DESCRIPTION OF RESEARCH FOCUS
My research focus can be divided into 2 main domains:
Since 1998 when I started my specialization in cardiology, my
research interest and therefore my studies were mainly clinically
oriented. During this period I got involved with research protocols
associated with acute cardiology (paroxysmal atrial fibrillation, acute
myocardial infarction). At the same time I participated in the study
ASSENT II (Α phase III, randomized, double-blind, parallel group,
international trial of single bolus of TNK-tissue plasminogen activator,
TNK-tPA, versus accelerated infusion of rt-PA in acute myocardial
infarction: Assessment of the Safety and Efficacy of a new
Thrombolytic Agent), 1998. My interest at the time, other than TNK-
tissue plasminogen was to record pharmacological therapies of atrial
9
fibrillation (including different formulations of amiodarone and
comparison of intravenous amiodarone and procainamide. In addition,
I studied and published the pathophysiology and physiology of the
myocardial tissue and specific biochemical markers, which can
predict neurological outcome. These activities hace led to publications
in International Journal of Cardiology, Minerva Cardioangiologica,
Resuscitation
Since 2002, my research focus shifted to more basic research. This is
reflected in my 13 year dedication in an experimental laboratory.
Since 2004, when I was placed at the Department of Experimental
Surgery and Surgical Research of the University of Athens, Medical
School, my research interest has been in experimental protocols of
“Cardiac Arrest-Cardiopulmonary Resuscitation-Physiology and
Pharmacology”.
• I organized the laboratory in order to sustain my research in
cardiopulmonary resuscitation, and I developed novel experimental
model of cardiac arrest induction. Moreover, I developed a model of
asphyxia and a model of severe hemorrhagic shock. These models
were published in Laboratory Animals and Comparative medicine.
In these models which were replicated world-wide were used to test
the efficacy, safety and pharmacodymanics of several
pharmacological agents.
My experimental activities are characterized mainly by:
Α. The development of animal models, where all my pharmacological
studies are conducted, namely:
1. The study of novel pharmaceutical agents in the management of
cardiac arrest, such as the beta-blockers and levosimendan for the
first time internationally, studies published in Am J Emerg Med,
Acta Anaesthesiol Scand, Circulation, multiple publications in
Resuscitation and pharmacological journals.
2. The supervision of clinical studies associated with clinical
pharmacology. These publications explored in depth the physiology
10
and pathophysiology of the cardiovascular system leading in
publications in Heart & Lung.
3. The application of multifactorial physiological cerebral
monitoring such as rSO2, TCD, and its association with molecular
indices such as S-100, NSE, IL-6, IL-9 in the blood, and tissues as
published in Resuscitation.
4. The comparison of the 2005 and 2000 guidelines on resuscitation a
study that was also published in Resuscitation.
5. The development of an experimental model of hemorrhagic shock
in which the effects of starches were studies in the management of
acute lung injury. This paper received the 1st Fresenius Cabi Award
and led to the a full paper publication in Anesthesiology.
The result of these research activities was:
a) The presentation of the results of our experiments in national and
international fora and
b) My invitation in the Scientific Meeting, March 2006 of the Αmerican
Heart Association (AHA) where I presented the possible beneficial
effects of beta-blockers in the management of cardiac arrest, an
application that was developed in our research facility.
This research activity is characterized by international originality as
we managed to restore spontaneous circulation after 8 and 10 min of
untreated cardiac arrest, with the addition of inotropes that do not
increase oxygen consumption by the myocardium. In addition we
started identifying the molecular basis of cardiopulmonary
resuscitation for the first time internationally. These activities have led
to the deeper understanding of the pathophysiology and pathogenesis
of post-resuscitation myocardial dysfunction and post-arrest injury
which for the first time combined all available data and provided a
theory that fully explains the physiological mechanisms of the
situation. These papers were published in Heart Failure Reviews and
Journal of Neurological Sciences. These activities attracted the media
11
in Greece and Internationally. For these activities, I received multiple
invitations for editorials in Journals such as Resuscitation, Acta
Physiologica and accepted many invitations to lecture in International
Fora. It was because of this increased recognition that collaborations
flourished with Universities from Europe and the United States of
America.
One of my research goals was to study all the pathophysiological
mechanisms of cardiac arrest and to improve the rates of return of
spontaneous circulation and survival in experimental models of
cardiac arrest with pharmacological agents. In addition, my focus was
restoration of normal cerebral function post cardiac arrest so as to
prevent permanent neurological damage. The result was 2 major
fundings from European funds, where I acted as the principal
investigator.
Pythagoras Ι: «Combination pharmacotherapy before defibrillation in a
swine model of prolonged cardiac arrest»
Pythagoras ΙΙ: «Co-administration of adrenaline, vasopressin and
nitroglycerin in a swine model of ventricular fibrillation»
In addition, I participated in more than 20 PhD theses with the same
research focus
• I am working closely with the departments of Microbiology of the
Aretaieon Hospital and the Department of Pathology of the
University of Athens and the department of informatics of the
University of Piraeus nationally and many other departments and
universities internationally.
Β. I created a new experimental model of cardiac arrest induction
following asphyxia, for the first time in Greece and I refined the
existing international protocol. Using this animal model we have
12
studied the pathophysiology of asphyxia and possible therapeutic
management, including multiple pharmacological agents.
• Our research group was one of the few internationally that
managed to increase survival in cardiac arrest. This is the reason
why I have been invited to many international for a, including ERC
and AHA congresses.
C. At the same time, I studied various aspects of medical education
with publications in international peer-reviewed journals. This led
to my recognition as a Fellow of the Academy of Medical Educators,
Fellow of the Higher Education Academy and the European
Resuscitation Educator for all Generic Instructor Courses. As I see
myself, as I lifelong learner more than an expert, I continue to
improve my qualification through formal education continuously. I
also studied in depth Toxicology receiving my Post-graduate
Diploma in Medical Toxicology from the University of Cardiff. As a
result of these studies, I have deeply comprehended the
pharmacodynamics and pharmacogenomics of all therapeutic
agents for all the systems of the human body
• I was the first to use videolaryngoscopes in didactic studies in the
airway management in simulated cardiac arrest scenarios in adults
and in neonates. This activity led to a deeper understanding of the
anatomy and physiology of the upper-airway anatomy and
physiology and proved that the standard Macintosh blade may not
be as efficient as alternatives in the management of the airway.
• Ι have examined the catastrophic results of excessive ventilation,
especially when chest compressions are interrupted in cardiac
arrest. That is the reason why we have developed a novel approach
of securing the airway without chest compressions interruption.
13
These studies have been published in international peer-reviewed
journals.
D. At the same time at the Research Laboratory of the Pharmaceutical
industry of ELPEN I developed a model of neonatal normocapnic
hypoxia in newborn swine for the first time internationally. During
these experiments we tested whether resuscitation of newborn
piglets could be achieved with various oxygen concentrations.
These studies have already been accepted in National and
International Congresses (17th European Workshop on
Neonatology, 15ο Panhellenic Perinatal Congress). Due to this
research activity we developed our collaboration with the university
of Cagliari Italy, where we have studied metabolomics in neonatal
resuscitation. In addition, due to my International recognition as a
pharmacology expert in all stages of the human development, I
worked closely with the University of Cagliari, the University of
Oslo, the Midewestern University of Chicago etc.
Simultaneously, I developed my interest for medical education
which went beyond simply publishing retention of skills, but was
formally developed by the post graduate Certificate and the Post
Graduate Diploma in Medical Education, where I had the chance to
see beyond the eyes of the Andragogy and understand and practice
and investigate various learning theories. I designed new curricula
and I published these findings in journals like Heart & Lung,
Resuscitation, etc. The continuous activity in the research lab has
led to many original publications, traditional reviews meta-
analyses and editorials in several International Journals.
At the same time, I went back to oversee and design and clinical
pharmacological studies. We have applied for a grant regarding
vasopressin and steroids in the critical care unit. This study will
have a clinical and a laboratory arm, where we will study the
14
effects of steroids in hypothalamic pituary axis clinically and
laboratory studies. The principal investigator of the clinical arm
will be Professor Zakynthinos and the principal investigator for the
laboratory arm will be me.
All these activities have led to several visiting professorships
several invited talks in International Congresses and several
collaborations with other universities of Europe and the USA and

TEACHING EXPERIENCE
I started teaching when I was an Internal Medicine Resident in the
Internal Medicine Department of the Psychiatric Hospital of Athens.
During this period, I taught all basic medical subjects in the Middle
grade Professional Nursing School of the psychiatric hospital of
Athens. I was the instructor responsible for the teaching and
preparation for the professional examination.
As a resident in Cardiology in the General Hospital of Nikea, I taught
Cardiology- Cardiopulmonary resuscitation for the surgical
specialty of the nurses. The same period I became an instructor for
the Hellenic Society of Cardiopulmonary Resuscitation, after
completing both Provider and Instructor Courses.
Later on, I started teaching for the European Resuscitation Council
courses, initially as an instructor candidate and later on as an
instructor. By participating in these courses I became initially Co-
Director, later on Director and I finally became Organizer, which is
the highest rank for teaching and organizing courses for the European
Resuscitation Council. The courses I organize are both at provider and
at instructor level. I have trained more than 2000 people in these
courses, both healthcare professionals and lay-people.
Because of my experience in teaching and my multiple publications in
the area of Cardiac Arrest-Cardiopulmonary Resuscitation, I was
invited by the Central Council of Health of the Ministry of Health to
participate in the National Committee of Cardiopulmonary
Resuscitation. This committee advises for the adequacy and
credibility of the resuscitation courses in Greece, and on the training
of the healthcare personnel of the Greek Hospitals. During my
presence in this Committee, we managed to legislate:
99
1. The compulsory training of all health care professionals in Basic
Life Support and Automated External Defibrillator and in
Immediate Life Support Course.
2. The application of the «Public Access Defibrillation Program»
which legally covers the rescuer to administer defibrillation in
public places, while for the first time in Greece we developed a
plan for automated external defibrillators in public places.
My participation in medical education covers a broad spectrum of
lectures and activities, initially at undergraduate level and later on in
post-graduate level. I have taught the subjects «Experimental
Surgery» «Cardiopulmonary resuscitation» «Descriptive Anatomy»
and «Human Anatomy» in the 2nd, 3rd, 4th, 5th and 6th year of the
medical curriculum. I was then motivated to organize an MSc program
entitled «Cardiopulmonary resuscitation» which is administered by the
Medical School of the University of Athens. In this program, medical
and nursing school graduates are normally accepted following a
formal interview. This MSc program aims at enhancing research on
cardiopulmonary resuscitation and to guide its students into a deeper
understanding of the way guidelines on resuscitation are created by
the various International Organizations. This MSc program admits
students for more than 7 years and is the MSc program that has the
largest number of applications.
Besides being the Scientific Moderator of the MSc, I lecture and I am
the Course Director of all the courses that are being held in the MSc.
These courses are always organized according to the principles set by
the International Organizations; our students apply in real time
simulation all the peri-arrest algorithms and the cardiac arrest
algorithm. Finally, I am the Scientific Director for the Experimental
Part of the MSc. During this course, our students are being trained in
small groups in the principles of basic research of Cardiopulmonary
100
resuscitation, and they practice in all 3 experimental models we have
elaborated and refined:
Ventricular fibrillation induction
Experimental Model of asphyxial cardiac arrest
Experimental Model of Hemorrhagic Shock
It was because of this work that the American College of Emergency
Physicians gave me permission to organize all of it courses in Greece.
Up to now, we have organized Advanced Cardiac Life Support
(ACLS), both at provider and instructor level, and we have managed to
create a critical core of instructors able to teach resuscitation. This
core of instructors consists both of doctors and nurses.
I was invited to give talks because of my research interest of
Pharmacology of Cardiopulmonary resuscitation and I was also invited
to give a lecture entitled “Beta-blockade in swine cardiac arrest” for
the American Heart Association, where distinguished scientists from
all over the world suggested therapeutic strategies to improve the
outcome of cardiac arrest victims. For all my teaching activities I keep
teaching and reflective portfolios.
