2 Flu Shot Vendors/Clinics

– TotalWellness Responses

o What are the minimum # of participants needed for the program (aka how many guaranteed shots must you give)? Let's say the minimum # does not sign up, is there a cost increase or fee added so that the clinic can still take place?

a. We need a minimum of 20 participants to come on-site. If 20 people do not show up on the day of the event, then we would still charge client for the 20 shots and $250 admin fee.

o What is the cost of the shot - all variations (trivalent, quad or another other anticipated forms for 2015 flu season)?

a. Please see attached pricing sheet.

o Are there any additional charges? Example include, hourly nurse fee, travel fee, minimum fees, others?

a. All prices are per person. Prices include shipping, supplies, travel costs, and biohazard disposal. The per shot rate includes a nurse to administer vaccine at a rate of 25 to 30 people per hour. There are no additional fees besides the ones that have been outlined on pricing sheet.

o What areas do you service? Local / National / Cities / States / Etc.a. TotalWellness has over 6,000 health professionals across

the country, so we can serve any location in the United States.

o Do you have coupons that can be given to spouses / employees to walk into clinics / pharmacies / or any other places to get shots that can be billed back to company / insurance? (Example: Rite-Aid/CVS/Meijer/ Costco/etc.)

a. TotalWellness provides two separate flu shot voucher programs. Vouchers are excellent for remote employees or employees working at very small sites. Clients only pay for the number of vouchers redeemed (above the minimum). You can choose the voucher expiration date; however, we recommend flu shot voucher programs run until Dec. 31, 2015 or until Feb. 28, 2016.

o Targeta. Employees can redeem flu shot vouchers at any of Target

Pharmacy’s over 1,600 locations across the United States. The corporate contact will have access to the Target Pharmacy Flu Shot Program website, where you can manage the details of your program.

o Extended Network a. TotalWellness partners with a national pharmacy benefit

manager to provide our Extended Network Voucher Program. The Extended Network encompasses over 63,500 pharmacies across the country including all major chains such as CVS, Walgreens, Wal-Mart, Kroger, and 99.9%of independent pharmacies. Participants can visit any pharmacy in the network to receive a trivalent flu vaccination. Utilization reports will be provided at the completion of the voucher program.

o Can you bill insurance direct? If so, which insurances do you accept / not accept?

a. We are currently setup to bill Blue Cross Blue Shield. However, for large clients (over 2,000 shots) we can work to set up insurance billing. Please note that the flu shot prices go up when we bill insurance direct. TotalWellness will bill for the maximum allowed by insurance, which is usually around $35 per shot. For this reason, many of our clients prefer to pay for flu shots directly.

o Is there any other information we should know that is critical to the business?

a. There is a very small window for employee flu shots (most companies want dates in September and October) so dates fill up very fast. It is always best to book early. We also require a minimum of four weeks’ notice from the time we receive a signed service agreement to the event date.

o Preservative-Free Vaccine a. According to the Centers for Disease Control and

Prevention (CDC), the flu shot is safe for pregnant or breast feeding women. However, we recommend offering preservative-free (mercury free/thimerosal free) vaccine to pregnant women. As a precautionary measure, some states have enacted legislation requiring the administration of

preservative-free vaccines to pregnant women and children. These states include: California, Delaware (Requires that everyone receive a preservative-free vaccine.), Missouri, New York, Washington

a. The minimum is 25 participants OR 90% of the guarantee, per clinic. This means the client is financially responsible for either 25 participants or 90% of their final guarantee, regardless of the number of people who actually receive a flu shot.

Examples:Guarantee 25 people. 17 receive shots. Client pays for 17 shots + 8 no shows.

Guarantee 100 people. (minimum would be 90% = 90.) 87 people get shots. Client pays for 87 shots + 3 no shows.

Guarantee 100 people (minimum would be 90% again.) 95 people receive shots. Client pays for 95 shots, no more.

a. This is currently TBD. Quest is collecting information from the major vaccine suppliers as we speak. I am hopeful a final decision will be made by the end of March at latest.

o Are there any additional charges? Example include, hourly nurse fee, travel fee, minimum fees, others?

a. Please see attached. These are 2014 prices. 2015 prices will be finalized when we receive final word on which vaccine(s) we are offering. At this point, I do not anticipate the additional charges will change from this document, but I cannot commit until all negotiations with our suppliers is complete.

o What areas do you service? Local / National / Cities / States / etc.a. Summit Health/Quest is a national company. We service

every zip code in the United States.

o Do you have coupons that can be given to spouses / employees to walk into clinics / pharmacies / or any other places to get shots that can be billed back to company / insurance? (Example: Rite-Aid/ CVS/Meijer/Costco/etc.)

a. We currently have a relationship with CVS pharmacies. They provide vouchers, which we share with clients. These are used as an offsite option, if participants or family members are unable to attend an onsite flu clinic. We invoice the client monthly for the cost of the shot plus a $6 administration fee. Last year, CVS charged $25 per voucher redeemed. If this price does not change, the cost back to the client would be $31 per voucher redeemed. There are no minimums on the voucher program.

o Can you bill insurance direct? If so, which insurances do you accept/not accept?

a. Yes, we can bill many insurance companies. This list changes continually, as we continue to negotiate agreements with various insurance companies. It would be best to provide us with the name of your primary insurance company, and we can let you know whether we are “in-network” with them, which means we can submit claims for flu shots.

a. We do not have the fees for the flu shot doses above and beyond the typical seasonal shot. We CAN get the vaccine though and pricing will be done as soon as we know our costs. I’m sorry I cannot give you a better answer at this time. We only had one request for flu shots beyond the regular season shot this year.

o Are there any additional charges? Example include, hourly nurse fee, travel fee, minimum fees, others?

a. Additional fees are tied to the numbers. Please see the attached fee sheet. It is nicely broken down.

a. Yes, we do have vouchers and the costs last year was $35. It includes just about any pharmacy but Walgreens.

a. At present we do not bill insurance carriers.

a. We would love to serve your clients. We have great references. Everything else is in the attached document.

a. We typically ask for 35 participants as a minimum, but can be more flexible with larger clients who also have some smaller locations.

a. By agreeing to the minimum, the client pays for that no matter if they do less. So, if the minimum is 35, and the client does only 20 shots, they are essentially paying for 15 additional shots that they did not receive. We review all of this up front with all of our clients so there are no surprises.

o What is the cost of the shot - all variations (trivalent, quad or any other anticipated forms for 2015 flu season)?

a. Costs vary based on amount of shots requested and by type. The larger the client the cheaper the shot, because we gain efficiencies at larger events.

a. Our pricing is all inclusive. We do charge minimums as explained above and we also have client guarantee 85% of the amount of vaccine requested. For example, if a client request 100 shots, they are guaranteeing us at least 85. If they do less they would pay the difference. While staffing is included, if a client requests an additional staff person based on their needs, our fee is $80/hour.

a. For large clients we offer a flu shot voucher program. Those vouchers can be used by anyone the client allows. These vouchers can be used nationwide at all CVS pharmacies.

a. Generally, our minimum is about 2o participants for a flu clinic. However, if there is a contract for multiple sites we will consider less. If the 20 minimum is not reached, we will still service the company but an administrative service charge of $50 will apply. Your clients are more than welcome to include their spouses or others to get to the 20 participant minimum and avoid the service charge.

o What is the cost of the shot - all variations (trivalent, quad or any other anticipated forms for 2015 flu season)?

a. We only provide quadrivalent vaccine and Flu mist. In essence, there is NO direct cost for employers or employees for a flu shot. We bill almost every insurance company there is. If there is self-pay or private pay because there is not insurance coverage, we are open to negotiation based upon number of participants. Generally, the price will range from $30 to $33.00 for onsite services.

o Are there any additional charges? Example include, hourly nurse fee, travel fee, minimum fees, others?

a. There are no other costs or hidden fees.

o What areas do you service? Local/National/Cities/States, etc.a. We service the state of Michigan. In fact, we have the state

of Michigan contract for state employees. Our primary focus has been the Lower Peninsula, however, we have done clinics in the UP depending on staffing availability.

a. We have 2 walk in sites: Southfield and Okemos, MI. We can provide coupons for spouses/employees to come to our offices and receive a flu shot.

a. Yes, see above. We bill insurances directly. There are very few insurances that we cannot bill.

o Is there any other information we should know that is critical to the business?

a. Of special interest to your clients is our online scheduling system which allows appointments within 5 minute ranges in order to reduce wait times and provide great service. Several of our larger clients utilized the system (350 to 450 employees) and were extremely pleased with the efficiency of the program.

These are the pricing as of February 6, 2015. This can change as we get closer and into the season. It also depends on volume.

a. The pricing includes: nursing fees; vaccine; ancillary supplies; documentation; online marketing tool kit; travel fees up to 30 miles, anything over that or fees like tools or parking fees will be called out prior.

depends on how long we have to resource the area.

a. While VaxAmerica does not have vouchers we are able to help coordinate vouchers through our partners.

a. We do not bill insurance. We bill directly to the company. What has worked in the past is developing a roster bill that companies can submit to their insurance company for reimbursement. Such a thing must be worked out prior to the clinic.

a. There are rare occasions where we may have to charge for mileage. With 280 branch offices nationwide, we typically have nursing staff in most major markets. In the event a nurse is traveling 60+ miles to a client, we will charge $0.55 per mile. This will be communicated to the client/broker prior to the client set up.

a. Yes, we have a partnership with Walgreens and their voucher program. The voucher can be used at any of the 8,000+ Walgreen locations nationwide. They can be given to spouses and employees depending on who the client opens up the program to. All vouchers will be billed to the company directly at a rate of $30.00 per redeemed voucher. They cannot be run through insurance.

o Can you bill insurance direct? If so, which insurance do you accept/ not accept?

a. Yes. Please see the attached list of insurance that we were able to bill in 2014. The list should look similar in 2015, however there may be a few additional carriers added prior to this year’s flu season.

o Is there any other information we should know that is critical to the business?

a. Maxim does not outsource. All of our nurses are W-2 Maxim employees. I will serve as your main point of contact for all programs. My program management team will coordinate schedules, supply/vaccine management and implementation of all programs. We run a turnkey program and will never send supplies or vaccine to a client. This will all be handled by our local branch offices.

Maxim has been providing flu shots for over 16 years. In 2014, we ran over 17,000 onsite flu clinics and immunized over 1.1M people.

We also provide onsite biometric screenings. If you have a client that is looking to provide screenings at the same time as flu shots, we can offer those as well.
