Event Report-1st Page
Event report 1st page divided into 4 sections:
• When? Event Identification-date & time of event and type of subject
• Who? Subject- identifies the subject
• Where? What? Event Context- location and description of event
• How severe? Event classification
– Verifies category and subcategory
– Requires user to enter severity of event
When? Event Identification
The date of event is the date the event occurred. Ideally the event report is entered as soon as possible so that more details are remembered.
The date of discovery defaults to today’s date. Please update if needed.
“Physician” is the default selection for occupational exposures. The category “Physician” covers ALL providers in UFCD: faculty, students, residents, and hygienists
If the subject of the event is a staff member, select employee. All DMD students are searchable under “Physicians”, so student is not used.
Who? Identify Subject
Identify the “Physician” (provider)
Either first or last name may be used to search for provider
Where? What? Event Context
Select the correct location of the event.
Please do not use “DN-Student Clinical Teams” for DMD Teams events. Instead select DN-Team. Expand list to select correct team.
ALL events that occur in the DMD Teams clinics should have the team number as the location!
What? Event Context
Enter the facts of the event in the “Event Description” field:
The field allows 500 characters. Event descriptions should be brief and factual.
How severe? Event Classification
Select the severity of the event.
The Category and Subcategory have auto-populated based upon earlier entries.

To add a document, such as a letter, select the “Attach Document” button.
Add PartyThis section allows the user to add any parties who were involved or witnessed the event:
Click on “Add Party”
A “party” could include patients, patient’s family members, passersby, providers, staff, etc.

Search for a provider by entering their last and/or first name:

Enter NameAlthough the option exists to enter events anonymously, if more information is needed, it is very helpful to have the name of the reporter of the event.
