25 mgto improve tolerability. Since there is a lag in getting the full onset of, notthe initial onset but the full effect of the drug, starting at a higher dose maynot actually get you further along and may get you some side effects. I willsay in clinical practice, different from the FDA label, many clinicians willstart at a somewhat lower dose because it does improve tolerability. Again,dosing is either qd or bid and the recommended maximum daily dose is100 mg a day and the duration of effect can be up to 24 hours.

Pharmacotherapy ofAdult ADHD andCoexisting PsychiatricDisordersSo now we’re going to talkabout the pharmaco -therapy of adult ADHDwith coexisting psychiatricdisorders—comor bidities.It’s important to know thatthis is off label use ofADHD compounds. There

Margaret Weiss reported on a placebo controlled trial ofparoxetine versus dextroamphetamine. There was one open label trial ofvenlafaxine +/- psychostimulants. Venlafaxine and the addition of the psy -chostimulants did have improvement in ADHD symptoms. And Rob Findlingreported an open label trial of the SSRI sertraline or fluoxetine + psy cho -stimulants. A lot of the data here shows that if you’re going to use an SSRIwith a psychostimulant the safety data from the small numbers of studiesindicate that they didn’t find a lot of drug interaction and the safety profilelooked okay. Remember the evidence base here is fairly small. One thing to think about when we’re talking about comorbidities is thereis no evidence base really to help guide you here but in medicine, whenyou’re treating more than one condition the general dictum is to treat themost impairing condition first, because that’s going to have the greatestimpact on our patients and not treating that condition will have the greatestimpairment. It’s important to keep this in mind, if you have a patient withADHD and Major Depression. Since Major Depression can be lifethreatening in terms of its suicidality and the ADHD is more lifelong manyclinicians will choose to treat the Major Depression first and then treat theADHD second.

Pharmacotherapy ofADHD and BipolarDisorderIn terms of the studies ofpharmacotherapy of ADHDand Bipolar Disorder, therewas one study of bupro -pion by Tim Wilens, whichfound improvement in theADHD symptoms and theworsening of the BipolarDisorder. Actually the STEP

study, the large scale multi-site NIH supported study actually found thatbuproprion induced lower switch rates in Bipolar Disorder. Even thoughit’s an antidepressant buproprion is the most commonly used medicationfor treating Bipolar Depression. In this study it was effective in treating theADHD and apparently did not worsen the Bipolar Disorder.

Pharmacotherapy of ADHD and Social Anxiety DisorderThere was one study thatwe reported investigatingthe pharmacotherapy ofadult ADHD with comorbidsocial anxiety disorder. Thisis a controlled study of ato -mox etine in dose ranges of40 to 100 mg a day vs.placebo. In this large study

of 218 adults, we actually found significant improvement in the ADHD andalso the social anxiety symptoms. The ADHD was rated by our standardmeasures of the 18 symptoms and the Liebowitz Scale was used formeasuring the improvement on social anxiety disorder.

Pharmacotherapy of ADHD and ExecutiveFunction DeficitsAnother way that we talkabout pharmacotherapyand co-existing condition is Execu tive Function Deficits.Exec u tive functioning refers to those neurocognitive process es that are neededto organize behavior across

time and sustain problem solving for future goals. There are two ways tolook at this, one by measuring the effects on neuropsychological tests andanother by looking at improvement on clinical measures of executivefunction deficits. The first study on neuropsychological tests wasconducted by Biederman, while the second was done by Tom Brown. Ofnote is that the Biederman study did find improvement in a number ofneuro psychological measures and that the executive function subset onthe Brown scale did show improvement with atomoxetine.

Pharmacotherapy ofADHD and SubstanceUse Disorders (SUD)Another group of studieslooked at pharmacother -apy of ADHD and sub -stance use disorders. Astudy by Tim Wilenslooked at atomoxetine inpatients with ADHD andalcohol use disorders. This

was a large study that looked at patients that were recently abstinent fromsignificant alcohol abuse or dependence and went into a controlled studyof atomoxetine versus placebo. There were significant effects of atomoxetineon the ADHD symptoms in this cohort of patients with very recentabstinence. And of note, subsequent publications investigating safety foundthat atomoxetine did not cause difficulties with liver function specifically inthis group of patients with significant alcohol use disorders. That is a veryimportant thing to keep in mind. Also atomoxetine, a nonstimulant, is not acontrolled substance and therefore it is something to think about as a firstline therapy for patients with active substance problems.A subsequent study by Fran Levin in patients with ADHD and cocaineabuse found that Atomoxetine was effective in this cohort. In a recentlyreleased e-publication from the clinical trials network looked at Orosmethylphenidate in patients with ADHD who were cigarette smokers. Dr. Winhusen found and reported that the Oros methylphenidatesubstantially improved the ADHD symptoms in adults who had ADHDand smoked. However, it did not increase abstinence rates in terms ofsmoking. These patients also had a behavioral program and patches toimprove treatment for their cigarette abuse.

Summary ofPharmacotherapy forTreating Adult ADHDTo summarize what weknow about the pharmaco -therapy of treating adultADHD, there are five FDAapproved medica tions fortreating adults with ADHD.Four of these are long-acting stimulants which

include Oros® methyl phen idate, dexmethyl phenidate XR, mixedamphetamine salts XR, lisdex amfetamine; and, there is one nonstimulant,atomoxetine hydrochloride. Research has shown that these drugs have agood safety profile and are efficacious in the treatment of adult ADHD. Thetreatment of adults with more complicated ADHD, meaning ADHD with acomorbid condition including Major Depression, Bipolar Disorder, AnxietyDisorder, or Executive Function Deficits, and substance use disorders canbe treated with a combination therapy or single use therapy, but theevidence basis here is much smaller than the evidence we have in termsof treating adults with ADHD without comorbidity. And additionally, the useof these ADHD medications in this population with comorbidity is off label. Thank you very much.
