Therapy Corner: Training Errors Revisited Gabe Yankowitz, PT, DPT

For more than 40 years now, one of the constants in the world of running-related injury (RRI) theories has been
the “conventional wisdom” claiming that the majority of said injuries can be blamed on training errors (TE).
As explained as far back as the second installment of this series [Therapy Corner #2], and reviewed/updated just a
few years ago [#114], the term training errors in this context refers to a training program that involves running too
much, too fast, too soon.

There have been a few numbers tossed around consistently over the years (e.g., 60% of all injuries are caused by
TEs; avoid increasing weekly mileage by more than 10% per week) that attempt to quantify the problem in some
manner, but the truth is these numbers have never been conclusively validated by solid research, for a couple of
reasons. First, all the research (up to now) has been retrospective and correlational. In other words, the numbers
were gleaned by questioning already-injured runners about their training leading up to the injury, so only an
association could be made between training factors and injury, not a claim of clear cause-and-effect. Second,
researchers had to completely rely on runners’ reports of their training regimen, which subjected the data to a fair
amount of unreliability.
Now however, a study published in the Journal of Orthopedic and Sports Physical Therapy [J Orthop Sports Phys
Ther 2018;48(10):740-748] describes an actual controlled research project that sought to determine if increasing
the volume or intensity of training results in increased risk for specific injuries thought to be associated with those
factors.
Prior research made speculative associations between the specific injuries of Runner’s Knee, ITB syndrome, and
patellar tendinitis and increased volume, as well as tying calf muscle strains, Achilles tendinitis, and plantar
fasciitis to increased intensity. But these associations were based exclusively on biomechanical models; there were
no studies showing an actual relationship between these factors in actual runners, so the investigators set up an
experimental study to see if this theory held.
Researchers in Denmark (The Happiest Country in the World – except for injured runners) randomly assigned
runners to one of two groups. The first group increased the volume (distance) of running over a 16-week period;
the second increased their intensity (speed) over the same period of time. Both parameters were closely controlled
and monitored, using runners’ smartphones to track distance and speed (something that would have been very
difficult to control just a couple of decades ago!), and the increased loads were purposefully greater than previous
studies indicated were probably “safe.”
The researchers were testing the specific hypothesis that those runners increasing distance would have a greater
number of injuries to the knee area, while those increasing intensity would show more problems in the ankle and
foot. If the hypothesis held up, it would bolster the long-held belief that training errors could be a major factor in
causing injuries.
Of course, that’s not what they found. The results showed that, of those runners who developed an injury, they
were pretty much evenly split in both groups – distance and speed – as far as the area of injury. Increased volume
caused an almost equal number of ankle/foot injuries as knee injuries, and the same was true for the increased
intensity group.
Does this mean the advice given for the past 4 decades to be cautious when increasing your training distance or
speed is invalid? No – not really. It just means we can’t blame those specific factors for specific injuries. We still
don’t know what the exact percentages of increased training are likely to cause problems for you; for now, the best
advice it to continue to abide by the general guidelines I outlined in those previous columns and, most importantly,
as always, listen to your body. If you feel unusually fatigued or sore, it’s probably a sign you need to back off a
bit in your training.

#19 Little Rock Marathon, Little Rock, AR 5:13:50 Kamal Jabbour, Ph.D
4 March 2012 --- I traveled uneventfully to Little Rock on Friday, a day in which 94 tornadoes devastated the
Midwest. I drove from the airport to the Convention Center, and picked up my race packet. The short-sleeve
technical shirt won the category of the ugliest marathon shirt ever. I walked around the expo, bought a few more
26.2 stickers, and retired to the Courtyard in West Little Rock.
I returned to the Expo Saturday morning to check out parking for race day. I saw Mike, Jenny and Tom, my
Columbus friends, hanging out in the sun. We chatted as we walked to the Expo. I did not see any other 50-staters.
At the Expo, I met Brooke, the pace leader for 4:55, and decided to join her team.
I spent much of Saturday resting. I napped after the trip to the Expo, ate lunch at Applebee’s, napped, drank
cappuccino at Barnes and Noble, napped, went to church in the evening, then ate dinner at the Butcher Shop. Their
sirloin steak and baked potato hit the mark.
I pinned bib number 130 on my Psalm 26:2 shirt, and stuffed a Builder Clif Bar and a Protein Powerbar in my
short pockets. I packed four Ibuprofens for miles 10 and 20, and broke the ring that held the rental car keys. I
mismatched my socks, and went early to sleep.
Glenda opened early for breakfast and got me coffee, a blueberry bagel and a banana. I grabbed a bottle of water,
and drove downtown. I parked close to the start and finish lines, and headed towards the port-a-potties. I saw
Mike, Jenny and Tom, and we chatted for several minutes. I spent the waiting hour in the car listening to K-Love
on the radio. I felt a lump in my throat hearing Matthew West sang “Strong Enough”.
Brooke radiated at the start line as she rallied the 4:55 wannabes. The sun ushered a pleasant day. The field of
9,000 runners sang a cappella the National Anthem. A count down from ten to zero sent us on our way. I heard no
horn, nor gun. We crossed the start line at 4 minutes, and shuffled onward.
A real life Memphis policewoman, our 31-year-old pace leader chatted cheerfully about war stories of marathons
and underage drinking busts. She rode a bicycle on patrol, not unlike the Santa Ana police that Jeff Dunham’s
Walter admired. I strived to throw my empty water cups in the garbage cans, not on the street, to avoid a charge of
littering.
Bill Rodgers said that “any marathon worth its salt has a few hills, and the Little Rock Marathon is a bit salty.”
The big hill started around 11 miles, and climbed steadily for the next five miles. I ran with Brooke and the pace
team through the half in 2:22, well ahead of pace. I lost the will to chase her around 16 miles, and contemplated
the last ten miles.
My left foot complained initially around mile 1, and got progressively worse in the following hours. I walked a
quarter mile to loosen the foot, then alternated running with walking. We saw an ambulance speeding by, and said
a quiet prayer. We learned later that a 37-year-old man collapsed around mile 11 of the half-marathon, and died
upon arrival to the hospital.
I met first-timer Barbara around mile 18. We walked together to the turnaround at mile 21, where she stopped
to visit with her family. We passed the 8-hour pace team – they started two hours before us. I saw Larry across the
road, also an early starter, then I saw Brooke. I resumed running into the wind, and dug deep for the last five miles.
I crossed the finish line in 5:18-ish, for a 5:13:50 chip time. The finish medal exceeded my expectations. At 8
inches across, it weighed 25.2 ounces, by far the largest medal in my 19-string collection. I felt good about my
time, but a bit concerned about my left foot. I also grew frustrated with my stubborn times north of 5 hours.
On the way to the car, I saw a table selling beautiful long-sleeve finisher shirts for $10. I carried no money, and
could not fathom the thought of walking to the car and back for one more shirt. I drove to the Courtyard, showered
and napped. I returned to the Convention Center for the post-race party. I partook in pulled pork and chicken
barbecue with all the fixings, then returned to the Courtyard for a good night’s sleep.

Last Chance Trail Run & Pancake Breakfast Skyline Lodge, Highland Forest, Fabius, NY

Run on beautiful snowy, icy trails. Fall down and get up. Step in ice water streams that have no safe crossing. Climb over, under and around fallen trees. Then return to the warmth of Skyline Lodge for aroma therapy. The smell of butter, pancakes, maple syrup, coffee and sausages fills the air. It is noisy with the chatter and laughing of your friends. An all-you-can-eat breakfast awaits you. This run is not for everyone. It is cold and the footing can be perilous. It is the kind of run that appeals to people who enjoy the wilderness. You're allowed to run, walk, skid, slide, wade, struggle and climb the trail. You may freeze. The trail is 9 miles but there are shortcuts allowing runs of various lengths. The full trail takes 80 to 120 minutes to complete. The trail is well-marked so you should not get lost. Time: Start any time between 8:00 and 9:15. You can start when you please and quit when you are tired and hungry. Run with a partner so that someone will hear you when you admire something interesting. It is safer and partners are plentiful. The first pancake is served at 9:00 and the last pancake is served at 10:30. The event has never been canceled; if the road to Fabius is open we'll run. If the snow is deep, we run on Park Roads. Directions: Highland Forest is in the SE corner of Onondaga County off Route 80, 4M East of Fabius. Entry: The cost is $10.00 per person (12 and older), $5.00 for ages 6-11 yrs., and 5yrs old and younger are FREE. Maximum for a family is $20.00. Everyone Can Run Beginners Program
Presented by the Syracuse Chargers Track Club and hosted by the Jewish Community Center of Syracuse, the
Everyone Can Run Beginners Program will begin on Wednesday January 9, 2019. This is a FREE eight-week
program designed to take you from walking to running and will start Wednesday, January 9, 2019 and continue
through Wednesday February 27, 2019. Greg Tuttle, a USATF Certified Coach, of the Syracuse Chargers Track
Club, will lead the Wednesday sessions. Information Presentations from Running Related Professionals, will be
held at most of the training sessions.
Time: Wednesdays from 5:30 p.m. – 6:30 p.m.
Place: Indoor track at the Jewish Community Center (JCC), which is located at 5655 Thompson Road in Dewitt
(13214). Participants should park at rear of building near the Fitness Center entrance. You will start with mostly
walking, increasing running intervals each week on a set schedule. All registrants will receive a Syracuse Chargers
Track Club Pedometer. All registrants will receive a 2 week trial pass to the JCC including use of their indoor track and
locker room. This program is open to Chargers, JCC members and prospective members, at least 14 years and older;
applicants under 18 years require a parent’s or guardian’s permission. All fitness levels welcome! If you can walk, you
can run! Important Note – Participants should consult their health care provider before beginning any new type of
exercise or fitness program. *All participants will be asked to sign waivers and registration forms before the first night
of the program.

2019 Syracuse Chargers Track Club Scholarship Application
The Syracuse Chargers Track Club, Inc. will award as many as three $1,000 scholarship grants to SCTC members
who have distinguished themselves through their: volunteer activities within our community (not limited to
Chargers events); academic achievement; and commitment to personal fitness. All applicants should have a record
of volunteer service to the athletic community. In order to qualify for consideration for one of these awards, an
applicant must be a graduating high school senior, an SCTC member at the time the application is submitted, and
the holder of at least a B average (80%) throughout high school. Anyone who wishes to be considered for one of
these awards should provide the information requested below and submit this form along with the following:
1. A letter of recommendation written by an adult who has supervised, coordinated, or is otherwise familiar
with your volunteer activities, particularly those that promote health and fitness.
2. A copy of your high school transcript.
3. A letter of recommendation from a teacher or your guidance counselor that focuses on your academic
achievement and merits.
4. A typed essay of 300-400 words in which you describe how your participation in athletics and the
volunteer activities that support athletics demonstrates your commitment to lifetime fitness.

The Charger Challenge The Charger Challenge is designed to provide guidance, incentive and long-term motivation to
runners and walkers through awards and recognition. Runners and walkers of all ages, abilities and levels of experience are welcome to
participate. Participants do not have to be Syracuse Chargers Track Club members. Three different challenges are available. Whether you
exercise for enjoyment, weight loss/maintenance, social or competitive reasons, you are encouraged to take on the Charger Challenge!

All Challenge Programs participants are required to fill out the application below and upon receipt, they are sent a log sheet for recording
miles run/walked along with instructions and guidelines for new runners. The participant submits completed log sheets for each award

I agree not to participate in Club activities unless I am medically able and properly trained. I agree to abide by any decision of an official concerning my ability to safely participate in any activity. I freely and voluntarily assume all risks associated with competing, volunteering, and all other activities in which the Club is involved. Such risks include, but are not limited to: falls, contact with other participants, the dangerous or negligent behavior of other participants, the effects of the weather, including high heat and/or humidity, the conditions of the road or other facilities, and traffic on the course, all such risks being known and appreciated by me. Having read this waiver and knowing these facts, and in consideration of you acceptance of my application for membership, I, for myself and anyone entitled to act on my behalf, waive and release the RRCA, USATF, Syracuse Chargers Track Club, Inc., and their officers, directors, and members, and all sponsors, their representatives and successors, from all claims or liabilities arising out of my participation in Club activities, even though that liability may arise out of negligence of carelessness on the part other persons named in this waiver. I grant permission to use for legitimate purpose any photographs, motion pictures, recordings, or other images of myself participating in Club activities.
