George Bacso Lifetime Achievement Award1. Divisions are asked to nominate at least one person each year for this award. A division or a

certified member may submit nominations. Self-nominations are acceptable. All information is held in strict confidence. Nominees must be USPTA-certified members (Master Pro, Elite Pro or Professional Level) in good standing. This award is from the beginning of your tennis career to current.

2. The following forms must be completed in detail (please type). Newspaper clippings, interviews or highlights of the applicant’s career are very important. Please include sufficient back-up material in terms of letters, fliers, pictures, certificates, newspaper articles, etc. via electronic only. Please use PDF format and attach separately.

3. Please include a color photograph and biographical information.

4. The Lifetime Achievement is judged in seven areas:

● Contributions to USPTA

● Contributions to USTA

● As a teaching professional

● As a player

● As a coach

● Education, research, publications

● Organizational affiliations, special achievements, contributions to the community

5. Layout/organization – If the nominee is not submitting his/her application through the online form, then please put the application in front of any supporting PDFs and highlight the seven areas above through the use of headers and detailed backup materials (for example, each of the seven areas above begins on a different page). Organization counts in your PDF/Word version.

6. ALL applications must be submitted via the online form or by email. To submit by email, download the application (Word document), fill it out electronically, and save it as a PDF. Applicants who complete the downloaded application should take care to keep all questions in their original format. Applicants should upload the application and supporting materials to www.dropbox.com, Google Drive, or a similar electronic service and email the link to [email protected]. Please indicate in the subject line which award is being applied for.

The deadline for the USPTA World Headquarters to receive all 2017 award applications is April 10, 2017. Applications submitted via email must be emailed by April 10, 2017 at 11:59 p.m. Eastern time. The entire application and ALL supporting materials must be received on or before the deadline date to be considered by the Awards Committee. The Awards Committee will not accept applications received after the deadline. Only electronic applications will be accepted.

7. The Awards Committee not only judges the award nominations, but also is available to answer questions throughout the year. If you have any questions, please direct them to Fred Viancos at the USPTA World Headquarters at 800-877-8248, ext. 128, or [email protected].

8. Some USPTA awards may or may not be given each year, at the committee’s discretion. Recipients will be announced/recognized during the 2017 USPTA World Conference in Orlando, FL. Award recipients are encouraged to attend the conference to receive their awards, however, not all awards will be presented at the conference.

9. Individuals may submit this application. To receive credit for the nomination, a division must include the completed application electronically as well.
