Dean’s Message Aloha.AsImoveintomysecondyearasdeanofTheSchoolofNursingandDentalHygiene(SONDH)attheUniversityofHawai‘iatMānoa,Icontinuetobeinspiredbythepride,vitalityanddeterminationofourfaculty,studentsandstaff.Thereisanexcitementintheair,notjustduetothechangesthatareoccurringinourschool,butalsoasaresultoftheexpandingrolesnursesanddentalhygienistsaretakingintheevolutionofhealthcare.

Asaschool,theSONDHisaggressivelyproactive.Byforgingstrongerbondswithourmedicalcounterparts,weareworkingtoaddresscriticalhealthissuesfacingourstate.AmongtheseistheacuteshortageofnursesastheBabyBoomergenerationmovesintoretirement,asituationwhichisfurtherexacerbatedbytheagingofHawai‘i’snursingworkforce.Theearthquakeof2006,thelitanyofworld-widenaturaldisastersandtheimpendingthreatofemergingdiseasesalsoserveasremindersthatHawai‘i’shealthcareprofessionalsmustbeforevervigilant.AndastheSONDHlookstothefuture,wecannotignoreHawai‘i’sat-risk,under-servedandunderrepresentedpopulationswhoneedourhelprightnow.

Inthisreport,you’llhearhowfacultyandstudentsattheSONDHareleadingthewaybyinitiatingresearchthataddressestheunderlyingcauseofpooroutcomesandimprovinglong-termhealthforentirepopulations.Additionally,theyarecreatinginnovativepartnershipsthatenabletheSONDHtoprovideourstudentswithrelevanthands-onopportunitieswhileallowingustofine-tuneourcurriculumtomeetthecurrentandanticipatedneedsofHawai‘i’sdental,medical,educationalandpublichealthcommunities.

Ontheeducationalforefront,theSONDHisleadingthewaywithsimulationtechnologyintheclassroom,onlineandaccelerateddegreeprograms,andaleadershiptrackthatprovidesjointnursingandbusinessdegrees.Throughoutthisreport,you’llalsodiscoverhowweareturningouruniquelocationandmulti-culturalstudentandpatientbaseintoopportunitiestoimprovelivesintheislandsandthroughoutthePacificRim.

Addingtoourschool’senergywasourFabulous50eventhonoring50ofthestate’stopnurses.Alsorecognizedattheeventwerefourproudmembersofourclassof’56.Theywerethefirstgraduatestoreceive4-yearbachelordegreesinnursingandpavedthewayforthetalentednursinganddentalhygieneprofessionalswhoarefollowingintheirfootsteps.

AttheSONDH,wearenotcontenttositbackandwaitforthefuture.Wearemeetingithead-onbyleadingthewayintheclassroomandoutinthefield.Weinviteyoutojoinusaswecontinueourquesttobecomeoneofthetop20nursingschoolsinthenation—aschoolwheretheworld’spremierfacultywillbedrawnandwhosegraduatesareamongthemostcovetedinHawai‘iandbeyond.

Q: Dean Boland, as the leader for the school of Nursing and Dental Hygiene, what would you say is the state of the school?

A:Thefirst75yearsoftheschoolwasdedicatedtobuildingoureducationalprograms.Today,ourgraduatesarewellrespectedandhighlyrecruited.Now,theSONDHisreadytobecomeaforceforchange.

Q: Where and what kind of change do you see?

A:Asapublicinstitution,it’sourresponsibility—ourkuleana—tofindwaystoimprovethehealthofindividualsandpopulationsinourstate.Unlikeprivateinstitutions,wetakeourleadfromandhavearesponsibilitytothecommunity.We’recontinuallyaskingourselveswhatourschoolcanbringtothetabletoimprovethehealthrelatedqualityoflifeforthepeopleofHawai‘i.

Q: Can you give me an example?

A:Onethatcomestomindisdevel-opingsolutionstotheepidemicofdiabetes.NativeHawaiianandFilipinogroupshaveamuchhigherrateofdiabetesascomparedtotheU.S.mainland.It’salsooneofthehardestdiseasestopreventandself-manage.Rightnow,nurseresearchersattheSONDHaredevelopingap-proachestohelpdiabeticstobecomemanagersoftheirowncare—doself-management—tolivehealthierlives.OurfacultyandstudentsembraceandrepresentthediverseculturesinHawai‘i.TheyseekinterventionsandsolutionsthatincorporatetraditionalnativeHawaiian,Japanese,ChineseandFilipinopractices.

Q: is research a new initiative for the school?

A:Researchhasbeenpartofourstrategicplan,butbeforewecould

beginweneededtolaytheground-

work.BeforeIcameaboard,the

schoolcreatedanOfficeofResearch

directedbyDr.JillianInouye.This

pastyear,thefacultydevelopeda

researchagenda.Followingonthat,

weaddedsupportthroughoutthe

entireprocessfromconceptdevelop-

menttoproposalsubmissionand

projectmanagement.

Q: Are there challenges and opportunities facing the school?

A:O‘ahuisreferredtoastheGather-

ingPlacewheremultiplepeopleand

forcescometogether.Therateof

changeinourworldwillcontinueto

accelerate.Bothhealthsciences

educationandhealthcarearein

transition.Thepublicisconcerned

andrightlysoaboutthequalityof

careinacountrywherethecostof

Q: What kind of support can make

a difference for the school?

Learning at the speed of lifethe soNDH leads the way with accelerated and online programs

TheSONDH,likeitsmainlandcounterparts,isrespondingtotheacutenationwidenursingshortagewithrigorousfast-trackdegreeprograms.Thisinnovativeapproachpropelsqualifiedgraduatesintothefieldinlesstimethanneededforatraditionalundergraduatedegree.Designedforsecond-degree,second-careerandgraduatestudents,fasttrackprogramsfulfillemployerrequestsformature,experiencednursespreparedforleadershiproles.

Accelerated Nursing Program and Master’s entry Program in Nursing

Pilotedin2004,theAcceleratedBachelor’sDegreeprogramenablesstudentswithapriorbachelor’sdegreeinanyfieldtocompletetheirBSinnursinginjusteighteenmonths.Itisdesignedforindividualswhodesirea“fasttrack”programtolaunchthemintoanewcareerinprofessionalnursing.

In2006,theprogramadmitted24students,ranginginagefrom23-37.Whilemoststudentsheldbachelordegrees,severalcamewithmaster’sdegreesinpublichealth,psychologyandbusinessadministration.Ouracceleratedstudentsarefocusedandhighlymotivated.Infact,thepresentclassscored87%ontheNationalLeagueforNursingachievementtestsinhealthassessment,wellabovethenationalaverageof50%.Theyarealsoadiversegroup:thepresentclassincludes3maleand21femalestudents;4studentsofHawaiianancestry,and3ofNativeAmericanheritage.

Buildingonsuccessfulmodelsacrossthenation,andinresponsetothedemandbyemployersandstudentsforamaster’slevelfasttrackprogram,theSONDHisrevisingtheacceleratedefforttocreateaMaster’sentryPrograminNursing(MePN)program.ThismovewillenabletheSONDHtostreamlinethecorerequirementprocesstobetteraccommodatestudentspursuingnursingastheirseconddegree.Simultaneously,theMePNwillincreaseefficiencyinthebachelor’sprogrambyreducingthenumberofmaster’scandidatesinundergraduatecourses.Byevolvingitscurriculum,theSONDHwilladdresstheimmediatedemandfromemployersandpreparenursesforeducation,leadershipandspecialtypractice.

Joint Ms Nursing/Master’s in Business Administration

InamovetoaddresstheshortageofnursesinleadershippositionstheSONDH,inconjunctionwiththeShidlerCollegeofBusinessattheUniversityofHawai‘i,offersajointdegreeinnursingandbusinessadministration.Thedegreeincludespreparationinmanagement,financeandplanningtoprovidenurseswithbusinessskillsforpositionsinmid-levelandseniorleadershippositions.

Theonlinedoctoraldegreebreaksnewgroundfortheuniversity.InnovativequalityprogrammingincreasesaccessforstudentsthroughoutHawai‘i.ThePhDprogrampreparesnursestoconductclinicalscholarshipandtoteachinnursingeducationprograms,especiallythosewithminoritystudentpopulations.ThecoursesusetheInternet,email,list-servesandotherweb-basedtechnologies.Inaddition,studentsparticipateinon-campusmeetingsthroughoutthecourseoftheprogramtofostertheirlearninginanacademicandscholarlycommunity.

The SONDH is the only nursing program in the State of Hawai‘i accredited by both the Commission on

Collegiate Nursing Education (CCNE) and the National League for Nursing Accrediting Commission (NLNAC).

Practice makes perfectsimulation technology takes real life situations into the classroom

TheSONDHiscounteringthenursingshortagebypreparinggraduatestoliterallyhitthegroundrunning.IntheSimulationLaboratory,twocomputerizedmannequins,SimMan™andSimBaby,™promptnursingstudentstotreatavarietyofacutecaresituationswithoutrealworldconsequences.Asthemannequinstalk,cry,experiencedifficultybreathing,exhibiterraticpulseandheartrates,makebowelsoundsandshowlipcyanosis,thestudentsrespondbyclearingobstructedairways,administeringIVs,takingbloodpressurereadingsandinsertingnasogastrictubes.eachsessioniscarefullymonitoredthroughaone-waymirrorbyinstructorLorrieWong,RN,MSN.Atthecloseofthesession,shewillfacilitateaninteractivedebriefingreviewingthescenariowiththestudents.

BythetimeSONDHstudentsbecomeregisterednurses,theywillhavehonedtheirclinical,technicalanddecision-makingskillsinpreparationforclinicalpractice.

ThegoalisfortheSONDH’sHealthWorkforceSimulationResourceLaboratorytobenetworkedtothefiveotherUniversityofHawai‘inursingprogramsacrossO‘ahu,theBigIsland,Mauiandkaua‘iforinterdisciplinarytrainingtoimprovecommunication,outcomes,andpatientsafety.

Fromabout1866to1969,HansenDisease(alsocalledleprosy)victimswerebanishedtotheremotekalaupapaPeninsulaontheislandofMoloka‘i.Recognizingthepotentiallyprofoundeffortofsuchavisit,instructorNancyMcGuckinorganizedtripstokalaupapaforstudentsandfaculty.

In2006,DeanMaryBolandaccompaniedkrisQureshi,RN,DNSc,StephanieMarshall,RN,MSandagroupoffifthsemesternursingstudentstovisitkalaupapa,Moloka‘i.IntheearlyyearsofHIV/AIDSwork,nursesonthefrontlinesfoundlessonsinthehistoryofsocialresponsetoinfectiousdiseasewhiledealingwithissuesofstigmaforcareprovidersandpatients.Visitingthiscommunity,wherelong-termresidentshavechosentoliveouttheirremainingyears,touchedeachoneofus.

‘Ike AO PONOinnovative cultural program at soNDH makes history

InHawai‘i,diversityisawayoflife.Andyet,despitethebeautifulmixofpeoples,NativeHawaiiansoftheindigenousculture,aresorelyunderrepresentedinhealthcareprofessions.AlthoughHawaiiansmakeupabout25%ofthestate’spopulation,theyrepresentonly5%ofHawai‘i’snursingprofessionals.TheSONDHiscorrectingthatdisparitybysupportinganinnovativeprogramcalled‘IkeAOPONO,directedbyNalaniMinton,MA.

Nalaniexplainstheculturalmeaningofitsmissionasreflectedinthenameshegavetheprogram:‘iKeisasharedvisionheldandnurturedbyall;Aoistheeverlastingqualityofcontinuation;andPoNoisharmonyinlifesustainedbyintegrity:rightrelationship,rightintentionandrightaction.Together,theseconceptspresentthecollectivevisionoflastinghealthandwellnessforthewholecommunity.

TheSONDHiscommittedtocreatingpositivechangethrough‘IkeAOPONO,asupportandculturalenrichmentprogramforNativeHawaiianandPacificIslandnursingstudents.Itsprimarypurposesare:1)toimprovethehealthofNativeHawaiianandPacificIslandpeoplesthroughculturallysensitivecare;2)toincreasethenumberofNativeHawaiianandPacificIslandhealthprofessionals;and3)toprovideinspirationtoyoungergenerationsofNativeHawaiiansandPacificIslandersintheformofpositiverolemodels.

RecentstatisticsshowthatcomparedtotheU.S.mainland,NativeHawaiians(inHawai‘i)haveagreaterrateofcancer,ahigherrateofdiabetesandobesity,ahigherrateofheartdiseaseandanalarminghigherrateofearlymortality.SamoanandPacificIslandersarealsoathighriskforthesediseases,and32%ofSamoanhouseholdincomesinHawai‘iarebelowthepovertylevel.Allfacebarrierstohighereducation,positivehealthandsocioeconomicoutcomes.

‘IkeAOPONOhelpsNativeHawaiianandPacificIslandstudentsapplyto,succeedinandgraduatefromtheSONDH.Whilemanyofthesestudentscomefromat-riskandunder-servedcommunities,theyareinspiredandencouragedtosucceedthroughauniquesupportbase,consistingofacademiccounseling,astrongstudent‘ohana(family),scholarships,financialaid,healthconferencesandprofessionalopportunities.

Atthecoreoftheprogramis‘IkeAOPONO’suniquelyHawaiianapproachtowellnesss.Throughcommunityservice,studentsspendtimetendingthelo‘i(taropatches),visitingsacredsitesandreconnectingwiththeirpast.Throughtheseexperiences,studentsdevelopaculturalunderstandingofhealththatenrichestheirpeers,faculty,thehealthcarecommunityandtheNativeHawaiiancommunityatlarge.

In2001,‘IkeAOPONObeganwith6students.AsofFall2006,theprogramhasgraduated40RNsandtodaysupports45undergraduatenursingstudents,15graduateNativeHawaiianandPacificIslandstudentspersemester,andthefirstNativeHawaiiangraduatetoearnaPhDinNursingfromtheSONDH.ThesenumbersrepresentaremarkabletransformationandcontributiontothenursingprofessionandculturallycompetenthealthcareinHawai‘i.

‘IKE AO PONO is supported by PL-103-150, the Native Hawaiian Health Care Act and the Native Hawaiian Education Act enacted by the U.S. Congress. Program funding is provided through a Social Justice Initiative of the UH Mānoa Chancellor’s Office, UH President and the Kuali‘i Council. Funding for student scholar-ships, cultural program needs, supplies and equipment is provided through partnerships with over 50 local organizations, among them, Queen’s Medical Center and Kamehameha Schools.

TheSONDHispursuingresearchthataddressestheneedsofHawai‘i’sdiversepopulationaswellasthoseofthePacificBasinandAsia.Hawai‘i’smulti-culturalsettingaffordstheSONDHauniqueopportunitytoconductculturallyrelevantinvestigationsthathavethepotentialtoimprovethequalityoflifeinourcommunities,ourislands,andtheworld.

AccordingtoDr.JillianInouye,therearemorethan102stepstomanagingdiabetes,makingitoneofthemostchallenging—andoverwhelming—lifestylestomaintain.Becauseitiswidelyacceptedthatknowledgedoesnotpredictorchangebehavior,Dr.Inouyeidentifiedtheneedforanalternateapproach,onethattookintoconsiderationaperson’sculturalandpersonalvalues.

TheCognitiveBehaviorInterventionStudyisafive-yearresearchinitiativetodetermineifthesystematicmodifica-tionofbehaviorscanresultinimprovedhealthoutcomes,betterquality-of-lifeandenhancedadherencetoself-managementforAsianandPacificIslanderindividualswithdiabetes.Or,inotherwords,canpatientsbemotivatedtotakebettercareofthemselvesbyperceivingtheircondi-tionas“halffull”insteadof“halfempty?”

TheCognitiveBehavioralIntervention(CBT)StudyforDiabetesSelfManagementisa$1.2millionfive-yeargrantfromtheNationalInstituteofNursingResearch,NIH.Currentlyinitsthirdyear,studyresultswillcontributetounderstandingsuccessfuldiabetesmanagementinAsianPacificIslanders.

Dr.Boydexplainsthatconceptsof“me”suchas“mybloodpressurereading,myweight,myage”arenotimportanttoNativeHawaiians.Instead,theirhealthismeasuredbytheirabilitytocareforothers,forexample,“Iamraisingfourchildrenandtakingcareofmymother,sothatmustmeanI’mhealthy.”

Howcantwosuchdisparatepointsofviewmeet?Dr.Boydstressesthatthefirststepistounderstandapatient’scorebeliefssothatthedesiredsolutioncanbeexplainedthroughaculturallyacceptablepointofview.Insteadoftellingapatienthemustlowerhisglucoseleveltoaspecificnumber,anursemighttellhimthatifhewantstobeabletoplaywithhisgrandchildreninfiveyears,hewillneedtoeatmorefoodsthatcomedirectlyfromthelandandsea.

In2006,theDepartmentofDentalHygieneenrolled25newstudentsforatotalstudentcountof67.ThedentalhygieneprogramatUniversityofHawai‘iatMānoaistheonlybaccalaureatedegreeintheislands.ItofferstheeducationalpreparationrequiredbydentalhygieniststoprovideoralhealthcareandeducationtothepeopleofHawai‘i,thecontinentalU.S.andthePacific-Asiaregion.

Inawell-equippedclinicalsetting,studentsreceivehands-onexperienceandsupervisedpatientcareexperiencestopreparethemforrealworldcareers.Theprogramalsoincludescoursesinliberalartsandbasicsciences.

Aswiththeirnursingcounterparts,dentalhygienistsareespeciallyneededincommunitieswithat-risk,underrepresentedandunder-servedpopulations,primarilyNativeHawaiianandPacificIslandpopulations.Tothisend,theSONDHandDepartmentofDentalHygienearecommittedtothereductionofhealthdisparitiesamongallthepeopleofHawai‘i.

Studentscompletingthebachelor’sofscienceindentalhygienequalifyforadmissiontothenationalandstateboarddentalhygieneexaminationsforlicensuretopracticedentalhygieneinprivate,publichealthandotherdentalhygienesettings.Itoffersprofessionaltrainingtobroadenanddeepenknowledgeandskills,aswellasprovidingafoundationforpersonalandprofessionaldevelopmentandopportunitiesforadmissiontograduatework.

TheeffortprovidedtheSchoolandDepartmentwithdiscipline-specificstrategiestosupporttheSchool’smissionfordentalhygieneeducation.Mostimportantly,localandnationalstakeholdersexpressedadeepcommitmenttosupportingdentalhygieneeducationatUH.

The Fabulous 50Honoring Hawai‘i’s brightest stars

In2006,incelebrationof50yearsofachievement,theSONDHpaidtributeto50nursesfromthroughoutthestatewhorepresentthebestofnursinginHawai‘i.Theserviceanddedicationofthese“Fabulous50”broughthonortothenursingprofession,advancementsinhealthcare,andextendedthespiritofalohatocountlesspatientsandfamiliesinourcommunity.

AlsohonoredwerefourmembersoftheClassof’56,theremarkableyoungwomenwhowerethestartofanewdirec-tioninnursingasthefirstgraduatesfromabeginningnursingbaccalaureateprograminHawai‘i.Theywentontorichcareersinhealthcare,administrationandresearch,andsetthestagefortheevolutionofoneoftheleadinghealthcareinstitutionsinthestate:theUniversityofHawai‘iSchoolofNursingandDentalHygiene.

Startingwiththatfirstgraduationceremonyin1956,theSONDHatUHMānoahasawardednearly3,000bachelor’sdegrees,750master’sdegrees,and18doctoratesinnursing.OurstudentsrepresenttherichculturalandethnictapestrythatdefinesHawai‘i:theirancestryisHawaiian,PacificIslander,Filipino,Japanese,Chineseandeuropean,amongothers.Afull25%percentofourstudentsaremale,farexceedingthenationalaverageof8%.MostgraduatesstayinHawai‘ibutmanygoontocareersonthemainlandandabroad.

Healthcareisateamsport.Whenitcomestothechallengesfacingournumberonepatient,theStateofHawai‘i,nosingleorganizationcandoitalone.AtSONDH,wearefortunatetoliveinacommunitywhereorganizationsfromallbranchesofhealthcareunderstandandappreciatethevalueofteamwork.ThisallowsSONDHstudentstolearninsettingsalongthecontinuumofcare.

Overtheyears,wehavecreatedcommunitypartnershipswithHawai‘i’sleadinghospitals,dentalcareproviders,publichealthdepartments,rehabilitationandlong-termcarefacilities.WithintheUniversityofHawai‘i,partnershipswithJohnA.BurnsSchoolofMedicine(JABSOM),DepartmentofPublicHealth,ShidlerCollegeofBusiness,andtheRichardsonSchoolofLawenrichtheschoolwithresearchopportunities,on-sitetraining,accesstonewtechnologies,skilldevelopment,leadershipdirectionanddevelopment.Inreturn,theSONDHisgraduatingtop-notchnursesanddentalhygienists—professionalswhostrengthentheiremployersbyprovidingquality,committedandcompassionatecare.

TheQueen’sMedicalCenter(QMC)continuestobeastrongsupporteroftheSONDHandin2006provided$150,000ineducationawardsfornineacceleratednursingstudents,supportedthe‘IkeAoPonoprogram,andfundedaresearchpartnershipbetweenQMCandtheSONDH.

TheSONDHhasalsofoundanextraordinarypartnerintheHawai‘iHealthSystemsCorporation.Thankstotheirefforts,SONDHgraduatestudentswillbeparticipatinginclinicalrotationswiththeJABSOMfamilymedicineresidents.
