1.0 Introduction and Site HistoryThe Lindy Boggs Medical Center complex (also known as Memorial Medical Center) is located at 301 N. Jefferson Davis Parkway, New Orleans, Louisiana 70119. The total property (including the parking area) is approximately 8.7 acres in size. The geographic coordinates at the entrance to the site (at Jefferson Davis Parkway and Bienville Street) are 29º 58' 19.67", 90º 05' 33.93." The site has been identified as occupying City Square 476A (which is 6.675 acres), and is bordered by Conti Street to the north, Jefferson Davis Parkway to the east, Bienville Street to the south, and S. Cortez Street to the west. See Figure 1, Site Location Map, in Appendix A. The property is divided into three buildings which have been designated with separate addresses: Building A is on the western end of the complex (3525 Bienville Street); Building B is the center of the complex (3425 Bienville Street); and Building C is the largest portion, the eastern side of the complex (301 N. Jefferson Davis Parkway). See Figure 2, Site Survey, in Appendix A, prepared by BFM Corporation and provided by St. Margaret's Daughters.

This workplan describes the Voluntary Remediation Program (VRP) Voluntary Remedial Investigation to be completed at the site to address previously identified contamination.This version of the workplan is a draft pending the approval of LDEQ. Previous investigations have been conducted at the site as part of property transfers and plans for redevelopment. This workplan has been prepared in accordance with the Risk Evaluation/Corrective Action Program (RECAP) 2003 guidance document, Appendix B.The workplan will be kept onsite during the field investigation and will be made available to all project personnel.

1.1 Previous Land UseThe complex was developed in the 1950's and was previously called Mercy Hospital and subsequently Memorial Hospital. Past site use also included a lumber yard. The previous property owners (based on a 2009 Phase I Environmental Site Assessment (PIESA)), beginning with the most recent, wereVictory Real Estate Investments, LLC, Tenet Health Care Corporation, and Southern Lumber & Demolishing Co., Inc.

1.2 Current UseCurrently the site is an inactive medical complex, Lindy Boggs Medical Center.The NAISC code for the property use is 62211. The property has been closedsince Hurricane Katrina. It is surrounded with chain link fence, although there are several openings and there are signs of vagrants and vandalism at the site. In addition, parts of the facility remain flooded from the hurricane. The buildings on the property have been designated as A, B, and C (see Figure 2, Appendix A); Building A is owned by St. Margaret's Daughters, while Buildings B and C are owned by St. Margaret's Foundation. Neighboring properties are a mix of residential and commercial use.

Previous investigations have identified two 10,000-gallon underground storage tanks (USTs) at the site. The USTs are located adjacent (to the west) to the chiller and generator building, between Buildings B and C on the north side of the site.

According to UST registration documents, the USTs stored diesel fuel used for emergency generators. The tanks were installed in January 2000. The material of construction and system type are not indicated. There is no record of known release at the site. The USTs are not in use, as the site is currently not in use.

1.3 Future Land UseThe property is being redeveloped as a medical complex, with a nursing home, specialty clinics, and ultimately a small surgical hospital. Building A is being redeveloped as condominiums, and Buildings B and C will be reopened as medical facilities.

1.4 ZoningThe site is zoned for commercial and residential use.

1.5 Description of ReleaseAs indicated in Section 1.2, no known release was ever reported or documented. Based on the analytical results from a Phase II Environmental Site Assessment (PIIESA), diesel was likely released from the tank(s) or tank appurtenances. The date and quantity of the release(s) are unknown. The extent of the release(s) and the associated plume are also unknown.

1.6 Results of the Preliminary Evaluation(s)There have been two PIESAs and two PIIESAs conducted at the property, by two different consultants for two different clients: The Culpepper Group, LLC (TCG) conducted a PIESA in November 2009 and a PIIESA in April 2010 on behalf of St. Margaret's Daughters; and Professional Service Industries, Inc. (PSI) conducted a PIESA in January 2007 and a PIIESA in February 2007 on behalf of Victory Real Estate Investments.

The 2009 TCG PIESA revealed the following Recognized Environmental Conditions (RECs):

� The presence of two 10,000-gallon underground storage tanks (USTs) used to store diesel fuel. Previous investigation (2007 PIIESA) has indicated a release of petroleum hydrocarbons from these tanks.

� The presence of bags of solid and medical waste remaining inside the facility.

� The presence of open containers (including 55-gallon drums) with unknown contents at the site.

The PSI 2007 PIESA had similar findings. In addition, a neighboring site was identified as an REC: Anatole's Garage had three USTs used for storage of gasoline; releases from this site could have impacted the subject property.

The 2010 TCG PIESA reviewed the data presented in the PSI PIESA. These data indicated that diesel and oil petroleum hydrocarbons were present in the

groundwater at the site (near the location of the diesel USTs; diesel concentrations in two locations and the oil concentration in one location exceeded the LDEQ RECAP screening standards for groundwater.

2.0 Emergency/Interim Corrective ActionThere has been no emergency or interim corrective action at the property, such as soil excavation and disposal. The USTs onsite are currently not in use due to site occupancy status.

3.0 Investigation DescriptionThe Voluntary Remedial Investigation will be conducted to determine the extent of contamination in soil and groundwater at the site and will meet the requirements of the Voluntary Remediation Program and RECAP 2003. The site assessment will include sampling and analysis from 10 soil borings and nine monitoring wells as described below. In addition, the USTs will be delineated (orientation, size, etc.) and sampled to characterize the contents.

3.1 Sample Collection and Screening Rationale

The site assessment will include soil sampling around the diesel USTs as well as at the corners of the site and along the property boundary close to the neighboring UST site to meet VRP requirements. Direct push technology will be used to advance the 10 soil borings to 12 feet below ground surface (bgs). Boreholes B1-B9 will be sampled for the contaminants of concern (COCs), while borehole B10 will be advanced in an unimpacted area for geotechnical analysis. All sample intervals will be field screened with a photoionization detector (PID) using the headspace method (samples will be placed in a plastic baggie, sealed, and allowed to equilibrate; the PID then measures organic vapors in the headspace of the baggie). Soil samples at boreholes B1-B9 will be collected from the surface interval (0-3 feet bgs), the water table interval, and the bottom of the borehole (9-12 feet bgs). The sample for geotechnical analysis at B10 will be collected from the water table interval. Soil samples will be collected directly from the disposable sample liner from the drill rig. The Geologist will log the soil descriptions according to the ASTM Unified Soil Classification System. All samples will be sent to a LELAP-accredited laboratory for analysis; standard turn around times will be requested. The analysis for samples from B1-B9 will be: fractionated petroleum hydrocarbons (volatile and extractable ranges – VPH and EPH) (MADEP), RECAP volatile organic compounds (VOCs) (EPA SW-846 Method 5035, 8260), RECAP semi-volatile organic compounds (SVOCs) (EPA SW-846 Method 8270), and RECAP metals (EPA SW-846 Method 6010, 7400). The sample collected at B10will be analyzed for geotechnical parameters: Atterberg Limits, foc, Particle Size, Unified Soil Classification, and Hydraulic Conductivity.

Soil Investigation

In addition to the primary samples indicated above, the appropriate QC samples required under RECAP will be collected. These will include two split samples, tworinsate blanks, one field blank for each day of the soil sampling, and one trip blank for each sample cooler containing volatile samples. In addition, matrix spike/matrix spike duplicate (MS/MSD) analysis will be requested on two samples.

The site assessment will also include groundwater sampling from all nine soil boring locations; the boreholes will be converted to temporary monitoring wells. The monitoring wells will be advanced using a hollow stem auger to allow for installation of the well materials (well screen and riser). Following installation, the wells will be developed by purging three well volumes (or until dry) as well as

monitoring stabilization parameters including pH, temperature, and conductivity. The wells will be purged using a peristaltic pump and monitored using a flow cell meter. Within 24 hours of well development, groundwater samples will be collected using bailers or the peristaltic pump. The samples will be sent to a LELAP-accredited laboratory for analysis for VPH, EPH, RECAP VOCs, RECAP SVOCs, and RECAP metals (total and dissolved concentrations). Total dissolved solids (TDS) will also be analyzed on one of the groundwater samples for groundwater classification purposes. Standard turn around times will be requested.

The analytical results will be evaluated using RECAP standards. Therefore, inaddition to the primary samples indicated above, the appropriate QC samples required under RECAP will be collected. These include one duplicate sample, one field blank, one trip blank, and MS/MSD analysis will be requested on one sample.

The two 10,000-gallon USTs will require removal prior to site redevelopment. The site investigation will include investigation of potential contents and characterization of any contents for disposal. A LDEQ-certified UST worker will conduct this investigation. The UST investigation will include probing the subsurface for the USTs to determine the orientation and to confirm the size of the tanks. This will be conducted manually with a UST probe. Depth to the tops of the USTs will also be probed. Concrete and soil/fill above the tanks will be removed using a backhoe or other equipment, with care taken to only scrape the top soil and not damage the tanks. Once the tanks have been revealed, the fill ports will be opened. Continuous air monitoring with a photoionization detector (PID) and combustible gas indicator - LEL O

UST Investigation

2 meter - will be conducted for organic vapors and explosive gases. The interior of the tanks will be probed and the contents will be gauged to determine a volume estimate of contents. If anything other than sand (product, water, sludge, mix) is encountered, one sample will be collected from each tank for analysis for total petroleum hydrocarbons - gasoline, diesel, and oil ranges (TPH-G, TPH-D, and TPH-O). One duplicate sample will be collected from one tank as well. In addition, a composite sample from the two tanks will be collected for TCLP analysis for waste characterization, if necessary. Upon completion of sampling, the tanks will be closed and the fill will be scraped back into the tankhold until UST removal.

3.2 Boring and Monitoring Well Placement RationaleThe boreholes/monitoring wells are located on a nonrandom basis to define the nature and extent of the contamination around the USTs as well as the evaluate the property boundaries to meet VRP requirements. Boreholes B1-B4 (MW1-MW4) are located around the UST tankhold to define the extent of contamination from the UST release. Borehole B5 (MW5) is located at the property line close to Anatole's Garage (the neighboring UST site). Borehole B6 (MW6) will be advanced southeast of B5, along Bienville Street at the ramp entrance to the emergency room. Borehole B7 (MW7) will be advanced at the southeastern corner of the site (Bienville Street and N. Jefferson Davis Parkway). Borehole B8 (MW8) will be advanced at the northeastern corner of the site (N. Jefferson Davis Parkway and Conti Street). Borehole B9 (MW9) will be advanced at the northwestern corner of the site (Conti Street and S. Cortez Street). Finally, borehole B10 will be

advanced in an unimpacted area near the UST investigation area. See Figure 3 in Appendix A for the proposed borehole/monitoring well locations.

3.3 Sample Designation and ManagementA sample numbering system will be used to identify each sample. This numbering system will identify soil samples, groundwater samples, splits, duplicates, trip blanks, rinsate blanks, temperature blanks, and field blanks. Each sample number will consist of the following format:X-project id-text-depth-suffixWhere:

X is the sample matrix: S = soil, W = water, WA = waste.Project id is the job number and site identifier.text is the borehole or monitoring well number: B2 = borehole 2 (B1-B10for soil), MW1 = monitoring well 1 (MW1-MW9 for groundwater).depth is the sample interval for soil: 0-3 = 0-3 feet bgs, etc.suffix is only added to the sample name if it is a split or duplicate. It will consist of letter a (lowercase).

All QC samples (trip blanks, field blanks, rinsate blanks) will be identified by the date they were collected. Rinsate blanks will also indicate the borehole after which they were collected.

Sample containers will be handled with gloves. Upon completion of sample collection, all sample containers will be wiped down (labels will be completed in the office prior to initiating field work). The lids will be tightened to prevent spilling or cross-contamination, and all containers will be wrapped in bubble wrap and placed on ice in coolers. A temperature blank will be added to the cooler to confirm that samples remain at 4 degrees Celsius until arrival at the laboratory. Trip blanks will be added to the cooler when volatile analysis is required to ensure that cross-contamination during shipment has not occurred. The completed Chain-of-Custody will be placed in a plastic bag and taped inside of the cooler lid. The cooler will be sealed with duct or shipping tape. A custody seal will be placed over the crack between the cooler lid and side to ensure the samples are not tampered with during transport to the laboratory. All labels and seals will be covered with clear tape.

3.4 DecontaminationA direct push drill rig will be used to advance the boreholes from which the soil samples will be collected. The soil intervals will be collected within disposable liners inside the sample tubes, however, the sample tubes will still be decontaminated between borehole locations to prevent potential cross contamination of samples. Disposable bailers or disposable tubing will be used to collect the groundwater samples, so decontamination is not necessary.

Personnel decontamination will involve washing exposed areas with soap and water and/or moistened towels. Disposable PPE will be removed and decontamination will take place prior to any employee leaving the work area and/or eating, drinking, smoking, etc.

3.5 Investigation Derived WastesSoil cuttings from the borehole/monitoring well installation will be placed in drums. Likewise, purge water from well development and decontamination water will be drummed and staged onsite. Waste characterization analysis (by TCLP) will be conducted to prepare a waste profile for disposal of the investigation-derived waste. All other wastes (such as PPE, disposable equipment, etc.) will be disposed of as general waste (in other words, it will be disposed of with the regular trash).

3.6 Geology/HydrologyThe property is situated within the Mississippi River Deltaic Plain. The structure and physiography of the entire region have been significantly affected by several geologic phenomena: the Gulf Coast geosyncline; the Mississippi structural trough; salt dome diapirism; and the sea level fluctuations during the Quaternary period.

The property is located on natural levee and backswamp sediments associated with the Mississippi River. Underlying these deposits are prodelta silty clay, delta front silty clay, and silty sand deposits associated with the Mississippi River Complex. Although the Pleistocene and Holocene sediments are of the same general origin, the Pleistocene deposits are more consolidated and usually have been oxidized during periods of lower sea level.

In the area, there are four major fresh water aquifers. These include the shallow aquifers, Gramercy aquifer, Norco aquifer, and the Gonzales-New Orleans aquifer. The shallow aquifers of Pleistocene and Holocene age include the point bar and abandoned channels of the Mississippi River and localized shallow sand bodies present within Holocene natural levee and deltaic deposits and undifferentiated Pleistocene deposits. These sands are of little importance as aquifers, and supply only small quantities of water (less than 50 gallons per minute) (Dial 1983). The quality is generally poor with high hardness and iron concentrations.

The Gramercy aquifer ("200-foot" sand) of Pleistocene age is composed of a series of sand lenses and channel fills. The top of the aquifer is generally found at 150 to 220 feet below mean sea level. In the New Orleans area, the Gramercy yields fresh water with a chloride content of approximately 300-ppm (Rollo 1966).

The Norco aquifer ("400-foot" sand) is composed of a fine to coarse-grained sand (Dial 1983). The top of the aquifer occurs at approximately 300 feet below mean sea level. In the New Orleans area, the Norco generally yields groundwater with chloride concentrations of 250 to 500 ppm (Rollo 1966).

The Gonzales-New Orleans aquifer ("700-foot" sand) is composed of a fine to medium-grained sand of uniform texture (Dial 1983). The top of the aquifer occurs at approximately 600 feet below mean sea level. In the New Orleans area, the Gonzales-New Orleans aquifer yields fresh groundwater; however, the aquifer has not been extensively utilized due to a characteristic yellow water color.

Groundwater in the New Orleans area is not used for drinking water. A DOTD well survey is included in Appendix B.

3.7 Off Site ImpactThe site assessment will define the extent of groundwater contamination and determine the direction of groundwater flow. The results of the investigation will provide information on the potential for offsite impact from the UST release at the site.

3.8 Off Site SourcesA neighboring site was identified as an REC in the 2007 PIESA: Anatole's Garage had three USTs used for storage of gasoline; releases from this site could have impacted the subject property.

3.9 Instrument/Equipment Testing, Inspection, and MaintenanceAll equipment (including sampling equipment and personal protective equipment) will be inspected by the Project Manager or designee prior to the site activities (during mobilization) and each day of site activities. All equipment is also maintained (proper storage, inspections, calibration, etc.) when not in use for a project. In this manner, all potential equipment failures are identified early on and corrected before use in the field.

Laboratory equipment is the responsibility of the LELAP-accredited laboratory and will be maintained according to laboratory SOPs.

Instrument/equipment testing and calibration will be conducted in accordance with standard SOPs and manufacturer recommendations.

3.10 Audits and Corrective ActionAudits and corrective action will be conducted in accordance with standard procedures under any quality management program.

4.0 QA/QC RequirementsThe sampling effort will require collection of quality control (QC) samples in accordance with RECAP 2003 as indicated in Section 3.0. The QC samples include the split and duplicate samples previously mentioned. In addition, rinsate blanks, trip blanks, field blanks, and matrix spike/matrix spike duplicate (MS/MSD) samples will be required. The following table shows the required QC samples.

The data collected under this site assessment will not only be evaluated under RECAP QA/QC requirements, but will also be evaluated for the following characteristics: precision, accuracy, representativeness, completeness, comparability, and sensitivity (PARCCS). These characteristics are reviewed below.

Precision is a measure of the reproducibility of measurements under a given set of conditions. Precision is usually stated in terms of relative percent difference (RPD). Results of QC samples will be used to calculate the precision of the analytical process, with a maximum acceptable RPD of 20%. Duplicates and splits will be used to determine the precision of the sampling process. A RPD guideline of 50% will be used in the evaluation of any field results, however, the evaluation will be sample dependent and will take into account field conditions at the time of sampling, field sample descriptions, and sample anomalies noted by the laboratory.

Accuracy is a measure of the bias in a measurement system, and is defined as the closeness of the reported value to the true value. Accuracy will be evaluated based on laboratory matrix spike recoveries, matrix spike duplicate recoveries, and laboratory control samples.

sampling strategies. It will be achieved by use of the appropriate sampling techniques. Reviewing the precision data of the field and laboratory data will assess representativeness.

Comparability will be achieved by using standard techniques to collect and analyze representative samples and by reporting analytical results in appropriate units. When such procedures are followed then precision and accuracy are known for a data set. Thus it is possible to compare data sets.

Completeness is a measure of the amount of valid data obtained from an analytical data set compared to the intended amount of data to be obtained under normal sampling and analytical conditions. The completeness goal for this project is 100%.

Sensitivity in terms of (Method Detection Limits) MDLs and (Practical Quantitation Limits) PQLs achievable by the laboratory is available upon request. The objective of sensitivity is to have a PQL or MDL (a PQL is preferable because it is a measurement of known quality, however reporting down to the MDL is sometimes necessary to meet RECAP screening standards) below the applicable regulatory or guidance value. In this case, the applicable values are the LDEQ RECAP screening standards.

Standard calculations for precision, accuracy, and completeness will be used.

All analytical methods to be used are EPA-approved methods as described in EPA publication SW-846, “Test Methods for Evaluating Solid Waste.” Briefly, the methods to be used are: MADEP, 5035, 8260, 8270, 6010, and 7400. The laboratory to be used will be accredited by LELAP.

In addition, the quantitation limits will meet the RECAP screening standards (the reporting limits will be equal to or lower than the RECAP screening standards to ensure the usability of the data).

7.0 Special Training/CertificationPersonnel working at the site have general safety training as well as training specific to the tasks that will be completed. Environmental training includes geology, chemistry, risk assessment, and UST worker training. In addition, the laboratory maintains Louisiana Environmental Laboratory Accreditation Program (LELAP) certification (to be provided) and the driller is state-certified. Safety training and certifications are also maintained; training certificates are available upon request.

Site-specific training will be given prior to start up of the project at a Site Start-Up Meeting. This meeting will address the objectives of the job and the potential hazards associated with the tasks to be completed. The Project Manager and Site Safety and Health Officer will conduct the start up and daily training for the project.

8.0 Data Management and EvaluationAnalytical results will be emailed and mailed from the LELAP-accredited laboratory. The data generated will be reviewed to ensure that all samples have been analyzed as specified in the method. Recoveries of laboratory matrix spike/matrix spike duplicate and laboratory control samples will be checked for consistency with method accuracy. RPDs of laboratory MS/MSDs will be checked for consistency with method precision. Where sample results fall outside of acceptable ranges for accuracy and precision as given in the methods, discrepancies will be reported immediately to the laboratoryProject Manager and corrective actions will be documented.

Data verification will include confirming that all analyses for samples submitted to the laboratory have been completed, proper and consistent units have been used for reporting analytical results, and the appropriate methods were used for analysis.

If any issues arise during data verification that require resolution, the laboratory will be contacted. Issues requiring resolution may include missing documentation or changes to the analytical program.

Data validation will be performed by the laboratory to determine the usability of the data using guidance from EPA method requirements for standard EPA analytical methods.

The data will be reported in a formal analytical report from the laboratory that will be included in the Site Investigation Report. Additionally, the data will be summarized in tabular form in the report.

The LDEQ RECAP screening standards are obtained from the RECAP document. These data will be used in the decision making process to determine if the contaminants of concern are present in unacceptable concentrations (above the established risk levels) and to define the nature and extent of contamination. One limitation of the screening standards is that there is a set of standards developed, in other words, standards have not been developed for every known chemical. When a chemical is detected for which there are no screening standards, one must be developed using LDEQ guidance. The screening standards are also intended for sites of one-half acre or less in size. The VOC and TPH acceptable risk levels decrease as the acreage increases. If the area of contamination is greater than one-half acre, more specific VOC and TPH screening standards will have to be developed to account for increased site size.

9.0 Documentation and Records9.1 Field Documentation

The Geologist will maintain a field logbook while on the site. All entries into the logbook will be written in indelible ink in an objective, factual manner to provide the reader with a clear description of the sequence of events as they happened in the field. All entries will be signed and dated by the writer. The logbook will be hard bound with pages sequentially numbered. No pages will be removed from the logbook. Corrections must be lined out and initialed. The logbook will contain information such as sample descriptions, sketches, persons on site, names and phone numbers of field contacts, exceptions and deviations from the work plans, and weather and field conditions.All field activities will be documented with photographs as well as the logbook. Errors or mistakes in the original field data will be crossed out with a single line, and initialed by the person making the correction. No data will be erased. Some original documents will be transcribed, allowing for corrections of errors. In these cases, the original documents will be kept in the project file. All field logbooks will be kept on site during field activities, and moved to the project files upon completion of the project.

9.2 ReportA VRP Remedial Investigation Report will be prepared in accordance with the RECAP 2003 document. The report will include site background and will document the work performed, methods used, data obtained and evaluated, all quality control information to satisfy data quality requirements, exposure assessment, groundwater classification, risk evaluation, and copies of all field documentation (including pictures and chains-of-custody). The report will be generated in both hard copy and electronic copy. The RPCwill determine the distribution list and location and retention time for the final report.

10.0 Reconciliation with User RequirementsThe data generated under this VRP Remedial Investigation need to be sufficient to support a decision about redevelopment of the property and whether remediation is required. If so, the data must meet data requirements for risk assessment and the data must be usable under RECAP. A Risk Assessor will review the data to ensure its usability. In this way potentially unusable data such as COC reporting limits above RECAP screening standards will be identified. If problems such as this are encountered, the problem will be documented. Any limitations on the use of the data will be included in the final report.
