Our mission is to help maintain an individual’s independence and dignity in their home and community.
The Weld County Area Agency on Aging (AAA), located in the Department of Human Services, primarily provides services to Weld County residents who are age 60 and older. The Weld County AAA also houses Options for Long-Term Care, Aging and Disability Resources for Colorado (ADRC) and Adult Protective Services for those age 18 and older.
We offer a wide range of programs, both through our office as well as with community partners, that provide supportive services and opportunities for older and vulnerable adults to enhance their health and maintain their independence.
The Weld County AAA is one of 16 AAAs in the state of Colorado. Area Agencies on Aging are part of a national network of agencies that provide a range of services to help older adults remain in their homes and communities as long as possible.

Weld County AAA offers a wide range of internal services to help vulnerable older adults live with independence and dignity in their homes and communities. To aid in this mission, there are five core services provided by Weld County AAA: Caregivers, Elder Rights, Health and Wellness, Nutrition and Supportive Services. Weld County AAA contracts with community partners or directly administers the programs to deliver these core services. The internal services offered by Weld County AAA are listed below and can also be found in the main resource guide.
Adult Protective ServicesAdult Protective Services (APS) receives reports of abuse, neglect and exploitation of at-risk adults, conducts an investigation into the allegations, and implements services to reduce and prevent further mistreatment. An at-risk adult is a person 18 years of age or older who is unable to provide or obtain the services necessary for their health, safety and welfare OR who lacks the capacity to make or understand responsible decisions.
Any concerned individual can make a report to APS by calling our intake screeners at (970) 346-7676. Each report will be evaluated to determine the appropriate response. APS also provides community education regarding abuse/neglect of at-risk adults and the services provided. Please contact (970) 400-6109 to arrange a presentation.
APS mandatory reporters must call law enforcement dispatch at (970) 350-9600 option 4 to make an initial report of physical abuse, caretaker neglect and/or financial exploitation. Mandatory reporters must call reports in on adults age 70 years of age or older or adults 18 years of age or older with an intellectual or developmental disability.
Chore ServiceThe Chore Service program provides funds to seniors for chore services which they are unable to do themselves. The program is designed to increase safety of individuals living at home. Services may include deep cleaning the home, grass/weed cutting, snow removal, etc. Weld County residents age 60 and over are eligible to apply. For more information, please contact (970) 400-6130.
Dental, Vision and HearingThe Dental, Vision and Hearing program provides financial assistance to seniors in need of dental, vision and/or hearing aid services. These may include exams, extractions, dentures, frames and/or lenses, and hearing aids. Weld County residents age 60 and over are eligible to apply.
The Visually Impaired program provides financial assistance and support to access assistive devices such as, but not limited to, personal magnifiers, talking watches or clocks, and other devices to enhance quality of life. Weld County residents age 60 and over who have a visual impairment not correctable with glasses are eligible to apply. For more information on either program, please contact (970) 400-6629.

Family Caregiver SupportThe Family Caregiver Support program offers support, resources and information to assist caregivers. We can help you find relief and assistance through a variety of respite and support programs. As you provide care for an elderly family member, many questions will arise, such as: Are they getting what they need? What other services are available to help them? How can I get some time off? We can help you answer these and other vital questions.
Support for your family member can be provided in their home, your home, a facility, or in a supportive group setting in the community. Among the choices are the following:
• Adult Day Programs: Daytime care for older or disabled adults offering social activities as well as personal care, transportation and meals.
• In-Home Respite Care: Companionship, supervision and assistance for older adults in their home.
• Out-Home Respite Care: Overnight stays in a community facility.
Weld County residents caring for an individual age 60 and over or age 55 and over with a dementia diagnosis are eligible to apply. For more information, please contact (970) 400-6130.
Friendly Fork (Senior Nutrition)The Senior Nutrition program, also known as Friendly Fork, provides nutritious and delicious lunches for seniors served at 23 sites throughout Weld County. Each meal meets 33 1/3 percent of the daily recommended dietary allowance. Social and educational opportunities are also available before and after meals.
Participants have the opportunity to contribute towards their meal. Our suggested donation is currently $4.00. Services are not denied to eligible participants who are unable to make a donation to the program.
Who can dine with us?
• If you are at least 60 years of age; or
• If you are a spouse of a person at least 60 years of age; or
• If you are under the age of 60 and are currently residing in a subsidized housing complex that offers a Senior Nutrition program meal on location.
• Guests are welcome to attend the program but are required to pay a guest fee to cover the cost of their lunch.

Reservations and cancellations are required and must be made in advance by 11:00 a.m. the previous business day to the site coordinator, or by calling (970) 346-6955.

Grandparents Raising GrandchildrenThe Grandparents Raising Grandchildren program provides support and tools to grandparents and older adult caregivers of children. The program offers support groups and educational workshops focusing on various challenges grandparents face when raising their grandchildren. Information and Assistance/Aging and Disability Resources for Colorado (ADRC)The Information and Assistance program is a link to community services, both public and private, for older adults and their families who are interested in becoming aware of the available resources in Weld County.
The Aging and Disability Resources for Colorado (ADRC) program is a single point of entry to assist people age 60 and over, or age 18 and older with a disability, who need guidance through the long-term care process. Long-term support refers to a wide range of in-home, community-based and institutional services and programs designed to help individuals access services. The ADRC program can provide Weld County residents with the following:

• Benefits Counseling
• Long-Term Futures Planning
The ADRC resource specialist is very knowledgeable about available programs and services to help meet your needs. Options Counseling is also available to give a more person-centered approach to help individuals gain an understanding of the benefits and limitations of long-term care support and service options. The goal of Options Counseling is to provide an individual with all the information needed for them to make an informed decision about long-term care. In-Home ServicesThe In-Home Services program provides financial assistance to seniors in need of homemaker or personal care assistance to remain in their homes and to live independently. Homemaker assistance may consist of meal preparation, shopping, light housework, money management or using the telephone. Personal care assistance could consist of bathing, dressing, walking, eating and other personal care activities. Weld County residents age 60 and over are eligible to apply. Long-Term Care OmbudsmanAn Ombudsman is an advocate who protects the civil and human rights of people in long-term care and assisted living facilities. They serve as a problem-solver and mediator; a resource and educator about long-term care; and an objective investigator of complaints and concerns in long-term care and assisted living facilities.
People that wish to use an Ombudsman may be residents of long-term care and assisted living facilities; friends and relatives of residents; staff members of facilities; and the community at large. For more information, please contact (970) 400-6128.

Options for Long-Term CareOptions for Long-Term Care (OLTC) provides functional assessments and ongoing case management for low-income clients of all ages who are in need of Medicaid long-term care services in their own homes, alternative care facilities, or for people in need of placement in a skilled nursing facility. In conjunction with the client, the case manager creates a plan of care and assists to arrange services. The case manager provides follow-up monitoring and links clients and families to community resources.
Weld County residents in need of long-term care services must first apply for Medicaid through Weld County Department of Human Services or online through Colorado PEAK at www.colorado.gov/PEAK. The OLTC goal is to provide long-term care services and supports in a setting requested by clients and their families while utilizing Medicaid in the most cost-effective manner.

Volunteer OpportunitiesThere are multiple opportunities to volunteer for the Weld County Area Agency on Aging:
• Senior Nutrition: Serve food, take reservations and complete paperwork at a meal site.
• Weld Aging Well: Become a trained leader and offer workshops in your community.
• Administration: Perform general office and clerical duties or become a board member.

Weld Aging WellThe Weld Aging Well program offers multiple workshops and classes for older adults to take control of their health and remain active in the community and at home. Topics covered include:
Fall Prevention Programs
• A Matter of Balance: Learn ways to reduce the fear of falling and break the fear of falling cycle by increasing activity levels. The program focuses on practical coping strategies. A variety of activities address physical, social and cognitive factors affecting the fear of falling.
• Tai Chi: Moving for Better Balance: A group fall prevention program that uses a unique combination of eight single forms that are derived from the traditional 24-form Yang-style Tai Chi. The program goals are to improve functional ability such as balance and physical function; reduce fall-related risks; and reduce fall frequency.

• Strong People: A strength-training program that builds bone density and muscle mass and improves joint health. The program also reduces risk for diabetes, heart disease, arthritis, depression and obesity. Learn appropriate exercises and proper technique at an adequate intensity level specific for you.
Self-Management Programs
• Healthier Living: Learn tools to become a better self-manager of your chronic health condition(s). People with heart disease, arthritis, diabetes, asthma, COPD or any other health condition(s) and/or their caregiver are encouraged to attend. Participants learn how to reduce pain and stress, eat well and exercise, manage symptoms and communicate better with others.
• Healthier Living Diabetes: Learn tools to become a better self-manager of your diabetes. People with diabetes and/or their caregiver are encouraged to attend. Participants learn how to reduce stress, read nutrition labels, prevent low blood sugar, delay complications and understand the importance of exercise.
Caregiver Programs
• Powerful Tools for Caregivers: This workshop empowers the family caregiver to find ways to reduce stress, manage emotions such as guilt and anger, improve communication with others and to take better care of themselves.
• Stress-Busting: Learn new stress management techniques, relaxation and coping skills. The evidence-based workshop is for family caregivers only, and helps them gain the tools to thrive while caregiving.

Hispanic Elderly OutreachBilingual workers assist Hispanic seniors by connecting them to community resources. For more information, contact Catholic Charities Hispanic Elderly Outreach in Fort Lupton at (303) 857-0521.
Home Delivered MealsSpecially prepared, nutritious meals are delivered to the home five days per week. For more information, contact Meals on Wheels in Greeley at (970) 353-9738.
Legal ServicesLegal services for Weld County residents age 60 and over to assist with public assistance benefits, preparation of living wills and powers of attorney, landlord or rent issues, senior housing and other personal rights issues. Peer CounselingCounseling services help older adults cope with the emotional stresses related to aging. The service is provided in the senior’s home by trained counselors who are also seniors. For more information, contact North Range Behavioral Health at (970) 347-2420.
TransportationVolunteers provide transportation for appointments, grocery shopping, trips to senior centers and more for seniors in need. For more information about the 60+ Ride program, contact Senior Resource Services at (970) 352-9348.

Volunteer Opportunities
The Weld County Area Agency on Aging has dedicated staff that manages programs, oversees community partnerships and provides leadership in the planning and coordination of services for older adults and adults living with disabilities in Weld County.
AAA Advisory BoardThe Weld County AAA has a dedicated advisory board comprised of 18 community members and representatives of community organizations who help to enhance the leadership role of the Weld County AAA.
The board provides advisory functions to the Weld County AAA and county commissioners while assisting in the planning, coordination and monitoring of Older Americans Act programs.
The board also provides feedback on such issues as healthcare, transportation and housing needs; suggests ideas for improvement in services; and assists with public hearings, focus groups and the development of the four-year plan, along with other agency activities and events.
