97 Households/152 individuals moved off streets and on the path to permanent housing since Jan 1 through collaborative case management

$44,375 new resources given by community ($11,750 cash) for ongoing continuum of care

1,882 volunteer hours donated

Anaheim PD Police calls reduced 11 YTD for homeless related calls (336 fewer calls in first 6 months of 2014 vs 2013)

Christina Hall: OC Food Access Coalition OC Food Access Coalition convenes multiple collaboratives in order to build the community

and resources to support access to healthy, local food options for Orange County's most nutritionally vulnerable residents. These include:

OC Farm to School Task Force - Food Service Directors, Nutritionists, Farmers, Retailers, Distributors working to increase the consumption of healthy whole foods which also expanding the amount of local purchasing of these foods to support local farmers.

Food Providers Forum - Food Pantries, Community Centers, Senior Centers and other agencies with food distribution programs meet for capacity building workshops and networking. Most similar to the Homeless Providers Forum.

Food Access Partners - Agencies, University Researchers, Retail and other organizational reps that are interested in changing the food system so that it is equitable and allows everyone to eat the healthy foods that they want to.

Food Policy Council - Advocates for policy change that will support the changes that need to be made in the overall food system at the municipal, state or federal level. Examples include work on the Federal Farm Bill or making the process easier for CalFresh recipients at the state level.

Working through county and regional collaboration, our vision is to: Create healthy food access today, for our low-income residents Build lasting solutions for food insecurity, especially ones based in systemic chance (farm to fork programs) Invest in efforts that will transform the local food system, and therefore transform the food environment in our county and region. And, the long-term vision we have is for transformed food system that supports all Orange County residents with nutritious, affordable and sustainable food options..

Our current priorities are largely focused on building an advocacy culture in Orange County. This is a major gap that exists and we need to build awareness of the issues facing our community as well as the network of representative citizens and professionals to help build a voice. We are now focusing on growing F2S in OC. At present, about half of all the school districts participate at some level but very little is really known about the procurement practices and integration with nutrition education and even garden education. We look forward to spending part of this year assessing current procurement practices.

With Orange County Health Care Agency we: Sit on the County Nutrition Action Plan as the hunger advocate group. We try to support the use of EBT and WIC at farmer’s markets as best we can. And, we are currently working with OCHCA on a tool that will help train service providers on federal nutrition programs and the importance of offering all appropriate services to clients. Through NEOP funded partners, we’re connecting the dots on Farm to School Serving as a liaison on regional food policy issues or projects folks at the County should be aware of, or be participating in.

David Gillanders: Financial Stability Alliance

Orange County Financial Stability Alliance (OCFSA) meets second Monday of each month at 3:30pm at Orange County United Way.

Pulls together partners in employment services, financial literacy, career pathways, financial institutions, partners in Orange County Free Tax Preparation program, any/all agencies geared towards helping families achieve financial stability.

Income Coalition of funded United Way partners from our first RFP offered through our strategic plan FACE2024. Includes WHW, Working Wardrobes, Goodwill Industries Orange County, And Taller San Jose. Purpose is to foster collaboration with agencies doing similar work and help create linkage to employment and career partnerships.

Participate:

Currently being informed on issues related to the incarcerated formerly incarcerated…on service providers & provision to the formerly incarcerated and the community at large.

Outcomes:

Currently this is limited to those who provide direct services to the formerly incarcerated, but may open up to all who are interested in this population.

Effective Collaboration:

Collaborative meetings remove barriers to service and people/organizations, increases networks (more people to do the work), increases collaborative capability, clarifies and increases limited knowledge, identifies select partners, maximize the utilization of existing processes, systems, people and organization assets that will facilitate a quick and productive start to the relationship, co-create new value, increased profitability and competiveness through joint creative innovation and continual improvement programs and projects…

Maricela Rios-Faust: Health & Domestic Violence Task Force THE NEED:

Nationally, between 1 in 4 and 1 in 3 women have experienced Domestic Violence as adults. Orange County is no different – according to the most recent California Health Interview Survey, 26.3% of OC women respondents have experienced physical or sexual violence by an intimate partner in their adult lifetimes. CHIS 2009.

Domestic violence creates significant short and long-term health consequences for the victim and exposed children, burdens the healthcare system, and interferes with workplace productivity. ACOG Committee Opinion No. 518, Obstet. Gynecol. 119:412-417 (Feb 2012).

In Orange County, there is no comprehensive, countywide system for addressing Domestic Violence as a health priority.

There is a growing support for DV screening and counseling by healthcare providers:

Some research and an increasing number of professional medical associations endorse DV screening and counseling of women of reproductive age.

The Affordable Care Act requires DV screening and counseling to be covered as preventive women's health service, with no cost sharing to the patient.

According to a 2013 survey funded by Blue Shield of California Foundation, California residents trust their doctors and nurses to talk with them about DV.

While some OC hospital departments are beginning to screen and counsel, there is no consistent approach for local hospitals, community clinics, and private medical offices to train providers to screen and counsel, no common resource directory nor referral mechanism, and little guidance on how to document, seek reimbursement, and/or mandatory reporting.

THE RESPONSE:

In the spring of 2013, the OCWHP partnered with the four OC-based state-funded DV agencies (Human Options, Interval House, Laura’s House, and Women’s Transitional Living Center) to launch a Health & Domestic Violence (HDV) Task Force. Approximately 40 organizations participate in the HDV Task Force, including:

CalOptima (Medi-Cal administrator)

Coalition of OC Community Health Centers

Large Hospital Systems in OC (Hoag, Kaiser, Memorial, Tenet and UCIMC)

Multiple nonprofit organizations (Casa de la Familia, CSP, MOMS, SAHARA, etc.)

OC Health Care Agency & Social Services Agency

Orange County Medical Association and American Academy of Pediatrics

Faculty from Chapman, CSU Fullerton and UC Irvine

THE PLANNING:

In October 2013, BSCF funded the OCWHP, on behalf of the HDV Task Force, to complete a six-month planning project to develop a vision for a countywide, integrated and collaborative Health & Domestic Violence System in Orange County. During the planning project the HDV Task Force:

Outreached to and engaged key stakeholders

Completed a needs assessment to identify significant gaps between the healthcare sector and the other social, legal and shelter services that support DV victims in OC

Review existing data, hospitals’ needs assessments, CBO program info

Surveys of DV shelter clients, healthcare providers, social service providers

Interviews with 13 key informants

Focus group of 7 social service providers

Review the literature and best practices

Evaluated different approaches and ultimately recommend a set of four strategies to establish such a countywide, integrated & collaborative HDV System in OC

Cross-Disciplinary Training for Healthcare and Social Service Providers

Central Clearinghouse for DV Resources and Referrals

Public Health Campaign re: DV as a Health Issue

Assess the need and the county's capacity to deliver Mental Health and Substance Abuse services for DV victims

THE OPPORTUNITY:

BSCF subsequently invited the OCWHP to apply for a grant to implement, coordinate and evaluate the strategies over a four-year period. The application was submitted in late March, a site visit was held in June, and a decision is expected this fall.

If awarded, the OCWHP will re-grant approximately $1.15 million for strategy implementation through a competitive RFP process beginning in 2015. The OCWHP will also coordinate the timing, messaging, outreach and other implementation issues for the strategies and ensure they are working together to establish the countywide, integrated, and collaborative HDV System contemplated during the planning project. Finally, the OCWHP will evaluate the collective impact of the strategies in establishing such a system and disseminate its findings in Orange County and beyond.
