Itiswithgreatpleasurethatwepresenttoyouthisreporthighlighting
thedepthandbreadthofactivitieswhichUICCanditsmembershave
undertakenin2009.Wehopethatyouwillenjoythecasestudies,theinsightintoourfuturedirectionandbecome
inspiredtobeanactivememberofUICC–aunionoforganisationsthatseekstoeliminatecancerasalife-threateningdiseaseforfuture
generations.
Wedelightinthefactthatourmembershiphassuchadiverseandimportantimpactoncancerglobally.Wecountasourmembers:patientgroups,hospitals,scientificresearchestablishments,ministriesofhealth,largeandsmallvoluntarycancercharities.Eachofthemisplayingafundamentalroleinfightingcancerintheircountryandalso,formany,runningorparticipatingininitiativeswhichhelpthoseoutsidetheirowncountry.
In2009,UICCcoordinatedasuccessfulWorldCancerDayon4Februaryunderthetheme“Ilovemyhealthyactivechildhood”,drawingattentiontotheimportance
Our programme activity continued in 2009 whereby:
• theCervicalCancerInitiativemovedintoitssecondyearandimplementedapilotprojectinTanzania;
• theMyChildMattersprogrammefunded7projectsindevelopingcountries,bringingthetotalnumberofprojectsto39spreadover26countries;
• theCapacityBuildingFundwasabletoprovidefinancialsupportto16projectsindevelopingcountries;
• weawardedmorethan150fellowshipsacrosstheglobe,allowingindividualstoshareknowledgeandtolearnfromtheexperiencesandexpertiseofothers.
In2010,cancerwillbethelargestkillerontheplanet.ItwilloutstripthetotalnumberofpeoplekilledbyHIV/AIDS,tuberculosisandmalariacombined.Inthedevelopedworld,approximately1in4peoplewilldevelopcancerintheirlifetimeandweknowthat1in10peopleworldwidewilldiefromthedisease(takingalifeexpectancyof75)1.UICCisdeterminedtoreversethistrend.
InDecember,theUICCBoardofDirectorsdiscussedandagreeduponanewwayforwardfortheorganisation.ThisnewapproachwillfocusondeliveringtheWorldCancerDeclarationtargetsbyextendingUICCmembershiparoundtheworld,byfurtherdevelopingitsadvocacyroleworkingwithourchosenpartnersandbysubstantiallyincreasingthefundingandactivityoftheorganisationatagloballevel.

UICCnowhasover350organisationalmembersinmorethan110countries,withambitionstogrowthisnumbertomorethan750membersinthecomingyears.UICCended2009withnearly200,000signatoriestotheWorldCancerDeclaration;ourgoalistoincreasethisnumbertomorethan1million.Ouraspirationsarehigh,butreflectthesizeoftheproblemandtheworkthatmustbedoneifwewanttogiveourchildrenabetterchancetoavoidandsurvivecancer.
InSeptember2009,theUICCBoardofDirectorsappointedCaryAdamsasUICC’snewCEO.Hejoinstheorganisationatatimewhendrive,enthusiasmandcommitmentbecomethehallmarksofourcommunity.
Inthisreport,wedrawontheachievementsofourmemberstoshowhowwearecollectivelymakingadifferencearoundtheworld.

Approximately40%ofcancerscanbepreventedandthroughthesupportofUICC’smembersandpartners,WorldCancerDayhasbecomeahugelysuccessfulcampaign–raisingawarenessaboutthesimplestepseveryonecantaketoreducetheirriskofcancer.
In2010,thecampaign’smessage“Cancercanbepreventedtoo”reachedanestimated2.4billionpeople.Thiswasachievedthroughanambitiousonlineandmediacampaignincludinge-cards,areporthighlightingthelinksbetweeninfectionsandcancerandapostercampaign(seesamplebelow).
CancerCAN BE PREVENTED TOO
UICChasevengreaterambitionsfor2011andbeyond,buildingonthepreventiontheme,andwewillexploreriskfactorsandcontinuetoencouragepeopletoadopthealthylifestylechoices,focusingontopicssuchas:
• Tobaccouseandpassivesmoking
• Alcoholconsumption
• Physicalactivity
• Healthydietandweight
• Sunexposureand
• Infections
UICCmembershaveaccesstoawiderangeofmaterialsandtoolstohelpthemrunsuccessfulcampaigns.PleasefindbelowanexampleofaposterprintedforWorldCancerDay2010.
UICCisresponsibleforcoordinatingWorldCancerDay–aninternationaleventrecognisedbytheWorldHealthOrganization-thattakesplaceonthe

2009provedverybusyfortheprogrammeasitmovedintoitssecondyear.Inparalleltobuildingandstrengtheningpartnershipsandcoalitionefforts,keyactivitiesincludedsupportingLEEPworkshopswithinaWHOseeandtreatdemonstrationprojectinsixAfricancountries;workingwithpartnersinNicaraguaandLatinAmericaontrainingacoregroupofhealthprofessionals;initiatinganopportunisticreviewofthemostfrequentlyusedtrainingmaterialsforcervicalcancer;anddevelopingtheUICCcervicalcancercurriculum.
TheroleofUICCistoconnect,mobiliseandsupportmemberstodeliverlocalsolutions.ThekeyfocusoftheTanzanianpilotprojectwastoencourageeffectivecoordinationofstakeholdersandbuildlocalcapacity.AsituationanalysisonthefeasibilityofintroducingtheHPVvaccinationwascarriedoutwiththesupportoftheSwissAgencyforDevelopmentandCooperation.Keypartnersandstakeholderswerealsomappedtoidentifyareasofoverlapandpotentialsynergy.Strengtheninglocalcapacitytoachieveasustainablecervicalcancercontrolprogrammewasprovidedviafellowshipsandtrainingworkshops.
UndertheleadershipoftheMinisterofHealthandSocialWelfare,atechnicalworkinggroupareworkingtofinaliseanationalplanforcervicalcancercontrol.TheUICCwillcontinuetosupportthisprojectinTanzania,mainlythroughfellowshipsandcapacitybuilding.
WhiletheeffortsinTanzaniaareongoing,in2010thefocuswillshifttoimplementingapilotprojectinNicaragua.UICCwillworkwithlocalmembersandpartnerstostrengthenpartnershipsforprofessionaleducationinLatinAmerica.
Within the framework of advocacy|partnership|localsolutionsthe programme focuses on four priorites:
1. Advocacyforaffordableandcomprehensivecervicalcancerpreventionprogrammes;
2. Raisingawareness;
3. Providingeducationalresourcesandtrainingtohealthprofessionalsandpolicy-makers;
4. Pilotingpreventionprojectswithmemberorganisationsandpartners.
CervicalCANCER INITIATIVE
TheUICCCervicalCancerInitiative(CCI)isafour-yearprogramme,
whichaimstosupportcomprehensivecervicalcancerpreventionandcontrol
worldwide.
The key activity was the implementation of a pilot project in Tanzania. The aim of
this project is:
• tofacilitatetheintroductionofnewpreventiontechnologiesforcervicalcancer,includingscreeningandvaccination,
• topromoteandsupporttheformulationofnationalpoliciesoncervicalcancerpreventionandcontrol.
Key recommendations for the pilot project in Tanzania include the need for:
• ministerialapprovalforanationalcancercontrolstrategyandadedicatedlineinthenationalbudget,
• developingandstrengtheningactivitieswhicharecross-cuttingand/orcanbenefitthewholesystem(e.g.cancerprevention/healthpromotion/NonCommunicableDiseasescontrol;cancerregistration,HealthManagementInformationSystems;andthedevelopmentandimplementationofnationalguidelines).

MyChildMatters,ajointinitiativebetweensanofi-aventisandUICC,nowcounts39fundedprojectsfrom26countries.Havinggrowntobeoneofthelargestchildhoodcancerinitiativesinlow-andmiddle-incomecountries,thefourthyearwelcomedsevenadditionalprojectsinCameroon,Ecuador,Mozambique,PanamaandThailand.
Thescientificevidenceillustratesthatthemajorityofthe160,000childrenaged14andunderdiagnosedwithcancerannually,couldanticipateacureifthecancerisdetectedearlyandappropriatetreatmentisavailable.TheMCMprogrammewasdesignedtoaddressashockingdisparity:inwellresourcedsettingssurvivalratesforchildhoodcancercanreach80%ormore,whileinlow-andmiddle-incomecountriesthispercentagecandropto50%orevenaslowas10%.
Withanambitiontoimprovethesurvivalratesindevelopingcountries,theprogrammehasfundedavarietyofprojectsinthelast4years,whichhavecontributedtotheimprovementofcancercareby:
• raisingpublicawareness,
• improvingprofessionaleducation,
• accesstoearlierdiagnosisandtreatment,
• painmanagementandpalliativecare,
• aswellassupportingthesocialandeconomicaspectsofthediseaseforchildrenandtheirfamilies.
TheMCMprojectportfolioaddressesmultipletargetsdescribedintheUICCWorldCancerDeclaration.
Thesuccessoftheprogrammereliesonasolidpartnershipwithsanofi-aventis,effectivementorshipthroughanetworkofinternationalexpertsinpaediatriconcology,fundamentalresearch,publichealthandtheleadershipprovidedbylocalprojectcoordinators.
InJuly2009,MCMawardeesandsupportersmetinDakar,Senegaltoreportonprogressandsharetheirexperiences.Representativesfrominternationalorganisations,nationalinstitutionsandhospitalsfrom21
countriesengagedwithgovernmentalrepresentativestoraisetheprofileofchildhoodcancerinSenegal.KeyachievementsoftheMCMprojectedwerepresented,including:asignificantreductioninthenumberofpatientsabandoningtreatmentinHonduras;thecreationofthefirstpalliativecarecentreinBangladesh;improvingaccesstotreatmentandsurvivalratesforchildrenwithBurkitt’slymphomainTanzania;andtheimplementationofanationalpainmanagementstrategyinMorocco.
Inearly2010,sanofi-aventisreneweditscommitmenttotheMyChildMattersprogrammefortheperiod2010-2013throughthepromiseoffinancialsupporttothesumof3millionEuros.
TheaimoftheMyChildMattersprogrammeistodemonstratesuccessinthefightagainstchildhoodcancer,whichcanbebuiltonbylocalprojectcoordinatorsinalong-termsustainablemanner.TheprogrammeinTanzaniaisanexampleofthatsuccesshavingreceivedbothlocalsupportandadditionalfundingthroughtheSymphasisFoundationtotreatBurkitt’sLymphoma,otherchildhoodcancers,aswellasexpandingaccesstocare.
TheMyChildMattersprogrammehasdemonstratedtheeffectivenessofthetwinningmodelinsupportingprofessionaldevelopment.UICChasbeenabletoexpanduponthisconcepttocreateanewpaediatriconcologyteaminHue,Vietnam.TheprogrammesupportedbytheSchellenbergFoundationandtheCantonVaud,Switzerlandwillcommencein2010attheUniversityofLausanne,totrainateamofpaediatriconcologists,nursesandpathologists,thuscreatingadedicatedchildhoodcancerwardinaregionofVietnamthathasnospecialisedcentres.

CapacityBUILDING FUND
TheCapacityBuildingFundwaslaunchedin2007toassistUICC
memberorganisationsinlow-andmiddle-incomecountriesdeliversmall-scalepreventionprojects.UICCoverseesandmanagesthe
Fund,whichisdirectedbyaSteeringCommitteewithrepresentativesfrommajorcontributorsandtheAmerican
CancerSociety.
ThepurposeoftheFundistostrengthenthecapacityofvoluntaryorganisationstodeliversustainablecommunity-basedprojects.TheprogrammeisopentoUICCmemberorganisationslocatedinlow-andmiddle-incomecountries,andislimitedtocommunity-based,non-governmentalorganisations.Ingeneral,projectsarerelatedtoeducation,raisingawareness,andtrainingforhealthprofessionalsandthegeneralpublic.

Todate,theCapacityBuildingFundhassupported56projectsin24countries,deliveringpreventionservicesandtrainingforhealthworkers.Since2007,atotalofUS$680,000hasbeencommittedtocommunityprojectsinlow-andmiddle-incomecountriesthroughtheFund.Recipientsoffundinglearntocollaboratewithprivateinstitutions,governmentsandotherNGO’s.
TheFundcontributestoUICCinanumberofways,includingincreasingtheprofileoftheorganisation,generatingvaluablememberparticipation,andraisingUICCcredibilitywithlocalNGOs.
Inaddition,theFundcontributesdirectlytotheWorldCancerDeclaration,withrecentProjectsawardedtospecificWorldCancerDeclarationTargets.
TheUICCCapacityBuildingFundhasmadeasignificantimpactinlow-andmiddle-incomecountriesinjust3years,withpositivefeedbackfromparticipatingorganisations.Itservesasamodelprogrammeforincreasingcapacityofcommunity-basedorganisationsandhasproventobeaneffectivevehicleforengagingtheglobalcancercommunityintheUICCmissionandWorldCancerDeclaration.

trainingofhealthcareproviders,concernaboutdiversion,addiction,andabuse,
andculturalmisperceptionsaboutpaincreateawebofbarriersthatforcemillionsofpeopletoliveanddiewith
treatablepain.Accesstopaintreatmentisparticularlyacuteinlowandmiddle-incomecountrieswhere70%ofcancerdeathsand99%ofHIVdeathsinthe
worldoccur,butjust9%ofthemorphineisconsumed.23
GAPRIGlobal Access to Pain Relief
Thecancercommunity’scommitmenttoaddressinguntreatedpainwascodifiedintheWorldCancerDeclaration’starget8:Effectivepaincontrolmeasureswillbeavailableuniversallytoallcancerpatientsinpainby2020.In2009,theUICCdecidedtofocusontheissueofuntreatedcancerpain.WithsupportfromtheAmericanCancerSociety,UICCestablishedtheGlobalAccesstoPainReliefInitiative(GAPRI),headedbyUICCPresident,ProfessorDavidHill.
UICCconvenedmeetingsinMayandDecemberwithhigh-levelpractitionersfrompalliativecareorganisations,othercancerorganisations,governments,andtheWorldHealthOrganization(WHO)toassistindesigningastrategyforGAPRI.AnAdvisoryCommitteewasestablishedandtheinitiativewaspubliclylaunchedbyProfessorHillattheLIVESTRONGGlobalCancerSummitinAugust2009.
OneoftheearlyareasoffocusforGAPRIwasthebarrierstoaccesstoopioidanalgesicscreatedbytheinternationaldrugregulatorysystem.AttheMarch2010meetingoftheUNCommissiononNarcoticDrugsinVienna,UICCco-hostedaside-eventwithHumanRightsWatch,theViennaNGOCommitteeonDrugs,theSwissGovernmentandtheWHO,whichdrewattentiontotheneedforbalancedpoliciesthatbothpromoteaccesstoopioidsformedicaluseandpreventmisuse.Historically,effortstopreventmisusehavereceivedmoreattentionthanthoseaimedatensuringuseformedicalpurposes.GAPRIhasreachedouttogovernments,theInternationalNarcoticsControlBoardandtheUnitedNationsOfficeonDrugsandCrimetoadvocateforagreaterfocusonensuringaccess.
GAPRIalsocommissionedacomprehensivebackgroundreportbyProfessorAllynTaylor,oftheGeorgetownLawSchool,whohaspublishedontheinternationallegalbarrierstoopioidavailability.Thisreportprovidesacontemporaryandcomprehensiveoverviewoftheglobalpaintreatmentlandscape,thedisparityinaccesstotreatment,andthemultiplechallengesthatmustbeaddressedtoeffectivelysolvetheproblem.ThereportwillbemadeavailabletothepublicontheGAPRIwebsiteandwillbeusedtoshapeandinformfutureGAPRIactivities.
Theyearendedwiththedevelopmentofplansfor2010,whichispoisedtobeaveryactiveandfruitfulyearforGAPRI.Wehavedevelopedastrategicplanandarefundraisingaroundsixflagshipprojects:
1. TheGAPRIFellowshipprogrammepairsatechnicalcatalystwithagovernmentofficialinalowormiddle-incomecountryforoneyeartoassistthegovernmentwithincreasingaccesstoanddemandforopioidanalgesics.
2. TheGAPRIPainTreatmentAdvocacyProjectraisesawarenessaboutthelargeunmetneedforpainrelieftogettheissueontheagendaofglobalhealthprogrammes,donors,nationalgovernments,andthegeneralpublic.
3. TheGAPRIInternationalAdvocacyProjectworkswithinternationalpolicysettingbodiestocreatecoherenceinregulatoryguidancearoundnarcoticprocurementanddistribution.
4. TheGAPRIProjectonDiversionandAbuseseekstobringtogetherorganisationsworkingondifferentaspectsofnarcoticcontrolandaccesstodevelopaclearunderstandingofthepracticalrequirementstoimproveaccesstoopioidsforscientificandmedicaluseinamannerthatminimizestheriskfordiversionandabuse.
5. TheGAPRIEnvoyProgrammeleveragesthemembershipbaseofUICCtocreateagroupofpartnersfromvariousorganisationsinourfocuscountrieswhosupportGAPRIeffortsthroughnetworkbuilding,advocacy,anddatagathering.
6. TheGAPRISupplierNetworkseekstoengagepainmedicationsupplierstodevelopmeaningfulandeffectivesupply-sidesolutionstoimproveaccesstoopioidanalgesics.

“TheAmericanCancerSociety’sBrazilBreastCancerActionprogrammeisaimedatstrengtheningbreastcancercontrolinBrazilthroughadvocacy,educationandinformation.Aspartoftheprogramme,theSocietyheldthefirstIntersectorialBreastCancerControlForumofSaoPauloState,convening80participants,includingrepresentativesofgovernment,healthcareandpatientorganisations.Participantsdiscussedhealthpoliciesandpractices,identifiedbarrierstoeffectivetreatment,anddevelopedpolicyrecommendationsforbreastcancerdesignedtoimprovetheavailabilityandqualityofearlydetection,diagnosis,treatment,andpalliativecare.
Aspartofarelatedmediaoutreacheffort,theSocietytrained44journalistsfromthefiveBrazilregionsinordertoimprovethequalityofmediacoverageandincreaseawarenessaboutcancerintheBrazilianpopulation.JournalistsdiscussedcommoncancermythsandwereeducatedontheBraziliancancerburden,cancerprevention,earlydetection,andtreatment.”

“CervicalcanceristheleadingcancerinTanzania.In2009,ORCIworkedtogetherwithUICCtofacilitateanHPVvaccinationfeasibilitystudyaimingtoassessthecapacityandbarriersfortheimplementationofacomprehensiveHPVpreventioninterventionthatincludesvaccination,advocacy,publicinformation,professionaleducationandscreening.

AsaresultofattendingtheLIVESTRONGGlobalCancerSummit,97%ofsurveyrespondentslearnedsomethingormetsomeonewhowillhelpthemaccomplishtheirgoals.99%agreedtheSummitprovidedauniqueplatformwhichwillsignificantlyadvancetheglobalfightagainstcancer.100%saidthattheyfeltempoweredwithnewwaysofengagingothersintheglobalfightagainstcancer.

TheCancerSocietyofFinlandheldameetingformediaandresearchersinterestedinusingtheFinnishversionoftheNORDCANdatabase.Asuccessfulmeetingdemonstratedtheneedofsuchworkshopsinthefuture.
AnotherInternettoolwaspublishedinJune2009whenavaststudyoncancerandprofessionswaspublished.ProfessorEeroPukkala,DirectorofStatisticsattheFinnishCancerRegistry,ledtheNordicresearchteamincollectingthedataon15millionpeoplebyprofession.Somethreemillioncasesofcancerwererecordedinthis
population.Informationonoccupationswasamassedfromcensusescarriedoutfromthe1960stothe1990sinFinland,Sweden,Norway,DenmarkandIceland.Thefollow-upextendedto2005.ThestudyfindingshavebeenpublishedintheJune2009ActaOncologica,theofficialjournalofthefiveNordiconcologicalsocieties.”

ThefinancialpositionofUICCremainedstableduringthereportingyear.UICC’soperatingexpenditurewaswellbalancedversusitsrevenuesandthefinancialyearclosedwithamodestsurplus.
ThefundsreceivedbyUICCweresupportingawiderangeofactivitieswithfellowships,theglobalpreventioncampaignandcapacitybuildingrepresentingthelargestshareofthetotalexpenditureforprojectsandactivities.
RestrictedincomefromgrantsandotherdesignatedcontributionsforspecificactivitiesundertakenbyUICCrepresentedthemajorityofthetotalincome.Theother,unrestricted,revenueswereprimarilyderivedfrommemberandcorporatepartnercontributionsandincomefromscientificpublications,helpingtosupportthegeneralorganisationalcosts.
