What is Direct Debit? Direct Debits are a way to pay your international health insurance premium from your current or savings account. You can set up a direct debit by signing a Direct Debit Mandate form; Now Health on behalf of AXA Gulf then takes money from your bank account automatically according to your instructions. What are the advantages of Direct Debit? As a customer you can take advantage of this service by making your recurring payments through the UAE Direct Debit System (UAEDDS). The key benefits are: It is secure and there is a one-time set up It is mandate driven There are no worries about remembering to pay at the right time. The only thing you need to remember is that there is sufficient cash in the account to cover the payment There is no need to write post-dated cheques, It is automated so there is no need for manual tracking to meet payment deadlines It eliminates the need for paper-based payment instructions for recurring payments Greater convenience Would there be a reference number or acknowledgement against a direct debit set up request? Yes, once the Direct Debit has been set up there would be a 23-digit unique identifier code, which can be obtained by contacting your bank. What are the charges that would be levied for each direct debit into my account? If sufficient funds are held in your account to honour the direct debit requests, then there will be no charges. If the direct debit is returned unpaid, they you may get charged from your bank, however they will notify you of this. Please note the standard surcharge for monthly (5%) or quarterly (3%) payment frequency will still apply What if my Direct Debit is returned unpaid? The Now Health team will contact you to notify you that the payment was unsuccessful and they will confirm a date to retake the Direct Debit payment Would I be allowed to change my paying bank details, if I have enrolled for direct debit? All direct debits must be to taken from a bank registered with the UAE Central Bank, based in any of the UAE Emirates. Can I setup direct debit from my joint account? Yes, direct debits can be setup from a joint account. At the time of setting up the direct debit make sure to imprint the signatures of all those authorised as per the mandate that you have setup at your bank.

Will there be direct debit on Fridays? What if I am not able to fund my account due to a public holiday(s)? Direct Debit instructions will not be executed on Fridays and other declared UAE Banking Holidays. If the due date for a payment is during the holidays, then the payment will be due any time within seven days immediately following the holidays. You should always ensure sufficient funds are available in your account to honour direct debit commitments

Will you take all my payments by Direct Debit? Due to the set up time required between Now Health, UAE Central Bank and the other banks involved, the first policy instalment payment will need to be made by cheque; bank transfer or credit card and we will take the following instalment premiums by direct debit.

Why do you require additional ID? Central bank requires additional ‘Know Your Customer’ (KYC) information, during the verification process; this can be your passport number, Emirates ID number, Driving licence number or Family book number, which will be completed on the application form under Customer ID & Type

How to set up a Direct Debit to pay your insurance premium

The following information must be completed by the customer on Direct Debit form

• Bank Name (The name of your bank i.e. Citi Bank, Emirates NBD, ADIB) • Title of Account (Your name on the account) • IBAN (IBAN number from the account you would like debiting) • Customer ID & Type please provide one of the following identification numbers,

using the abbreviation codes & then the corresponding number from your ID o Passport number - PASSP o Emirates ID number - EIDAC o Driving licence number - DRVLN o Family Book number - FAMBK

• Please read the terms and conditions, sign and date the form

For the following information can be completed by the now Now Health team on behalf of AXA, alternatively the client can complete it (all information is as per the invoice)

• Commences on (Policy start date) • Expires on (Policy expiry date) • Payment frequency (Please tick your payment frequency: monthly, quarterly,

half-yearly)

• Fixed Amount - depending on your payment frequency this is your policy amount divided by:

⇒ 12, if you are paying monthly ⇒ 4, if you are paying quarterly ⇒ 2, if you are paying half-yearly

Please note the value must be in AED and not USD, please use exchange rate of 3.68 AED to 1 USD – please do not round this figure up or down as it will need to be the exact amount.

If the payee is not the planholder, please inform your customer service team. Please note the form must be completed with the payee’s details and not the planholder’s details.

What happens now?

• The original form needs to be sent to Now Health (please note copies of the form will not be accepted by UAE Central bank)

Customer Service Now Health International Ground Floor, Al Shaiba Building, PO Box 502163, Dubai

• You will make your 1st premium payment by credit card, cheque or bank transfer and we will issue you your policy pack

• Now Health will submit your direct debit form to UAE Central Bank for set up, who will contact your bank to check all the details

• We will then receive confirmation of set up from your bank and update our system

• Your 2nd premium instalment onwards will be taken by direct debit on your policy inception day, every month or quarter i.e. your policy starts on 1st month, we will take your monthly payments on the 1st of each month

• Please note you cannot change the payment due date, as this is governed by the original invoice, which is submitted to the Central Bank

• We will only contact you if there is any missing information with your direct debit form or we were unable to process a payment
