Assessment and Management of Liver Fibrosis and Cirrhosis

Dr Jones & Partners is able to provide Liver Elastography scans (a non-invasive, ultrasound scan). This scan can help accurately assess patients with minimal fibrosis or with severe fibrosis/cirrhosis.

Several new medications have been made available on the PBS for the treatment of patients with Hepatitis C. These medications enable GPs to effectively treat and manage affected patients to minimise liver damage.

Elastography of the liver can be referred for both the initial assessment of disease (baseline) and monitoring of response to treatment (progression/response).

An Elastography Ultrasound scan is Medicare rebated. It can be referred by both

GPs and Specialists.

Elastography is used for the initial assessment and ongoing management of patients with:

+ Hepatitis B + Hepatitis C + Chronic liver disease + Liver fibrosis + History of heavy alcohol use/abuse + Potential cirrhosis from non-alcoholic

fatty liver disease.

The referring clinician should request “Liver Elasto” - one of the clinical indications above should be detailed on the referral form.

Patient Preparation - Standard preparation for an Upper Abdominal Ultrasound. Patients are required to fast for 6 hours prior to the exam.

For bookings contact the nearest Dr Jones & Partners Medical Imaging clinic.

Example of Cirrhosis

Grey scale changes to the appearance of liver

Internal and surface nodularity

Liver elastography measurement performed

Elastography/Stiffness measurement of 12.1 kPa. This equates to F4 on the Metavir score (Cirrhosis)

Example of Fibrosis

Minimal grey scale changes noted except for increased echogenicity of the liver

Liver Elastography measurement performed with confidence map displayed

Elastography/Stiffness measurement of 6.57 kPa, that equates to F2 on the Metavir score. This is an example of significant fibrosis despite minimal changes noted on grey scale ultrasound imaging

Liver Fibrosis Classification

Metavir ScoreShear Wave kPa

Range

Normal – Mild F0 – F1 < 6.43

Significant F2 6.43 – 9.54

Advanced F3 9.54 – 11.34

Cirrhosis F4 > 11.34
