Before In-Processing (April/May)Residency Program or VA service completes:

• RCVL (credentialing letter with list of residents)

• Individual appointment letters (mail-merge)

• IRM 1-B Form (for computer access)• TMS certificates of completion

(Examples of all forms will be shown on later slides)

Submit to sponsoring VA service

send you proof of MTT

completion in TMS.

Police (fingerprints)

In-Processing (end of June)Documents/Swearing-In:• Appointment Affidavit (SF-61)

• Employment Eligibility Verification (I-9)

• Pharmacy Prescription Card

• CPRS Training

• PIV BadgeSchedule with Police.

* Must bring two forms ID. *Not required to start July 1st.

Fingerprints expire > 120 days.

After In-Processing

VA Service coordinates:• Pick up PIV Badge

• Link PIV Badge

• CPRS Training for electronic medical record

• Locked doorways (ER, OR, Psych, etc.)• Narcotic prescribing

Maintenance• MTT in TMS must be completed every year.

• Passwords expire > 90 days

• If > 1 year, needs Service Chief approval.

• Upgrade information and PGY level in VISTA.

• CPRS Refresher Training available(must reactivate expired accounts before arrival).
