Overview of Patient Perio This Overview section includes purpose, benefits, features and requirements introductory information.
Purpose of Patient Perio
The Patient Perio tab provides a means for providers to create, edit, print and delete Periodontal charts and create DMF (Decayed/Missing/Filled) scores.
Benefits of Patient Perio
Convenience of all data on a single charting screen
This is not only for on screen viewing in EVOLUTION, but also for printouts and emailed charts.
Speed
Providers can use a combination of mouse-clicks and keypad entries to rapidly record on the Perio chart.
Integration
The Perio data is integrated with Charting data to present a total picture.
Convenience of customising the order of data entry
Providers have options on the order of data entry as they move around the mouth. Each provider can either
Know and follow the default order as configured in EVOLUTION, or
Customise this order generically, or
Quickly customise this order as required on a per-patient basis.
Features of Patient Perio
NvvP compatible
The charting form is now compatible with NvvP requirements.
FULL perio charting, with all data on a single screen
Having all data on a single screen is convenient for both online viewing and printout viewing.
Print out paper forms or Email digital forms

Forms can be physically printed or digitally emailed.
You can also print out a blank chart with just the patient name and date, showing only teeth that are present:
Colour coding
Some chart fields are colour-coded to represent different values.
For example, notice how different values have associated colours in these fields:
Configure EVOLUTION to follow your particular sequence as you move around the mouth

You can specify the order in which you will enter data as you proceed around the mouth, so that EVOLUTION automatically associates the data in that order.
EVOLUTION needs to know the order in which each provider moves around the mouth to correctly apply the data that the provider enters. Each provider therefore either needs to know and follow the default order, or needs to know how to customise this order generically, or as required on a per-patient basis.
Option to ignore the eights (treat as non-existent) as you move around the mouth
You can optionally configure EVOLUTION to ignore the eights in your sequential movement around the mouth.
Tooth comment field
Each tooth has an optional comment field of up to 10 characters.
(Note that not all 10 characters are visible on the chart)

Enter data by means of keypad
To speed data entry, providers can enter data by means of keypad keys.
Many keys function as a toggle, to cycle through the values for that particular field.

Requirements for Patient Perio
Perio charting requires the following:
EVOLUTION version 11.4 or later.
Functional colour printer in order to print completed forms (a completed Perio Chart includes colour-coded fields)
If the practice only intends printing greyscale blank forms, a monotone printer will suffice.
Functional Email in order to email forms.
Printers and Email configured in EVOLUTION.
Normal keyboard Numerical Keypad in order to enter data in the multiple fields of the Perio chart.

Each provider therefore either needs to know and follow the default order, or needs to know how to customise this order generically, or as required on a per-patient basis. Viewing a Patient Perio chart
To view a Patient Perio Chart
1. In the Patients desktop, select a Patient and click the Perio tab:
2. The main Perio screen displays.
3. Select a chart in the Perio workspace at top left.
4. The chart will immediately display in read-only format at right. It includes completed treatment from
Charting, plus any completed and saved Perio work for that chart.
5. From the Perio workspace you can also add a new chart, edit, print or delete the currently selected chart.

Adding, editing, deleting a Perio Chart
To Add a Perio Chart
1. Click the Add button in the Perio workspace area:
2. The Add Perio Chart window will display, overlaid on the Perio tab area:
3. TRype a name for the chart.
4. Press the Tab key to tab to the 1st section, as defined in the tab order in the Configure Perio Chart Screen (see "The Configure Perio Chart screen" on page 20)
5. Complete it as required (see Entering data on the Perio Chart (on page 16))
6. Click OK to save it.
The chart name will then be listed in the Perio workspace area:
From here you can
o Select it to view the read only chart (see "Viewing a Patient Perio chart" on page 10) at right
o Edit, Print (see "Printing or Emailing a completed Perio Chart" on page 12) or Delete it

To Edit a Perio Chart
1. Select a chart in the Perio workspace area and then click the Edit button:
2. This opens the existing chart with data.
3. Complete it as required (see Entering data on the Perio Chart (on page 16))
4. Click OK to save it.
To Delete a Perio Chart
Use the Delete button to mark a selected perio chart as deleted:

To Print or Email a completed Perio Chart
1. In the Patients desktop, select a Patient and click the Perio tab:
2. The main Perio screen displays.
3. Select a chart in the Perio workspace at top left, and click the Print button:

4. The standard print dialogue will appear. Optionally Edit the Layout, Preview, Email or Print the chart.

Printing a blank Perio Chart per Patient
Use the Print Blank button to print a blank copy of the grid, with the patient's name and date on the paper.
Please note:
The grid adapts to only show teeth that are present and this is represented on the blank form.
The restorative tooth chart shown in the center of the grid is also printed with the patient's current data filled in.
There are no options when printing the blank form, it cannot be sent to email, or exported, it just prints on the chosen printer.
An appropriately configured printer is required.

To print a blank Perio Chart per patient
(Requires appropriately configured printer)
1. In the Perio Chart workspace, select a chart.
2. Click the Print Blank button.
3. The Print Blank Perio Chart printer dialogue displays, from where you can optionally edit the layout and
Preview before printing in the normal manner.

Functional areas:
Some chart fields are colour-coded. For example:
Changing the width of the Perio workspace (white area)
To change the width of the Patient Perio workspace
1. Click Configure > User Settings.
2. In the User settings screen, select a text size from the Text Size dropdown menu:

Small text will display on the narrowest workspace, and Large text on the widest workspace.
Interaction between Chart and Perio
Completed treatments in the Charting screen appear in the Perio display.
Pending (incomplete) treatments in the Charting screen do not appear in the Perio display.
Entering data on the Perio Chart
IMPORTANT: EVOLUTION needs to know the order in which the provider moves around the mouth to correctly apply the data that the provider enters. See The Configure Perio Chart screen (on page 20) to either be aware of the default order, or for instructions on how to customise this order per provider. Providers can also quickly customise the order on a per-patient basis.

To enter data on the Perio Chart
Press the Tab key to tab to the pocket depth of the 1st section (section order is as defined in the tab order in the Configure Perio Chart Screen (see "The Configure Perio Chart screen" on page 20)).
Enter the data with procedures indicated below:
To enter a number on the Perio chart and move the cursor to the next box
Press a number key to enter a value (from 0-9) in the current box. This also moves the cursor on to the next box.
Hold a number key to cycle: Add 10 to the number > negate the number > back to the number itself
To move cursor focus to the previous / next box without entering a number on the Perio chart
Left arrow key or NMLK+4 moves the cursor to the previous box without adding an entry in the current box.
Right arrow key or NMLK+6 moves the cursor to the next box without adding an entry in the current box.
To move cursor focus to the Recession or Pocket Depth field above or below the currently focussed one
Up arrow key or NMLK+8 moves the cursor to the Recession or Pocket Depth field above the one that currently has focus.
Down arrow key or NMLK+2 moves the cursor to the Recession or Pocket Depth field below the one that currently has focus.
To record plaque on or off at the current position
Left-click in the Plaque field to cycle: plaque on > plaque off at that position.
- OR -
Press '-' to cycle: plaque on > plaque off.
To record pus (suppuration) on or off at the current position
Press "/" to cycle: pus on > pus off at the current position.
To record bleeding at the current position
Press "*" to cycle point bleeding > full bleeding > no bleeding.
To record tooth prognosis state
Press '.' to cycle the tooth through the prognosis states: positive > negative > neutral.
To record the Mobility at the current position

Left-click in the Mobility field to cycle: blank > 0 > 1 > 2 > 3 and then back to blank
- OR -
NMLK+5 cycles the Mobilty field at the current position: blank > 0 > 1 > 2 > 3 and then back to blank.
NOTE: Blank is unrecorded, zero is 'No Mobility'
To record Furcation at the current position
Left-click in the Furcation field to cycle: blank > 0 > I > II > III
- OR -
Press '+' to cycle the Furcation field at the current position: blank > 0 > I > II > III.
NOTE: On teeth where there are two furcation positions, if the cursor is in a 'middle' position, the key will cycle the left most field.
To delete the entry at the current cursor position, and keep the cursor in that position
Press NMLK+'.' to delete the entry in the current field and revert it to blank. The cursor will not move.
Summary of mouse left-click options:
Plaque field cycle: plaque on > plaque off
Mobility field cycle: blank > 0 > 1 > 2 > 3 > blank
Furcation field cycle: blank > 0 > I > II > III
Prognosis field cycle: + > - > /

DMF (BPE) Score - creating, editing, deleting
To create a DMF (Decayed/Missing/Filled) Score
1. Click the button at the bottom of the Perio tab. The Basic Perio Examination (BPE) window will open:
2. Enter the appropriate numbers for decayed, missing and filled teeth for the patient.
The numbers will be totalled for the Total and Total Sound fields, and are not able to be edited.
3. Click in the ‘Oral Hygn Score’ field and use the button to select a rating for hygiene and add any notes. The window will now look similar to the following:
4. Click to save the DMF score.

To edit a DMF score
Select the item in the Perio tab, click the button to re-display the Basic Perio Examination (BPE) screen, then make and save the changes as required.
To delete a DMF score
Select the item in the Perio tab, and click the Delete button .
Configuring Patient Perio This section describes how to set up and customise the software.
The Configure Perio Chart screen
Purpose
EVOLUTION needs to know the order in which the provider moves around the mouth to correctly apply the data that the provider enters.
IMPORTANT: Each provider needs to either know the default order, or how to customise this order generically, or as required on a per-patient basis. This affects the tab order when charting information.
Customisation
This can be customised per provider login, or providers can simply use the default settings.
NOTE: If they use the default settings, they need to check that this is indeed how they move around the mouth.
Parameters that can be customised:
Attachment level threshold
How recession data is entered (either after each pocket depth, after each quadrant, or after all pocket depths)
Whether or not to include the eights in the progression around the mouth
The order of progression through all pieces of the dental arch: four quadrants, each with outside and inside
The direction of progression for each piece of the dental arch.

To open the Configure Perio Chart window
Click the Configure button in either the background (read-only) chart, or in the Add/Edit Perio Chart window:

The chart includes instructions:
Attachment Level Threshold
When pocket depth and recession are recorded, EVOLUTION calculates an attachment figure and automatically populates this field. 4 and over will highlight with a red colour (the threshold has been breached).
Enter recession data This refers to the point at which EVOLUTION automatically tabs over to recession. Default is after each pocket depth is recorded.

Skip the 8s Check (tick) to disregard (and have EVOLUTION disregard) the eights.
Tab Order selectors
Set the dropdown selectors to specify the order in which you want to proceed around the mouth.
As you select a position for a quadrant, the quadrant that already has that position will swap over with the one you that chose.
Direction
Click the blue arrows to toggle their direction.

Skipping the Eights in the order of data entry
EVOLUTION provides an option to remove the eights (wisdom teeth) from the tab order. In this case dentists chart the wisdom teeth manually.
If you do not routinely collect data for wisdom teeth, check (tick) the Skip the 8s check box. When the cursor gets to wisdom tooth, whether or not the tooth is present, the cursor will move on to the next tooth in sequence.
You can manually click on a wisdom tooth to record data against it if you wish, even with this option ticked. With the Skip the 8s checkbox ticked, the blue arrows will shorten to indicate show that the 8s are not included.
To skip the eights (wisdom teeth) in order of data entry in Perio examination
1. Add/edit a Perio chart (see "Adding, editing, deleting a Perio Chart" on page 11).
2. To display the , select the Configure button at bottom right of the Perio Chart:
3. In the Configure Perio Chart window, under the Order of Data Entry graphic, check (tick) the Skip the 8s
checkbox:
You will see that the blue line indicating order of data entry no longer covers the eights. As you enter data, the eights will be dis-counted in the series.
