Q: I’m not familiar with the Quest Diagnostic registration site. How do I schedule an appointment for my wellness screening? A: Click “Sign Up Now” in the green box. NOTE: Unless you have a username and password from a previous Quest Diagnostics screening, you will need to register on this website as a new user, and create a new login account by clicking “Sign Up Now” in the green box.

Personal Health Information Confidentiality and Security Q: Will NXP ever see or have access to my personal test results? A: No. Protecting your privacy and that of your spouse/domestic partner is as important to NXP as it is to you. Individual screening results and specific health risks are confidential. Your screening results are managed independently by Quest Diagnostics and will never be shared with NXP. Quest Diagnostics does provide NXP an aggregate report of the entire screened population. This is de-identified aggregated data that enables us to shape our plans and programs to meet the health needs of U.S. employees. Quest Diagnostics also provides notification to NXP Rewards Customer Service confirming your completion status which allows us to determine your eligibility for the annual wellness incentive award. Q: How secure is my data? A: NXP’s careful review and due diligence process have ensured that Quest Diagnostics is compliant with all HIPAA (Health Insurance Portability and Accountability Act of 1996) and Federal policies for safeguarding your medical data. Security measures are in place to prevent the loss, misuse or alteration of the information you submit and all intranet communications with the websites are done under SSL encryption to protect the data in transit. Quest Diagnostics web pages are designed with security and privacy as their primary criteria and the site’s servers are constantly, automatically monitored for intrusions. Their network and computer security is regularly maintained and has been audited by an external security team. Q: What happens with my data once I register or complete the wellness screenings? A: Your personal screening results are not shared with anyone but you. Once you complete the wellness screening, Quest Diagnostics notifies NXP Rewards Customer Service of your completion status. The completion status indicates that you have completed the wellness screening. Your personal information is kept private in accordance with the Notice of Privacy Practice and HIPAA requirements. Q. What happens if I do not agree to the Terms of Service? Will I receive the wellness incentive? A. If you do not agree to the terms of service, you are confirming that you don’t want to be screened. Your name will not appear on the file sent to NXP Rewards Customer Service and you will not be eligible for the wellness incentive. Q: Can I opt out of the NXP Employee Health Benefit Plan entirely so my data is not stored anywhere? A: Participation in the annual wellness screening is included as part of the NXP Employee Health Benefit Plan. If you would like to opt out of the NXP Employee Health Benefit Plans entirely, you must make that

election annually by calling NXP Rewards Customer Service at (888) 375-2367. It may take up to 30 days to process your opt-out request. Please remember that if you choose to opt-out, you will not be eligible for the wellness incentive. Q: Can I access my results online, or after I receive my onsite screening results at my home address? A: Yes. If you registered with Quest online and provided an email address you will receive an email when your results are available to view online, usually about 5 days after your screening, if you took the optional health questionnaire. If you choose not to take the optional Health Questionnaire, your screening results will be available online following the wellness incentive deadline of August 31, 2016. To view your results online go to My.QuestForHealth.com, if you previously created a username and password enter your user name and password you created when registering for an appointment. If you did not register for an appointment, click the Sign Up Now button in the green box and follow the instructions on establishing a username and password carefully. Review this FAQ for tips on registering. Q: How can I get my past screening history? A: Former Freescale: You can access your previous year’s screening results by completing this form and mailing or faxing it to the address or fax number on the form. Q: If I choose to visit my health care provider, what information does the Physician Results Form ask for and is my personal health data shared or stored with NXP or Quest Diagnostics? A: The Physician Results Form includes the exam date, lab values, healthcare provider signature, address, phone number and office stamp (if available) and date it was faxed to Quest Diagnostics. Your doctor is asked to measure your blood pressure, height, weight, lipid panel and glucose. Your personal results will never be shared with NXP. If you do not want to share your results with Quest Diagnostics, your doctor only needs to sign and date the form. Additionally, you are responsible for ensuring your provider submits the Physicians Results Form to Quest Diagnostics. We highly recommend you personally fax the completed form using the contact information listed at the top of the form to ensure your form is submitted by the August 31, 2016 deadline.

Who is eligible? Q: Who is eligible to participate in the NXP wellness screening?

A: All U.S. active employees regularly scheduled for work at least 20 hours per week. If you are eligible for NXP benefits, you may participate in the wellness screening. In addition, all spouses/domestic partners that are enrolled in an NXP medical plan are eligible to participate in the wellness screening. Please ensure your spouse/domestic partner is escorted to the onsite screening as required by your location. Only employees enrolled in the NXP medical plan (Kaiser and Aetna) are eligible for the wellness incentive. Spousal/domestic partner screening participation is not required to receive the wellness incentive. Q: Can a retiree and their spouse participate in the onsite wellness screenings? A: No. Insured retirees and their spouses who are insured on NXP’s medical plan are strongly encouraged to visit their health care provider for their regular, recommended wellness check-ups. Q: Who is eligible to receive the wellness incentive?

Employees enrolled in the 2017 medical plan. Spouses aren’t eligible to earn the wellness incentive.

Regardless of the screening option you choose, the 2017 wellness incentive will be earned as long as the biometric values were collected between August 8, 2015 and August 31, 2016 and Quest Diagnostics has received them by August 31, 2016 no later than 11:59 p.m. in your time zone.

If you were hired or rehired between July 1 and Dec. 31, 2016, you will automatically receive the 2017 wellness incentive if you are enrolled in the 2017 medical plan.

If you are not currently enrolled in the NXP medical plan, but might enroll in 2017 as a result of a life event such as marriage, birth of a child, etc., you should participate in a wellness screening by Aug. 31, 2016 in order to receive the 2017 wellness incentive.

Q: Does the 2017 wellness incentive apply to all NXP Medical Plans? A: The 2017 wellness incentive will apply to all NXP medical plans offered in 2017. It’s too early for us to share details about the 2017 medical plan offering, but we will communicate more about the plan changes and offering during the Annual Enrollment meetings in October. Q: Can my spouse/domestic partner participate in the onsite wellness screenings? A: All employees and spouses/domestic partners that are enrolled in an NXP medical plan are eligible and encouraged to participate in the wellness screening and optional health risk assessment at no cost. Only employees enrolled in the NXP medical plan are eligible for the wellness incentive. Spousal/domestic partner participation is not required to receive the wellness incentive. Please ensure your spouse/domestic partner is escorted to the onsite wellness screening as required by your location. Q: My spouse and I both work for NXP. How do we both receive the wellness incentive? A: In order for you both to receive the wellness incentive, you are each required to complete the wellness screening by the deadline, August 31, 2016. Children of all ages, including those covered by NXP, are strongly encouraged to visit their pediatrician/health care provider for their regular, recommended wellness check-ups. Q: My child(ren) and I both work for NXP. How do we both receive the wellness incentive? A: In order for you both to receive the wellness incentive you are each required to complete the wellness screening by the deadline, August 31, 2016. In addition, you each need to enroll in the NXP medical plan individually during NXP’s Annual Enrollment period. Q. I am not enrolled in the NXP medical plan. Do I need to participate in a wellness screening? A. No. But if you are considering enrolling in the medical plan in 2017 for any reason, this is a valuable opportunity to complete the screening required to earn the wellness incentive, at no charge. If you are not enrolled in the medical plan, you are not eligible for the wellness incentive. Q. I am not enrolled in the NXP medical plan. Can I participate in a wellness screening? A. Yes. But if you are not enrolled in the medical plan, you are not eligible for the wellness incentive. Q: Can contractors participate in the wellness screenings? A: No. Contractors are not eligible to participate in the wellness screenings. We encourage you to check with your employer regarding screening opportunities or visit your health care provider.

Screening Process Questions Q: Where do I register for my wellness screening? A: Register at https://my.questforhealth.com. Q: The system requires a registration key. What is it? A: The registration key for scheduling a wellness screening is NXP. Q: What are my options for completing a wellness screening? A: There are three options to complete the wellness screening – onsite, screened at one of many Quest Patient Service Centers, or with your personal health care provider. Q: What is the deadline for completing the screening to earn the 2017 wellness incentive? A: The 2017 wellness incentive is based on your completion of a wellness screening by Aug. 31, 2016. Q: Do I need to fast before being screened? A: Yes. For accurate results, you should fast (not eat or drink anything other than water) for eight (8) hours before your wellness screening appointment, though it is not mandatory. You should take any

regularly scheduled medicines as usual. You should drink at least two glasses of water one hour prior to your screening. Q: Is there anything that changed between last year’s process and this year’s process for Former Freescale employees? A: There are differences for Former Freescale employees:

Former Freescale employees used a different screening provider in previous years. To earn the 2017 wellness incentive please register through the Quest Diagnostics registration site using NXP as your registration key.

Former Freescale employees were required to complete the online Health Power Profile, also known as Health Risk Questionnaire, in addition to the physical/biometric screening to earn their wellness incentive. You no longer are required to complete the online Health Questionnaire to earn their wellness incentive; you just need to complete the physical/biometric screening either onsite, through your health provider or through Quest Patient Service Center. If you do not to take the optional Health Questionnaire, your screening results will be sent to you following the wellness incentive deadline of August 31, 2016.

In previous years, it was not a requirement to be enrolled in an NXP medical plan for a spouse/domestic partner to be eligible for the onsite screening. In 2016, spouse/domestic partners are able to participate onsite only if they are enrolled as a dependent in an NXP medical plan.

Q: Is there anything that changed between last year’s process and this year’s process for Legacy NXP employees? A: There are differences for Legacy NXP employees:

Legacy NXP employees haven’t had a medical premium discount tied to wellness screenings in previous years. Starting this year legacy NXP employees will have to register and complete a wellness screening through Quest to earn the 2017 medical premium discount

The onsite screenings will include a venipuncture blood draw instead of using a finger-stick blood collection.

The process for completing the wellness screening is different as well. You must register through the Quest Diagnostics registration site at https://my.questforhealth.com using registration key NXP to schedule your screening. You have three options for completing your screening and all are initiated through the Quest Diagnostics registration site:

1. Onsite screening, if available in your location 2. Download the Physician Results Form and have your personal physician complete and

sign that for you. It is your responsibility to fax or mail in your signed health provider form to the contact listed at the top of the form. In addition, you are responsible for any fees associated with the office visit and/or testing.

3. Download the Patient Service Center Form and take that form with you to a local Quest Patient Service Center.

Regardless of the option you choose, the 2017 wellness incentive will be earned as long as the biometric values were collected between August 8, 2015 and August 31, 2016 and Quest Diagnostics has received them by August 31, 2016 no later than 11:59 p.m. in your time zone.

Q: What is involved with the wellness screening? A: Your wellness screening is a two part process, the physical/biometric screening and an optional online Health Questionnaire, also referred to as a health risk assessment (HRA). The physical/biometric screening includes blood pressure, height, weight and waist measurement, BMI calculation and a fasting lipid and glucose venipuncture blood draw. The optional Health Questionnaire is an online assessment related to lifestyle, nutrition and exercise. The results from the physical/biometric screening and the Health Questionnaire are combined in a personal report to provide you an overall review of your health and lifestyle profile. Q: What is involved with the onsite wellness screening? A: The wellness screening includes: blood pressure, height and weight measurement, waist circumference measurement, fasting lipid panel and glucose test via venipuncture. The lipid panel includes total cholesterol, HDL (good cholesterol), LDL (bad cholesterol), triglycerides, total

Q: What additional tests are available during the onsite screening? And what is the cost? A: Additional tests will not be available during the onsite events. You are encouraged to visit your health care provider for additional tests. Q: What is involved with the Patient Service Center screening? A: To complete a Patient Service Center (PSC) screening, you must register for this option on the Quest Diagnostics registration site. Once registered, download and print the lab order to present to the lab indicated on your form at the time of service. The wellness screening includes a fasting lipid panel and glucose test via venipuncture. The panel includes total cholesterol, HDL (good cholesterol), LDL (bad cholesterol), triglycerides, total cholesterol/HDL ratio and glucose. Blood pressure, height and weight are also reported. In order to receive the 2017 wellness incentive, you must register for the Patient Service

Center Screening through the Quest Diagnostics registration site and complete the screening by August

31, 2016. Q: What is Sonoma Quest Laboratories? A: Quest Diagnostics Health & Wellness Services screenings in the State of Arizona are collected at Sonora Quest Laboratories facilities and forwarded to Quest Diagnostics for testing. Q: How do I schedule the Patient Service Center Screening through Sonoma Quest Laboratories? A: You must register at My.QuestForHealth.com before visiting a Sonora Quest facility for collection, or contacting Sonora Quest to schedule an appointment. To schedule an appointment, call (855) 367-2778 Monday through Friday from 7 a.m. to 5 p.m., email [email protected] or schedule online at www.SonoraQuest.com. Appointments should be scheduled as Biometrics (Blueprint). For a list of participating Sonoma Quest Laboratories click HERE. For all other questions, contact the Participant Service Center at [email protected] or by phone (855) 623-9355. Q: Can I request additional tests with the Patient Service Center screening? A: No. You are encouraged to visit your health care provider for additional tests. Q: What is involved with the Physician Results Form? Q: Can I request additional tests with the Physician Results Form? A: You are encouraged to discuss additional testing needs with your health care provider. In most cases, additional testing is covered by your NXP medical plan. For details, please contact your medical carrier directly. Q: What is involved in the optional Health Questionnaire? A: The optional Health Questionnaire, also referred to as a health risk assessment (HRA), is an online assessment pertaining to lifestyle, nutrition and exercise that should take about 10-15 minutes to complete. To receive a comprehensive results packet (MyHealth Profile), you must complete the optional Health Questionnaire online. You are not required to complete the optional Health Questionnaire to earn the 2017 wellness incentive. If you choose not to take the Health Questionnaire, just your screening results (MyGuide to Health Profile) will be sent to you following the wellness incentive deadline of August 31, 2016.

Q: What is My Health Profile? A: MyHealth Profile is a personalized report of health status and risk based upon integrated screening results and Health Questionnaire data. My Health communicates a Health Quotient Score to reflect a participant’s degree of health risk for developing chronic illness. To receive this profile, participants must complete the optional Health Questionnaire along with their wellness screening. Q: What is MyGuide to Health? A: MyGuide to Health is a personalized report of a participant’s health status based upon their screening results. Employees who choose not to complete the optional Health Questionnaire will receive this report following the wellness incentive deadline of August 31, 2016. Q: What is a glucose test? A: A glucose test measures the amount of sugar in your blood. This test is used to help detect diabetes and other glucose metabolism disorders. Q: What is a lipid screening? A: A lipid screening includes total cholesterol, triglycerides, high density lipoprotein (HDL, or “good cholesterol”), low density lipoprotein (LDL, or “bad cholesterol”) and total cholesterol to HDL ratio (predicts risk of heart disease). Q. Can I get a flu shot during my onsite wellness screening? A: Flu vaccinations are not available during the onsite wellness screenings. However, flu vaccinations will be offered in the Fall of 2016 – stay tuned to your NXP email for more information. Q. I completed the wellness screening in April; can I repeat the wellness screening in June? A. No. The onsite wellness screening is offered at no cost to you, and spouse/domestic partners covered by the NXP medical plan once a year. If you are monitoring your results or a condition, you are encouraged to follow up with your health care provider.

General Program Questions Q: What is the 2017 wellness incentive? A: The 2017 wellness incentive is based on your completion of a wellness screening by Aug. 31, 2016. It’s too early for us to discuss the 2017 healthcare strategy, but we do know that wellness will continue as a key component, and we will communicate more about the incentive strategy during the Annual Enrollment meetings in October. The incentive is designed to contribute toward the health and well-being of all NXP employees. Q: Why is NXP offering the wellness incentive? A: Living a healthy lifestyle has many advantages for you and NXP. Helping you know your numbers and improve your health is the right thing to do, both from a personal and business perspective. Just as NXP is always striving for improved quality and customer service, your wellness program aims to take advantage of the personal and financial benefits of better health. Q: How much is the 2017 wellness incentive? A: Your NXP medical plan design for 2017, including premium payments and covered expenses, will be communicated in October 2016. It is anticipated that the wellness incentive amount for 2017 will be similar in value to previous year incentive amounts. Because NXP is self-insured and pays approximately $69 million for medical plan expenses each year, the premium amounts that you pay depend on the cost of the plan. NXP remains focused on providing you with a cost competitive and high quality benefits. Q: Why should I complete a wellness screening and the optional Health Questionnaire? A: The wellness screening gives you an overall look at your biometric values. The optional Health Questionnaire takes those values, and self-reported information about your lifestyle and health choices, to provide you an overall review of your health and lifestyle profile (MyHealth Profile). This is critical to help you understand what you can do to improve your health and other factors that could possibly be impacting your overall health, such as your stress levels, nutrition, and exercise activities. You are not required to complete the Health Questionnaire to earn the 2017 wellness incentive. If you choose not to

take the questionnaire, just your screening results (MyGuide to Health Profile) will be sent to you following the wellness incentive deadline of August 31, 2016. Q: Will my benefits be impacted if I elect not to participate in a wellness screening? A: If you elect not to participate in a wellness screening, you will not receive the 2017 wellness incentive. In previous years this has meant you will pay more in medical premiums. Again, it’s too early for us to discuss the 2017 healthcare strategy, but we do know that wellness will continue as a key component and will communicate more about the incentive strategy during the Annual Enrollment meetings in October 2016. Q: Where are the onsite wellness screenings being held and how do I register? A: Wellness screenings will be held onsite at the Austin, TX, Detroit, MI, Chandler and Tempe, AZ, and San Jose, CA NXP locations. To register, visit the Quest Diagnostics registration site and follow the instructions. Q: I am a remote employee and cannot make any of the scheduled onsite wellness screenings. How can I qualify for the 2017 wellness incentive? A: You can complete your wellness screening and receive the 2017 wellness incentive by selecting either the Physician Results Form or at a Quest Patient Service Center by Aug. 31, 2016. Q: Will I receive an email confirmation after registering for an onsite wellness screening? A: Yes, you will receive an email confirmation within 24 - 48 hours after registering. If you do not receive the email, first check your email quarantine/spam folder then contact Quest Diagnostics at (855) 623-9355, if needed. To ensure you receive personal attention at your wellness screening, the schedule is designed to accommodate a limited number of sign-ups. If your first choice is unavailable, please check for a different date or time. If the schedule reaches capacity, we will evaluate the possibility of additional wellness screenings. Q: What if I need to reschedule my wellness screening? A: To reschedule login to the Quest Diagnostics website, enter the requested information, and then select “Your Screening” tab. From the confirmation screen, click on cancel this appointment. You will then need to go back in and schedule an appointment. Q: How do I sign up for a new screening appointment if I missed my previously scheduled time? To check the status of the optional Health Questionnaire, log in using the username and password you created when registering as a new user. Once logged in, select the Health Questionnaire tab to check the status. If you choose not to take the questionnaire, just your screening results (MyGuide to Health Profile) will be sent to you following the wellness incentive deadline of August 31, 2016. Q: I cannot make any of the scheduled onsite wellness screenings. How can I qualify for the 2017 wellness incentive? A: If you are unable to complete an onsite wellness screening, you can complete your wellness screening and receive the 2017 wellness incentive by selecting either the Physician Results Form or at a Patient Service Center by Aug. 31, 2016.

Q. What if I am unable to participate in a wellness screening or it is not recommended by my health care provider due to a medical condition? A: If you are unable to participate due to a medical condition, download and submit the Physician Results Form from the Quest Diagnostics website by Aug. 31, 2016. Simply ask your health care provider to sign and date the form before submitting to Quest. The form must be faxed to Quest Diagnostics at the number at the top of the form by Aug. 31, 2016 to receive your 2017 wellness incentive. With health care provider approval, you do not need to complete the optional Health Questionnaire. Q: Is there anything else I need to know or do when completing my wellness screening at a Patient Service Center or with my health care provider? A: No. Make sure you submit your form by the deadline, Aug. 31, 2016 to receive the 2017 wellness incentive. Q: How do I arrange my wellness screening at a Patient Service Center? A: Visit the Quest Diagnostics registration site, enter the requested information, and select “At a Patient Service Center”. Complete the registration process, print the lab order and remember to take it to the lab location listed on the form on the day of your screening. On the day of your wellness screening you should fast (not eat or drink anything other than water) for eight (8) hours before your appointment. You should take any regularly scheduled medicines as usual. You should drink at least two glasses of water one hour prior to your appointment. There is no cost for the wellness screening when performed at the lab you are instructed to use. Q: I just visited Quest Diagnostics and had my blood drawn. When will I receive the results? A. You will receive your results in the mail within three weeks of your wellness screening date at Quest Diagnostics, if you completed the optional Health Questionnaire. If you did not complete the optional health Questionnaire, your results will be sent to you following the wellness incentive deadline of August 31, 2016. Q. What if I recently completed a lipid panel and glucose blood test? A: If you completed the required tests with your health care provider on or after August 8, 2015 you may download the Physician Results Form from the Quest Diagnostics registration site then ask your health care provider to fax the completed and signed form to Quest Diagnostics at the number at the top of the form. You are responsible for coordinating the delivery of your wellness screening data from your provider to Quest by the deadline. Q: I participate in medical surveillance testing coordinated through the NXP Occupational Health Nurse and EHS department. Does the physical I complete count towards the 2017 wellness incentive? A: No, if your physical included blood lab work and was completed between 8/8/2015 and 8/31/2016, you must select the Physicians Result Form from the Quest Diagnostics registration site. Q: Can I complete the optional Health Questionnaire before my wellness screening appointment at my facility? A: Yes. This year, there will not be an opportunity to complete the optional Health Questionnaire during your onsite wellness screening events. If you wish to complete the optional online Health Questionnaire in advance, you may do so when you register for your onsite appointment. Q: Can I complete my optional Health Questionnaire during an onsite wellness screening? A: No. Unfortunately Quest Diagnostics will not have devices for participants to take the optional Health Questionnaire at the onsite events. Q: If I choose to take the optional Health Questionnaire online after completing the onsite wellness screening, will I still receive my lab values in my results packet? A: Yes. Once the optional Health Questionnaire is complete, you will receive a comprehensive results packet. If you decide not to take the optional Health Questionnaire, your results will be sent to you following the wellness incentive deadline of August 31, 2016.

Q: How do I ensure I receive the wellness incentive?

A: You can view your incentive status at www.nxp.com/rewards. Please allow up to five weeks after your

screening and Health Questionnaire for your incentive status to be updated with NXP Rewards Customer Service and on the NXP Advantage website. If you did not complete the optional Health Questionnaire, your completer status will not show up until 5 weeks following the wellness incentive deadline of August 31, 2016. Q: If I participated in the wellness screening last year, do I have to participate again this year? A: The wellness screening is optional, but if you want to earn the wellness incentive you need to participate in the wellness screening. If you participate in the wellness screening and submit your documentation between 8/8/2015 and 8/31/2016 it will count toward the 2017 wellness incentive. Q. How long is my 2017 wellness incentive status valid? A: Your 2017 wellness incentive is valid if you are enrolled in the NXP medical plan from Jan. 1 through Dec. 31, 2017.

Q: My health care provider faxed in my form. When will I receive the results? A: A results packet will be sent to you 3 weeks after submitting your Physicians Result Form to your doctor, if you have already completed the optional Health Questionnaire. If you did not complete the optional Health Questionnaire, your results will be sent to you following the wellness incentive deadline of August 31, 2016. Q: Will I have any out-of-pocket expenses related to the wellness screenings? A: NXP will pay for the cost of one onsite or lab wellness screening per year for employees and ensured spouses/domestic partners. If you choose to complete your wellness screening at your physician’s office, your standard medical benefits apply. This means you will pay the applicable copay or deductible and coinsurance. Q: If my physician charges to complete the provider form, will NXP pay for it? A: No. You are responsible for that cost if applicable. The onsite and lab programs are available to you at no cost. Q: What is the deadline to complete the wellness screenings? A: The final series of onsite wellness screenings is July 5-8 (TX locations) and July 19-21 (AZ locations). Please check Quest Diagnostics website for dates. The last day to complete and submit at a Patient Service Center or Physician Results Form Patient Service Center is Aug. 31, 2016 by 11:59 p.m. in your time zone. Q: What happens if I miss the deadline to complete the wellness screenings? A: If your wellness screening is not completed by the close of business Aug. 31, 2016 you will not be eligible for the 2017 wellness incentive and will not receive the discount on your 2017 medical premiums. Q: I am enrolled in the NXP medical savings plan with single coverage so my payroll deduction is $0 each pay period. Will I receive a credit if I complete the wellness screening and become eligible for the wellness incentive? A: If you participate in the wellness screening by the deadline you can avoid paying an additional amount for your 2017 medical premiums. The $0 employee-only coverage cost you are paying now, in 2016, is based on completing a wellness screening in 2015. If you did not complete the wellness screening in 2015 you would be paying $9 per pay period. Q. Will my wellness screening results be used to update my tobacco user status for medical and life insurance premiums during Annual Enrollment in October? A: No. You are required to verify your status when you enroll during Annual Enrollment. Q. Will you charge me more for premiums in future years if my wellness screening results are not ideal or if I have a specific chronic health condition such as diabetes? A: No. The purpose of the program is to offer you awareness programs and services to enhance your health and well-being. Annual premiums are impacted each year by legislative actions and claims costs. You will be updated as these occur. Q: How will medical premiums be determined in future years? A: NXP’s medical premiums are determined in part by the annual cost of medical claims. NXP is self-insured and pays more than $69 million per year for medical claims. If this amount changes, the medical premium you pay is reflected in your paycheck. The healthier our employees are, the lower our claims costs, which results in lower costs overall for you and for NXP. Q: Will my doctor receive a copy of the wellness screening results? A: No. You are encouraged to provide a copy of your results to your physician.
