Division of Nursing (DN), Bureaus of Health Professions (BHPr), Health Resources and Services Administration (HRSA), Department of Health and Human Services (HHS) under grant number 6D09HP25938 and title “Advancing Nursing Education: Interprofessional Collaboration addressing Multiple Chronic Conditions in Rural Communities” for $997,173.

Focus Training Grant

The focus is on the implementation of an IPE model that prepares students with requisite knowledge needed to address MCC through Interprofessional Grand Round teams

The ANE-IPC-MMC proposes to advance nursing education and practice by engaging NP students and other graduate health trainees in shared interprofessional clinical and didactic experiences, to prepare them to work in rural communities.

IPGR Process Referrals from Community Partners

Patient participates for up to a year, with weekly check-in’s with NP and other team members as needed

NP students – Initial H&P Utilizing portable TM equipment Store and Forward data collected Enter info into EHR

All IPGR team members meet and utilize technology to communicate with each other and the patient (patient-centered care) Live feed facilitated by the NP on site with patient

IPGR teams develop IP Plan of Care IPGR teams present to lead faculty in all professions Revise as needed Present to PCP Implement approved IP Plan of Care with patient F/U up to a year with patient, weekly basis

Orientation to Technology $227,030 TM Equipment 6 Mobile Telemedicine Packages Videoconferencing Infrastructure iPads with 3G/4G Bandwidth in 4 Rural Clinics
