s Compex in close proximity to anyother medical device, please consultits manufacturer or your doctor.■Do not disconnect the stimulatorelectrode cables during the sessionwithout first switching off the stimulator.■Never connect stimulation cables toan external electrical power supplybecause of risk of electrical shock.■Never use the Compex if it is dam-aged (case, cables, etc.) or if the bat-tery compartment is open. There is arisk of electrical discharge.■Keep the Compex and its acces-sories out of the reach of children.■Do not allow any foreign items(soil, water, metal, etc.) to enter intothe Compex and the battery compartment.■Sudden temperature variations cancreate droplets in the device due tocondensation. Use the device onlyonce it reached ambient temperature.■Do not use the Compex while driv-ing or operating machinery.■Do not use the stimulator in themountains at an altitude higher than3.000 metres.

Where you should never applyelectrodes…■To the head.■Counter-laterally: Do not use thetwo poles of the same channel (oneelectrode connected to thepositive/red pole and one electrodeconnected to the negative/blackpole) on either side of the body cen-treline (eg: right arm and left arm).■To or in the vicinity of skin lesionsof any kind (wounds, inflammations,burns, irritations, eczema, etc.).

Precautions when using the electrodes■Only use the electrodes suppliedby Compex. Other electrodes may

have electrical properties that areunsuitable for the Compex stimulator.■Do not remove or move the elec-trodes during a stimulation sessionwithout first switching off the instrument.■Do not plunge the electrodes intowater.■Do not apply a solvent of anykind to the electrodes.■Before applying the electrodes, itis recommended to wash and de-grease the skin, and then dry it.■Apply the whole surface of theelectrodes to the skin.■For very important reasons of hy-giene, each user must have his ownelectrode set. Do not use the sameelectrodes on different people.■Do not use a set of electrodes formore than fifteen sessions, as thequality of the contact between theelectrode and the skin, which is important for the comfort and effectiveness of stimulation, pro-gressively degrades.■ In certain people with very sensi-tive skin, redness may be observedunder the electrodes after a stimula-tion session. Generally, this rednessis completely harmless and disap-pears after 10 to 20 minutes. How-ever, avoid starting a stimulationsession on the same area until theredness is no longer evident.

Precautions when using the batteries■Do not open the battery compart-ment without first switching off thestimulator.■Use only alkaline batteries 1.5 V,size AA, LR6. Never use batterieswith other characteristics.■All the four batteries must be replaced at the same time.

I. WARNINGS

1. Counter-indications

Important counter-indications■Cardiac stimulator (pacemaker)■Epilepsy■Pregnancy (no positioning in theabdominal area)■Serious arterial circulatory prob-lems in the lower limbs■Abdominal or inguinal hernia

Precautions when using theCompex■After trauma or a recent operation(less than 6 months)■Muscular atrophy■Persistent pains■Need for muscular rehabilitation

In all of the above cases, you are advised to:■never use the Compex for pro-longed periods without medical advice;■ consult your doctor if you are inany doubt whatsoever;■ read this manual carefully, in par-ticular chapter VI, which provides in-formation concerning the effects andindications of each stimulation pro-gramme.

Osteosynthesis equipmentThe presence of osteosynthesis equipment(metallic equipment in contact with thebone: pins, screws, plates, prostheses, etc.)is not a contra-indication for the use ofCompex programmes. The electrical cur-rent of the Compex is specially designed tohave no harmful effect with regard to os-teosynthesis equipment.

2. Safety measures

What you should not do withthe Compex■Do not use the Compex in water orin a humid atmosphere (sauna, hy-drotherapy, etc.).■Never carry out an initial stimula-tion session on a person who isstanding. The first five minutes ofstimulation must always be per-formed on a person who is sitting orlying down. In rare instances, veryhighly strung people may have avagal reaction. This is of psychologi-cal origin and is connected with afear of the muscle stimulation as wellas surprise at seeing one of theirmuscles contract without having in-tentionally contracted it themselves.This vagal reaction produces a feel-ing of weakness with a fainting ten-dency, slowing down of the heartrateand a reduction in arterial bloodpressure. If this does occur, all that isrequired is to stop the stimulationand lie down with the legs raised for the time it takes for the feeling of weakness to disappear (5 to10 minutes).■Never allow movement resultingfrom muscular contraction during astimulation session. You should al-ways stimulate isometrically; thismeans that the extremities of the limbin which a muscle is being stimulatedmust be firmly fixed, so as to preventthe movement that results from contraction.■Do not use the device at less than1.5 metres from a short-wave device,a micro-wave device or high fre-quency (H.F.) surgical equipment toavoid the risk of causing skin irrita-tions or burns under the electrodes. Ifyou are in any doubt about using the

s ■Batteries must be positioned ac-cording to the diagram inside the bat-tery compartment.■Do not allow any foreign items(object, water, sand, dust, etc.) toenter into the battery compartment.■Always close the battery compart-ment before switching off the device.■Remove immediately the batteries ifthe Compex “bleeps” continuously,

in case of abnormal heating or smell,or if smoke comes from the device.■Never leave the batteries insideyour device if you do not intend touse it for a long period of time. Bat-teries may leak and damage yourunit. If the batteries show signs of aleak, avoid contact with skin, eyesand clothing. In case of contact, washsurface with water and contact yourdoctor.

5. Storage conditions

The storage conditions must not ex-ceed the following figures:Storage temperature: from -20°C to 45°C

Never leave the batteries inside yourdevice if you do not intend to use itfor a long period of time.

6. Use conditions

Do not use in an explosion risk area.

For environmental protection the de-vice, the batteries and its accessorieshave to be eliminated with specialwaste.

The Compex is based directly onmedical technology.To guarantee your safety, the design,manufacturing and distribution ofCompex are in conformity with therequirements of the European Direc-tive 93/42/CEE.The device is in conformity with thestandard for general safety rules forelectromedical devices IEC 60601-1,the standard for electromagneticcompatibility IEC 60601-1-2 and thestandard for special safety rules fornerve and muscle stimulators IEC60601-2-10.Current international standards (IEC60601-2-10 AM1 2001) require that awarning be given concerning the ap-plication of electrodes to the thorax(increased risk of cardiac fibrillation).

for the stimulation pulses can exceed10 mA and 10 V. The informationgiven in this handbook must bestrictly observed.

3. Guarantee

The Compex stimulators are contrac-tually covered with a guarantee of 2(two) years. Register on our web site:www.compex.info.The Compex guarantee comes intoeffect on the date of purchase of thedevice.The Compex guarantee applies to thestimulator (goods and work) anddoes not cover cables and electrodes.It covers all the defects resulting froma problem of quality of the materialor from a defective manufacture. Theguarantee does not apply if the de-vice was damaged further to a shock,an accident, a false operation, an in-sufficient protection against the hu-midity, a dumping or a repair notmade by our after-sales services.

The guarantee is valid only on pres-entation of proof of purchase.

Legal rights are not affected by thisguarantee.

4. Maintenance

To clean your unit, use a soft dusterand an alcohol-based cleaning prod-uct, which does not contain any sol-vents. In fact, solvents could damagethe plastic parts, especially the panelcovering the screen of your Compex.The user must not attempt any repairsto the device or any of its accessories.Never dismantle the Compex contain-ing high-voltage parts because of riskof electrical discharge.Compex Médical SA declines all re-sponsibility for damages and conse-quences resulting from any attempt toopen, modify or repair the device orany of its components by a person ora service centre not officially ap-proved by Compex Médical SA.Compex stimulators do not requirecalibration or verification of perform-

3b To set the contrast of the display, press the“+”/“–” key of channel 2 (“+” to increase the pre-ferred percentage and “–” to reduce it).

3c To set the sound volume, press the “+”/“–” ofchannel 3 (“+” to increase the preferred percent-age and “–” to reduce it).

3d To confirm the parameters selected, pressthe “On/Off” switch. Your stimulator saves youroptions. It is now ready for use with the settingsyou selected.

4. Selecting your motivation

To switch on your stimulator, brieflypress the “On/Off” switch on the leftof the Compex. A musical signal canbe heard and a screen showing thedifferent motivations is displayed.Before choosing a programme, youshould select the desired motivation.

The electrical impulses generated bythe Compex are transmitted to the

nerves by self-adhesive electrodes.The choice of the size, the connection andthe correct positioning of the electrodes areessential factors for ensuring effective andcomfortable stimulation. So they requireparticular care.To do all this – and for information on therecommended stimulation positions – referto the pictures and pictograms on theposter, and to the “Table with placementsof electrodes and stimulation positions” inchapter VII. The specific applications con-tained in chapter VI also provide usefulrelevant information.

1. Inserting and replacingthe batteries

Before opening the battery compart-ment, you are strongly advised to takecareful note of the counter-indica-

tions and safety measures detailed at thebeginning of this manual (chapter I:“Warnings”).

Before using the unit for the first timeand to make it work, you must insertthe four batteries delivered with yourdevice into the battery compartmenton the back of your device.

III. DIRECTIONS FOR USE ANDINSTRUCTIONS

Before using the unit for the first time, you are strongly advised to take careful note ofthe counter-indications and safety measures detailed at the beginning of this manual(chapter I: “Warnings”), as this powerful equipment is neither a toy nor a gadget!

Use only alkaline batteries 1.5 V, sizeAA, LR6. Never use batteries with

other characteristics

Connecting the electrodes and cablesThe electrode cables are connected tothe stimulator through sockets on thebase of the device.Four cables can be connected simul-taneously to the four channels of thedevice.For easier use and better identifica-tion of the four channels, we adviseyou to follow the colour of the elec-trode cables and the stimulator sockets:blue = channel 1 yellow = channel 3green = channel 2 red = channel 4

3. Preliminary settings:language, light intensity and sound volume

Before using the unit for the firsttime, you should select the workinglanguage of the device which is dis-played on the options screen. Pro-ceed as per the instructions describedbelow.Afterwards, for the greatest comfort,Compex offers you a number of set-ting options (operating language se-lection, display contrast setting andsound volume setting).To do this, display the options screenby pressing the “On/Off” switch onthe left of the Compex and hold itdown for a few seconds.

Setting stimulation energyWhen the programme starts, the Com-pex prompts you to increase the stim-ulation energy, a key factor for theeffectiveness of the stimulation.

7e By pressing the “On/Off” key, you can returnto the previous screen.

Programme progressionStimulation does really start whenthe stimulation energy has been in-creased. The screens that are dis-played during stimulation allow youto observe the progression of the ses-sion. These screens may differ fromone programme to another. The ex-amples reproduced below are suffi-cient for you to understand thegeneral rules.

7a The Compex “bleeps” and the symbols of thefour channels flash, changing from “+” to “000”:the four channels are at 0 energy. You must in-crease the stimulation energy so that the stimula-tion can start. To do this, press the “+” of the keysof the relevant channels, until the desired settingis reached.If you want to increase the energy level of all fourchannels simultaneously, press the “i” button, lo-cated below the “On/Off” key. You can also increasethe energy level of three first channels, by press-ing the “i” button twice, or the energy level of twofirst channels only, by pressing three times thisbutton.Once you activate the “i” button, the associatedchannels are highlighted in white on a black background.

6. Personalising a programme

The programme personalisationscreen is not available for all

programmes!

6a Certain programmes require manual selec-tion of the muscular group that you want to stim-ulate and a figurine is displayed above channel 1.To select your desired group, press the “+”/“–”key of channel 1 (to go up or down). The sevenmuscular groups are displayed in succession inblack on the figurine:

Current international standards re-quire that a warning be given con-cerning the application of electrodes

to the thorax (increased risk of cardiacfibrillation).

6b The warming-up sequence suggested forsome programmes is active by default (an ani-mated rising convection symbol above theheater). If you prefer to skip this sequence, pressthe “+”/“–” key of channel 2.

Before using your stimulator for thefirst time, insert the four batteries intothe battery compartment (see section1: “Inserting and replacing the batteries”).The symbol of a small battery indi-cates the charge level of the batteries.12a The small battery is empty and it is ani-mated: batteries are flat. You must replace them(see section 1: “Inserting and replacing the batteries).

peared ( ): The batteries are completelyflat. It is no longer possible to use the device.Change the batteries immediately (see section 1:“Inserting and replacing the batteries”).

13a-14a The Compex “bleeps” andalternatively displays the symbol of a couple ofelectrodes and an arrow pointing to the channelwhere a problem has been detected. Above, thestimulator has detected an electrode fault onchannel 2. This message may mean that:1) there is no electrode connected to this channel;2) the electrodes are old, worn out and/or thecontact is poor: try using new electrodes;3) the electrode cable is defective: try it usinganother channel. If cable shows a default again,please replace.

8b-9b A small animated symbol indicating themotivation to which the programme belongs.

8c-9c Session progression scale:The progression scale is composed of three dis-tinct parts: the programme consists of threestimulation sequences, i.e., warming up (lighthatching /left), the actual work (black area/cen-tre) and relaxation at the end of the programme(heavier hatching/right).The progression scale can be also entirelyhatched: the programme then consists of a sin-gle stimulation sequence.The small cursor located just below the scalemoves horizontally and indicates precisely thestate of progression of the programme (here:start of the first sequence, i.e., the work sequence).

8d-9d Contraction and active rest time bar:This bar is displayed at the right of the progres-sion scale, but only during the work sequence. Itindicates the duration of the contraction and ac-tive rest times.8d The cursor is located under the black (left)part of the bar: you are in a muscular contrac-tion phase.9d The cursor is located under the hatched(right) part of the bar: you are in an active restphase.

The stimulation energy during theactive rest phase is automatically set

at 50% of the stimulation energy duringthe muscular contraction.

8e-9e Bar charts:8e The muscular contraction phase is repre-sented by the bar charts of the stimulationchannels used (which become black during thecontraction phase).9e The active rest phase is represented by thebar charts of the stimulation channels used(which become hatched during the active restphase).

8f-9f You can interrupt the programme mo-mentarily ( = “PAUSE”) by pressing the“On/Off” key. If you wish to restart the session,simply press the “+”/“–” key of channel 4.

IV. HOW DOES IT WORK?

The principle of electrostimulation isto stimulate nerve fibres by means ofelectrical impulses transmitted byelectrodes.The electrical impulses generated byCompex stimulators are high-qualityimpulses – ensuring safety, comfortand efficiency – that stimulate differ-ent types of nerve fibres:– the motor nerves to stimulate a muscular re-sponse. The quantity of work and the benefits ob-tained depend on the stimulation parameters.This is known as electromuscularstimulation(EMS).

1. Motor nerve stimulation(EMS)

second, contraction time, rest time,total programme time) subject themuscles to different types of work,according to muscular fibres. In fact,different types of muscular fibresmay be distinguished according totheir respective contraction speed:slow, intermediate and fast fibres.The fast fibres will obviously pre-dominate in a sprinter, while amarathon runner will have more slow fibres. VI. PROGRAMMES AND SPECIFICAPPLICATIONSThe applications that follow are given by way of example. They provide a bet-ter understanding of how electrostimulation sessions can be combined withvoluntary activity.

You can of course choose a musculargroup different from the one indi-cated, depending on the activitiesyou practice or the body area youwish to stimulate.The specific applications provide youinformation on the placement ofelectrodes and the stimulation posi-tion to adopt. This information is rep-resented by numbers (placement ofelectrodes) and letters (stimulationposition). They refer to the “Tablewith placements of electrodes and

stimulation positions” in chapter VIIand to the poster with the picturesfor the placements of electrodes andthe pictograms for the body posi-tions. The table also indicates thestimulation energy to use, accordingto the programme.You must select in the device themuscular group indicated on the fig-urine. When no figurine appears (-),it is not necessary to choose the muscular group: the selection is automatic.

2. Stimulation positions

To determine the stimulation positionto adopt depending on the placement

of the electrodes and the chosen pro-gramme, refer to the pictures and pic-tograms on the poster and to the “Tablewith placements of electrodes and stimula-tion positions” in chapter VII.

The stimulation position depends onthe muscular group to be stimulatedand the programme chosen.For the programmes involving power-ful muscular contractions, the muscleshould always be stimulated in an iso-metric fashion. Consequently, youshould fix the extremity of the stimu-lated limb(s) firmly. In this way, youprovide maximum resistance to themovement and prevent any shorten-ing of the muscle during the contrac-tion, which could create cramp painsand serious stiffness after the session.For example, when stimulating thequadriceps, the user should be in aseated position with the ankles fixedwith straps to prevent extension ofthe knees.For the other types of programmes(for example, the Health programmesand the Active recovery programme),

which do not involve powerful mus-cular contractions, position yourselfas comfortably as possible.

3. Setting the stimulationenergy level

In a stimulated muscle, the numberof recruited fibres depends on thestimulation energy. For the pro-grammes involving powerful muscu-lar contractions, it is thereforeabsolutely necessary to use maximumstimulation energy, always at the limitof what you can support, so as to in-volve the greatest possible number offibres.The “Table with placements of elec-trodes and stimulation positions” inchapter VII provides information onthe energy to use, depending on theselected programme.

4. Progression in the levels

In general, it is not advisable to gothrough the different levels quicklywith the intention of reaching level 3as fast as possible. In fact, the differ-ent levels correspond to progresswith electrostimulation.The simplest and most usual proce-dure is to start with level 1 and raisethe level when changing to a newstimulation cycle.At the end of a cycle, you may eitherstart a new cycle at the next level upor do some maintenance at the rateof 1 session a week at the last levelreached.
